Add OpIgnoreIntersectionKHR/OpTerminateRayKHR related

* Remove redudant message in the header of check hlsl

* rebase against master (fix merge conflict)

* Create a new basic block after termination.

* Don't change the SPIRV-Headers hash.

* Add unit tests and add assertion.

* Fix missing ==
