Steven Kirk c2c81d5945 Fix akavache submodule.

Steven Kirk 2d55511c3c Remove PCL akavache and splat libs. 
Having them be PCL meant that we had to have both a PCL and .netfx version of Splat, which was causing problems with test runners because they would load the wrong version of the library at random.

Steven Kirk bcef0654bd Remove Rothko submodule. 
Usean updated Rothko nuget package. Also had to add an export wrapper as I prefer to define the export in `github/VisualStudio` rather than in our Rothko fork.

Andreia Gaita 8c89ef0133 Implement a variant of the clone repo dialog for re-cloning from the Start Page 
The Start Page re-acquisition code path needs to clone repositories, but it
already knows which repo to clone, so we need a variant of the clone dialog
for this purpose.

Steven Kirk bf5e87142c Update octokit to 0.22.0 
This fixes #534 via https://github.com/octokit/octokit.net/pull/1411. Previously the request to /site/sha to detect an enterprise instance wasn't getting sent due to octokit's implementation of `RedirectHandler`.

Andreia Gaita e11d832e15 Add a loader for external packages 
Add a way to load external packages that are included in the vsix and
expose the IUIProvider as a global service, so those packages can make
use of our services.
Load the StartPage package if we're running on Dev15 Preview 4
