Saves comitters' daily life by avoid #include-ing everything from
internal.h to make each file do so instead. This would significantly
speed up incremental builds.
We take the following inclusion order in this changeset:
1. "ruby/config.h", where _GNU_SOURCE is defined (must be the very
first thing among everything).
2. RUBY_EXTCONF_H if any.
3. Standard C headers, sorted alphabetically.
4. Other system headers, maybe guarded by #ifdef
5. Everything else, sorted alphabetically.
Exceptions are those win32-related headers, which tend not be self-
containing (headers have inclusion order dependencies).
* enc/encdb.c, enc/utf_16_32.h (ENC_DUMMY_UNICODE): Unicode with BOM
must be based on big endian variants, so that actual encodings would

* encoding.c (rb_enc_set_base): Add for setting base encoding
with their names. this is internal function.
* template/encdb.h.tmpl: specify ENC_SET_BASE for second
encodings in each encoding files.
* enc/encdb.c (rb_enc_set_base): add a declaration.
(ENC_SET_BASE): ditto.

* transcode.c (init_transcoder_table): moved to enc/trans/transdb.c.
* enc/depend (enc/encdb.o enc/trans/transdb.o): depend on
corresponding headers.
* common.mk (COMMONOBJS): moved transcode.o from OBJS
