Al Viro 8de5277879 vfs: check i_nlink limits in vfs_{mkdir,rename_dir,link}
New field of struct super_block - ->s_max_links.  Maximal allowed
value of ->i_nlink or 0; in the latter case all checks still need
to be done in ->link/->mkdir/->rename instances.  Note that this
limit applies both to directoris and to non-directories.

Djalal Harouni 34b0784056 ext2: protect inode changes in the SETVERSION and SETFLAGS ioctls
Unlock mutex after i_flags and i_ctime updates in the EXT2_IOC_SETFLAGS
ioctl.

Use i_mutex in the EXT2_IOC_SETVERSION ioctl to protect i_ctime and
i_generation updates and make the ioctl consistent since i_mutex is
also used in other places to protect timestamps and inode changes.

Paul Gortmaker 302bf2f325 ext2/3/4: delete unneeded includes of module.h
Delete any instances of include module.h that were not strictly
required.  In the case of ext2, the declaration of MODULE_LICENSE
etc. were in inode.c but the module_init/exit were in super.c, so
relocate the MODULE_LICENCE/AUTHOR block to super.c which makes it
consistent with ext3 and ext4 at the same time.

Jan Kara ef6919c283 ext2: Fix error handling on inode bitmap corruption
When insert_inode_locked() fails in ext2_new_inode() it most likely means inode
bitmap got corrupted and we allocated again inode which is already in use. Also
doing unlock_new_inode() during error recovery is wrong since the inode does
not have I_NEW set. Fix the problem by informing about filesystem error and
jumping to fail: (instead of fail_drop:) which doesn't call unlock_new_inode().

Al Viro 6b520e0565 vfs: fix the stupidity with i_dentry in inode destructors
Seeing that just about every destructor got that INIT_LIST_HEAD() copied into
it, there is no point whatsoever keeping this INIT_LIST_HEAD in inode_init_once();
the cost of taking it into inode_init_always() will be negligible for pipes
and sockets and negative for everything else.  Not to mention the removal of
boilerplate code from ->destroy_inode() instances...

changed the xyz_check_acl() functions into the more natural
xyz_get_acl() interface, we grew two copies of the

Christoph Hellwig 4e34e719e4 fs: take the ACL checks to common code
Replace the ->check_acl method with a ->get_acl method that simply reads an
ACL from disk after having a cache miss.  This means we can replace the ACL
checking boilerplate code with a single implementation in namei.c.

exactly the same thing.

Linus Torvalds e77819e57f vfs: move ACL cache lookup into generic code
This moves logic for checking the cached ACL values from low-level
filesystems into generic code.  The end result is a streamlined ACL
check that doesn't need to load the inode->i_op->check_acl pointer at
all for the common cached case.

The filesystems also don't need to check for a non-blocking RCU walk
case in their acl_check() functions, because that is all handled at a
VFS layer.

Wang Sheng-Hui 03b5bb3429 ext2: check xattr name_len before acquiring xattr_sem in ext2_xattr_get
In ext2_xattr_get(), the code will acquire xattr_sem first, later checks
the length of xattr name_len > 255. It's unnecessarily time consuming and
also ext2_xattr_set() checks the length before other checks. So move the
check before acquiring xattr_sem to make these two functions consistent.

Josef Bacik 02c24a8218 fs: push i_mutex and filemap_write_and_wait down into ->fsync() handlers
Btrfs needs to be able to control how filemap_write_and_wait_range() is called
in fsync to make it less of a painful operation, so push down taking i_mutex and
the calling of filemap_write_and_wait() down into the ->fsync() handlers.  Some
file systems can drop taking the i_mutex altogether it seems, like ext3 and
ocfs2.  For correctness sake I just pushed everything down in all cases to make
sure that we keep the current behavior the same for everybody, and then each
individual fs maintainer can make up their mind about what to do from there.

Christoph Hellwig aacfc19c62 fs: simplify the blockdev_direct_IO prototype
Simple filesystems always pass inode->i_sb_bdev as the block device
argument, and never need a end_io handler.  Let's simply things for
them and for my grepping activity by dropping these arguments.  The
only thing not falling into that scheme is ext4, which passes and
end_io handler without needing special flags (yet), but given how
messy the direct I/O code there is use of __blockdev_direct_IO
in one instead of two out of three cases isn't going to make a large
difference anyway.

Christoph Hellwig 562c72aa57 fs: move inode_dio_wait calls into ->setattr
Let filesystems handle waiting for direct I/O requests themselves instead
of doing it beforehand.  This means filesystem-specific locks to prevent
new dio referenes from appearing can be held.  This is important to allow
generalizing i_dio_count to non-DIO_LOCKING filesystems.

Mimi Zohar 9d8f13ba3f security: new security_inode_init_security API adds function callback
This patch changes the security_inode_init_security API by adding a
filesystem specific callback to write security extended attributes.
This change is in preparation for supporting the initialization of
multiple LSM xattrs and the EVM xattr.  Initially the callback function
walks an array of xattrs, writing each xattr separately, but could be
optimized to write multiple xattrs at once.

For existing security_inode_init_security() calls, which have not yet
been converted to use the new callback function, such as those in
reiserfs and ocfs2, this patch defines security_old_inode_init_security().

Robin Dong 4e299c1d91 ext2: fix error msg when mounting fs with too-large blocksize
When ext2 mounts a filesystem, it attempts to set the block device
blocksize with a call to sb_set_blocksize, which can fail for
several reasons.  The current failure message in ext2 prints:

  EXT2-fs (loop1): error: blocksize is too small

which is not correct in all cases.  This can be demonstrated
by creating a filesystem with

  # mkfs.ext2 -b 8192

on a 4k page system, and attempting to mount it.

Change the error message to a more generic:

  EXT2-fs (loop1): bad blocksize 8192

to match the error message in ext3.

Akinobu Mita f312eff816 bitops: remove ext2 non-atomic bitops from asm/bitops.h
As the result of conversions, there are no users of ext2 non-atomic bit
operations except for ext2 filesystem itself.  Now we can put them into
architecture independent code in ext2 filesystem, and remove from
asm/bitops.h for all architectures.

Josh Hunt e8a80c6f76 ext2: Fix link count corruption under heavy link+rename load
vfs_rename_other() does not lock renamed inode with i_mutex. Thus changing
i_nlink in a non-atomic manner (which happens in ext2_rename()) can corrupt
it as reported and analyzed by Josh.

In fact, there is no good reason to mess with i_nlink of the moved file.
We did it presumably to simulate linking into the new directory and unlinking
from an old one. But the practical effect of this is disputable because fsck
can possibly treat file as being properly linked into both directories without
writing any error which is confusing. So we just stop increment-decrement
games with i_nlink which also fixes the corruption.

Eric Paris 2a7dba391e fs/vfs/security: pass last path component to LSM on inode creation
SELinux would like to implement a new labeling behavior of newly created
inodes.  We currently label new inodes based on the parent and the creating
process.  This new behavior would also take into account the name of the
new object when deciding the new label.  This is not the (supposed) full path,
just the last component of the path.

This is very useful because creating /etc/shadow is different than creating
/etc/passwd but the kernel hooks are unable to differentiate these
operations.  We currently require that userspace realize it is doing some
difficult operation like that and than userspace jumps through SELinux hoops
to get things set up correctly.  This patch does not implement new
behavior, that is obviously contained in a seperate SELinux patch, but it
does pass the needed name down to the correct LSM hook.  If no such name
exists it is fine to pass NULL.

Eric Sandeen 40a063f669 ext2: speed up file creates by optimizing rec_len functions
The addition of 64k block capability in the rec_len_from_disk
and rec_len_to_disk functions added a bit of math overhead which
slows down file create workloads needlessly when the architecture
cannot even support 64k blocks, thanks to page size limits.

The directory entry checking can also be optimized a bit
by sprinkling in some unlikely() conditions to move the
error handling out of line.

bonnie++ sequential file creates on a 512MB ramdisk speeds up
from about 2200/s to about 2500/s, about a 14% improvement.

Nick Piggin fa0d7e3de6 fs: icache RCU free inodes
RCU free the struct inode. This will allow:

- Subsequent store-free path walking patch. The inode must be consulted for
  permissions when walking, so an RCU inode reference is a must.
- sb_inode_list_lock to be moved inside i_lock because sb list walkers who want
  to take i_lock no longer need to take sb_inode_list_lock to walk the list in
  the first place. This will simplify and optimize locking.
- Could remove some nested trylock loops in dcache code
- Could potentially simplify things a bit in VM land. Do not need to take the
  page lock to follow page->mapping.

The downsides of this is the performance cost of using RCU. In a simple
creat/unlink microbenchmark, performance drops by about 10% due to inability to
reuse cache-hot slab objects. As iterations increase and RCU freeing starts
kicking over, this increases to about 20%.

In cases where inode lifetimes are longer (ie. many inodes may be allocated
during the average life span of a single inode), a lot of this cache reuse is
not applicable, so the regression caused by this patch is smaller.

The cache-hot regression could largely be avoided by using SLAB_DESTROY_BY_RCU,
however this adds some complexity to list walking and store-free path walking,
so I prefer to implement this at a later date, if it is shown to be a win in
real situations. I haven't found a regression in any non-micro benchmark so I
doubt it will be a problem.

Jan Blunck 3e44f9f1dc BKL: Remove BKL from ext2 filesystem
The BKL is still used in ext2_put_super(), ext2_fill_super(), ext2_sync_fs()
ext2_remount() and ext2_write_inode(). From these calls ext2_put_super(),
ext2_fill_super() and ext2_remount() are protected against each other by
the struct super_block s_umount rw semaphore. The call in ext2_write_inode()
could only protect the modification of the ext2_sb_info through
ext2_update_dynamic_rev() against concurrent ext2_sync_fs() or ext2_remount().
ext2_fill_super() and ext2_put_super() can be left out because you need a
valid filesystem reference in all three cases, which you do not have when
you are one of these functions.

If the BKL is only protecting the modification of the ext2_sb_info it can
safely be removed since this is protected by the struct ext2_sb_info s_lock.

Jan Blunck db71922217 BKL: Explicitly add BKL around get_sb/fill_super
This patch is a preparation necessary to remove the BKL from do_new_mount().
It explicitly adds calls to lock_kernel()/unlock_kernel() around
get_sb/fill_super operations for filesystems that still uses the BKL.

I've read through all the code formerly covered by the BKL inside
do_kern_mount() and have satisfied myself that it doesn't need the BKL
any more.

do_kern_mount() is already called without the BKL when mounting the rootfs
and in nfsctl. do_kern_mount() calls vfs_kern_mount(), which is called
from various places without BKL: simple_pin_fs(), nfs_do_clone_mount()
through nfs_follow_mountpoint(), afs_mntpt_do_automount() through
afs_mntpt_follow_link(). Both later functions are actually the filesystems
follow_link inode operation. vfs_kern_mount() is calling the specified
get_sb function and lets the filesystem do its job by calling the given
fill_super function.

Therefore I think it is safe to push down the BKL from the VFS to the
low-level filesystems get_sb/fill_super operation.

Andreas Gruenbacher 2aec7c5232 mbcache: Remove unused features
The mbcache code was written to support a variable number of indexes,
but all the existing users use exactly one index.  Simplify to code to
support only that case.

There are also no users of the cache entry free operation, and none of
the users keep extra data in cache entries.  Remove those features as
well.

Christoph Hellwig 2c27c65ed0 check ATTR_SIZE contraints in inode_change_ok
Make sure we check the truncate constraints early on in ->setattr by adding
those checks to inode_change_ok.  Also clean up and document inode_change_ok
to make this obvious.

As a fallout we don't have to call inode_newsize_ok from simple_setsize and
simplify it down to a truncate_setsize which doesn't return an error.  This
simplifies a lot of setattr implementations and means we use truncate_setsize
almost everywhere.  Get rid of fat_setsize now that it's trivial and mark
ext2_setsize static to make the calling convention obvious.

Keep the inode_newsize_ok in vmtruncate for now as all callers need an
audit for its removal anyway.

Note: setattr code in ecryptfs doesn't call inode_change_ok at all and
needs a deeper audit, but that is left for later.

Christoph Hellwig f4e420dc42 clean up write_begin usage for directories in pagecache
For filesystem that implement directories in pagecache we call
block_write_begin with an already allocated page for this code, while the
normal regular file write path uses the default block_write_begin behaviour.

Get rid of the __foofs_write_begin helper and opencode the normal write_begin
call in foofs_write_begin, while adding a new foofs_prepare_chunk helper for
the directory code.  The added benefit is that foofs_prepare_chunk has
a much saner calling convention.

Note that the interruptible flag passed into block_write_begin is always
ignored if we already pass in a page (see next patch for details), and
we never were doing truncations of exessive blocks for this case either so we
can switch directly to block_write_begin_newtrunc.

Nick Piggin af5a30d8cf fix truncate inode time modification breakage
mtime and ctime should be changed only if the file size has actually
changed. Patches changing ext2 and tmpfs from vmtruncate to new truncate
sequence has caused regressions where they always update timestamps.

There is some strange cases in POSIX where truncate(2) must not update
times unless the size has acutally changed, see 6e656be89.

This area is all still rather buggy in different ways in a lot of
filesystems and needs a cleanup and audit (ideally the vfs will provide
a simple attribute or call to direct all filesystems exactly which
attributes to change). But coming up with the best solution will take a
while and is not appropriate for rc anyway.

So fix recent regression for now.

Christoph Hellwig 1b061d9247 rename the generic fsync implementations
We don't name our generic fsync implementations very well currently.
The no-op implementation for in-memory filesystems currently is called
simple_sync_file which doesn't make too much sense to start with,
the the generic one for simple filesystems is called simple_fsync
which can lead to some confusion.

This patch renames the generic file fsync method to generic_file_fsync
to match the other generic_file_* routines it is supposed to be used
with, and the no-op implementation to noop_fsync to make it obvious
what to expect.  In addition add some documentation for both methods.

Christoph Hellwig e0ccfd959c quota: move unmount handling into the filesystem
Currently the VFS calls into the quotactl interface for unmounting
filesystems.  This means filesystems with their own quota handling
can't easily distinguish between user-space originating quotaoff
and an unount.  Instead move the responsibily of the unmount handling
into the filesystem to be consistent with all other dquot handling.

Note that we do call dquot_disable a lot later now, e.g. after
a sync_filesystem.  But this is fine as the quota code does all its
writes via blockdev's mapping and that is synced even later.

Christoph Hellwig c79d967de3 quota: move remount handling into the filesystem
Currently do_remount_sb calls into the dquot code to tell it about going
from rw to ro and ro to rw.  Move this code into the filesystem to
not depend on the dquot code in the VFS - note ocfs2 already ignores
these calls and handles remount by itself.  This gets rid of overloading
the quotactl calls and allows to unify the VFS and XFS codepaths in
that area later.

Dmitry Monakhov 12755627bd quota: unify quota init condition in setattr
Quota must being initialized if size or uid/git changes requested.
But initialization performed in two different places:
in case of i_size file system is responsible for dquot init
, but in case of uid/gid init will be called internally in
dquot_transfer().
This ambiguity makes code harder to understand.
Let's move this logic to one common helper function.

Jan Blunck e0a5cbac02 BKL: Remove BKL from ext2 filesystem
The BKL is still used in ext2_put_super(), ext2_fill_super(), ext2_sync_fs()
ext2_remount() and ext2_write_inode(). From these calls ext2_put_super(),
ext2_fill_super() and ext2_remount() are protected against each other by
the struct super_block s_umount rw semaphore. The call in ext2_write_inode()
could only protect the modification of the ext2_sb_info through
ext2_update_dynamic_rev() against concurrent ext2_sync_fs() or ext2_remount().
ext2_fill_super() and ext2_put_super() can be left out because you need a
valid filesystem reference in all three cases, which you do not have when
you are one of these functions.

If the BKL is only protecting the modification of the ext2_sb_info it can
safely be removed since this is protected by the struct ext2_sb_info s_lock.

Jan Blunck c15271f4e7 ext2: Add ext2_sb_info s_lock spinlock
Add a spinlock that protects against concurrent modifications of
s_mount_state, s_blocks_last, s_overhead_last and the content of the
superblock's buffer pointed to by sbi->s_es. The spinlock is now used in
ext2_xattr_update_super_block() which was setting the
EXT2_FEATURE_COMPAT_EXT_ATTR flag on the superblock without protection
before. Likewise the spinlock is used in ext2_show_options() to have a
consistent view of the mount options.

This is a preparation patch for removing the BKL from ext2 in the next
patch.

Jan Blunck 20da9baf4c ext2: Remove duplicate code from ext2_sync_fs()
Depending in the state (valid or unchecked) of the filesystem either
ext2_sync_super() or ext2_commit_super() is called. If the filesystem is
currently valid (it is checked), we first mark it unchecked and afterwards
duplicate the work that ext2_sync_super() is doing later. Therefore this
patch removes the duplicate code and calls ext2_sync_super() directly after
marking the filesystem unchecked.

There is no point in loading bitmap for groups which are completely full.
This causes noticeable performance problems (and memory pressure) on small
systems with large full filesystem

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Christoph Hellwig 907f4554e2 dquot: move dquot initialization responsibility into the filesystem
Currently various places in the VFS call vfs_dq_init directly.  This means
we tie the quota code into the VFS.  Get rid of that and make the
filesystem responsible for the initialization.   For most metadata operations
this is a straight forward move into the methods, but for truncate and
open it's a bit more complicated.

For truncate we currently only call vfs_dq_init for the sys_truncate case
because open already takes care of it for ftruncate and open(O_TRUNC) - the
new code causes an additional vfs_dq_init for those which is harmless.

For open the initialization is moved from do_filp_open into the open method,
which means it happens slightly earlier now, and only for regular files.
The latter is fine because we don't need to initialize it for operations
on special files, and we already do it as part of the namespace operations
for directories.

Add a dquot_file_open helper that filesystems that support generic quotas
can use to fill in ->open.

and rationalize the wrappers around it to move as much as possible
code into the common block for CONFIG_QUOTA vs not.  Also rename
all these helpers to be named dquot_* instead of vfs_dq_*.

Christoph Hellwig 431547b3c4 sanitize xattr handler prototypes
Add a flags argument to struct xattr_handler and pass it to all xattr
handler methods.  This allows using the same methods for multiple
handlers, e.g. for the ACL methods which perform exactly the same action
for the access and default ACLs, just using a different underlying
attribute.  With a little more groundwork it'll also allow sharing the
methods for the regular user/trusted/secure handlers in extN, ocfs2 and
jffs2 like it's already done for xfs in this patch.

Also change the inode argument to the handlers to a dentry to allow
using the handlers mechnism for filesystems that require it later,
e.g. cifs.

Jan Kara 48bde86df0 ext2: report metadata errors during fsync
When an IO error happens while writing metadata buffers, we should better
report it and call ext2_error since the filesystem is probably no longer
consistent.  Sometimes such IO errors happen while flushing thread does
background writeback, the buffer gets later evicted from memory, and thus
the only trace of the error remains as AS_EIO bit set in blockdevice's
mapping.  So we check this bit in ext2_fsync and report the error although
we cannot be really sure which buffer we failed to write.

Stephen Hemminger 2074abfeb8 ext2: clear uptodate flag on super block I/O error
This fixes a WARN backtrace in mark_buffer_dirty() that occurs during
unmount when a USB or floppy device is removed. I reported this a kernel
regression, but looks like it might have been there for longer
than that.

The super block update from a previous operation has marked the buffer
as in error, and the flag has to be cleared before doing the update.
(Similar code already exists in ext4).

Andi Kleen aa261f549d HWPOISON: Enable .remove_error_page for migration aware file systems
Enable removing of corrupted pages through truncation
for a bunch of file systems: ext*, xfs, gfs2, ocfs2, ntfs
These should cover most server needs.

I chose the set of migration aware file systems for this
for now, assuming they have been especially audited.
But in general it should be safe for all file systems
on the data area that support read/write and truncate.

Caveat: the hardware error handler does not take i_mutex
for now before calling the truncate function. Is that ok?

Nicolas Pitre 9de6886ec6 ext2: fix unbalanced kmap()/kunmap()
In ext2_rename(), dir_page is acquired through ext2_dotdot().  It is
then released through ext2_set_link() but only if old_dir != new_dir.
Failing that, the pkmap reference count is never decremented and the
page remains pinned forever.  Repeat that a couple times with highmem
pages and all pkmap slots get exhausted, and every further kmap() calls
end up stalling on the pkmap_map_wait queue at which point the whole
system comes to a halt.

Bryan Donlan 4d6c13f87d ext2: return -EIO not -ESTALE on directory traversal through deleted inode
ext2_iget() returns -ESTALE if invoked on a deleted inode, in order to
report errors to NFS properly.  However, in ext[234]_lookup(), this
-ESTALE can be propagated to userspace if the filesystem is corrupted such
that a directory entry references a deleted inode.  This leads to a
misleading error message - "Stale NFS file handle" - and confusion on the
part of the admin.

The bug can be easily reproduced by creating a new filesystem, making a
link to an unused inode using debugfs, then mounting and attempting to ls
-l said link.

This patch thus changes ext2_lookup to return -EIO if it receives -ESTALE
from ext2_iget(), as ext2 does for other filesystem metadata corruption;
and also invokes the appropriate ext*_error functions when this case is
detected.

Jan Kara 39fe7557b4 ext2: Do not update mtime of a moved directory
One of our users is complaining that his backup tool is upset on ext2
(while it's happy on ext3, xfs, ...) because of the mtime change.

The problem is:

    mkdir foo
    mkdir bar
    mkdir foo/a

Now under ext2:
    mv foo/a foo/b

changes mtime of 'foo/a' (foo/b after the move).  That does not really
make sense and it does not happen under any other filesystem I've seen.

More complicated is:
    mv foo/a bar/a

This changes mtime of foo/a (bar/a after the move) and it makes some
sense since we had to update parent directory pointer of foo/a.  But
again, no other filesystem does this.  So after some thoughts I'd vote
for consistency and change ext2 to behave the same as other filesystems.

Do not update mtime of a moved directory.  Specs don't say anything
about it (neither that it should, nor that it should not be updated) and
other common filesystems (ext3, ext4, xfs, reiserfs, fat, ...) don't do
it.  So let's become more consistent.

Christoph Hellwig 6cfd014842 push BKL down into ->put_super
Move BKL into ->put_super from the only caller.  A couple of
filesystems had trivial enough ->put_super (only kfree and NULLing of
s_fs_info + stuff in there) to not get any locking: coda, cramfs, efs,
hugetlbfs, omfs, qnx4, shmem, all others got the full treatment.  Most
of them probably don't need it, but I'd rather sort that out individually.
Preferably after all the other BKL pushdowns in that area.

removed since we don't do lock_super() at all in generic_shutdown_super()

Christoph Hellwig 8c85e12512 remove ->write_super call in generic_shutdown_super
We just did a full fs writeout using sync_filesystem before, and if
that's not enough for the filesystem it can perform it's own writeout
in ->put_super, which many filesystems already do.

Move a call to foofs_write_super into every foofs_put_super for now to
guarantee identical behaviour until it's cleaned up by the individual
filesystem maintainers.

Exceptions:

 - affs already has identical copy & pasted code at the beginning of
   affs_put_super so no need to do it twice.
 - xfs does the right thing without it and I have changes pending for
   the xfs tree touching this are so I don't really need conflicts
   here..

Jan Kara 316cb4ef3e ext2: fix data corruption for racing writes
If two writers allocating blocks to file race with each other (e.g.
because writepages races with ordinary write or two writepages race with
each other), ext2_getblock() can be called on the same inode in parallel.
Before we are going to allocate new blocks, we have to recheck the block
chain we have obtained so far without holding truncate_mutex.  Otherwise
we could overwrite the indirect block pointer set by the other writer
leading to data loss.

The below test program by Ying is able to reproduce the data loss with ext2
on in BRD in a few minutes if the machine is under memory pressure:

For a reason that I was unable to understand in three months of debugging,
mount ext2 -o remount stopped working properly when remounting from
regular operation to xip, or the other way around.  According to a git
bisect search, the problem was introduced with the VM_MIXEDMAP/PTE_SPECIAL
rework in the vm:

    xip: support non-struct page backed memory

In the failing scenario, the filesystem is mounted read only via root=
kernel parameter on s390x.  During remount (in rc.sysinit), the inodes of
the bash binary and its libraries are busy and cannot be invalidated (the
bash which is running rc.sysinit resides on subject filesystem).
Afterwards, another bash process (running ifup-eth) recurses into a
subshell, runs dup_mm (via fork).  Some of the mappings in this bash
process were created from inodes that could not be invalidated during
remount.

Both parent and child process crash some time later due to inconsistencies
in their address spaces.  The issue seems to be timing sensitive, various
attempts to recreate it have failed.

This patch refuses to change the xip flag during remount in case some
inodes cannot be invalidated.  This patch keeps users from running into
that issue.

Duane Griffin ef8b646183 ext2: tighten restrictions on inode flags
At the moment there are few restrictions on which flags may be set on
which inodes.  Specifically DIRSYNC may only be set on directories and
IMMUTABLE and APPEND may not be set on links.  Tighten that to disallow
TOPDIR being set on non-directories and only NODUMP and NOATIME to be set
on non-regular file, non-directories.

Introduces a flags masking function which masks flags based on mode and
use it during inode creation and when flags are set via the ioctl to
facilitate future consistency.

Duane Griffin 0e090f1e05 ext2: don't inherit inappropriate inode flags from parent
At present BTREE/INDEX is the only flag that new ext2 inodes do NOT
inherit from their parent.  In addition prevent the flags DIRTY, ECOMPR,
INDEX, IMAGIC and TOPDIR from being inherited.  List inheritable flags
explicitly to prevent future flags from accidentally being inherited.

This fixes the TOPDIR flag inheritance bug reported at
http://bugzilla.kernel.org/show_bug.cgi?id=9866.

Pekka J Enberg 18a82eb9f9 ext2: allocate ->s_blockgroup_lock separately
As spotted by kmemtrace, struct ext2_sb_info is 17024 bytes on 64-bit
which makes it a very bad fit for SLAB allocators.  The culprit of the
wasted memory is ->s_blockgroup_lock which can be as big as 16 KB when
NR_CPUS >= 32.

To fix that, allocate ->s_blockgroup_lock, which fits nicely in a order 2
page in the worst case, separately.  This shinks down struct ext2_sb_info
enough to fit a 1 KB slab cache so now we allocate 16 KB + 1 KB instead of
32 KB saving 15 KB of memory.

might pick it in half-baked state.

same inumber hashed at the same time; otherwise a bogus fhandle coming
from nfsd might race with inode creation:

nfsd: iget_locked() creates inode
nfsd: try to read from disk, block on that.
ext2_new_inode(): allocate inode with that inumber
ext2_new_inode(): insert it into icache, set it up and dirty
ext2_write_inode(): get the relevant part of inode table in cache,
set the entry for our inode (and start writing to disk)
nfsd: get CPU again, look into inode table, see nice and sane on-disk
inode, set the in-core inode from it

oops - we have two in-core inodes with the same inumber live in icache,
both used for IO.  Welcome to fs corruption...

Change some task->e?[ug]id to task_e?[ug]id().  In some places it makes more
sense to use RCU directly rather than a convenient wrapper; these will be
addressed by later patches.

Eric Sandeen bd39597cbd ext2: avoid printk floods in the face of directory corruption
A very large directory with many read failures (either due to storage
problems, or due to invalid size & blocks from corruption) will generate a
printk storm as the filesystem continues to try to read all the blocks.
This flood of messages can tie up the box until it is complete - which may
be a very long time, especially for very large corrupted values.

This is fixed by only reporting the corruption once each time we try to
read the directory.

Mingming Cao d707d31c97 ext2: fix ext2 block reservation early ENOSPC issue
We could run into ENOSPC error on ext2, even when there is free blocks on
the filesystem.

The problem is triggered in the case the goal block group has 0 free
blocks , and the rest block groups are skipped due to the check of
"free_blocks < windowsz/2".  Current code could fall back to non
reservation allocation to prevent early ENOSPC after examing all the block
groups with reservation on , but this code was bypassed if the reservation
window is turned off already, which is true in this case.

This patch fixed two issues:
1) We don't need to turn off block reservation if the goal block group has
0 free blocks left and continue search for the rest of block groups.

Current code the intention is to turn off the block reservation if the
goal allocation group has a few (some) free blocks left (not enough for
make the desired reservation window),to try to allocation in the goal
block group, to get better locality.  But if the goal blocks have 0 free
blocks, it should leave the block reservation on, and continues search for
the next block groups,rather than turn off block reservation completely.

2) we don't need to check the window size if the block reservation is off.

The problem was originally found and fixed in ext4.

Steven Whitehouse a447c09324 vfs: Use const for kernel parser table
This is a much better version of a previous patch to make the parser
tables constant. Rather than changing the typedef, we put the "const" in
all the various places where its required, allowing the __initconst
exception for nfsroot which was the cause of the previous trouble.

This was posted for review some time ago and I believe its been in -mm
since then.

Josef Bacik 68c9d702bb generic block based fiemap implementation
Any block based fs (this patch includes ext3) just has to declare its own
fiemap() function and then call this generic function with its own
get_block_t. This works well for block based filesystems that will map
multiple contiguous blocks at one time, but will work for filesystems that
only map one block at a time, you will just end up with an "extent" for each
block. One gotcha is this will not play nicely where there is hole+data
after the EOF. This function will assume its hit the end of the data as soon
as it hits a hole after the EOF, so if there is any data past that it will
not pick that up. AFAIK no block based fs does this anyway, but its in the
comments of the function anyway just in case.

Hisashi Hifumi 8ab22b9abb vfs: pagecache usage optimization for pagesize!=blocksize
When we read some part of a file through pagecache, if there is a
pagecache of corresponding index but this page is not uptodate, read IO
is issued and this page will be uptodate.

I think this is good for pagesize == blocksize environment but there is
room for improvement on pagesize != blocksize environment.  Because in
this case a page can have multiple buffers and even if a page is not
uptodate, some buffers can be uptodate.

So I suggest that when all buffers which correspond to a part of a file
that we want to read are uptodate, use this pagecache and copy data from
this pagecache to user buffer even if a page is not uptodate.  This can
reduce read IO and improve system throughput.

I wrote a benchmark program and got result number with this program.

This benchmark do:

  1: mount and open a test file.

  2: create a 512MB file.

  3: close a file and umount.

  4: mount and again open a test file.

  5: pwrite randomly 300000 times on a test file.  offset is aligned
     by IO size(1024bytes).

  6: measure time of preading randomly 100000 times on a test file.

The result was:
	2.6.26
        330 sec

	2.6.26-patched
        226 sec

On ext3/4, a file is written through buffer/block.  So random read/write
mixed workloads or random read after random write workloads are optimized
with this patch under pagesize != blocksize environment.  This test result

If the block allocator gets blocks out of system zone ext2 calls ext2_error.
But if the file system is mounted with errors=continue retry block allocation.
 We need to mark the system zone blocks as in use to make sure retry don't
pick them again

System zone is the block range mapping block bitmap, inode bitmap and inode
table.

Nick Piggin 70688e4dd1 xip: support non-struct page backed memory
Convert XIP to support non-struct page backed memory, using VM_MIXEDMAP for
the user mappings.

This requires the get_xip_page API to be changed to an address based one.
Improve the API layering a little bit too, while we're here.

This is required in order to support XIP filesystems on memory that isn't
backed with struct page (but memory with struct page is still supported too).

Jared Hulbert 30afcb4bd2 return pfn from direct_access, for XIP
Alter the block device ->direct_access() API to work with the new
get_xip_mem() API (that requires both kaddr and pfn are returned).

Some architectures will not do the right thing in their virt_to_page() for use
by XIP (to translate from the kernel virtual address returned by
direct_access(), to a user mappable pfn in XIP's page fault handler.

However, we can't switch it to just return the pfn and not the kaddr, because
we have no good way to get a kva from a pfn, and XIP requires the kva for its
read(2) and write(2) handlers.  So we have to return both.

Jan Kara 335e92e8a5 vfs: fix possible deadlock in ext2, ext3, ext4 when using xattrs
mb_cache_entry_alloc() was allocating cache entries with GFP_KERNEL.  But
filesystems are calling this function while holding xattr_sem so possible
recursion into the fs violates locking ordering of xattr_sem and transaction
start / i_mutex for ext2-4.  Change mb_cache_entry_alloc() so that filesystems
can specify desired gfp mask and use GFP_NOFS from all of them.

David Howells 52fcf70329 iget: stop EXT2 from using iget() and read_inode()
Stop the EXT2 filesystem from using iget() and read_inode().  Replace
ext2_read_inode() with ext2_iget(), and call that instead of iget().
ext2_iget() then uses iget_locked() directly and returns a proper error code
instead of an inode in the event of an error.

ext2_fill_super() returns any error incurred when getting the root inode
instead of EINVAL.

Aneesh Kumar K.V 14e11e106b ext2: change the default behaviour on error
ext2 file system was by default ignoring errors and continuing.  This is
not a good default as continuing on error could lead to file system
corruption.  Change the default to mark the file system readonly.  Debian
and ubuntu already does this as the default in their fstab.

Forbid user from changing file flags on quota files.  User has no bussiness
in playing with these flags when quota is on.  Furthermore there is a
remote possibility of deadlock due to a lock inversion between quota file's
i_mutex and transaction's start (i_mutex for quota file is locked only when
trasaction is started in quota operations) in ext3 and ext4.

Linus Torvalds 0b832a4b93 Revert "ext2/ext3/ext4: add block bitmap validation"
This reverts commit 7c9e69faa2, fixing up
conflicts in fs/ext4/balloc.c manually.

The cost of doing the bitmap validation on each lookup - even when the
bitmap is cached - is absolutely prohibitive.  We could, and probably
should, do it only when adding the bitmap to the buffer cache.  However,
right now we are better off just reverting it.

Peter Zijlstra measured the cost of this extra validation as a 85%
decrease in cached iozone, and while I had a patch that took it down to
just 17% by not being _quite_ so stupid in the validation, it was still
a big slowdown that could have been avoided by just doing it right.

Eric Sandeen ef2fb67989 remove unused bh in calls to ext234_get_group_desc
ext[234]_get_group_desc never tests the bh argument, and only sets it if it
is passed in; it is perfectly happy with a NULL bh argument.  But, many
callers send one in and never use it.  May as well call with NULL like
other callers who don't use the bh.

Miklos Szeredi 93d44cb275 ext2: show all mount options
Using mtab is problematic for various reasons, one of them is that
unprivileged mounts won't turn up in there.  So we want to get rid of it, and
use /proc/mounts instead.

But most filesystems are lazy, and are not showing all mount options.  Which
means, that without mtab, the user won't be able to see some or all of the
options.

It would be nice if the generic code could remember the mount options, and
show them without the need to add extra code to filesystems.  But this is not
easy, because different filesystems handle mount options given options, and
not tough the rest.  This is not taken into account by mount(8) either, so
/etc/mtab will be broken in this case.

This series fixes up ->show_options() in ext[234].

Hugh spotted that some code does:
  percpu_counter_add(&counter, -unsignedlong)

which, when the amount argument is of type s32, sort-of works thanks to
two's-complement. However when we'd change the type to s64 this breaks on 32bit
machines, because the promotion rules zero extend the unsigned number.

Provide percpu_counter_sub() to hide the s64 cast. That is:
  percpu_counter_sub(&counter, foo)
is equal to:
  percpu_counter_add(&counter, -(s64)foo);

Nick Piggin 03158cd7eb fs: restore nobh
Implement nobh in new aops.  This is a bit tricky.  FWIW, nobh_truncate is
now implemented in a way that does not create blocks in sparse regions,
which is a silly thing for it to have been doing (isn't it?)

ext2 survives fsx and fsstress. jfs is converted as well... ext3
should be easy to do (but not done yet).

Eric Sandeen 780dcdb211 fix inode_table test in ext234_check_descriptors
ext[234]_check_descriptors sanity checks block group descriptor geometry at
mount time, testing whether the block bitmap, inode bitmap, and inode table
reside wholly within the blockgroup.  However, the inode table test is off
by one so that if the last block in the inode table resides on the last
block of the block group, the test incorrectly fails.  This is because it
tests the last block as (start + length) rather than (start + length - 1).

This can be seen by trying to mount a filesystem made such as:

 mkfs.ext2 -F -b 1024 -m 0 -g 256 -N 3744 fsfile 1024

which yields:

 EXT2-fs error (device loop0): ext2_check_descriptors: Inode table for group 0 not in group (block 101)!
 EXT2-fs: group descriptors corrupted!

There is a similar bug in e2fsprogs, patch already sent for that.

(I wonder if inside(), outside(), and/or in_range() should someday be
used in this and other tests throughout the ext filesystems...)

Paul Mundt 20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

Satyam Sharma 3bd858ab1c Introduce is_owner_or_cap() to wrap CAP_FOWNER use with fsuid check
Introduce is_owner_or_cap() macro in fs.h, and convert over relevant
users to it. This is done because we want to avoid bugs in the future
where we check for only effective fsuid of the current task against a
file's owning uid, without simultaneously checking for CAP_FOWNER as
well, thus violating its semantics.
[ XFS uses special macros and structures, and in general looked ...
untouchable, so we leave it alone -- but it has been looked over. ]

The (current->fsuid != inode->i_uid) check in generic_permission() and
exec_permission_lite() is left alone, because those operations are
covered by CAP_DAC_OVERRIDE and CAP_DAC_READ_SEARCH. Similarly operations
falling under the purview of CAP_CHOWN and CAP_LEASE are also left alone.

Badari Pulavarty 2235219b77 ext2: statfs speed up
This is a patch that speeds up statfs.  It is very simple - the "overhead"
calculation, which takes a huge amount of time for large filesystems, never
changes unless the size of the filesystem itself changes.  That means we can
store it in memory and only recalculate if the filesystem has been resized
(almost never).

It also fixes a minor problem that we never update the on-disk superblock free
blocks/inodes counts until the filesystem is unmounted.  While not fatal, we
may as well update that on disk when we have the information, and it makes
things like debugfs and dumpe2fs report a bit more accurate info.

Carsten Otte d054fe3d10 xip sendfile removal
This patch removes xip_file_sendfile, the sendfile implementation for
xip without replacement. Those customers that use xip on s390 are not
using sendfile() as far as we know, and so far s390 is the only platform
this could potentially be used on so far.
Having sendfile is not a popular feature for execute in place file
systems, however we have a working implementation of splice_read() based
on fs/splice.c if anyone asks for it.
At this point in time, it does not seem preferable to merge
splice_read() for xip because it causes extra maintenence effort due to
code duplication and it requires struct page behind the xip memory
segment. We'd like to get rid of that in favor of supporting flash based
embedded platforms (Monta Vista work) soon.

This patch is now treating timestamps with the high bit set as negative
times (before Jan 1, 1970).  This means we lose 1/2 of the possible range
of timestamps (lopping off 68 years before unix timestamp overflow -
now only 30 years away :-) to handle the extremely rare case of setting
timestamps into the distant past.

If we are only interested in fixing the underflow case, we could just
limit the values to 0 instead of storing negative values.  At worst this
will skew the timestamp by a few hours for timezones in the far east
(files would still show Jan 1, 1970 in "ls -l" output).

That said, it seems 32-bit systems (mine at least) allow files to be set
into the past (01/01/1907 works fine) so it seems this patch is bringing
the x86_64 behaviour into sync with other kernels.

On the plus side, we have a patch that is ready to add nanosecond timestamps
to ext3 and as an added bonus adds 2 high bits to the on-disk timestamp so
this extends the maximum date to 2242.

Christoph Lameter 50953fe9e0 slab allocators: Remove SLAB_DEBUG_INITIAL flag
I have never seen a use of SLAB_DEBUG_INITIAL.  It is only supported by
SLAB.

I think its purpose was to have a callback after an object has been freed
to verify that the state is the constructor state again?  The callback is
performed before each freeing of an object.

I would think that it is much easier to check the object state manually
before the free.  That also places the check near the code object
manipulation of the object.

Also the SLAB_DEBUG_INITIAL callback is only performed if the kernel was
compiled with SLAB debugging on.  If there would be code in a constructor
handling SLAB_DEBUG_INITIAL then it would have to be conditional on
SLAB_DEBUG otherwise it would just be dead code.  But there is no such code
in the kernel.  I think SLUB_DEBUG_INITIAL is too problematic to make real
use of, difficult to understand and there are easier ways to accomplish the
same effect (i.e.  add debug code before kfree).

There is a related flag SLAB_CTOR_VERIFY that is frequently checked to be
clear in fs inode caches.  Remove the pointless checks (they would even be
pointless without removeal of SLAB_DEBUG_INITIAL) from the fs constructors.

This is the last slab flag that SLUB did not support.  Remove the check for
unimplemented flags from SLUB.

Nick Piggin 6fe6900e1e mm: make read_cache_page synchronous
Ensure pages are uptodate after returning from read_cache_page, which allows
us to cut out most of the filesystem-internal PageUptodate calls.

I didn't have a great look down the call chains, but this appears to fixes 7
possible use-before uptodate in hfs, 2 in hfsplus, 1 in jfs, a few in
ecryptfs, 1 in jffs2, and a possible cleared data overwritten with readpage in
block2mtd.  All depending on whether the filler is async and/or can return
with a !uptodate page.

Hugh Dickins 2e7842b887 [PATCH] fix umask when noACL kernel meets extN tuned for ACLs
Fix insecure default behaviour reported by Tigran Aivazian: if an ext2 or
ext3 or ext4 filesystem is tuned to mount with "acl", but mounted by a
kernel built without ACL support, then umask was ignored when creating
inodes - though root or user has umask 022, touch creates files as 0666,
and mkdir creates directories as 0777.

This appears to have worked right until 2.6.11, when a fix to the default
mode on symlinks (always 0777) assumed VFS applies umask: which it does,
unless the mount is marked for ACLs; but ext[234] set MS_POSIXACL in
s_flags according to s_mount_opt set according to def_mount_opts.

We could revert to the 2.6.10 ext[234]_init_acl (adding an S_ISLNK test);
but other filesystems only set MS_POSIXACL when ACLs are configured.  We
could fix this at another level; but it seems most robust to avoid setting
the s_mount_opt flag in the first place (at the expense of more ifdefs).

Likewise don't set the XATTR_USER flag when built without XATTR support.

Eric Sandeen d8adb9cef7 [PATCH] ext2: skip pages past number of blocks in ext2_find_entry

If a directory's i_size is corrupted, ext2_find_entry() will keep
processing pages until the i_size is reached, even if there are no more
blocks associated with the directory inode.  This patch puts in some
minimal sanity-checking so that we don't keep checking pages (and issuing
errors) if we know there can be no more data to read, based on the block
count of the directory inode.

This is somewhat similar in approach to the ext3 patch I sent earlier this
year.

David Howells f0d1b0b30d [PATCH] LOG2: Implement a general integer log2 facility in the kernel
This facility provides three entry points:

	ilog2()		Log base 2 of unsigned long
	ilog2_u32()	Log base 2 of u32
	ilog2_u64()	Log base 2 of u64

These facilities can either be used inside functions on dynamic data:

	int do_something(long q)
	{
		...;
		y = ilog2(x)
		...;
	}

Or can be used to statically initialise global variables with constant values:

	unsigned n = ilog2(27);

When performing static initialisation, the compiler will report "error:
initializer element is not constant" if asked to take a log of zero or of
something not reducible to a constant.  They treat negative numbers as
unsigned.

When not dealing with a constant, they fall back to using fls() which permits
them to use arch-specific log calculation instructions - such as BSR on
x86/x86_64 or SCAN on FRV - if available.

Vasily Averin 5a2b4062f5 [PATCH] ext2: errors behaviour fix
Current error behaviour for ext2 and ext3 filesystems does not fully
correspond to the documentation and should be fixed.

According to man 8 mount, ext2 and ext3 file systems allow to set one of 3
different on-errors behaviours:

  ---- start of quote man 8 mount ----

  errors=continue / errors=remount-ro / errors=panic

    Define the behaviour when an error is encountered.  (Either ignore
    errors and just mark the file system erroneous and continue, or remount
    the file system read-only, or panic and halt the system.) The default is
    set in the filesystem superblock, and can be changed using tune2fs(8).

  ---- end of quote ----

However EXT3_ERRORS_CONTINUE is not read from the superblock, and thus
ERRORS_CONT is not saved on the sbi->s_mount_opt.  It leads to the incorrect
handle of errors on ext3.

Then we've checked corresponding code in ext2 and discovered that it is buggy
as well:

- EXT2_ERRORS_CONTINUE is not read from the superblock (the same);

- parse_option() does not clean the alternative values and thus something
  like (ERRORS_CONT|ERRORS_RO) can be set;

- if options are omitted, parse_option() does not set any of these options.

Therefore it is possible to set any combination of these options on the ext2:

- none of them may be set: EXT2_ERRORS_CONTINUE on superblock / empty mount
  options;

- any of them may be set using mount options;

- 2 any options may be set: by using EXT2_ERRORS_RO/EXT2_ERRORS_PANIC on the
  superblock and other value in mount options;

- and finally all three options may be set by adding third option in remount.

Currently ext2 uses these values only in ext2_error() and it is not leading to
any noticeable troubles.  However somebody may be discouraged when he will try
to workaround EXT2_ERRORS_PANIC on the superblock by using errors=continue in
mount options.

This patch:

EXT2_ERRORS_CONTINUE should be read from the superblock as default value for
error behaviour.  parse_option() should clean the alternative options and
should not change default value taken from the superblock.

Dave Hansen 9a53c3a783 [PATCH] r/o bind mounts: unlink: monitor i_nlink
When a filesystem decrements i_nlink to zero, it means that a write must be
performed in order to drop the inode from the filesystem.

We're shortly going to have keep filesystems from being remounted r/o between
the time that this i_nlink decrement and that write occurs.

So, add a little helper function to do the decrements.  We'll tie into it in a
bit to note when i_nlink hits zero.

This patch cleans up generic_file_*_read/write() interfaces.  Christoph
Hellwig gave me the idea for this clean ups.

In a nutshell, all filesystems should set .aio_read/.aio_write methods and use
do_sync_read/ do_sync_write() as their .read/.write methods.  This allows us
to cleanup all variants of generic_file_* routines.

Final available interfaces:

generic_file_aio_read() - read handler
generic_file_aio_write() - write handler
generic_file_aio_write_nolock() - no lock write handler

__generic_file_aio_write_nolock() - internal worker routine
