[PATCH] drivers/scsi/initio.c: cleanups

This patch contains the following cleanups:
- make needlessly global code static
- remove or #if 0 the following unused functions:
  - tul_pop_pend_scb
  - tul_device_reset
  - tul_reset_scsi_bus
