Free dynamically allocated device data structures when device registration
fails. This fixes memory leakage when the registration fails.

The pseries PCI hotplug code has a number of issues, ranging from
incorrect resource setup to crashes, depending on what is added,
when, whether it contains a bridge, etc etc....
This fixes a whole bunch of these, while actually simplifying the code
a bit, using more generic code in the process and factoring out common
code between adding of a PHB, a slot or a device.

To properly fix PCI hotplug, it's useful to be able to make the fixup
passes on all devices whether they were just hot plugged or already
there.
The EEH code however used to not be very friendly with calling
eeh_add_device_late() multiple time, and not very rebust in the way it
generally tests whether a device is in the expected state vs. the EEH
code.
This improves it, along with cleaning up a couple of debug printk's.

The 'ibm,interrupt-server#-size' properties are not in the cpu nodes,
which is where we currently look for them, but rather live under the
interrupt source controller nodes (which have "ibm,ppc-xics" in their
compatible property).
This moves the code that looks for the ibm,interrupt-server#-size
properties from xics_update_irq_servers() into xics_init_IRQ().
Also this adds a check for mismatched sizes across the interrupt
source controller nodes. Not sure this is necessary as in this case
the firmware might be seriously busted.
This property only appears on POWER6 boxes and is only used in the
set-indicator(gqirm) call, and apparently firmware currently ignores
the value we pass. Nevertheless we need to fix it in case future
firmware versions use it.

The Freescale implementation of MPIC only allows a single CPU destination
for non-IPI interrupts. We add a flag to the mpic_init to distinquish
these variants of MPIC. We pull in the irq_choose_cpu from sparc64 to
select a single CPU as the destination of the interrupt.
This is to deal with the fact that the default smp affinity was
changed by commit 1840475676 ("genirq:
Expose default irq affinity mask (take 3)") to be all CPUs.

After the merge of the 32 and 64bit DMA code, dma_direct_ops lost
their map/unmap_single() functions but gained map/unmap_page(). This
caused a problem for Cell because Cell's dma_iommu_fixed_ops called
the dma_direct_ops if the fixed linear mapping was to be used or the
iommu ops if the dynamic window was to be used. So in order to fix
this problem we need to update the 64bit DMA code to use
map/unmap_page.
First, we update the generic IOMMU code so that iommu_map_single()
becomes iommu_map_page() and iommu_unmap_single() becomes
iommu_unmap_page(). Then we propagate these changes up through all
the callers of these two functions and in the process update all the
dma_mapping_ops so that they have map/unmap_page rahter than
map/unmap_single. We can do this because on 64bit there is no HIGHMEM
memory so map/unmap_page ends up performing exactly the same function
as map/unmap_single, just taking different arguments.
This has no affect on drivers because the dma_map_single_attrs() just
ends up calling the map_page() function of the appropriate
dma_mapping_ops and similarly the dma_unmap_single_attrs() calls
unmap_page().
This fixes an oops on Cell blades, which oops on boot without this
because they call dma_direct_ops.map_single, which is NULL.

Resources for PHB's that are dynamically added to a system are not
properly allocated in the resource tree.
Not having these resources allocated causes an oops when removing
the PHB when we try to release them.
The diff appears a bit messy, this is mainly due to moving everything
one tab to the left in the pcibios_allocate_bus_resources routine.
The functionality change in this routine is only that the
list_for_each_entry() loop is pulled out and moved to the necessary
calling routine.

linux/crash_dump.h defines is_kdump_kernel() to be used by code that
needs to know if the previous kernel crashed instead of a (clean) boot
or reboot.
This updates the just added powerpc code to use it. This is needed
for the next commit, which will remove __kdump_flag.

The i2c bus defn is broken on linkstation / kurobox machines since at
least 2.6.27. Fix it. Also remove CONFIG_SERIAL_OF_PLATFORM, which, if
enabled, breaks the serial console after the
"console handover: boot [udbg0] -> real [ttyS1]" message.

This adds relocatable kernel support for kdump. With this one can
use the same regular kernel to capture the kdump. A signature (0xfeed1234)
is passed in r6 from panic code to the next kernel through kexec_sequence
and purgatory code. The signature is used to differentiate between
kdump kernel and non-kdump kernels.
The purgatory code compares the signature and sets the __kdump_flag in
head_64.S. During the boot up, kernel code checks __kdump_flag and if it
is set, the kernel will behave as relocatable kdump kernel. This kernel
will boot at the address where it was loaded by kexec-tools ie. at the
address reserved through crashkernel boot parameter.
CONFIG_CRASH_DUMP depends on CONFIG_RELOCATABLE option to build kdump
kernel as relocatable. So the same kernel can be used as production and
kdump kernel.
This patch incorporates the changes suggested by Paul Mackerras to avoid
GOT use and to avoid two copies of the code.

Most of the platforms were printing the size of the memory
in their show_cpuinfo implementations. This moves that to
the common show_cpuinfo, so that all 32-bit platforms will
now print the size of memory. I also update the code
to deal with the fact that total_memory is now a phys_addr_t.

We used to assume that even numbered threads were the primary
threads, ie those that would be listed and started as a cpu from
open firmware. Replace a left over is even (% 2) check with a check
for it being a primary thread and update the comments.
Tested with a debug print on pseries, identical code found for cell.

The pfn of the memory to be removed should be validated prior to
attempting to remove the memory. In cases where the probe of a
memory section fails during hotplug add, the pfn for the lmb may
not be valid.

With most file readers (eg cat, dd), reading a context's regs file will
result in two reads: the first to read the data, and the second to
return EOF. Because each read performs a spu_acquire_saved, we end up
descheduling and re-scheduling the context twice.
This change does a simple check to see if we'd return EOF before
calling spu_acquire_saved(), saving the extra schedule operation.

Currently, read() on the sputrace log will block until the read buffer
is full. This makes it difficult to retrieve the end of the buffer, as
the user will need to read with the right-sized buffer.
In a similar method as 91553a1b5e0df006a3573a88d98ee7cd48a3818a, this
change makes the switch_log return if there has already been data
read.

Currently, we use ctx->mapping_lock and ctx->switch_log->lock for the
context switch log. The mapping lock only prevents concurrent open()s,
so we require the switch_lock->lock for reads.
Since writes to the switch log buffer occur on context switches, we're
better off synchronising with the state_mutex, which is held during a
switch. Since we're serialised througout the buffer reads and writes,
we can use the state mutex to protect open and release too, and
can now kfree() the log buffer on release. This allows us to perform
the switch log notify without taking any extra locks.
Because the buffer is only present while the file is open, we can use
it to prevent multiple simultaneous openers.

Currently, read() on the sputrace buffer will only return data when
the user buffer is exhausted. This may mean that we never see the
end of the event log, unless we read() with exactly the right-sized
buffer.
This change makes sputrace_read not block if we have data ready to
return.

Currently, sputrace will start logging to the event buffer before the
log buffer has been open()ed. This results in a heap of "lost samples"
warnings if the sputrace file hasn't yet been opened.
Since the buffer is reset on open() anyway, there's no need to enable
logging when no-one has opened the log.
Because open clears the log, make it return EBUSY for mutliple open
calls.

Due to confusion between the ftrace infrastructure and the gcc profiling
tracer "ftrace", this patch renames the config options from FTRACE to
FUNCTION_TRACER. The other two names that are offspring from FTRACE
DYNAMIC_FTRACE and FTRACE_MCOUNT_RECORD will stay the same.
This patch was generated mostly by script, and partially by hand.

This adds a common board file for almost all of the "simple" PowerPC 40x
boards that exist today. This is intended to be a single place to add
support for boards that do not differ in platform support from most of the
evaluation boards that are used as reference platforms. Boards that have
specific requirements or custom hardware setup should still have their own
board.c file.
The first board ported to this is the AMCC PowerPC 405EZ Acadia board.

MPC5200 needs to have pipelining disabled for ATA to work. MPC5200B does not.

Recently, indirect_pci was changed to test if the bus number requested
is the one hanging straight off the PHB, then it substitutes the bus
number with another one contained in a new "self_busno" field of the
pci_controller structure.
However, this breaks CHRP which didn't initialize this new field, and
which relies on having the right bus number passed to the hardware.
This fixes it by initializing this variable properly for all CHRP bridges

The detection of the IBM "Python" PCI host bridge on IBM CHRP
machines such as old RS6000 was broken when we changed
of_device_is_compatible() from strncasecmp to strcasecmp (dropped
the "n" variant) due to the way IBM encodes the chip version.
We fix that by instead doing a match on the model property like
we do for others bridges in that file. It should be good enough
for those machines. If yours is still broken, let me know.

This is a much better version of a previous patch to make the parser
tables constant. Rather than changing the typedef, we put the "const" in
all the various places where its required, allowing the __initconst
exception for nfsroot which was the cause of the previous trouble.
This was posted for review some time ago and I believe its been in -mm
since then.

Early versions of the Freescale DIU framebuffer driver depended on a bootmem
allocation of memory for the video buffer. The need for this feature was
removed in commit 6b51d51a, so now we can remove the platform-specific code
that allocated that memory.

In the standalone setup the board's CPLD disables the PCI internal
arbiter, thus any access to the PCI bus will hang the board.
The common way to disable particular devices in the device tree is to
put the "status" property with any value other than "ok" or "okay"
into the device node we want to disable.
So, when there is no PCI arbiter on the bus the u-boot adds status =
"broken (no arbiter)" property into the PCI controller's node, and so
marks the PCI controller as unavailable.

Specifying user-selectable option in the qe_lib/Kconfig was a bad idea
because the qe_lib/Kconfig is included into the top level Kconfig, and
thus the QE_GPIO option appears at the top level menu.
This patch effectively moves the QE_GPIO option under the platform menu
instead.

Modify the Kconfig so that Freescale QUICC Engine (QE) support is a selectable
option, thereby allowing users to compile kernels without any QE support.
The drawback is that QE support is now disabled by default on platforms that
have a QE, and so a defconfig is needed to enable QE and QE devices (like
UCC GETH). Fortunately, all the current relevant defconfigs do that already.

Support for the SBC610 VPX Single Board Computer from GE Fanuc (PowerPC MPC8641D).
A number of MPC8641D based route interrupts for on-board interrupts through
a FPGA based interrupt controller, which is chained with the
MPC8641D's mpic. This patch provides a basic driver to allow basic routing
of interrupts to the mpic.

A single full sync (mb()) is requrired to order the mmio to the qirr reg
with the set or clear of the message word. However, test_and_clear_bit
has the effect of smp_mb() and we are not doing any other io from here,
so we don't need a mb per bit processed.

EOI normally has the side effect of returning the cpu to the base
priority to recieve the next interrupt. This is actually controlled
by the top byte of the xirr register. When we are exiting the
kernel in kexec we must eoi the ipi for the next kernel because we
never return from the handler, but we want to leave interrupt
delivery blocked until the next kernel takes action.
Since the hardware ipi vector is fixed, its easiest to just do the
eoi explicitly.

This factors out processors joining and unjoining the Global Interrupt
Queue into a separate function.
There is a bit of math to calculate the arguments to rtas to join
or leave the global interrupt queue, and a warning on failure
afterwards. Make a helper for the 3 callers.

The xirr is 32 bits in hardware, but the hypervisor requries the upper
bits of the register to be clear on the hcall. By changing the type
from signed to unsigned int we can drop masking it back to 32 bits.

xics supports only one ipi per cpu, and expects software to use some
queue to know why the interrupt was sent. In Linux, we use a an array
of bitmaps indexed by cpu to identify the message. Currently the bits
are set in smp.c and decoded in xics.c, with the data structure in a
header file. Consolidate the code in xics.c similar to mpic and other
interrupt controllers.
Also, while making the the array static, the message word doesn't need
to be volatile as set_bit and test_clear_bit take care of it for us, and
put it under ifdef smp.

Currently, every time we determine which irq server to use, we check if
default_server, which is the id of the bootcpu, is still online. But
default_server is a hardware cpu, not the logical cpu id needed to index
cpu_online_map.
Since the default server can only go offline during a cpu hotplug event,
explicitly check the default server and choose the new one when we move
irqs away from the cpu being offlined.
This has the added benefit of only needing the boot_cpuid to be updated
and not relying on the cpu being marked offline during migrate_irqs_away.
Also, since xics_update_irq_servers only reads device tree information, we
can call it before xics_init_host in xics_init_IRQ and then default_server
will always be valid when we can reach get_irq_server via the host ops.

When reciving an irq vector that does not have a linux mapping, the kernel
prints a message and calls RTAS to disable the irq source. Previously
the kernel did not EOI the interrupt, causing the source to think it is
still being processed by software. While this does add an additional
layer of protection against interrupt storms had RTAS failed to disable
the source, it also prevents the interrupt from working when a driver
later enables it. (We could alternatively send an EOI on startup, but
that strategy would likely fail on an emulated xics.)
All interrupts should be disabled when the kernel starts, but this can
be observed if a driver does not shutdown an interrupt in its reboot
hook before starting a new kernel with kexec.
Michael reports this can be reproduced trivially by banging the keyboard
while kexec'ing on a P5 LPAR: even though the hvc_console driver request's
the console irq later in boot, the console is non-functional because
we're receiving no console interrupts.

Testing hotplug memory remove has revealed that we can oops in
pseries_lmb_remove(). The incorrect shift causes a NULL pointer
dereference in the page_zone() inline routine.
I have only been able to reproduce the oops on kernels with large pages
enabled.
Tested on Power5 and Power6 with and without large pages enabled.

A mutex_unlock(&gang->aff_mutex) in spufs_create_context() is missing
in case spufs_context_open() fails. As a result, spu_create syscall
and spu_get_idle() may block.
This patch adds the mutex_unlock.

Currently, an empty spufs root inode has nlink count of 1. However,
the directory has two links; / -> spu and /spu/ -> .
This change increments the link count of the root inode in spufs.

1. arch/powerpc/platforms/pasemi/gpio_mdio.c also needs to be
converted over to mdiobus_{alloc,free}().
2. drivers/net/phy/fixed.c used to embed a struct mii_bus into its
struct fixed_mdio_bus and then use container_of() to go from the
former to the latter. Since mii bus structures are no longer
embedded, we need to do something like use the mii bus private
pointer to go from mii_bus to fixed_mdio_bus instead.

In preparation of giving mii_bus objects a device tree presence of
their own, rename struct mii_bus's ->dev argument to ->parent, since
having a 'struct device *dev' that points to our parent device
conflicts with introducing a 'struct device dev' representing our own
device.

rtas_log_read() doesn't check file flags for O_NONBLOCK and blocks
non-blocking readers of /proc/ppc64/rtas/error_log when there is
no data available. This fixes it.
Also rtas_log_read() returns now with ENODATA to prevent suspending of
process in wait_event_interruptible() when logging facility was
switched off and log is already empty.

The Arches Evaluation board is based on the AMCC 460GT SoC chip.
This board is a dual processor board with each processor providing
independent resources for Rapid IO, Gigabit Ethernet, and serial
communications. Each 460GT has it's own 512MB DDR2 memory, 32MB NOR FLASH,
UART, EEPROM and temperature sensor, along with a shared debug port.
The two 460GT's will communicate with each other via shared memory,
Gigabit Ethernet and x1 PCI-Express.

Commit 00c5372d37 caused the MPC8544DS
board to hang at boot. The MPC8544DS is unique in that it doesn't use
the PCI slots on the ULI (unlike the MPC8572DS or MPC8610HPCD). So
the dummy read at the end of the address space causes us to hang.
We can detect the situation by comparing the bridge's BARs versus
the root complex.

Support for the SBC610 VPX Single Board Computer from GE Fanuc (PowerPC
MPC8641D).
Fixup to correctly reconfigure USB, provided by an NEC uPD720101, after
device is reset. This requires a set of chip specific registers in the
devices configuration space to be correctly written, enabling all ports
and switching the device to use an external 48-MHz Oscillator.

This rearranges a bit of code, and adds support for
36-bit physical addressing for configs that use a
hashed page table. The 36b physical support is not
enabled by default on any config - it must be
explicitly enabled via the config system.
This patch *only* expands the page table code to accomodate
large physical addresses on 32-bit systems and enables the
PHYS_64BIT config option for 86xx. It does *not*
allow you to boot a board with more than about 3.5GB of
RAM - for that, SWIOTLB support is also required (and
coming soon).

irq_radix_revmap() currently serves 2 purposes, irq mapping lookup
and insertion which happen in interrupt and process context respectively.
Separate the function into its 2 components, one for lookup only and one
for insertion only.
Fix the only user of the revmap tree (XICS) to use the new functions.
Also, move the insertion into the radix tree of those irqs that were
requested before it was initialized at said tree initialization.
Mutual exclusion between the tree initialization and readers/writers is
handled via a state variable (revmap_trees_allocated) set to 1 when the tree
has been initialized and set to 2 after the already requested irqs have been
inserted in the tree by the init path. This state is checked before any reader
or writer access just like we used to check for tree.gfp_mask != 0 before.
Finally, now that we're not any longer inserting nodes into the radix-tree
in interrupt context, turn the GFP_ATOMIC allocations into GFP_KERNEL ones.

This implements CONFIG_RELOCATABLE for 64-bit by making the kernel as
a position-independent executable (PIE) when it is set. This involves
processing the dynamic relocations in the image in the early stages of
booting, even if the kernel is being run at the address it is linked at,
since the linker does not necessarily fill in words in the image for
which there are dynamic relocations. (In fact the linker does fill in
such words for 64-bit executables, though not for 32-bit executables,
so in principle we could avoid calling relocate() entirely when we're
running a 64-bit kernel at the linked address.)
The dynamic relocations are processed by a new function relocate(addr),
where the addr parameter is the virtual address where the image will be
run. In fact we call it twice; once before calling prom_init, and again
when starting the main kernel. This means that reloc_offset() returns
0 in prom_init (since it has been relocated to the address it is running
at), which necessitated a few adjustments.
This also changes __va and __pa to use an equivalent definition that is
simpler. With the relocatable kernel, PAGE_OFFSET and MEMORY_START are
constants (for 64-bit) whereas PHYSICAL_START is a variable (and
KERNELBASE ideally should be too, but isn't yet).
With this, relocatable kernels still copy themselves down to physical
address 0 and run there.

Using LOAD_REG_IMMEDIATE to get the address of kernel symbols
generates 5 instructions where LOAD_REG_ADDR can do it in one,
and will generate R_PPC64_ADDR16_* relocations in the output when
we get to making the kernel as a position-independent executable,
which we'd rather not have to handle. This changes various bits
of assembly code to use LOAD_REG_ADDR when we need to get the
address of a symbol, or to use suitable position-independent code
for cases where we can't access the TOC for various reasons, or
if we're not running at the address we were linked at.
It also cleans up a few minor things; there's no reason to save and
restore SRR0/1 around RTAS calls, __mmu_off can get the return
address from LR more conveniently than the caller can supply it in
R4 (and we already assume elsewhere that EA == RA if the MMU is on
in early boot), and enable_64b_mode was using 5 instructions where
2 would do.

The return code from invocation of the notifier for
pSeries_reconfig_chain during update of the device tree is not
checked. This causes writes to /proc/ppc64/ofdt to update memory
properties (i.e. ibm,dyamic-reconfiguration-memory) to always
return success, instead of the result of the notifier chain.
This happens specifically when we remove/add memory from the
device tree on machines using memory specified in the
ibm,dynamic-reconfiguration-memory property of the device tree.

We currently have a race when scheduling a context to a SPE -
after we have found a runnable context in spusched_tick, the same
context may have been scheduled by spu_activate().
This may result in a panic if we try to unschedule a context that has
been freed in the meantime.
This change exits spu_schedule() if the context has already been
scheduled, so we don't end up scheduling it twice.

This change introduces a 'free_spu' flag to spu_unschedule, to indicate
whether or not the function should free the spu after descheduling the
context. We only set this flag if we're not going to re-schedule
another context on this SPU.

count on SPU contexts during find_victim, but this may cause a leak in
the reference count if we later find a better contender for a context to
unschedule.
Change the reference to after we've found our victim context, so we
don't do the extra get_spu_context().

This bug is causing random crashes
(http://bugzilla.kernel.org/show_bug.cgi?id=11414).
-fno-omit-frame-pointer is only needed on powerpc when -pg is also
supplied, and there is a gcc bug that causes incorrect code generation
on 32-bit powerpc when -fno-omit-frame-pointer is used---it uses stack
locations below the stack pointer, which is not allowed by the ABI
because those locations can and sometimes do get corrupted by an
interrupt.
This ensures that CONFIG_FRAME_POINTER is only selected by ftrace.
When CONFIG_FTRACE is enabled we also pass -mno-sched-epilog to work
around the gcc codegen bug.

This adds a common board file for almost all of the "simple" PowerPC 44x
boards that exist today. This is intended to be a single place to add
support for boards that do not differ in platform support from most of the
evaluation boards that are used as reference platforms. Boards that have
specific requirements or custom hardware setup should still have their own
board.c file.

Fix the ioremap of the spu shadow regs on the PS3.
The current PS3 hypervisor requires the spu shadow regs to be
mapped with the PTE page protection bits set as read-only (PP=3).
This implementation uses the low level __ioremap() to bypass the
page protection settings inforced by ioremap_flags() to get the
needed PTE bits set for the shadow regs.
This fixes a runtime failure on the PS3 introduced by the powerpc
ioremap_prot rework of commit a1f242ff46
("powerpc ioremap_prot").

If the firmware page size used for collaborative memory overcommit
is 4k, but the kernel is using 64k pages, the page loaning is currently
broken as it only marks the first 4k page of each 64k page as loaned.
This fixes this to iterate through each 4k page and mark them all as
loaned/active.

Currently, there is a possible reference-after-free in the spusched
code - contexts may be freed after we have released their state_mutex
in spusched_tick and find_victim.
This change takes a reference to the context before releasing the
mutex, so that the context doesn't get destroyed.

Currently, spu_run ignores the npc argument for contexts created with
SPU_CREATE_NOSCHED. While this is correct for isolated contexts,
there's no need to enforce the npc restriction on non-isolated NOSCHED
contexts.
This means that NOSCHED contexts can only ever run with an entry point
of 0x0.
This change to spu_run_init allows setting of the npc (and, while we're
at it, the privcntl) for non-isolated NOSCHED contexts. This allows
us to run NOSCHED contexts from any entry point.

This patch removes the i2c code which is now obsolete due to the new
ibm iic driver walking the device tree for child nodes.
There are two other small cleanups that came indirectly from the ad7414
code review. Make sure Tlow is correct and handle the case where
i2c_smbus_read_word_data fails.

This patch introduces baudrate setting support via the generic clock API.
When present the optional device tree clock property is used instead of
fsl-cpm-brg. Platforms can then define complex clock schemes, to output
the serial clock on an external pin for instance.

This removes the non-working code in legacy_serial that tried to handle
the powermac SCC ports, and instead add a (now working) function to the
powermac platform code to find the default serial console if any.

When using the "sccdbg" option to route early kernel messages and
xmon to the SCC serial port on PowerMacs, when this wasn't the
configured output port of Open Firmware, we initialize the baudrate
to 57600bps. This isn't a very good default on some powermacs where
both the FW and pmac_zilog will default to 38400. This fixes it to
use the same logic as pmac_zilog to pick a default speed.

Kmem cache passed to constructor is only needed for constructors that are
themselves multiplexeres. Nobody uses this "feature", nor does anybody uses
passed kmem cache in non-trivial way, so pass only pointer to object.

Add per-device dma_mapping_ops support for CONFIG_X86_64 as POWER
architecture does:
This enables us to cleanly fix the Calgary IOMMU issue that some devices
are not behind the IOMMU (http://lkml.org/lkml/2008/5/8/423).
I think that per-device dma_mapping_ops support would be also helpful for
KVM people to support PCI passthrough but Andi thinks that this makes it
difficult to support the PCI passthrough (see the above thread). So I
CC'ed this to KVM camp. Comments are appreciated.
A pointer to dma_mapping_ops to struct dev_archdata is added. If the
pointer is non NULL, DMA operations in asm/dma-mapping.h use it. If it's
NULL, the system-wide dma_ops pointer is used as before.
If it's useful for KVM people, I plan to implement a mechanism to register
a hook called when a new pci (or dma capable) device is created (it works
with hot plugging). It enables IOMMUs to set up an appropriate
dma_mapping_ops per device.
The major obstacle is that dma_mapping_error doesn't take a pointer to the
device unlike other DMA operations. So x86 can't have dma_mapping_ops per
device. Note all the POWER IOMMUs use the same dma_mapping_error function
so this is not a problem for POWER but x86 IOMMUs use different
dma_mapping_error functions.
The first patch adds the device argument to dma_mapping_error. The patch
is trivial but large since it touches lots of drivers and dma-mapping.h in
all the architecture.
This patch:
dma_mapping_error() doesn't take a pointer to the device unlike other DMA
operations. So we can't have dma_mapping_ops per device.
Note that POWER already has dma_mapping_ops per device but all the POWER
IOMMUs use the same dma_mapping_error function. x86 IOMMUs use device
argument.

Fixed up conflict in arch/powerpc/platforms/52xx/Kconfig manually.
This patch adds functionality to the gpio-lib subsystem to make it
possible to enable the gpio-lib code even if the architecture code didn't
request to get it built in.
The archtitecture code does still need to implement the gpiolib accessor
functions in its asm/gpio.h file. This patch adds the implementations for
x86 and PPC.
With these changes it is possible to run generic GPIO expansion cards on
every architecture that implements the trivial wrapper functions. Support
for more architectures can easily be added.

To support Cooperative Memory Overcommitment (CMO), we need to check
for failure from some of the tce hcalls.
These changes for the pseries platform affect the powerpc architecture;
patches for the other affected platforms are included in this patch.
pSeries platform IOMMU code changes:
* platform TCE functions must handle H_NOT_ENOUGH_RESOURCES errors and
return an error.
Architecture IOMMU code changes:
* Calls to ppc_md.tce_build need to check return values and return
DMA_MAPPING_ERROR for transient errors.
Architecture changes:
* struct machdep_calls for tce_build*_pSeriesLP functions need to change
to indicate failure.
* all other platforms will need updates to iommu functions to match the new
calling semantics; they will return 0 on success. The other platforms
default configs have been built, but no further testing was performed.

For Cooperative Memory Overcommitment (CMO), set the FW_FEATURE_CMO
flag in powerpc_firmware_features from the rtas ibm,get-system-parameters
table prior to calling iommu_init_early_pSeries.
With this, any CMO specific functionality can be controlled by checking:

At the moment the fixed mapping is by default strongly ordered (the
iommu_fixed=weak boot option must be used to make the fixed mapping weakly
ordered). If we're on a setup where the southbridge is being used in
endpoint mode (triblade and CAB boards) the default should be a weakly
ordered fixed mapping.
This adds a check so that if a node of type pcie-endpoint can be found in
the device tree the fixed mapping is set to be weak by default (but can be
overridden using iommu_fixed=strong).

Currenlt,, it is possible to lock aff_mutex and
cbe_spu_info[n].list_mutex in different orders, allowing a deadlock to
occur. With this change, aff_mutex is not taken within a list_mutex
critical section anymore.

This patch removes the old kgdb reminants from ARCH=powerpc and
implements the new style arch specific stub for the common kgdb core
interface.
It is possible to have xmon and kgdb in the same kernel, but you
cannot use both at the same time because there is only one set of
debug hooks.
The arch specific kgdb implementation saves the previous state of the
debug hooks and restores them if you unconfigure the kgdb I/O driver.
Kgdb should have no impact on a kernel that has no kgdb I/O driver
configured.

This allow to dynamically generate attributes and share show/store
functions between attributes. Right now most attributes are generated
by special macros and lots of duplicated code. With the attribute
passed it's instead possible to attach some data to the attribute
and then use that in shared low level functions to do different things.
I need this for the dynamically generated bank attributes in the x86
machine check code, but it'll allow some further cleanups.
I converted all users in tree to the new show/store prototype. It's a single
huge patch to avoid unbisectable sections.

This patch changes the EEH_MAX_FAILS action from panic to printing an
error message. Panicking under under this condition is too harsh.
Although performance will be affected and the device may not recover,
the system is still running, which at the very least should allow for a
more graceful shutdown. The patch also removes the msleep() within a
spinlock, which can lead to a deadlock and is not recommended.

Introduce a new dma attriblue DMA_ATTR_WEAK_ORDERING to use weak ordering
on DMA mappings in the Cell processor. Add the code to the Cell's IOMMU
implementation to use this code.
Dynamic mappings can be weakly or strongly ordered on an individual basis
but the fixed mapping has to be either completely strong or completely weak.
This is currently decided by a kernel boot option (pass iommu_fixed=weak
for a weakly ordered fixed linear mapping, strongly ordered is the default).

This patch adds support for the power button on future IBM cell blades.
It actually doesn't shut down the machine. Instead it exposes an
input device /dev/input/event0 to userspace which sends KEY_POWER
if power button has been pressed.
haldaemon actually recognizes the button, so a plattform independent acpid
replacement should handle it correctly.

This patch adds a cpufreq governor that takes the number of running spus
into account. It's very similar to the ondemand governor, but not as complex.
Instead of hacking spu load into the ondemand governor it might be easier to
have cpufreq accepting multiple governors per cpu in future.
Don't know if this is the right way, but it would keep the governors simple.

Jack Ren and Eric Miao tracked down the following long standing
problem in the NOHZ code:
scheduler switch to idle task
enable interrupts
Window starts here
----> interrupt happens (does not set NEED_RESCHED)
irq_exit() stops the tick
----> interrupt happens (does set NEED_RESCHED)
return from schedule()
cpu_idle(): preempt_disable();
Window ends here
The interrupts can happen at any point inside the race window. The
first interrupt stops the tick, the second one causes the scheduler to
rerun and switch away from idle again and we end up with the tick
disabled.
The fact that it needs two interrupts where the first one does not set
NEED_RESCHED and the second one does made the bug obscure and extremly
hard to reproduce and analyse. Kudos to Jack and Eric.
Solution: Limit the NOHZ functionality to the idle loop to make sure
that we can not run into such a situation ever again.

This was not the proper fix. As Scott Wood said CONFIG_FS_ENET has nothing
to do with the issue. The proper fix is to select PHYLIB for this board.
Its possible to build the phylib as a module, however this breaks the
board code because alloc_mdio_bitbang and mdiobus_register are not
available if we build as a module. These are needed by the board code
since it implements the low level mdio bitbang ops.
So we unconditionally select PHYLIB to ensure its built into the kernel
if we are building in EP8248E support.
Long term we should look at moving the mdio_ops into its own file so it
can be built as a module.

- Get rid of uses_fsl_uli_m1575, it does not scale for all cases.
Instead, let's explicitly use machine_is() for each fixup.
- Factor out MPC8610HPCD quirks to fsl_uli1575, and protect them with
machine_is(). One step closer to multiplatform kernels.
- Actually use fsl_uli1575 on MPC8610HPCD, so RTC quirk will be applied.
- RTC quirk applies to all boards though, so no machine_is() checks.

This patch fixes RTC on MPC8572DS boards: dummy read helps only when
reading at the end of the bridge's memory (i.e. outside of behind the
bridge devices' assigned regions).
With this change the quirk also makes RTC work on MPC8610HPCD, so it's
unlikely that this will break MPC8641HPCN or MPC8544DS boards.

Choosing PCI or not at config time is allowed on some
platforms via an if expression in arch/powerpc/Kconfig.
To add a new platform with PCI support selectable at
config time, you must change the if expression. This
patch makes this easier by changing:
bool "PCI support" if <long expression>
to
bool "PCI support" if PPC_PCI_CHOICE
and adding select PPC_PCI_CHOICE to all the config nodes that
were previously in the PCI if expression.
Platforms with unconditional PCI support continue to
just select PCI in their config nodes.

It is okay for both _PAGE_GUARDED and _PAGE_COHERENT (G and M) to be set
in the same pte. In fact, even if that were not the case, there doesn't
seem to be any place where G is set without also setting I (_PAGE_NO_CACHE),
so the test for I is sufficient as a condition to clear _PAGE_COHERENT
when filling the hash table.

There isn't any reason at this point that we can't build 82xx, 83xx & 86xx
support in with the other 6xx based boards. Twiddle the Kconfigs to allow
this.
This allows us to remove the machine type selection for related to 6xx.

Initialize I2C pins on boards with CPM1/CPM2 controllers and document the
i2c bus in booting-without-of.
The boards don't have any I2C chips connected to the I2C bus, so unless
some external chips are connected to the boards, this code is just an
example of setting everything else up.

Add support for the MPC8536 process and MPC8536DS reference board. The
MPC8536 is an e500v2 based SoC which eTSEC, USB, SATA, PCI, and PCIe.
The USB and SATA IP blocks are similiar to those on the PQ2 Pro SoCs and
thus use the same drivers.

On MPC5200 the PCI target control register (PCITCR) @ MBAR + 0xD6C is
initialized with only bit 7 (Latrule disable) set. The 8-Bit write
combine timer (Bits 24..31) should be also set to a reasonable value
_greater zero_ (0x08 = default) since setting it to 0x00 leads to
_very poor_ performance as a PCI target since external burst won't be
possible at all.
Setting the WCT to 0x08 (cache-line size) leads to good overall perfomance.

The following patch restores the PERR and SERR bits in the PCI
command register during an EEH device recovery. We have found
at least one case (an Agilent test card) where the PERR/SERR
bits are set to 1 by firmware at boot time, but are not restored
to 1 during EEH recovery. The patch fixes the Agilent card
problem. It has been tested on several other EEH-enabled cards
with no regressions.

The current low level hash code on LPAR configurations clears
_PAGE_COHERENT (M) when either _PAGE_GUARDED (G) or _PAGE_NO_CACHE (I)
is set. This conflicts with _PAGE_SAO which has M, I and W bits sets at
once (normally invalid combo) to indicate the new SAO attribute.
This changes the code to allow that case.

Make cell_dma_dev_setup_iommu() return a pointer to the struct iommu_table
(or NULL if no table can be found) rather than putting this pointer into
dev->archdata.dma_data (let the caller do that), and rename this function
to cell_get_iommu_table() to reflect this change.
This will allow us to get the iommu table for a device that doesn't have
the table in the archdata.

Update powerpc to use the new dma_*map*_attrs() interfaces. In doing so
update struct dma_mapping_ops to accept a struct dma_attrs and propagate
these changes through to all users of the code (generic IOMMU and the
64bit DMA code, and the iseries and ps3 platform code).
The old dma_*map_*() interfaces are reimplemented as calls to the
corresponding new interfaces.

As nr_active counter includes also spus waiting for syscalls to return
we need a seperate counter that only counts spus that are currently running
on spu side. This counter shall be used by a cpufreq governor that targets
a frequency dependent from the number of running spus.

Currently, the .ctx debug file in spu context directories is always
present.
We'd prefer to prevent users from relying on this file, so add a
"debug" mount option to spufs. The .ctx file will only be added to
the context directories when this option is present.

Currently, spufs never specifies the i_size for the files in context
directories, so stat() always reports 0-byte files.
This change adds allows the spufs_dir_(nosched_)contents arrays to
specify a file size. This allows stat() to report correct file sizes,
and makes SEEK_END work.

An spu context shouldn't get an extra tick if the time slice code
couldn't find something else to run. This means contexts that are not
within spu_run (ie, SPU_SCHED_SPU_RUN is cleared) will not receive
extra ticks while we have no other contexts waiting.

Add a ctxt file to spufs that shows spu context information that is used
in scheduling. This info can be used for debugging spufs scheduler
issues, and to isolate between application and spufs problems as it
shows a lot of state such as priorities and dispatch counts.
This file contains internal spufs state and is subject to change at any
time, and therefore no applications should depend on it. The file is
intended for the use of spufs kernel developers.

This updates the device tree manipulation routines so that memory
add/remove of lmbs represented under the
ibm,dynamic-reconfiguration-memory node of the device tree invokes the
hotplug notifier chain.
This change is needed because of the change in the way memory is
represented under the ibm,dynamic-reconfiguration-memory node. All lmbs
are described in the ibm,dynamic-memory property instead of having a
separate node for each lmb as in previous device tree layouts. This
requires the update_node() routine to check for updates to the
ibm,dynamic-memory property and invoke the hotplug notifier chain.
This also updates the pseries hotplug notifier to be able to gather information
for lmbs represented under the ibm,dynamic-reconfiguration-memory node and
have the lmbs added/removed.

Use the base address of the lmb to derive the starting page frame number
instead of trying to extract it from the drc index of the lmb. The drc
index should not be used for this as it will, and did, break.
Until this point, systems that have had memory represented in the device
tree with a node for each lmb the drc index would (luckily) closely
track the base address of the lmb. For example a lmb with a drc index
of 8000000a would have a base address of a0000000. This correlation
allowed the current code to derive the starting page frame number from
the drc inddex
Device tree layouts where lmbs are represented under the
ibm,dynamic-reconfiguration-memory node in the ibm,dynamic-memory
property do not have this correlation between the drc index and base
address of the lmb.

Allow the phandle passed to the /proc/ppc64/ofdt file to be specified
in formats other than decimal. This allows us to easily specify phandle
values in hex that would otherwise appear as negative integers.
This is an issue on systems where the value of
/proc/device-tree/ibm,dynamic-reconfiguration-memory.ibm,phandle is
fffffff9. Having to pass this to the ofdt file as a string results in
a large negative number, and simple_strtoul() does not handle negative
numbers.

Suspend/resume ("echo mem > /sys/power/state") does not work with
vanilla kernels -- the system does not suspend correctly and just
hangs. This patch fixes this so suspend/resume works:
1) of_iomap does not map the whole 0xC000 of the MPC5200 immr so
saving registers does not work.
2) PCI registers need to be saved and restored.

Currently create_branch() creates a branch instruction for you, and
patches it into the call site. In some circumstances it would be nice
to be able to create the instruction and patch it later, and also some
code might want to check for errors in the branch creation before
doing the patching. A future commit will change create_branch() to
check for errors.
For callers that don't care, replace create_branch() with
patch_branch(), which just creates the branch and patches it directly.
While we're touching all the callers, change to using unsigned int *,
as this seems to match usage better. That allows (and requires) us to
remove the volatile in the definition of vector in powermac/smp.c and
mpc86xx_smp.c, that's correct because now that we're passing vector as
an unsigned int * the compiler knows that it's value might change
across the patch_branch() call.

We currently have a few routines for patching code in asm/system.h, because
they didn't fit anywhere else. I'd like to clean them up a little and add
some more, so first move them into a dedicated C file - they don't need to
be inlined.
While we're moving the code, drop create_function_call(), it's intended
caller never got merged and will be replaced in future with something
different.

At present, if we have a kernel with a 64kB page size, and some
process maps something that has to be mapped with 4kB pages (such as a
cache-inhibited mapping on POWER5+, or the eHCA infiniband queue-pair
pages), we change the process to use 4kB pages everywhere. This hurts
the performance of HPC programs that access eHCA from userspace.
With this patch, the kernel will only demote the slice(s) containing
the eHCA or cache-inhibited mappings, leaving the remaining slices
able to use 64kB hardware pages.
This also changes the slice_get_unmapped_area code so that it is
willing to place a 64k-page mapping into (or across) a 4k-page slice
if there is no better alternative, i.e. if the program specified
MAP_FIXED or if there is not sufficient space available in slices that
are either empty or already have 64k-page mappings in them.

There is a delay in the transition to the stopped state for class 2
interrupts. In some cases, the controlling thread detects the state of
the spu as running, and goes back to sleep resulting in a hung
application as the event is missed.
This change detects the stop condition and re-generates the wakeup event
after a context save.

Time slicing can occur at the same time as spu exception handling
resulting in the wakeup of the wrong thread.
This change uses the the spu's register_lock to enforce synchronization
between bind/unbind and spu exception handling so that they are
mutually exclusive.

According to the CBEA, the SPU dsisr is not updated for class 0
exceptions.
spu_stopped() is testing the dsisr that was passed to it from the class
0 exception handler, so we return a false positive here.
This patch cleans up the interrupt handler and erroneous tests in
spu_stopped. It also removes the fields from the csa since it is not
needed to process class 0 events.

The "ndfc-chip" device doesn't need any resources. All resources
are handled by the "ndfc-nand" device. Registering the same memory
resource twice causes "cat /proc/iomem" to go into an infinite loop
displaying NDFC memory addresses.

The MPC85xx MDS board requires some board level tweaks of the PHYs that
either the eTSEC (gianfar) or UCC ethernet controllers are connected to.
Its possible to build the phylib as a module, however this breaks the
board level fix ups because phy_read and phy_write are not available
if we build as a module.
So we unconditionally select PHYLIB to ensure its built into the kernel
if we are building in MPC85xx MDS support. This was determined to be
the easiest soultion even though it prevents the user from removing
PHYLIB support if they decide they don't want it.

When I changed irq_alloc_host() to take an of_node
(52964f87c64e6c6ea671b5bf3030fb1494090a48: "Add an optional
device_node pointer to the irq_host"), I botched the reference
counting semantics.
Stephen pointed out that it's irq_alloc_host()'s business if
it needs to take an additional reference to the device_node,
the caller shouldn't need to care.

A change was made to walk_memory_resource() in commit
4b119e21d0 that added a
check of find_lmb(). Add the coresponding lmb_add()
call to ps3_mm_add_memory() so that that check will
succeed.
This fixes the condition where the PS3 boots up with
only the 128 MiB of boot memory, and doesn't see the
other 128MiB that is available.

This adds some debugging code to the Axon MSI driver. It creates a
file per MSIC in /sys/kernel/debug/powerpc, which allows the user to
trigger a fake MSI interrupt by writing to the file.
This can be used to test some of the MSI generation path. In
particular, that the MSIC recognises a write to the MSI address,
generates an interrupt and writes the MSI packet into the ring buffer.
All the code is inside #ifdef DEBUG so it causes no harm unless it's
enabled.

The following adds local bus, flash and MTD partition nodes for
sbc8548. As well, a compatible field for the soc node, so that
of_platform_bus_probe() will pick it up.
Something that is provided through this newly added epld node
is the Hardware Revision which is now being utilized.

DIU platform code should not just write to the PIXIS' BRDCFG0 register,
it should set and clear its own bits only, otherwise it will break
firmware setup (in fact it breaks second uart).
Also get rid of magic numbers in the related code.

The 8568 MDS needs some configuration changes to the PHY in order to
work properly. These are done in the firmware, normally, but Linux
shouldn't need to rely on the firmware running such things (someone
could disable the PHY support in the firmware to save space, for instance).

We should not requeue the victim context in find_victim if the owner is
not in spu_run. It's first not needed because leaving the context on
the spu is an optimization and second is harmful because it means the
owner could re-enter spu_run when the context is on the runqueue and
trip the BUG_ON in __spu_update_sched_info.

Currently, page fault handlers don't issue a mfc restart if the context
switch pending flag is set, which can leave us with a hanging DMA after
a context restore.
This patch introduces fault pending flag that is set by the fault
handler and read by the context switch code, so that the latter can add
the restart bit at the right spot, after it has successfuly saved the
state of the mfc control register.

SPU class 0 & 1 exceptions may occur in parallel, so we may end up
overwriting csa.dsisr.
This change adds dedicated fields for each class to the spu and the spu
context so that fault data is not overwritten.

Currently, we re-route SPU interrupts to the current cpu, which may be
on a remote node. In the case of time slicing, all spu interrupts will
end up routed to the same cpu, where the spusched_tick occurs.
This change routes mfc interrupts to the cpu where the controlling
thread last ran, provided that cpu is on the same node as the spu
(otherwise don't reroute interrupts).
This should improve performance and provide a more predictable
environment for processing spu exceptions. In the past we have seen
concurrent delivery of spu exceptions to two cpus. This eliminates that
concern.

synchronize_irq() provides the serialization for
SPU_CONTEXT_SWITCH_PENDING which is read with a simple load. This
routine guarantees that the relevant interrupt handlers are not running,
so that the next time they do run they will see the update
memory value.
This must be done correctly so that exception handling code does not
restart the mfc in the middle of a context switch while we are trying
to atomically stop it and save state.

There's currently no way to tell if spu_process_callback has
returned with the state mutex held, as -EINTR may be returned
by either the syscall or the spu_acquire fail case.
Instead, just do a non-interruptible mutex_lock here.

Currently, we update the SPU master run control bit (ie,
spu_enable_spu) in spufs_run_spu before we grab the context mutex. This
can result in races with other processes accessing this context's
resources.
This change moves the spu_enable_spu to after we have acquired the
context lock.

We currently have two issues with the MFC save code:
* save_mfc_decr doesn't handle a transition of 1 -> 0 of the Ds bit
* The Q bit may be stale in the CSA
This change fixes the first issue by clearing the relevant bits from
the MFC_CNTL value in the CSA before or-ing in the updated status.
Also, we add the Q bit to the updated status.

Currently, we can introduce invalid entries into the MFC queues:
1) context starts a DMA
2) context gets scheduled out during a DMA
- kernel saves MFC queue to CSA
- kernel saves 0x0 in csa->mfc_control_RW
3) context gets scheduled in
- csa->mfc_control[Q] ('queues empty') isn't set, so DMA queues are
restored from the CSA
4) context's DMA is completed
5) context gets scheduled out again, no DMA occuring this time
- kernel sees that MFC_CNTL[Q] ('queues empty') is set, so doesn't
touch saved queue data in CSA
- kernel saves 0x0 in csa->mfc_control_RW
6) context gets scheduled in
- csa->mfc_control[Q] ('queues empty') isn't set (we saved is as 0!),
so DMA queues are restored from the CSA
In this last restore, we've restored the queue status from step 2,
which are now invalid.
This change makes save_mfc_cntl() closer to the save/restore sequence,
as specified in the CBE handbook.

When we issue a MFC purge request, we may inadvertantly clear the
suspended status.
This change adds the MFC_CNTL_SUSPEND_MASK when we issue a purge
request, so that the suspend bit is masked out.

We may currently lose interrupts during SPE context switch, as we alter
the INT_Route register. Because the IIC uses a per-thread priority
status, changing the interrupt routing to a different thread means that
the IRQ is no longer masked by the priority status, so we end up with
two fasteoi IRQ handlers executing for the one irq_desc. The fasteoi
handler doesn't handle multiple IRQs, so drops the second one.
Fix this by using our own flow handler. This is based on
handle_edge_irq, but issues an eoi after IRQs are handled, and doesn't
do any mask/unmasking.

There are userspace instrumentation tools that need to monitor spu
context switches. This patch adds a new file called 'switch_log' to
each spufs context directory that can be used to monitor the context
switches.
Context switch in/out and exit from spu_run are monitored after the
file was first opened and can be read from it.

The powerpc kernel maintains information about logical memory blocks
in the lmb.memory structure, which is initialized and updated at boot
time, but not when memory is added or removed while the kernel is
running.
This adds a hotplug memory notifier which updates lmb.memory when
memory is added or removed. This information is useful for eHEA
driver to find out the memory layout and holes.
NOTE: No special locking is needed for lmb_add() and lmb_remove().
Calls to these are serialized by caller. (pSeries_reconfig_chain).

Hotplug memory remove notifier for 64-bit powerpc. This gets invoked
by writing to /proc/ppc64/ofdt the string "remove_node " followed by
the firmware device tree pathname of the node that needs to be removed.
In response, this adjusts the sections and removes sysfs entries by
calling __remove_pages(). Then it calls arch-specific code to get rid
of the hardware MMU mappings for the section of memory.

This adds support for PCI Express port on Celleb. I/O space of this
PCI Express port is not mapped in memory space. So we use the

Add a DEBUG config setting which turns on all (most) of the debugging
under platforms/pseries.
To have this take effect we need to remove all the #undef DEBUG's, in
various files. We leave the #undef DEBUG in platforms/pseries/lpar.c,
as this enables debugging printks from the low-level hash table routines,
and tends to make your system unusable. If you want those enabled you
still have to turn them on by hand.
Also some of the RAS code has a DEBUG block which causes a functional
change, so I've keyed this off a different (non-existant) debug #define.
This is only enabled if you have PPC_EARLY_DEBUG enabled also.

In pseries/lpar.c, fix some printf specifier mismatches, and add
a newline to one printk.
In pseries/rtasd.c add "rtasd" to some messages to make it clear
where they're coming from.
In pseries/scanlog.c remove the hand-rolled runtime debugging support
in there. This file has been largely unchanged for eons, if we need to
debug it in future we can recompile.

On pseries LPAR we can call the udbg routines, and the udbg console very
early. So mark the udbg console as safe to call early in boot, and register
the udbg console as soon as the udbg routines are hooked up.
This allows platforms/pseries code to use printk() and pr_debug() rather
than needing to call udbg_printf() directly for early debugging. This is
nice because a) it's standard, b) it goes via the printk buffer, and c)
you can get printk time stamps.

Currently all iSeries secondary CPUs spin directly on the cpu_start
field in their paca. Make them spin on the global
__secondary_hold_spinloop until after the pacas have been initialised.
As Stephen Rothwell points out, this works at the moment because
__secondary_hold_spinloop is being set already, but iSeries isn't
looking at it :)

This lets us check if the device is really added to the driver core or
not, which is what we need when walking some of the bus lists. The flag
is there in anticipation of getting rid of the other PCI device list,
which is what we used to check in this situation.

None of these files use any of the functionality promised by
asm/semaphore.h. It's possible that they rely on it dragging in some
unrelated header file, but I can't build all these files, so we'll have
fix any build failures as they come up.

The xics code currently has a direct and lpar variant of
xics_host_map, the only difference being which irq_chip they use. If
we remember which irq_chip we're using we can combine these two
routines. That also allows us to have a single irq_host_ops instead
of two.

pseries_mpic_init_IRQ() implements the same logic as the xics code did to
find the i8259 cascade irq. Now that we've pulled that logic out into
pseries_setup_i8259_cascade() we can use it in the mpic code.
