Martin Schwidefsky 1f44a22577 s390: convert interrupt handling to use generic hardirq
With the introduction of PCI it became apparent that s390 should
convert to generic hardirqs as too many drivers do not have the
correct dependency for GENERIC_HARDIRQS. On the architecture
level s390 does not have irq lines. It has external interrupts,
I/O interrupts and adapter interrupts. This patch hard-codes all
external interrupts as irq #1, all I/O interrupts as irq #2 and
all adapter interrupts as irq #3. The additional information from
the lowcore associated with the interrupt is stored in the
pt_regs of the interrupt frame, where the interrupt handler can
pick it up. For PCI/MSI interrupts the adapter interrupt handler
scans the relevant bit fields and calls generic_handle_irq with
the virtual irq number for the MSI interrupt.

The PCI code is the first user of adapter interrupts vectors.
Add a set of helpers to airq.c to separate the adatper interrupt
code from the PCI bits. The helpers allow for adapter interrupt
vectors of any size.

Martin Schwidefsky f4eae94f71 s390/airq: simplify adapter interrupt code
There are three users of adapter interrupts: AP, QDIO and PCI. Each
registers a single adapter interrupt with independent ISCs. Define
a "struct airq" with the interrupt handler, a pointer and a mask for
the local summary indicator and the ISC for the adapter interrupt
source. Convert the indicator array with its fixed number of adapter
interrupt sources per ISE to an array of hlists. This removes the
limitation to 32 adapter interrupts per ISC and allows for arbitrary
memory locations for the local summary indicator.

Martin Schwidefsky 48f6b00c6e s390/irq: store interrupt information in pt_regs
Copy the interrupt parameters from the lowcore to the pt_regs structure
in entry[64].S and reduce the arguments of the low level interrupt handler
to the pt_regs pointer only. In addition move the test-pending-interrupt
loop from do_IRQ to entry[64].S to make sure that interrupt information
is always delivered via pt_regs.

Sebastian Ott 3ad19b8fb0 s390/cio: fix irq statistics
When we fetch an interrupt on the CCW console using tsch (via
ccw_device_wait_idle formerly known as wait_cons_dev) we increment
the irq count for the affected interruption class but it is not
accounted as an IO interrupt.

This is broken since commit b603d258a4
"s390: remove superfluous tpi from wait_cons_dev"

Fix it so that the sum of the individual interrupts per class matches
the number of IO interrupts again.

Sebastian Ott 0ad8f714a1 s390/cio: fix early init counter usage
Via ccw_device_init_count we keep track of how many devices are in
asynchronous device recognition/initialization. For early devices this
variable was not only used prior to its initialization but used
incorrectly (incremented but never decremented). Fix this by using static
initialization for this variable (and friends), make them visible to
device.c only, and decrement the counter after recognition of early
devices is finished.

wait_cons_dev is used to busy wait for an interrupt on the console
ccw device. Stop using the static console_subchannel and add a
parameter to this function to specify on which ccw device/subchannel
we have to do the polling.

While at it rename the function to ccw_device_wait_idle and
move it to device.c

Sebastian Ott 88e7616e40 s390/cio: handle unknown pgroup state
When an attempt to query or modify the grouping state of a channel path
fails due to a timeout, we cannot be sure about its state. To get back to
a defined state, disband the whole path group and try again while excluding
the offending path.

Sebastian Ott e6a0b7c90f s390/cio: skip broken paths
Omit known to be broken paths (those set in path_noirq_mask) for the
sense/set PGID and nop IO commands.

Note: path_noirq_mask will be reset in ccw_device_verify_start (the
paths could be healthy again). However if we restart a path
verification via verify_start this mask will not be reset (there is
no need to let the wait for an interrupt time out again - plus we
do not want to loop once we deal with the paths in unknown path group
state).

happend to be during an operation where we query or alter the state of
path groups set the pgid_unknown flag.

With this change we allow usage of devices which have such ill-behaved
paths (if at least one path is operational).

Sebastian Ott 509d97b6f9 s390/chsc: fix SEI usage
cbc0dd1 "s390/pci: CHSC PCI support for error and availability events"
introduced a new SEI notification type as part of pci support.
The way SEI was called with nt2 and nt0 consecutive broke the nt0
stuff used for channel subsystem notifications.

The reason why this was broken with the mentioned patch is that you
cannot selectively disable type 0 notifications (so even when asked
for type 2 only, type 0 could be presented).

The way to do it is to tell SEI which types of notification you can
process and -this is the important part- look at the SEI result which
notification type you actually received.

Heiko Carstens 420f42ecf4 s390/irq: remove split irq fields from /proc/stat
Now that irq sum accounting for /proc/stat's "intr" line works again we
have the oddity that the sum field (first field) contains only the sum
of the second (external irqs) and third field (I/O interrupts).
The reason for that is that these two fields are already sums of all other
fields. So if we would sum up everything we would count every interrupt
twice.
This is broken since the split interrupt accounting was merged two years
ago: 052ff461c8 "[S390] irq: have detailed
statistics for interrupt types".
To fix this remove the split interrupt fields from /proc/stat's "intr"
line again and only have them in /proc/interrupts.

This restores the old behaviour, seems to be the only sane fix and mimics
a behaviour from other architectures where /proc/interrupts also contains
more than /proc/stat's "intr" line does.

Heiko Carstens add9bde216 s390/irq: enable irq sum accounting for /proc/stat again
For more than two years, since f2c66cd8ee
"/proc/stat: scalability of irq num per cpu" the output of /proc/stat is
broken.
The first field in the "intr" line should contain the sum of all interrupts,
however since the above mentioned change it is always zero.

The reason for that is that a per cpu irq sum variable had been introduced
which got incremented when calling kstat_incr_irqs_this_cpu(). However
on s390 we directly incremented only the per cpu per irq counter by accessing
the array element via kstat_cpu(smp_processor_id()).irqs[...].
So fix this and use the kstat_incr_irqs_this_cpu() wrapper which increments
both: the per cpu per irq counter and the per cpu irq sum counter.

Jan Glauber 6726a807c3 s390/qdio: rename the misleading PCI flag of qdio devices
For each detected qdio device a line like to following is printed:
qdio: 0.0.4102 OSA on SC 1045 using AI:1 QEBSM:0 PCI:1 TDD:1 SIGA: W AP

The PCI flag is misleading as this stands for "program controlled interrupt".
Rename it to PRI "program requested interrupt" which is more accurate and
does not interfere with another popular piece of technology.

Leave the pci string in the code since changing that would result in a huge
patch.

Sebastian Ott d99e79ec55 s390/cio: fix pgid reserved check
The check to whom a device is reserved is done by checking the path
state of the affected channel paths. If it turns out that one path is
flagged as reserved by someone else the whole device is marked as such.

However the meaning of the RESVD_ELSE bit is that the addressed device
is reserved to a different pathgroup (and not reserved to a different
LPAR). If we do this test on a path which is currently not a member of
the pathgroup we could erroneously mark the device as reserved to
someone else.

To fix this collect the reserved state for all potential members of the
pathgroup and only mark the device as reserved if all of those potential
members have the RESVD_ELSE bit set.
