phy: qcom-qmp: new qmp phy driver for qcom-chipsets

Qualcomm SOCs have QMP phy controller that provides support
to a number of controller, viz. PCIe, UFS, and USB.
Add a new driver, based on generic phy framework, for this
phy controller.
