filemap: update offset check in filemap_fault()

With THP, current check of offset:

    VM_BUG_ON_PAGE(page->index != offset, page);

is no longer accurate. Update it to:

    VM_BUG_ON_PAGE(page_to_pgoff(page) != offset, page);
