As null_blk is a very good start point to test block layer, this patch
adds description and comments to 'timeout', 'requeue' and 'init_hctx' to
explain how to use fault injection with null_blk.
The nvme has similar with nvme_core.fail_request in the form of comment.

It will start failing randomly including IO to unrelated zones because of
->error "reuse". Trigger can be partition detection as well if test is not
run immediately which is even more entertaining.
The fix is of course to clear ->error where necessary.

In commit 2da22da573 (nbd: fix zero cmd timeout handling v2),
it is allowed to reset timer when it fires if tag_set.timeout
is set to zero. If the server is shutdown and a new socket
is reconfigured, the request should be requeued to be processed by
new server instead of waiting for response from the old one.

Nbd server with multiple connections could be upgraded since
560bc4b (nbd: handle dead connections). But if only one conncection
is configured, after we take down nbd server, all inflight IO
would finally timeout and return error. We could requeue them
like what we do with multiple connections and wait for new socket
in submit path.

__loop_update_dio() can be called as a part of loop_set_fd(), when the
block queue is not yet up and running; avoid freezing the block queue in
that case, since that is an expensive operation.

Instead of initializing null_blk hardware queues explicitly after the
request queue has been created, provide .init_hctx() and .exit_hctx()
callback functions. The latter functions are not only called during
request queue allocation but also when the number of hardware queues
changes. Allocate nr_cpu_ids queues during initialization to support
increasing the number of hardware queues above the initial hardware
queue count.
This change fixes increasing the number of hardware queues above the
initial number of hardware queues and also keeps nullb->nr_queues in
sync with the number of hardware queues.

Since nobody else is going to restart our hw_queue for us, the
blk_mq_start_stopped_hw_queues() is in virtblk_done() is not sufficient
necessarily sufficient to ensure that the queue will get started again.
In case of global resource outage (-ENOMEM because mapping failure,
because of swiotlb full) our virtqueue may be empty and we can get
stuck with a stopped hw_queue.
Let us not stop the queue on arbitrary errors, but only on -EONSPC which
indicates a full virtqueue, where the hw_queue is guaranteed to get
started by virtblk_done() before when it makes sense to carry on
submitting requests. Let us also remove a stale comment.

Jordy Zomer reported a KASAN out-of-bounds read in the floppy driver in
wait_til_ready().
Which on the face of it can't happen, since as Willy Tarreau points out,
the function does no particular memory access. Except through the FDCS
macro, which just indexes a static allocation through teh current fdc,
which is always checked against N_FDC.
Except the checking happens after we've already assigned the value.
The floppy driver is a disgrace (a lot of it going back to my original
horrd "design"), and has no real maintainer. Nobody has the hardware,
and nobody really cares. But it still gets used in virtual environment
because it's one of those things that everybody supports.
The whole thing should be re-written, or at least parts of it should be
seriously cleaned up. The 'current fdc' index, which is used by the
FDCS macro, and which is often shadowed by a local 'fdc' variable, is a
prime example of how not to write code.
But because nobody has the hardware or the motivation, let's just fix up
the immediate problem with a nasty band-aid: test the fdc index before
actually assigning it to the static 'fdc' variable.

Pull vfs file system parameter updates from Al Viro:
"Saner fs_parser.c guts and data structures. The system-wide registry
of syntax types (string/enum/int32/oct32/.../etc.) is gone and so is
the horror switch() in fs_parse() that would have to grow another case
every time something got added to that system-wide registry.
New syntax types can be added by filesystems easily now, and their
namespace is that of functions - not of system-wide enum members. IOW,
they can be shared or kept private and if some turn out to be widely
useful, we can make them common library helpers, etc., without having
to do anything whatsoever to fs_parse() itself.
And we already get that kind of requests - the thing that finally
pushed me into doing that was "oh, and let's add one for timeouts -
things like 15s or 2h". If some filesystem really wants that, let them
do it. Without somebody having to play gatekeeper for the variants
blessed by direct support in fs_parse(), TYVM.
Quite a bit of boilerplate is gone. And IMO the data structures make a

fs_parse() analogue taking p_log instead of fs_context.
fs_parse() turned into a wrapper, callers in ceph_common and rbd
switched to __fs_parse().
As the result, fs_parse() never gets NULL fs_context and neither
do fs_context-based logging primitives

As it is, vfs_parse_fs_string() makes "foo" and "foo=" indistinguishable;
both get fs_value_is_string for ->type and NULL for ->string. To make
it even more unpleasant, that combination is impossible to produce with
fsconfig().
Much saner rules would be
"foo" => fs_value_is_flag, NULL
"foo=" => fs_value_is_string, ""
"foo=bar" => fs_value_is_string, "bar"
All cases are distinguishable, all results are expressable by fsconfig(),
->has_value checks are much simpler that way (to the point of the field
being useless) and quite a few regressions go away (gfs2 has no business
accepting -o nodebug=, for example).

In brd_init func, rd_nr num of brd_device are firstly allocated
and add in brd_devices, then brd_devices are traversed to add each
brd_device by calling add_disk func. When allocating brd_device,
the disk->first_minor is set to i * max_part, if rd_nr * max_part
is larger than MINORMASK, two different brd_device may have the same
devt, then only one of them can be successfully added.
when rmmod brd.ko, it will cause oops when calling brd_exit.

Currently when an error code -EIO or -ENOSPC in the for-loop of
writeback_store the error code is being overwritten by a ret = len
assignment at the end of the function and the error codes are being
lost. Fix this by assigning ret = len at the start of the function and
remove the assignment from the end, hence allowing ret to be preserved
when error codes are assigned to it.

The worst-case scenario on finding same element pages is that almost all
elements are same at the first glance but only last few elements are
different.
Since the same element tends to be grouped from the beginning of the
pages, if we check the first element with the last element before
looping through all elements, we might have some chances to quickly
detect non-same element pages.
1. Test is done under LG webOS TV (64-bit arch)
2. Dump the swap-out pages (~819200 pages)
3. Analyze the pages with simple test script which counts the iteration
number and measures the speed at off-line
Under 64-bit arch, the worst iteration count is PAGE_SIZE / 8 bytes =
512. The speed is based on the time to consume page_same_filled()
function only. The result, on average, is listed as below:

The result shows that the average iteration count decreases by 13% and
the speed increases by 25% with this patch. This patch does not

Today the Xen blkfront driver allocates memory for one struct
blkfront_ring_info for each communication ring. This structure is
statically sized for the maximum supported configuration resulting
in a size of more than 90 kB.
As the main size contributor is one array inside the struct, the
memory allocation can easily be limited by moving this array to be
the last structure element and to allocate only the memory for the
actually needed array size.

When kzalloc fail, may cause trying to destroy the
workqueue from inside the workqueue.
If num_connections is m (2 < m), and NO.1 ~ NO.n
(1 < n < m) kzalloc are successful. The NO.(n + 1)
failed. Then, nbd_start_device will return ENOMEM
to nbd_start_device_ioctl, and nbd_start_device_ioctl
will return immediately without running flush_workqueue.
However, we still have n recv threads. If nbd_release
run first, recv threads may have to drop the last
config_refs and try to destroy the workqueue from
inside the workqueue.
To fix it, add a flush_workqueue in nbd_start_device.

Switching to struct_size for the allocation in fifo_alloc avoids
hard-coding the type of fifo_buffer.values in fifo_alloc. It also
provides overflow protection; to avoid pessimistic code being
generated by the compiler as a result, this patch also switches
fifo_size to unsigned, propagating the change as appropriate.

Each `blkif` has a free pages pool for the grant mapping. The size of
the pool starts from zero and is increased on demand while processing
the I/O requests. If current I/O requests handling is finished or 100
milliseconds has passed since last I/O requests handling, it checks and
shrinks the pool to not exceed the size limit, `max_buffer_pages`.
Therefore, host administrators can cause memory pressure in blkback by
attaching a large number of block devices and inducing I/O. Such
problematic situations can be avoided by limiting the maximum number of
devices that can be attached, but finding the optimal limit is not so
easy. Improper set of the limit can results in memory pressure or a
resource underutilization. This commit avoids such problematic
situations by squeezing the pools (returns every free page in the pool
to the system) for a while (users can set this duration via a module
parameter) if memory pressure is detected.
Discussions
===========
The `blkback`'s original shrinking mechanism returns only pages in the
pool which are not currently be used by `blkback` to the system. In
other words, the pages that are not mapped with granted pages. Because
this commit is changing only the shrink limit but still uses the same
freeing mechanism it does not touch pages which are currently mapping
grants.
Once memory pressure is detected, this commit keeps the squeezing limit
for a user-specified time duration. The duration should be neither too
long nor too short. If it is too long, the squeezing incurring overhead
can reduce the I/O performance. If it is too short, `blkback` will not
free enough pages to reduce the memory pressure. This commit sets the
value as `10 milliseconds` by default because it is a short time in
terms of I/O while it is a long time in terms of memory operations.
Also, as the original shrinking mechanism works for at least every 100
milliseconds, this could be a somewhat reasonable choice. I also tested
other durations (refer to the below section for more details) and
confirmed that 10 milliseconds is the one that works best with the test.
That said, the proper duration depends on actual configurations and
workloads. That's why this commit allows users to set the duration as a
module parameter.
Memory Pressure Test
====================
To show how this commit fixes the memory pressure situation well, I
configured a test environment on a xen-running virtualization system.
On the `blkfront` running guest instances, I attach a large number of
network-backed volume devices and induce I/O to those. Meanwhile, I
measure the number of pages that swapped in (pswpin) and out (pswpout)
on the `blkback` running guest. The test ran twice, once for the
`blkback` before this commit and once for that after this commit. As
shown below, this commit has dramatically reduced the memory pressure:

To find a best squeezing duration, I repeated the test with three
different durations (1ms, 10ms, and 100ms). The results are as below:

As expected, the memory pressure decreases as the duration increases,
but the reduction become slow from the `10ms`. Based on this results, I
chose the default duration as 10ms.
Performance Overhead Test
=========================
This commit could incur I/O performance degradation under severe memory
pressure because the squeezing will require more page allocations per
I/O. To show the overhead, I artificially made a worst-case squeezing
situation and measured the I/O performance of a `blkfront` running
guest.
For the artificial squeezing, I set the `blkback.max_buffer_pages` using
the `/sys/module/xen_blkback/parameters/max_buffer_pages` file. In this
test, I set the value to `1024` and `0`. The `1024` is the default
value. Setting the value as `0` is same to a situation doing the
squeezing always (worst-case).
If the underlying block device is slow enough, the squeezing overhead
could be hidden. For the reason, I use a fast block device, namely the
rbd[1]:

directly to the device as below and collect the 'MB/s' results.

In short, even worst case squeezing on ramdisk based fast block device
makes no visible performance degradation. Please note that this is just
a very simple and minimal test. On systems using super-fast block
devices and a special I/O workload, the results might be different. If
you have any doubt, test on your machine with your workload to find the
optimal squeezing duration for you.

Each driver calling scsi_ioctl() gets an equivalent compat_ioctl()
handler that implements the same commands by calling scsi_compat_ioctl().
The scsi_cmd_ioctl() and scsi_cmd_blk_ioctl() functions are compatible
at this point, so any driver that calls those can do so for both native
and compat mode, with the argument passed through compat_ptr().
With this, we can remove the entries from fs/compat_ioctl.c. The new
code is larger, but should be easier to maintain and keep updated with
newly added commands.

Various block drivers implement the CDROMMULTISESSION,
CDROM_GET_CAPABILITY, and CDROMEJECT ioctl commands, relying on the
block layer to handle compat_ioctl mode for them.
Move this into the drivers directly as a preparation for simplifying
the block layer later.
When only integer arguments or no arguments are passed, the
same handler can be used for .ioctl and .compat_ioctl, and
when only pointer arguments are passed, the newly added
blkdev_compat_ptr_ioctl can be used.

compat handler. Making the normal implementation do the
right thing is actually very simply, so just do that by

There is only one implementation of this ioctl, so move the handling out
of the common block layer code into the place where it's actually needed.
It also gets called indirectly through pktcdvd, which needs to be aware
of this change.
As I noticed, the old implementation of the compat handler failed to
convert the structure on the way out, so the updated fields never got
written back to user space. This is either not important, or it has
never worked and should be fixed now.

These drivers implement the HDIO_GET_IDENTITY and CDROMVOLREAD ioctl
commands, which are compatible between 32-bit and 64-bit user space and
traditionally handled by compat_blkdev_driver_ioctl().
As a prerequisite to removing that function, make both drivers use
blkdev_compat_ptr_ioctl() as their .compat_ioctl callback.

By simply re-attaching to shared rings during connect_ring() rather than
assuming they are freshly allocated (i.e assuming the counters are zero)
it is possible for vbd instances to be unbound and re-bound from and to
(respectively) a running guest.
This has been tested by running:

nbd: fix max number of supported devs
where we can deadlock during device shutdown. The problem occurs if
the recv_work's nbd_config_put occurs after nbd_start_device_ioctl has
returned and the userspace app has droppped its reference via closing
the device and running nbd_release. The recv_work nbd_config_put call
would then drop the refcount to zero and try to destroy the config which
would try to do destroy_workqueue from the recv work.
This patch just has nbd_start_device_ioctl do a flush_workqueue when it
wakes so we know after the ioctl returns running works have exited. This
also fixes a possible race where we could try to reuse the device while
old recv_works are still running.

Queue dma alignment limit requires users(fs, target, ...) of block layer
to pass aligned buffer.
So far brd doesn't support un-aligned buffer, even though it is easy
to support it.
However, given brd is often used for debug purpose, and there are other
drivers which can't support un-aligned buffer too.
So add warning so that brd users know what to fix.

Now we depend on blk_queue_split() to respect most of queue limit
(the only one exception could be dma alignment), however
blk_queue_split() isn't used for brd, so this limit isn't respected
since v4.3.
Also max_hw_sectors limit doesn't play a big role for brd, which is
added since brd is added to tree for unknown reason.
So remove it.

For each I/O request, blkback first maps the foreign pages for the
request to its local pages. If an allocation of a local page for the
mapping fails, it should unmap every mapping already made for the
request.
However, blkback's handling mechanism for the allocation failure does
not mark the remaining foreign pages as unmapped. Therefore, the unmap
function merely tries to unmap every valid grant page for the request,
including the pages not mapped due to the allocation failure. On a
system that fails the allocation frequently, this problem leads to
following kernel crash.

The current zone revalidation code has a major problem in that it
doesn't update the zone size and q->nr_zones atomically, leading
to a short window where an out of bounds access to the zone arrays
is possible.
To fix this move the setting of the zone size into the crticial
sections blk_revalidate_disk_zones so that it gets updated together
with the zone bitmaps and q->nr_zones. This also slightly simplifies
the caller as it deducts the zone size from the report_zones.
This change also allows to check for a power of two zone size in generic
code.

bio based drivers only need to update q->nr_zones. Do that manually
instead of overloading blk_revalidate_disk_zones to keep that function
simpler for the next round of changes that will rely even more on the
request based functionality.

of it and move it all into drivers.
This series incorporates some of Al's work and many patches of my own,
but in the end stops short of actually removing the last part, which is
the scsi ioctl handlers. I have patches for those as well, but they need
more testing or possibly a rewrite.

For a read-only mapping, ask for a set of features that make the image
only unwritable rather than both unreadable and unwritable by a client
that doesn't understand them. As of today, the difference between them
for krbd is journaling (JOURNALING) and live migration (MIGRATING).
get_features method supports read_only parameter since hammer, ceph.git
commit 6176ec5fde2a ("librbd: differentiate between R/O vs R/W RBD
features").

Since infernalis, ceph.git commit 281f87f9ee52 ("cls_rbd: get_features
on snapshots returns HEAD image features"), querying and checking that
is pointless. Userspace support for manipulating image features after
image creation came also in infernalis, so a snapshot with a different
set of features wasn't ever possible.

RBD_DEV_FLAG_EXISTS check in rbd_queue_workfn() is racy and leads to
inconsistent behaviour. If the object (or its snapshot) isn't there,
the OSD returns ENOENT. A read submitted before the snapshot removal
notification is processed would be zero-filled and ended with status
OK, while future reads would be failed with IOERR. It also doesn't
handle a case when an image that is mapped read-only is removed.
On top of this, because watch is no longer established for read-only
mappings, we no longer get notifications, so rbd_exists_validate() is
effectively dead code. While failing requests rather than returning
zeros is a good thing, RBD_DEV_FLAG_EXISTS is not it.

A read-only mapping should be usable with read-only OSD caps, so
neither the header lock nor the object map lock can be acquired.
Unfortunately, this means that images mapped read-only lose the
advantage of the object map.
Snapshots, however, can take advantage of the object map without
any exclusionary locks, so if the object map is desired, snapshot
the image and map the snapshot instead of the image.

If an image is mapped read-only, don't allow setting its partition(s)
to read-write via BLKROSET: with the previous patch all writes to such
images are failed anyway.
If an image is mapped read-write, its partition(s) can be set to
read-only (and back to read-write) as before. Note that at the rbd
level the image will remain writeable: anything sent down by the block
layer will be executed, including any write from internal kernel users.

Even though -o ro/-o read_only/--read-only options are very old, we
have never really treated them seriously (on par with snapshots). As
a first step, fail writes to images mapped read-only just like we do
for snapshots.
We need this check in rbd because the block layer basically ignores
read-only setting, see commit a32e236eb9 ("Partially revert "block:
fail op_is_write() requests to read-only partitions"").

rbd_dev->opts is not available for parent images, making checking
rbd_dev->opts->read_only in various places (rbd_dev_image_probe(),
need_exclusive_lock(), use_object_map() in the following patches)
harder than it needs to be.
Keeping rbd_dev_image_probe() in mind, move the initialization in
do_rbd_add() up. snap_id isn't filled in at that point, so replace
rbd_is_snap() with a snap_name comparison.

Avoid the need to allocate a potentially large array of struct blk_zone
in the block layer by switching the ->report_zones method interface to
a callback model. Now the caller simply supplies a callback that is
executed on each reported zone, and private data for it.

Currently, if the loop device receives a WRITE_ZEROES request, it asks
the underlying filesystem to punch out the range. This behavior is
correct if unmapping is allowed. However, a NOUNMAP request means that
the caller doesn't want us to free the storage backing the range, so
punching out the range is incorrect behavior.
To satisfy a NOUNMAP | WRITE_ZEROES request, loop should ask the
underlying filesystem to FALLOC_FL_ZERO_RANGE, which is (according to
the fallocate documentation) required to ensure that the entire range is
backed by real storage, which suffices for our purposes.

nbd requires socket families to support the shutdown method so the nbd
recv workqueue can be woken up from its sock_recvmsg call. If the socket
does not support the callout we will leave recv works running or get hangs
later when the device or module is removed.
This adds a check during socket connection/reconnection to make sure the
socket being passed in supports the needed callout.

This was preceded by us timing out everything and shutting down the
sockets for the device. The problem is we had a request in the queue at
the same time, so we completed the request twice. This can actually
happen in a lot of cases, we fail to get a ref on our config, we only
have one connection and just error out the command, etc.
Fix this by checking cmd->status in nbd_read_stat. We only change this
under the cmd->lock, so we are safe to check this here and see if we've
already error'ed this command out, which would indicate that we've
completed it as well.

We already do this for the most part, except in timeout and clear_req.
For the timeout case we take the lock after we grab a ref on the config,
but that isn't really necessary because we're safe to touch the cmd at
this point, so just move the order around.
For the clear_req cause this is initiated by the user, so again is safe.

pkt_ioctl() implements the generic SCSI_IOCTL_SEND_COMMAND
and some cdrom ioctls by forwarding to the underlying block
device. For compat_ioctl handling, this always takes a
roundtrip through fs/compat_ioctl.c that we should try
to avoid, at least for the compatible commands.
CDROM_SEND_PACKET is an exception here, it requires special
translation in compat_blkdev_driver_ioctl().
CDROM_LAST_WRITTEN has no compat handling at the moment.

A zoned block device maintains a write pointer within a zone, and reads
beyond the write pointer are undefined. Fill data buffer returned above

There is a warning message in my test with below steps:
# rbd bench --io-type write --io-size 4K --io-threads 1 --io-pattern rand test &
# sleep 5
# pkill -9 rbd
# rbd map test &
# sleep 5
# pkill rbd
The reason is that the rbd_add_acquire_lock() is interruptable,
that means, when we kill the waiting on ->acquire_wait, the lock_dwork
could be still running.

Then when we reach the rbd_dev_free(), WARN_ON is triggered because
lock_state is not RBD_LOCK_STATE_UNLOCKED.
To fix it, this commit make sure the lock_dwork was finished before
calling rbd_dev_image_unlock().
On the other hand, this would not happend in do_rbd_remove(), because
after rbd mapped, lock_dwork will only be queued for IO request, and
request will continue unless lock_dwork finished. when we call
rbd_dev_image_unlock() in do_rbd_remove(), all requests are done.
That means, lock_state should not be locked again after
rbd_dev_image_unlock().

1. nbd_put takes the mutex and drops nbd->ref to 0. It then does
idr_remove and drops the mutex.
2. nbd_genl_connect takes the mutex. idr_find/idr_for_each fails
to find an existing device, so it does nbd_dev_add.
3. just before the nbd_put could call nbd_dev_remove or not finished
totally, but if nbd_dev_add try to add_disk, we can hit:
debugfs: Directory 'nbd1' with parent 'block' already present!
This patch will make sure all the disk add/remove stuff are done
by holding the nbd_index_mutex lock.

The return code from null_handle_zoned() sets the cmd->error value.
Returning OK status when an error occured overwrites the intended
cmd->error. Return the appropriate error code instead of setting the
error in the cmd.

The loop driver assumes that if the passed in fd is opened with
O_DIRECT, the caller wants to use direct I/O on the loop device.
However, if the underlying block device has a different block size than
the loop block queue, direct I/O can't be enabled. Instead of requiring
userspace to manually change the blocksize and re-enable direct I/O,
just change the queue block sizes to match, as well as the io_min size.

Add RB_DECLARE_CALLBACKS_MAX, which generates augmented rbtree callbacks
for the case where the augmented value is a scalar whose definition
follows a max(f(node)) pattern. This actually covers all present uses of
RB_DECLARE_CALLBACKS, and saves some (source) code duplication in the
various RBCOMPUTE function definitions.

The module load should fail only if there is something wrong with the
configuration or if an error prevents it to work properly. The module
should be able to be loaded with (nr_device == 0), since it will not
trigger errors or be in malfunction state. Preventing loading with zero
devices also breaks applications that configures this module using
configfs API. Remove the nr_device check to fix this.

A negative number of devices is nonsensical, so change the type to
unsigned. If the number of devices is 0, it is impossible for userspace
to interact with the module, so refuse loading the driver for that case.

Using the helper blk_queue_required_elevator_features(), set the
elevator feature ELEVATOR_F_ZBD_SEQ_WRITE as required for the request
queue of null_blk devices created with zoned mode enabled.
This feature requirement can always be satisfied as the mq-deadline
elevator is always selected for in-kernel compilation when
CONFIG_BLK_DEV_ZONED (zoned block device support) is enabled.

This patch creates a helper function for handling zoned block device
operations.
This patch also restructured the code for null_blk_zoned.c and uses the
pattern to return blk_status_t and catch the error in the function
null_handle_cmd() into cmd->error variable instead of setting it up in
the deeper layer just like the way it is done for flush, badblocks and
memory backed case in the null_handle_cmd(). We also move
null_handle_zoned() to the null_blk_zoned.c to keep the zoned code
separate.

This is a preparation patch which moves the duplicate code for sectors
and nr_sectors calculations for bio vs request mode into their
respective callers (null_queue_bio(), null_qeueue_req()). Now the core
function only deals with the respective actions and commands instead of
having to calculte the bio vs req operations and different sector
related variables. We also move the flush command handling at the top
which significantly simplifies the rest of the code.

nbd: add multi-connection support
that limited the number of devices to 256. Before the patch we could
create 1000s of devices, but the patch switched us from using our
own thread to using a work queue which has a default limit of 256
active works.
The problem is that our recv_work function sits in a loop until
disconnection but only handles IO for one connection. The work is
started when the connection is started/restarted, but if we end up
creating 257 or more connections, the queue_work call just queues
connection257+'s recv_work and that waits for connection 1 - 256's
recv_work to be disconnected and that work instance completing.
Instead of reverting back to kthreads, this has us allocate a
workqueue_struct per device, so we can block in the work.

nbd: allow block mq to deal with timeouts
where before the patch userspace would set the timeout to 0 to disable
it. With the above patch, a zero timeout tells the block layer to use
the default value of 30 seconds. For setups where commands can take a
long time or experience transient issues like network disruptions this
then results in IO errors being sent to the application.
To fix this, the patch still uses the common block layer timeout
framework, but if zero is set, nbd just logs a message and then resets
the timer when it expires.

Pull kernel thread signal handling fix from Eric Biederman:
"I overlooked the fact that kernel threads are created with all signals
set to SIG_IGN, and accidentally caused a regression in cifs and drbd
when replacing force_sig with send_sig.
This is my fix for that regression. I add a new function
allow_kernel_signal which allows kernel threads to receive signals
sent from the kernel, but continues to ignore all signals sent from
userspace. This ensures the user space interface for cifs and drbd
remain the same.
These kernel threads depend on blocking networking calls which block
until something is received or a signal is pending. Making receiving
of signals somewhat necessary for these kernel threads.
Perhaps someday we can cleanup those interfaces and remove
allow_kernel_signal. If not allow_kernel_signal is pretty trivial and
clearly documents what is going on so I don't think we will mind
carrying it"

My recent to change to only use force_sig for a synchronous events
wound up breaking signal reception cifs and drbd. I had overlooked
the fact that by default kthreads start out with all signals set to
SIG_IGN. So a change I thought was safe turned out to have made it
impossible for those kernel thread to catch their signals.
Reverting the work on force_sig is a bad idea because what the code
was doing was very much a misuse of force_sig. As the way force_sig
ultimately allowed the signal to happen was to change the signal
handler to SIG_DFL. Which after the first signal will allow userspace
to send signals to these kernel threads. At least for
wake_ack_receiver in drbd that does not appear actively wrong.
So correct this problem by adding allow_kernel_signal that will allow
signals whose siginfo reports they were sent by the kernel through,
but will not allow userspace generated signals, and update cifs and
drbd to call allow_kernel_signal in an appropriate place so that their
thread can receive this signal.
Fixing things this way ensures that userspace won't be able to send
signals and cause problems, that it is clear which signals the
threads are expecting to receive, and it guarantees that nothing
else in the system will be affected.
This change was partly inspired by similar cifs and drbd patches that
added allow_signal.

In read_per_ring_refs(), after 'req' and related memory regions are
allocated, xen_blkif_map() is invoked to map the shared frame, irq, and
etc. However, if this mapping process fails, no cleanup is performed,
leading to memory leaks. To fix this issue, invoke the cleanup before
returning the error.

Enabling Direct I/O with loop devices helps reducing memory usage by
avoiding double caching. 32 bit applications running on 64 bits systems
are currently not able to request direct I/O because is missing from the
lo_compat_ioctl.
This patch fixes the compatibility issue mentioned above by exporting
LOOP_SET_DIRECT_IO as additional lo_compat_ioctl() entry.
The input argument for this ioctl is a single long converted to a 1-bit
boolean, so compatibility is preserved.

A deadlock with this stacktrace was observed.
The loop thread does a GFP_KERNEL allocation, it calls into dm-bufio
shrinker and the shrinker depends on I/O completion in the dm-bufio
subsystem.
In order to fix the deadlock (and other similar ones), we set the flag
PF_MEMALLOC_NOIO at loop thread entry.

Commit 33ec3e53e7 ("loop: Don't change loop device under exclusive
opener") made LOOP_SET_FD ioctl acquire exclusive block device reference
while it updates loop device binding. However this can make perfectly
valid mount(2) fail with EBUSY due to racing LOOP_SET_FD holding
temporarily the exclusive bdev reference in cases like this:

Fix the problem by not getting full exclusive bdev reference in
LOOP_SET_FD but instead just mark the bdev as being claimed while we
update the binding information. This just blocks new exclusive openers
instead of failing them with EBUSY thus fixing the problem.

Building with clang and KASAN, we get a warning about an overly large
stack frame on 32-bit architectures:

We already allocate other data dynamically in this function, so

Merge floppy ioctl verification fixes from Denis Efremov.
This also marks the floppy driver as orphaned - it turns out that Jiri
no longer has working hardware.
Actual working physical floppy hardware is getting hard to find, and
while Willy was able to test this, I think the driver can be considered
pretty much dead from an actual hardware standpoint. The hardware that
is still sold seems to be mainly USB-based, which doesn't use this
legacy driver at all.
The old floppy disk controller is still emulated in various VM
environments, so the driver isn't going away, but let's see if anybody
is interested to step up to maintain it.
The lack of hardware also likely means that the ioctl range verification
fixes are probably mostly relevant to anybody using floppies in a
virtual environment. Which is probably also going away in favor of USB
storage emulation, but who knows.

function of the floppy driver.
The FDDEFPRM ioctl allows one to set the geometry of a disk. The sect
and head fields (unsigned int) of the floppy_drive structure are used to
compute the max_sector (int) in the make_raw_rw_request function. It is
possible to overflow the max_sector. Next, max_sector is passed to the
copy_buffer function and used in one of the memcpy calls.
An unprivileged user could trigger the bug if the device is accessible,
but requires a floppy disk to be inserted.
The patch adds the check for the .sect * .head multiplication for not
overflowing in the set_geometry function.
The bug was found by syzkaller.

be used to call the drive_name. A floppy disk is not required to be
inserted.
CAP_SYS_ADMIN is required to call FDSETDRVPRM.
The patch adds the check for a value of the native_format field to be in
the '0 <= x < ARRAY_SIZE(floppy_type)' range of the floppy_type array
indices.

This fixes a divide by zero error in the setup_format_params function of
the floppy driver.
Two consecutive ioctls can trigger the bug: The first one should set the
drive geometry with such .sect and .rate values for the F_SECT_PER_TRACK
to become zero. Next, the floppy format operation should be called.
A floppy disk is not required to be inserted. An unprivileged user
could trigger the bug if the device is accessible.
The patch checks F_SECT_PER_TRACK for a non-zero value in the
set_geometry function. The proper check should involve a reasonable
upper limit for the .sect and .rate fields, but it could change the
UAPI.
The patch also checks F_SECT_PER_TRACK in the setup_format_params, and
cancels the formatting operation in case of zero.
The bug was found by syzkaller.

Rename the blockdev documentation files to ReST, add an
index for them and adjust in order to produce a nice html
output via the Sphinx build system.

Add those too to the documentation.
At its new index.rst, let's add a :orphan: while this is not linked to

Only GFP_KERNEL and GFP_NOIO are used with blkdev_report_zones(). In
preparation of using vmalloc() for large report buffer and zone array
allocations used by this function, remove its "gfp_t gfp_mask" argument
and rely on the caller context to use memalloc_noio_save/restore() where
necessary (block layer zone revalidation and dm-zoned I/O error path).

If the device is setup with ioctl we can resize the device after the
initial setup, but if the device is setup with netlink we cannot use the
resize related ioctls and there is no netlink reconfigure size ATTR
handling code.
This patch adds netlink reconfigure resize support to match the ioctl
interface.

Pull force_sig() argument change from Eric Biederman:
"A source of error over the years has been that force_sig has taken a
task parameter when it is only safe to use force_sig with the current
task.
The force_sig function is built for delivering synchronous signals
such as SIGSEGV where the userspace application caused a synchronous
fault (such as a page fault) and the kernel responded with a signal.
Because the name force_sig does not make this clear, and because the
force_sig takes a task parameter the function force_sig has been
abused for sending other kinds of signals over the years. Slowly those
have been fixed when the oopses have been tracked down.
This set of changes fixes the remaining abusers of force_sig and
carefully rips out the task parameter from force_sig and friends
making this kind of error almost impossible in the future"

Speed up reads, discards and zeroouts through RBD_OBJ_FLAG_MAY_EXIST
and RBD_OBJ_FLAG_NOOP_FOR_NONEXISTENT based on object map.
Invalid object maps are not trusted, but still updated. Note that we
never iterate, resize or invalidate object maps. If object-map feature
is enabled but object map fails to load, we just fail the requester
(either "rbd map" or I/O, by way of post-acquire action).

Snapshot object map will be loaded in rbd_dev_image_probe(), so we need
to know snapshot's size (as opposed to HEAD's size) sooner.
