percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

directly, which now lose the number argument which is always 1.

it can just call into it's own routine directly.
Move shared logic into the common __dquot_alloc_space,
dquot_claim_space_nodirty and __dquot_free_space low-level methods,
and rationalize the wrappers around it to move as much as possible
code into the common block for CONFIG_QUOTA vs not. Also rename
all these helpers to be named dquot_* instead of vfs_dq_*.

The next step in divorcing metadata I/O management from struct inode is
to pass struct ocfs2_caching_info to the journal functions. Thus the
journal locks a metadata cache with the cache io_lock function. It also
can compare ci_last_trans and ci_created_trans directly.
This is a large patch because of all the places we change
ocfs2_journal_access..(handle, inode, ...) to
ocfs2_journal_access..(handle, INODE_CACHE(inode), ...).

We are really passing the inode into the ocfs2_read/write_blocks()
functions to get at the metadata cache. This commit passes the cache
directly into the metadata block functions, divorcing them from the
inode.

ocfs2_read_virt_blocks() does BUG when we try to read a block from a file
beyond its end. Since this can happen due to filesystem corruption, it
is not really an appropriate answer. Make ocfs2_read_quota_block() check
the condition and handle it by calling ocfs2_error() and returning EIO.

In OCFS2, allocator locks rank above transaction start. Thus we
cannot extend quota file from inside a transaction less we could
deadlock.
We solve the problem by starting transaction not already in
ocfs2_acquire_dquot() but only in ocfs2_local_read_dquot() and
ocfs2_global_read_dquot() and we allocate blocks to quota files before starting
the transaction. In case we crash, quota files will just have a few blocks
more but that's no problem since we just use them next time we extend the
quota file.

syncjiff is just a converted value of syncms. Some places which
are updating syncms forgot to update syncjiff as well. Since the
conversion is just a simple division / multiplication and it does
not happen frequently, just remove the syncjiff field to avoid
forgotten conversions.

Change i_size of global quota files so that it always remains aligned to block
size. This is mainly because the end of quota block may contain checksum (if
checksumming is enabled) and it's a bit awkward for it to be "outside" of quota
file (and it makes life harder for ocfs2-tools).

It is not possible to get a read lock and then try to get the same write lock
in one thread as that can block on downconvert being requested by other node
leading to deadlock. So first drop the quota lock for reading and only after
that get it for writing.

It could happen that some limit has been set via quotactl() and in parallel
->mark_dirty() is called from another thread doing e.g. dquot_alloc_space(). In
such case ocfs2_write_dquot() must not try to sync the dquot because that needs
global quota lock but that ranks above transaction start.

dqptr_sem anymore, we don't have to start a transaction and obtain locks

The per-metadata-type ocfs2_journal_access_*() functions hook up jbd2
commit triggers and allow us to compute metadata ecc right before the
buffers are written out. This commit provides ecc for inodes, extent
blocks, group descriptors, and quota blocks. It is not safe to use
extened attributes and metaecc at the same time yet.
The ocfs2_extent_tree and ocfs2_path abstractions in alloc.c both hide
the type of block at their root. Before, it didn't matter, but now the
root block must use the appropriate ocfs2_journal_access_*() function.
To keep this abstract, the structures now have a pointer to the matching
journal_access function and a wrapper call to call it.
A few places use naked ocfs2_write_block() calls instead of adding the
blocks to the journal. We make sure to calculate their checksum and ecc
before the write.
Since we pass around the journal_access functions. Let's typedef them
in ocfs2.h.

Add block check calls to the read_block validate functions. This is the
almost all of the read-side checking of metaecc. xattr buckets are not checked
yet. Writes are also unchecked, and so a read-write mount will quickly fail.

ocfs2_bread() has become ocfs2_read_virt_blocks(), with a prototype to
match ocfs2_read_blocks(). The quota code, converting from
ocfs2_bread(), wraps the call to ocfs2_read_virt_blocks() in
ocfs2_read_quota_block(). Unfortunately, the prototype of
ocfs2_read_quota_block() matches the old prototype of ocfs2_bread().
The problem is that ocfs2_bread() returned the buffer head, and callers
assumed that a NULL pointer was indicative of error. It wasn't. This
is why ocfs2_bread() took an int*err argument as well.
The new prototype of ocfs2_read_virt_blocks() avoids this error handling
confusion. Let's change ocfs2_read_quota_block() to match.

For each quota type each node has local quota file. In this file it stores
changes users have made to disk usage via this node. Once in a while this
information is synced to global file (and thus with other nodes) so that
limits enforcement at least aproximately works.
Global quota files contain all the information about usage and limits. It's
mostly handled by the generic VFS code (which implements a trie of structures
inside a quota file). We only have to provide functions to convert structures
from on-disk format to in-memory one. We also have to provide wrappers for
various quota functions starting transactions and acquiring necessary cluster
locks before the actual IO is really started.
