Now that the SPDX tag is in all driver core files, that identifies the
license in a specific and legally-defined manner. So the extra GPL text
wording can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the driver core files files with the correct SPDX license
identifier based on the license text in the file itself. The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

Most of these files were implicitly getting EXPORT_SYMBOL via
device.h which was including module.h, but that path will be broken
soon.

We don't need a kset here, a simple kobject will do just fine, so
dynamically create the kobject and use it.
We also rename hypervisor_subsys to hypervisor_kset to catch all users
of the variable.

We don't need a "default" ktype for a kset. We should set this
explicitly every time for each kset. This change is needed so that we
can make ksets dynamic, and cleans up one of the odd, undocumented
assumption that the kset/kobject/ktype model has.

To have a home for all hypervisors, this patch creates /sys/hypervisor.
A new config option SYS_HYPERVISOR is introduced, which should to be set
by architecture dependent hypervisors (e.g. s390 or Xen).
