Armen Baloyan 63ea923a97 [SCSI] qla2xxx: Properly set result field of bsg_job reply structure for success and failure.
FC transport on receiving bsg_job submission failure, calls bsg_job->job_done()
and sets the bsg_job->reply->result the returned value. In contrast, when the
success code (0) is returned fc transport doesn't call bsg_job->job_done() and
doesn't populate bsg_job->reply->result.

Aaron Lu 691e3d3175 [SCSI] sd: update sd to use the new pm callbacks
Update sd driver to use the callbacks defined in dev_pm_ops.

sd_freeze is NULL, the bus level callback has taken care of quiescing
the device so there should be nothing needs to be done here.
Consequently, sd_thaw is not needed here either.

suspend, poweroff and runtime suspend share the same routine sd_suspend,
which will sync flush and then stop the drive, this is the same as before.

resume, restore and runtime resume share the same routine sd_resume,
which will start the drive by putting it into active power state, this
is also the same as before.

Aaron Lu 80d2fd48cc [SCSI] scsi_pm: use callbacks from dev_pm_ops for scsi devices
Use of pm_message_t is deprecated and device driver is not supposed
to use that. This patch migrates the SCSI bus level pm callbacks
to call device's pm callbacks defined in its driver's dev_pm_ops.

This is achieved by finding out which device pm callback should be used
in bus callback function, and then pass that callback function pointer
as a param to the scsi_bus_{suspend,resume}_common routine, which will
further pass that callback to scsi_dev_type_{suspend,resume} after
proper handling.

The special case for freeze in scsi_bus_suspend_common is not necessary
since there is no high level SCSI driver has implemented freeze, so no
need to runtime resume the device if it is in runtime suspended state
for system freeze, just return like the system suspend/hibernate case.

Since only sd has implemented drv->suspend/drv->resume, and I'll update
sd driver to use the new callbacks in the following patch, there is no
need to fallback to call drv->suspend/drv->resume if dev_pm_ops is NULL.

Aaron Lu a01475637c [SCSI] sd: put to stopped power state when runtime suspend
When device is runtime suspended, put it to stopped power state to save
some power.

This will also make the behaviour consistent with what the scsi_pm.c
thinks about sd as the comment says:
sd treats runtime suspend, system suspend and system hibernate identical.
With this patch, it is now identical.
And sd_shutdown will also do nothing when it finds the device has been
runtime suspended, if we do not spin down the disk in runtime suspend
by putting it into stopped power state, the disk will be shut down
incorrectly.
And the the same problem can be solved for runtime power off after
runtime suspended case by this change.

With the current runtime scheme for disk, it will only be runtime
suspended when no process opens the disk, so this shouldn't happen a
lot, which makes it acceptable to spin down the disk when runtime
suspended. If some day a more aggressive runtime scheme is used, like
the 'request based runtime pm for disk' that Alan Stern and Lin Ming
has been working, we can introduce some policy to control this. But for
now, make it simple and correct by spinning down the disk.

Xi Wang beecadea1b [SCSI] mvsas: fix undefined bit shift
The macro bit(n) is defined as ((u32)1 << n), and thus it doesn't work
with n >= 32, such as in mvs_94xx_assign_reg_set():

	if (i >= 32) {
		mvi->sata_reg_set |= bit(i);
		...
	}

The shift ((u32)1 << n) with n >= 32 also leads to undefined behavior.
The result varies depending on the architecture.

This patch changes bit(n) to do a 64-bit shift.  It also simplifies
mv_ffc64() using __ffs64(), since invoking ffz() with ~0 is undefined.

Steve Hodgson 06e97b4890 qla2xxx: Look up LUN for abort requests
Search through the list of pending commands on the session list to find
the command the initiator is actually aborting, so that we can pass the
correct LUN to the core TMR handling code.

Force large capacity (> 0xFFFFFFFF blocks) drives to use READ/WRITE(16) instead
of READ/WRITE(10). Some(most/all?) USB enclosures do not like READ(10) commands

Eric Northup b56d100351 [SCSI] virtio_scsi: fix memory leak on full queue condition.
virtscsi_queuecommand was leaking memory when the virtio queue was full.

Tested: Guest operates correctly even with very small queue sizes, validated
we're not leaking kmalloc-192 sized allocations anymore.

Fixed boot from san failure when SLI4 FC device presented on the same PCI bus

The request_firmware interface can induce delays while looking
for firmware files, even if no fw file is present. In some situations
the delays exceeded scan_wait timeouts, resulting in situations in which
the boot device had not been discovered in time.  Boot Device does not
need to be on a lpfc device.

Change request_firmware use to be module paramater driven. Default is to
not attempt firmware download on boot. Add sysfs parameter to invoke
firmware update.

type can easily be read from the corresponding cnic->flags.  This will

Kernel used to panic while running IO is disable mode, as there
was an issue with getting the correct EQ on which completion has come.
Fix done is create workqueue per hba and work item for each EQ created.

Boaz Harrosh 51976a8c85 [SCSI] osd_uld: Add osdname & systemid sysfs at scsi_osd class
This patch adds the support for the following two read-only sysfs attributes
to scsi_osd class members : osdname & systemid

These attributes will show up as below in sysfs class hierarchy:
/sys/class/scsi_osd/osdX/osdname
/sys/class/scsi_osd/osdX/systemid

The osdname & systemid are OSD device attributes which uniquely
identify a device on the network, while it's IP and certainly
it's /dev/osdX device path might change.
Userspace utilities (e.g. mkfs.exofs) can parse these attributes to
identify the correct OSD in safer and faster way.

(Today osd apps open each device in the system and send a
 attributes query for these, in order to access the user
 requested device)

Joel D. Diaz afd5e34b2b [SCSI] sd: Reshuffle init_sd to avoid crash
scsi_register_driver will register a prep_fn() function, which
in turn migh need to use the sd_cdp_pool for DIF.
Which hasn't been initialised at this point, leading to
a crash. So reshuffle the init_sd() and exit_sd() paths
to have the driver registered last.

Martin K. Petersen 5db44863b6 [SCSI] sd: Implement support for WRITE SAME
Implement support for WRITE SAME(10) and WRITE SAME(16) in the SCSI disk
driver.

 - We set the default maximum to 0xFFFF because there are several
   devices out there that only support two-byte block counts even with
   WRITE SAME(16). We only enable transfers bigger than 0xFFFF if the
   device explicitly reports MAXIMUM WRITE SAME LENGTH in the BLOCK
   LIMITS VPD.

 - max_write_same_blocks can be overriden per-device basis in sysfs.

 - The UNMAP discovery heuristics remain unchanged but the discard
   limits are tweaked to match the "real" WRITE SAME commands.

 - In the error handling logic we now distinguish between WRITE SAME
   with and without UNMAP set.

The discovery process heuristics are:

 - If the device reports a SCSI level of SPC-3 or greater we'll issue
   READ SUPPORTED OPERATION CODES to find out whether WRITE SAME(16) is
   supported. If that's the case we will use it.

 - If the device supports the block limits VPD and reports a MAXIMUM
   WRITE SAME LENGTH bigger than 0xFFFF we will use WRITE SAME(16).

 - Otherwise we will use WRITE SAME(10) unless the target LBA is beyond
   0xFFFFFFFF or the block count exceeds 0xFFFF.

 - no_write_same is set for ATA, FireWire and USB.

Martin K. Petersen 3c6bdaeab4 [SCSI] Add a report opcode helper
The REPORT SUPPORTED OPERATION CODES command can be used to query
whether a given opcode is supported by a device. Add a helper function
that allows us to look up commands.

We only issue RSOC if the device reports compliance with SPC-3 or
later. But to err on the side of caution we disable the command for ATA,
FireWire and USB.

Maciej Patelczyk 49bd665c54 [SCSI] isci: copy fis 0x34 response into proper buffer
SATA MICROCODE DOWNALOAD fails on isci driver. After receiving Register
Device to Host (FIS 0x34) frame Initiator resets phy.
In the frame handler routine response (FIS 0x34) was copied into wrong
buffer and upper layer did not receive any answer which resulted in
timeout and reset.
This patch corrects this bug.

Christoph Hellwig de103c93af target: pass sense_reason as a return value
Pass the sense reason as an explicit return value from the I/O submission
path instead of storing it in struct se_cmd and using negative return
values.  This cleans up a lot of the code pathes, and with the sparse
annotations for the new sense_reason_t type allows for much better

Roland Dreier c8292d1da5 qla2xxx: Update target lookup session tables when a target session changes
It is possible for the target code to change the loop_id or s_id of a
target session in reaction to an FC fabric change.  However, the
session structures are stored in tables that are indexed by these two
keys, and if we just change the session structure but leave the
pointers to it in the old places in the table, havoc can ensue.  For
example, a new session might come along that should go in the old slot
in the table and overwrite the old session pointer.

To handle this, add a new tgt_ops->update_sess() method that also
updates the "by loop_id" and "by s_id" lookup tables when a session
changes, so that the keys where a session pointer is stored in these
tables always matches the keys in the session structure itself.

My draft of SPC-4 says the following about the SCSI name string in
inquiry VPD page 83h:

    The SCSI NAME STRING field starts with either:

    a) the four UTF-8 characters 'eui.' concatenated with 16, 24, or
       32 hexadecimal digits (i.e., the UTF-8 characters 0 through 9
       and A through F) for an EUI-64 based identifier (see
       7.8.6.5). The first hexadecimal digit shall be the most
       significant four bits of the first byte (i.e., most significant
       byte) of the EUI-64 based identifier;
    b) the four UTF-8 characters 'naa.' concatenated with 16 or 32
       hexadecimal digits for an NAA identifier (see 7.8.6.6). The
       first hexadecimal digit shall be the most significant four bits
       of the first byte (i.e., most significant byte) of the NAA
       identifier; or
    c) the four UTF-8 characters 'iqn.' concatenated with an iSCSI
       Name for an iSCSI-name based identifier (see iSCSI).

However, the .tpg_get_wwn method for tcm_qla2xxx formats the WWN so
the SCSI name string looks like "52:4a:93:7d:24:5f:b2:12,t,0x0001".
This patch corrects the code so that VPD 83h gives a SPC-compliant
SCSI name string like "naa.524a937d245fb212,t,0x0001" while leavig
other uses alone (so configfs will still work with ':' separated WWNs).

Properly account for I/O in transit before returning from the RESET call.
In the absense of this patch, we could have a situation where the host may
respond to a command that was issued prior to the issuance of the RESET
command at some arbitrary time after responding to the RESET command.
Currently, the host does not do anything with the RESET command.

Lukas Czerner bc977749e9 [SCSI] scsi_debug: Fix off-by-one bug when unmapping region
Currently it is possible to unmap one more block than user requested to
due to the off-by-one error in unmap_region(). This is probably due to
the fact that the end variable despite its name actually points to the
last block to unmap + 1. However in the condition it is handled as the
last block of the region to unmap.

The bug was not previously spotted probably due to the fact that the
region was not zeroed, which has changed with commit
be1dd78de5. With that commit we were able
to corrupt the ext4 file system on 256M scsi_debug device with LBPRZ
enabled using fstrim.

Since the 'end' semantic is the same in several functions there this
commit just fixes the condition to use the 'end' variable correctly in
that context.

Li Zhong 329a402cb0 [SCSI] Shorten the path length of scsi_cmd_to_driver()
This patch tries to shorten the path length of scsi_cmd_to_driver(). As only
REQ_TYPE_BLOCK_PC commands can be submitted without a driver, so we could
avoid the related NULL checking, as long as we make sure we don't use it for
REQ_TYPE_BLOCK_PC type commands. Plus, this fixes a bug where you get
different behaviors from REQ_TYPE_BLOCK_PC commands when a driver is and isn't
attached.

virtio-scsi needs to report LUNs greater than 256 using the "flat"
format.  Because the Linux SCSI layer just maps the SCSI LUN to
an u32, without any parsing, these end up in the range from 16640
to 32767.  Fix max_lun to account for the possibility that logical
unit numbers are encoded with the "flat" format.

Yi Zou 3b64b18811 [SCSI] libfc: fix lun reset failure bugs in fc_fcp_resp handling of FCP_RSP_INFO
In LUN RESET testing involving NetApp targets, it is observed that LUN
RESET is failing. The fc_fcp_resp() is not completing the completion
for the LUN RESET task since fc_fcp_resp assumes that the FCP_RSP_INFO
is 8 bytes with the 4 byte reserved field, where in case of NetApp targets
the FCP_RSP to LUN RESET only has 4 bytes of FCP_RSP_INFO. This leads
fc_fcp_resp to error out w/o completing the task completion, eventually
causing LUN RESET to be escalated to host reset, which is not very nice.

Per FCP-3 r04, clause 9.5.15 and Table 23, the FCP_RSP_INFO field can be either
4 bytes or 8 bytes, with the last 4 bytes as "Reserved (if any)". Therefore it
is valid to have 4 bytes FCP_RSP_INFO like some of the NetApp targets behave.
Fixing this by validating the FCP_RSP_INFO against both the two spec allowed
length.

Neerav Parikh 31c37a6f21 [SCSI] fcoe: Fix write errors on NPIV ports
SCSI errors were generated while writing to LUNs
connected via NPIV ports.

Debugging this it was found that the FCoE packets
transmitted via the NPIV ports were not tagged with
correct user priority as negotiated with peer by DCB
agent. This resulted in FCoE traffic going with priority
zero(0) that did not have priority flow control (PFC)
enabled for it. The initiator after transferring data
to the target never saw any reply indicating the transfer
was complete. This resulted in error recovery (ABTS) and
SCSI command retries by the scsi-mid layer; eventually
resulting in I/O errors.

This patch fixes this issue by keeping the FCoE user
priority information in the fcoe_interface instance
that is common for both the physical port as well as
NPIV ports connected to that physical port; instead
of storing it in fcoe_port structure that has a per
port instance.

Martin Michlmayr 0f6d93aa9d drivers/scsi/atp870u.c: fix bad use of udelay
The ACARD driver calls udelay() with a value > 2000, which leads to to
the following compilation error on ARM:

  ERROR: "__bad_udelay" [drivers/scsi/atp870u.ko] undefined!
  make[1]: *** [__modpost] Error 1

This is because udelay is defined on ARM, roughly speaking, as

	#define udelay(n) ((n) > 2000 ? __bad_udelay() : \
		__const_udelay((n) * ((2199023U*HZ)>>11)))

The argument to __const_udelay is the number of jiffies to wait divided
by 4, but this does not work unless the multiplication does not
overflow, and that is what the build error is designed to prevent.  The
intended behavior can be achieved by using mdelay to call udelay
multiple times in a loop.

  round including considerable API and behavior cleanups.

   * delayed_work combines a timer and a work item.  The handling of the
     timer part has always been a bit clunky leading to confusing
     cancelation API with weird corner-case behaviors.  delayed_work is
     updated to use new IRQ safe timer and cancelation now works as
     expected.

   * Another deficiency of delayed_work was lack of the counterpart of
     mod_timer() which led to cancel+queue combinations or open-coded
     timer+work usages.  mod_delayed_work[_on]() are added.

     These two delayed_work changes make delayed_work provide interface
     and behave like timer which is executed with process context.

   * A work item could be executed concurrently on multiple CPUs, which
     is rather unintuitive and made flush_work() behavior confusing and
     half-broken under certain circumstances.  This problem doesn't
     exist for non-reentrant workqueues.  While non-reentrancy check
     isn't free, the overhead is incurred only when a work item bounces
     across different CPUs and even in simulated pathological scenario
     the overhead isn't too high.

     All workqueues are made non-reentrant.  This removes the
     distinction between flush_[delayed_]work() and
     flush_[delayed_]_work_sync().  The former is now as strong as the
     latter and the specified work item is guaranteed to have finished
     execution of any previous queueing on return.

   * In addition to the various bug fixes, Lai redid and simplified CPU
     hotplug handling significantly.

   * Joonsoo introduced system_highpri_wq and used it during CPU
     hotplug.

  There are two merge commits - one to pull in IRQ safe timer from
  tip/timers/core and the other to pull in CPU hotplug fixes from
  wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."

Fixed a number of trivial conflicts, but the more interesting conflicts
were silent ones where the deprecated interfaces had been used by new
code in the merge window, and thus didn't cause any real data conflicts.

Martin K. Petersen fe542396da [SCSI] sd: Ensure we correctly disable devices with unknown protection type
We set the capacity to zero when we discovered a device formatted with
an unknown DIF protection type. However, the read_capacity code would
override the capacity and cause the device to be enabled regardless.

Make sd_read_protection_type() return an error if the protection type is
unknown. Also prevent duplicate printk lines when the device is being
revalidated.

Don't restart lookup of more stargets in the multi-target case, just

Benjamin Herrenschmidt 9965c2f19b [SCSI] ibmvscsi: Remove backend abstraction
Now that the iSeries code is gone the backend abstraction
in this driver is no longer necessary, which allows us to
consolidate the driver in one file.

The side effect is that the module name is now ibmvscsi.ko
which matches the driver hotplug name and fixes auto-load
issues.

Martin Pitt d986788b26 [SCSI] scsi_debug: Add "removable" parameter
Add "removable" module parameter to set the "removable" attribute of any
subsequently created debug block device. It is a writable driver option, so
that you can switch between removable and "fixed" media block devices in
between the add_host calls.

This is useful for being able to test the different behaviour/required
privileges in e. g. the udisks test suite.

while processing the stop firmware mailbox command on driver unload,
a interrupt is processed which causes kernel panic as the response
queue is not valid.

Dave Jiang 44ef2bf72d [SCSI] isci: Fix interrupt coalescing assumption of active TCs
We always assign a dummy task context to a port in order to address a
silicon issue. We have 4 ports per controller. So when idle, there are always
exactly 4 TCs "active". The adaptive interrupt coalescing code uses number of
active TCs to figure out the coalescing values. However, we never hit "0" TCs
because of the 4 dummy TCs. Putting in fix so that we calculate this correctly.

Jeff Skirvin 54b4667775 [SCSI] isci: Allow SSP tasks into the task management path.
This commit fixes a driver bug for SSP tasks that require task management
in the target after they complete in the SCU hardware.  The problem was
manifested in the function "isci_task_abort_task", which tests
to see if the sas_task.lldd_task is non-NULL before allowing task
management; this bug would always NULL lldd_task in the SCU I/O completion
path even if target management was required, which would prevent
task / target manangement from happening.

Note that in the case of SATA/STP targets, error recovery is provided by
the libata error handler which is why SATA/STP device recovery worked
correctly even though SSP handling did not.

Brian King 5cdf162654 [SCSI] ibmvfc: Ignore fabric RSCNs when link is dead
This fixes an issues seen where a Fabric RSCN event was received
while the link was down, which resulted in repeated attempts to log back
into the fabric, which then failed, resulting in the ibmvfc driver
taking the host offline. Fix this by delaying taking any action
regarding the fabric RSCN until the link comes back up.

Brian King a077c7faf3 [SCSI] ibmvfc: Fix double completion on abort timeout
If an abort request times out to the virtual fibre channel adapter,
the ibmvfc driver will kick off a reset of the adapter. This
patch ensures we wait for the both the abort request and the
request being aborted to be completed prior to exiting the
eh_abort handler. This fixes a bug where the ibmvfc driver
was erroneously returning success to the eh_abort handler
then later sending back a response to the same command.

- Changes to avoid discovering NPIV port as remote port by the other

  way in which the initiator and target ports are zoned in the fabric.

Stale information in the temporary fcport created in
qla2x00_configure_local_loop() causes qla2x00_get_port_database() call
to fail. This reschedules scan, which gets stuck continuously in the
rescheduling-of-scan loop due to the failure.

Roland Dreier e4b11b89f9 qla2xxx: Fix endianness of task management response code
The qla2xxx firmware actually expects the task management response
code in a CTIO IOCB with SCSI status mode 1 to be in little-endian
byte order, ie the response code should be the first byte in the
sense_data[] array.  The old code erroneously byte-swapped the
response code, which puts it in the wrong place on the wire and leads
to initiators thinking every task management request succeeds (since
they see 0 in the byte where they look for the response code).

Stephen M. Cameron 256d0eaac8 [SCSI] hpsa: fix handling of protocol error
If a command status of CMD_PROTOCOL_ERR is received, this
information should be conveyed to the SCSI mid layer, not
dropped on the floor.  CMD_PROTOCOL_ERR may be received
from the Smart Array for any commands destined for an external
RAID controller such as a P2000, or commands destined for tape
drives or CD/DVD-ROM drives, if for instance a cable is
disconnected.  This mostly affects multipath configurations, as
disconnecting a cable on a non-multipath configuration is not
going to do anything good regardless of whether CMD_PROTOCOL_ERR
is handled correctly or not.  Not handling CMD_PROTOCOL_ERR
correctly in a multipath configaration involving external RAID
controllers may cause data corruption, so this is quite a serious
bug.  This bug should not normally cause a problem for direct
attached disk storage.

Roland Dreier 9c58b7ddd7 target: Simplify fabric sense data length handling
Every fabric driver has to supply a se_tfo->set_fabric_sense_len()
method, just so iSCSI can return an offset of 2.  However, every fabric
driver is already allocating a sense buffer and passing it into the
target core, either via transport_init_se_cmd() or target_submit_cmd().

So instead of having iSCSI pass the start of its sense buffer into the
core and then later tell the core to skip the first 2 bytes, it seems
easier for iSCSI just to do the offset of 2 when it passes the sense
buffer into the core.  Then we can drop the se_tfo->set_fabric_sense_len()
everywhere, and just add a couple of lines of code to iSCSI to set the
sense data length to the beginning of the buffer right before it sends
it over the network.

(nab: Remove .set_fabric_sense_len usage from tcm_qla2xxx_npiv_ops +
      change transport_get_sense_buffer to follow v3.6-rc6 code w/o
      ->set_fabric_sense_len usage)

This patch fixes the following kernel panic invoked by uninitialized fields
in the chip initialization for the 1G bnx2 iSCSI offload.

One of the bits in the chip initialization is being used by the latest
firmware to control overflow packets.  When this control bit gets enabled
erroneously, it would ultimately result in a bad packet placement which would
cause the bnx2 driver to dereference a NULL ptr in the placement handler.

This can happen under certain stress I/O environment under the Linux
iSCSI offload operation.

This change only affects Broadcom's 5709 chipset.

Jeff Mahoney 6ed33a4a0b [SCSI] st: raise device limit
The device limit of 128 tape drives was established in 2003 as a
significant increase from the 8 tape drives allowed previously.

We're seeing customer sites that between a large number of drives
and multipath are discovering more than 128 devices and running
into problems.

Now that we're not stuck having to store a pointer in array
and aren't limited by kmalloc failing on higher order allocs we can
lift the limit to fill the entire minor range based on the number
of modes.

Based on the current code, that's 2^17 devices.

Jeff Mahoney 6c648d95a6 [SCSI] st: get rid of scsi_tapes array
st currently allocates an array to store pointers to all of the
scsi_tape objects. It's used to discover available indexes to use as the
base for the minor number selection and later to look up scsi_tape
devices for character devices.

We switch to using an IDR for minor selection and a pointer from
st_modedef back to scsi_tape for the lookups.

John Soni Jose df5d0e6ee9 [SCSI] be2iscsi: Format the MAC_ADDR with sysfs_format_mac.
The MAC_ADDR stored in driver private structure is of
unsigned char data type but strlcpy parameters is of
signed char data type. This conversion of data types
lead to change in the value.This changed value is passed
to the upper layer and junk characters were displayed
when "iscsiadm -m iface" command was run.

In case of iSCSI boot, since the the MAC_ADDR was coming
junk the boot was also not working

John Soni Jose 99bc5d55c0 [SCSI] be2iscsi: Added Logging mechanism for the driver.
Added new log level mechanism for different events. These
log levels can be set at driver load time/run time. The
log level is set for each Scsi_host.

Fixed few multi-line print warning to get over the new checkpatch.pl
warnings on multi-line strings.

John Soni Jose 9aef4200ee [SCSI] be2iscsi: Issue MBX Cmd for login to boot target in crashdump mode
When the driver comes up in crashdump mode, it has to explicitly
issue command to FW for logging to the boot target. This fix issues
MBX Cmd to login to boot target in crashdump mode.

John Soni Jose afd96fa45b [SCSI] be2iscsi: Removing the iscsi_data_pdu setting.
The setting of iscsi_data_pdu is not required anymore,
as this was required for BE1 adapters only. The BE1 adapter
were not supported in any previous versions of the kernel.
