(1) recorded_lock_rec > current_lock_rec should not be occurred
on rb_ec_vm_lock_rec_release().
(2) should be release VM lock at EXEC_TAG(), not POP_TAG().
(3) some refactoring.
If a ractor getting a VM lock (monitor) raises an exception,
unlock can be skipped. To release VM lock correctly on exception
(or other jumps with JUMP_TAG), EC_POP_TAG() releases VM lock.
Looking at the list of symbols inside of libruby-static.a, I found
hundreds of functions that are defined, but used from nowhere.
There can be reasons for each of them (e.g. some functions are
specific to some platform, some are useful when debugging, etc).
However it seems the functions deleted here exist for no reason.
This changeset reduces the size of ruby binary from 26,671,456
bytes to 26,592,864 bytes on my machine.
Without this patch, "raise" event invoked twice when raise an
exception in "load"ed script.

moved from eval_intern.h.
* compile.c iseq.c, vm.c: use UNALIGNED_MEMBER_PTR for `entries`
in `struct iseq_catch_table`.
* vm_eval.c, vm_insnhelper.c: use UNALIGNED_MEMBER_PTR for `body`
in `rb_method_definition_t`.
We see several occurrence of "diagnostic push/pop" so why not
make them macros. Tested on GCC8 / Clang 6.
Note that ruby.h is intentionally left untouched because we don't
want to introduce new public macros.

* vm_core.h (rb_thread_t): rb_thread_t::ec is now a pointer.
There are many code using `th` to represent execution context
(such as cfp, VM stack and so on). To access `ec`, they need to
use `th->ec->...` (adding one indirection) so that we need to
replace them by passing `ec` instead of `th`.
* vm_core.h (GET_EC()): introduced to access current ec. Also
remove `ruby_current_thread` global variable.
* cont.c (rb_context_t): introduce rb_context_t::thread_ptr instead of
rb_context_t::thread_value.
* cont.c (ec_set_vm_stack): added to update vm_stack explicitly.
* cont.c (ec_switch): added to switch ec explicitly.
* cont.c (rb_fiber_close): added to terminate fibers explicitly.

Return value of EXEC_TAG() is saved by "int state".
Instead of "int", use "enum ruby_tag_type". First EXEC_TAG()
value should be 0, so that define TAG_NONE (= 0) and use it.
Some code used "status" instead of "state". To make them clear,
rename them to state.
We can change variable name from "state" to "tag_state", but this
ticket doesn't contain it.

Qundef rather than 0 which is equal to Qfalse. Since Kernel#throw(obj)
searches a tag with rb_vm_tag::tag == obj, throw(false) can

* Introduce concept of "Block Handler (BH)" to represent
passed blocks.
* move rb_control_frame_t::flag to ep[0] (as a special local
variable). This flags represents not only frame type, but also
env flags such as escaped.
* rename `rb_block_t` to `struct rb_block`.
* Make Proc, Binding and RubyVM::Env objects wb-protected.
