Are you the copyright holder or authorized to act on the copyright owner's behalf?
Yes, I am the copyright holder.
Please describe the nature of your copyright ownership or authorization to act on the owner's behalf.
I'm the developer of a platform fighting game engine known internally as "Super Smash Flash 2 Engine", a proprietary, closed-source game engine associated with my company McLeodGaming Inc. I am also the leader behind Super Smash Flash 2, a fan game built using this engine. Both code from the engine and game itself were de-compiled and uploaded to GitHub without my permission, and neither are licensed for distribution by third-parties.
Also note that I do keep the core source code on a private Github repository that has never been made public at any point, which can serve as proof of ownership and the fact that the code from the game was de-compiled:

Please provide a detailed description of the original copyrighted work that has allegedly been infringed. If possible, include a URL to where it is posted online.
This is the second occurrence of the offending user uploading the source code for my proprietary, closed-source engine known as "Super Smash Flash 2 Engine" without permission. Note that this engine is different from the Super Smash Flash 2 game itself, which is a Super Smash Bros. fan game built using the engine. The engine itself does not contain any multimedia content or playable game, rather it is purely a platform fighting game engine in its raw form designed to ship platform fighters with for Flash and Adobe AIR.
An example official project built utilizing this engine may be found here:
https://www.supersmashflash.com/play/ssf2
The offending repository has de-compiled the source code of this game in order to upload both an unauthorized modified version of the game itself and engine to GitHub.

What files should be taken down? Please provide URLs for each file, or if the entire repository, the repository’s URL.

Have you searched for any forks of the allegedly infringing files or repositories? Each fork is a distinct repository and must be identified separately if you believe it is infringing and wish to have it taken down.
N/A
Is the work licensed under an open source license? If so, which open source license? Are the allegedly infringing files being used under the open source license, or are they in violation of the license?
No, it is a closed-source, proprietary project.
What would be the best solution for the alleged infringement? Are there specific changes the other person can make other than removal? Can the repository be made private?
Only removal of the content will be accepted, the source code for the engine and game are not licensed for distribution to public or private third parties.
Do you have the alleged infringer’s contact information? If so, please provide it.
I do not have their direct contact information, just the GitHub profile of the first person who uploaded it:

I have a good faith belief that use of the copyrighted materials described above on the infringing web pages is not authorized by the copyright owner, or its agent, or the law.
I have taken fair use into consideration.
