- Do not assume IPv6 is not working when getaddrinfo is not present.
The check to see if IPv6 actually works is now independent of whether
there is any resolver that can potentially resolve a hostname to IPv6.
Prior to this change if getaddrinfo() was not found at compile time then
Curl_ipv6works() would be defined as a macro that returns FALSE.
When getaddrinfo is not found then libcurl is built with CURLRES_IPV4
defined instead of CURLRES_IPV6, meaning that it cannot do IPv6 lookups
in the traditional way. With this commit if libcurl is built with IPv6
support (ENABLE_IPV6) but without getaddrinfo (CURLRES_IPV6), and the
IPv6 stack is actually working, then it is possible for libcurl to
resolve IPv6 addresses by using DoH.

In some situations, it was possible that a transfer was setup to
use an specific IP version, but due do DNS caching or connection
reuse, it ended up using a different IP version from requested.
This commit changes the effect of CURLOPT_IPRESOLVE from simply
restricting address resolution to preventing the wrong connection
type being used, when choosing a connection from the pool, and
to restricting what addresses could be used when establishing
a new connection.
It is important that all addresses versions are resolved, even if
not used in that transfer in particular, because the result is
cached, and could be useful for a different transfer with a
different CURLOPT_IPRESOLVE setting.

- We need the transfer handle to log, store data and more. Everything in
libcurl is driven by a transfer (the CURL * in the public API).
- This work clarifies and separates the transfers from the connections
better.
- We should avoid "conn->data". Since individual connections can be used
by many transfers when multiplexing, making sure that conn->data
points to the current and correct transfer at all times is difficult
and has been notoriously error-prone over the years. The goal is to
ultimately remove the conn->data pointer for this reason.

Previously it was stored in a global state which contributed to
curl_global_init's thread unsafety. This boolean is now instead figured
out in curl_multi_init() and stored in the multi handle. Less effective,
but thread safe.

Instead of using a fixed 256 byte buffer in the connectdata struct.
In my build, this reduces the size of the connectdata struct by 11.8%,
from 2160 to 1904 bytes with no functionality or performance loss.
This also fixes a bug in schannel's Curl_verify_certificate where it
called Curl_sspi_strerror when it should have called Curl_strerror for
string from GetLastError. the only effect would have been no text or the
wrong text being shown for the error.

The function does not return the same value as snprintf() normally does,
so readers may be mislead into thinking the code works differently than
it actually does. A different function name makes this easier to detect.

curl_printf.h defines printf to curl_mprintf, etc. This can cause
problems with external headers which may use
__attribute__((format(printf, ...))) markers etc.
To avoid that they cause problems with system includes, we include
curl_printf.h after any system headers. That makes the three last
headers to always be, and we keep them in this order:
curl_printf.h
curl_memory.h
memdebug.h
None of them include system headers, they all do funny #defines.

For consistency, as we seem to have a bit of a mixed bag, changed all
instances of ipv4 and ipv6 in comments and documentations to use the
correct case.
Solaris with the SunStudio Compiler is reportedly missing this define,
but as we're using it without any good reason on all the places it was
used I've now instead switched to just use sensible buffer sizes that
fit a 32 bit decimal number. Which also happens to be smaller than the
common NI_MAXSERV value which is 32 on most machines.

1 - lib/setup_once.h used __SETUP_ONCE_H macro as header inclusion guard
up to commit ec691ca3 which changed this to HEADER_CURL_SETUP_ONCE_H,
this single inclusion guard is enough to ensure that inclusion of
lib/setup_once.h done from lib/setup.h is only done once.
Additionally lib/setup.h has always used __SETUP_ONCE_H macro to
protect inclusion of setup_once.h even after commit ec691ca3, this
was to avoid a circular header inclusion triggered when building a
c-ares enabled version with c-ares sources available which also has
a setup_once.h header. Commit ec691ca3 exposes the real nature of
__SETUP_ONCE_H usage in lib/setup.h, it is a header inclusion guard
foreign to libcurl belonging to c-ares's setup_once.h
The renaming this commit does, fixes the circular header inclusion,
and as such removes the need and usage of a header inclusion guard
foreign to libcurl. Macro __SETUP_ONCE_H no longer used in libcurl.
2 - Due to the circular interdependency of old lib/setup_once.h and the
c-ares setup_once.h header, old file lib/setup_once.h has carried
back from 2006 up to now days an alarming and prominent notice about
the need of keeping libcurl's and c-ares's setup_once.h in sync.
Given that this commit fixes the circular interdependency, the need
and presence of mentioned notice is removed.
All mentioned interdependencies come back from now old days when
the c-ares project lived inside a curl subdirectory. This commit
removes last traces of such fact.

Introduced the initial setup to allow closesocket callbacks by making
sure sclose() is only ever called from one place in the libcurl source
and still run all test cases fine.
Move ipv6-functional-probe into a single function that is used from all
places that need to know.
Make the probe function store the result in a static variable so that
subsequent invokes just returns the previous result and won't have to
probe again.
The IP version choice was previously only in the UserDefined struct
within the SessionHandle, but since we sometimes alter that option
during a request we need to have it on a per-connection basis.
I also moved more "init conn" code into the allocate_conn() function
which is designed for that purpose more or less.
