Patch series "Fix CONT-PTE/PMD size hugetlb issue when unmapping or migrating", v4.
presently, migrating a hugetlb page or unmapping a poisoned hugetlb page,
we'll use ptep_clear_flush() and set_pte_at() to nuke the page table entry
and remap it, and this is incorrect for CONT-PTE or CONT-PMD size hugetlb
page, which will cause potential data consistent issue. This patch set
will change to use hugetlb related APIs to fix this issue.
Note: Mike pointed out the huge_ptep_get() will only return the one
specific value, and it would not take into account the dirty or young bits
of CONT-PTE/PMDs like the huge_ptep_get_and_clear() [1]. This
inconsistent issue is not introduced by this patch set, and this issue
will be addressed in another thread [2]. Meanwhile the uffd for hugetlb
case [3] pointed out by Gerald also needs another patch to address.

It is incorrect to use ptep_clear_flush() to nuke a hugetlb page table
when unmapping or migrating a hugetlb page, and will change to use
huge_ptep_clear_flush() instead in the following patches.
So this is a preparation patch, which changes the huge_ptep_clear_flush()
to return the original pte to help to nuke a hugetlb page table.

Itanium defines a get_cycles() function, but it does not do the usual
`#define get_cycles get_cycles` dance, making it impossible for generic
code to see if an arch-specific function was defined. While the
get_cycles() ifdef is not currently used, the following timekeeping
patch in this series will depend on the macro existing (or not existing)
when defining random_get_entropy().

The last remaining implementation of arch_ptrace_attach is ia64's
ptrace_attach_sync_user_rbs which was added at the end of 2007 in
commit aa91a2e900 ("[IA64] Synchronize RBS on PTRACE_ATTACH").
Reading the comments and examining the code ptrace_attach_sync_user_rbs
has the sole purpose of saving registers to the stack when ptrace_attach
changes TASK_STOPPED to TASK_TRACED. In all other cases arch_ptrace_stop
takes care of the register saving.
In commit d79fdd6d96 ("ptrace: Clean transitions between TASK_STOPPED and TRACED")
modified ptrace_attach to wake up the thread and enter ptrace_stop normally even
when the thread starts out stopped.
This makes ptrace_attach_sync_user_rbs completely unnecessary. So just
remove it.
I read through the code to verify that ptrace_attach_sync_user_rbs is
unnecessary. What I found is that the code is quite dead.
Reading ptrace_attach_sync_user_rbs it is easy to see that the it does
nothing unless __state == TASK_STOPPED.
Calling arch_ptrace_attach (aka ptrace_attach_sync_user_rbs) after
ptrace_traceme it is easy to see that because we are talking about the
current process the value of __state is TASK_RUNNING. Which means
ptrace_attach_sync_user_rbs does nothing.
The only other call of arch_ptrace_attach (aka
ptrace_attach_sync_user_rbs) is after ptrace_attach.
If the task is running (and PTRACE_SEIZE is not specified), a SIGSTOP
is sent which results in do_signal_stop setting JOBCTL_TRAP_STOP on
the target task (as it is ptraced) and the target task stopping
in ptrace_stop with __state == TASK_TRACED.
If the task was already stopped then ptrace_attach sets
JOBCTL_TRAPPING and JOBCTL_TRAP_STOP, wakes it out of __TASK_STOPPED,
and waits until the JOBCTL_TRAPPING_BIT is clear. At which point
the task stops in ptrace_stop.
In both cases there are a couple of funning excpetions such as if the
traced task receiveds a SIGCONT, or is set a fatal signal.
However in all of those cases the tracee never stops in __state
TASK_STOPPED. Which is a long way of saying that ptrace_attach_sync_user_rbs
is guaranteed never to do anything.

Many architectures have similar install.sh scripts.
The first half is really generic; it verifies that the kernel image
and System.map exist, then executes ~/bin/${INSTALLKERNEL} or
/sbin/${INSTALLKERNEL} if available.
The second half is kind of arch-specific; it copies the kernel image
and System.map to the destination, but the code is slightly different.
Factor out the generic part into scripts/install.sh.

Add fn and fn_arg members into struct kernel_clone_args and test for
them in copy_thread (instead of testing for PF_KTHREAD | PF_IO_WORKER).
This allows any task that wants to be a user space task that only runs
in kernel mode to use this functionality.
The code on x86 is an exception and still retains a PF_KTHREAD test
because x86 unlikely everything else handles kthreads slightly
differently than user space tasks that start with a function.
The functions that created tasks that start with a function
have been updated to set ".fn" and ".fn_arg" instead of
".stack" and ".stack_size". These functions are fork_idle(),
create_io_thread(), kernel_thread(), and user_mode_thread().

The architectures ia64 and parisc have special handling for the idle
thread in copy_process. Add a flag named idle to kernel_clone_args
and use it to explicity test if an idle process is being created.
Fullfill the expectations of the rest of the copy_thread
implemetations and pass a function pointer in .stack from fork_idle().
This makes what is happening in copy_thread better defined, and is
useful to make idle threads less special.

With io_uring we have started supporting tasks that are for most
purposes user space tasks that exclusively run code in kernel mode.
The kernel task that exec's init and tasks that exec user mode
helpers are also user mode tasks that just run kernel code
until they call kernel execve.
Pass kernel_clone_args into copy_thread so these oddball
tasks can be supported more cleanly and easily.

Patch series "Convert vmcore to use an iov_iter", v5.
For some reason several people have been sending bad patches to fix
compiler warnings in vmcore recently. Here's how it should be done.
Compile-tested only on x86. As noted in the first patch, s390 should take
this conversion a bit further, but I'm not inclined to do that work
myself.
This patch (of 3):
Instead of passing in a 'buf' and 'userbuf' argument, pass in an iov_iter.
s390 needs more work to pass the iov_iter down further, or refactor, but
I'd be more comfortable if someone who can test on s390 did that work.
It's more convenient to convert the whole of read_from_oldmem() to take an
iov_iter at the same time, so rename it to read_from_oldmem_iter() and add
a temporary read_from_oldmem() wrapper that creates an iov_iter.

Pass a boolean flag to indicate if swiotlb needs to be enabled based on
the addressing needs, and replace the verbose argument with a set of
flags, including one to force enable bounce buffering.
Note that this patch removes the possibility to force xen-swiotlb use
with the swiotlb=force parameter on the command line on x86 (arm and
arm64 never supported that), but this interface will be restored shortly.

The IOMMU table tries to separate the different IOMMUs into different
backends, but actually requires various cross calls.
Rewrite the code to do the generic swiotlb/swiotlb-xen setup directly
in pci-dma.c and then just call into the IOMMU drivers.

This set of changes removes tracehook.h, moves modification of all of
the ptrace fields inside of siglock to remove races, adds a missing
permission check to ptrace.c
The removal of tracehook.h is quite significant as it has been a major
source of confusion in recent years. Much of that confusion was
around task_work and TIF_NOTIFY_SIGNAL (which I have now decoupled
making the semantics clearer).
For people who don't know tracehook.h is a vestiage of an attempt to
implement uprobes like functionality that was never fully merged, and
was later superseeded by uprobes when uprobes was merged. For many
years now we have been removing what tracehook functionaly a little
bit at a time. To the point where now anything left in tracehook.h is
some weird strange thing that is difficult to understand.

... and call node_dev_init() after memory_dev_init() from driver_init(),
so before any of the existing arch/subsys calls. All online nodes should
be known at that point: early during boot, arch code determines node and
zone ranges and sets the relevant nodes online; usually this happens in
setup_arch().
This is in line with memory_dev_init(), which initializes the memory
device subsystem and creates all memory block devices.
Similar to memory_dev_init(), panic() if anything goes wrong, we don't
want to continue with such basic initialization errors.
The important part is that node_dev_init() gets called after
memory_dev_init() and after cpu_dev_init(), but before any of the relevant
archs call register_cpu() to register the new cpu device under the node
device. The latter should be the case for the current users of
topology_init().

Prior to "mm: handle uninitialized numa nodes gracefully" memory hotplug
used to allocate pgdat when memory has been added to a node
(hotadd_init_pgdat) arch_free_nodedata has been only used in the failure
path because once the pgdat is exported (to be visible by NODA_DATA(nid))
it cannot really be freed because there is no synchronization available
for that.
pgdat is allocated for each possible nodes now so the memory hotplug
doesn't need to do the ever use arch_free_nodedata so drop it.
This patch doesn't introduce any functional change.

We have had several reports [1][2][3] that page allocator blows up when an
allocation from a possible node is requested. The underlying reason is
that NODE_DATA for the specific node is not allocated.
NUMA specific initialization is arch specific and it can vary a lot. E.g.
x86 tries to initialize all nodes that have some cpu affinity (see
init_cpu_to_node) but this can be insufficient because the node might be
cpuless for example.
One way to address this problem would be to check for !node_online nodes
when trying to get a zonelist and silently fall back to another node.
That is unfortunately adding a branch into allocator hot path and it
doesn't handle any other potential NODE_DATA users.
This patch takes a different approach (following a lead of [3]) and it pre
allocates pgdat for all possible nodes in an arch indipendent code -
free_area_init. All uninitialized nodes are treated as memoryless nodes.
node_state of the node is not changed because that would lead to other
side effects - e.g. sysfs representation of such a node and from past
discussions [4] it is known that some tools might have problems digesting
that.
Newly allocated pgdat only gets a minimal initialization and the rest of
the work is expected to be done by the memory hotplug - hotadd_new_pgdat
(renamed to hotadd_init_pgdat).
generic_alloc_nodedata is changed to use the memblock allocator because
neither page nor slab allocators are available at the stage when all
pgdats are allocated. Hotplug doesn't allocate pgdat anymore so we can
use the early boot allocator. The only arch specific implementation is
ia64 and that is changed to use the early allocator as well.

Patch series "mm, memory_hotplug: handle unitialized numa node gracefully".
The core of the fix is patch 2 which also links existing bug reports. The
high level goal is to have all possible numa nodes have their pgdat
allocated and initialized so
for_each_possible_node(nid)
NODE_DATA(nid)
will never return garbage. This has proven to be problem in several
places when an offline numa node is used for an allocation just to realize
that node_data and therefore allocation fallback zonelists are not
initialized and such an allocation request blows up.
There were attempts to address that by checking node_online in several
places including the page allocator. This patchset approaches the problem
from a different perspective and instead of special casing, which just
adds a runtime overhead, it allocates pglist_data for each possible node.
This can add some memory overhead for platforms with high number of
possible nodes if they do not contain any memory. This should be a rather
rare configuration though.

We need to use this function in common code, so define it for
architectures and/or configrations that miss it. The result of
pmd_pfn() will only be used if TRANSPARENT_HUGEPAGE is enabled,
but a function or macro called pmd_pfn() must be defined, even
on machines with two level page tables.

Christoph Hellwig and a few others spent a huge effort on removing
set_fs() from most of the important architectures, but about half the
other architectures were never completed even though most of them don't
actually use set_fs() at all.
I did a patch for microblaze at some point, which turned out to be fairly
generic, and now ported it to most other architectures, using new generic
implementations of access_ok() and __{get,put}_kernel_nocheck().
Three architectures (sparc64, ia64, and sh) needed some extra work,

ia64 only uses set_fs() in one file to handle unaligned access for
both user space and kernel instructions. Rewrite this to explicitly
pass around a flag about which one it is and drop the feature from
the architecture.

There are many different ways that access_ok() is defined across
architectures, but in the end, they all just compare against the
user_addr_max() value or they accept anything.
Provide one definition that works for most architectures, checking
against TASK_SIZE_MAX for user processes or skipping the check inside
of uaccess_kernel() sections.
For architectures without CONFIG_SET_FS(), this should be the fastest
check, as it comes down to a single comparison of a pointer against a
compile-time constant, while the architecture specific versions tend to
do something more complex for historic reasons or get something wrong.
Type checking for __user annotations is handled inconsistently across
architectures, but this is easily simplified as well by using an inline
function that takes a 'const void __user *' argument. A handful of
callers need an extra __user annotation for this.
Some architectures had trick to use 33-bit or 65-bit arithmetic on the
addresses to calculate the overflow, however this simpler version uses
fewer registers, which means it can produce better object code in the
end despite needing a second (statically predicted) branch.

Provide a generic alloc_thread_stack_node() for IA64 and
CONFIG_ARCH_THREAD_STACK_ALLOCATOR which returns stack pointer and sets
task_struct::stack so it behaves exactly like the other implementations.
Rename IA64's alloc_thread_stack_node() and add the generic version to the
fork code so it is in one place _and_ to drastically lower the chances of
fat fingering the IA64 code. Do the same for free_thread_stack().

There is a regular need in the kernel to provide a way to declare
having a dynamically sized set of trailing elements in a structure.
Kernel code should always use “flexible array members”[1] for these
cases. The older style of one-element or zero-length arrays should
no longer be used[2].
This code was transformed with the help of Coccinelle:

dereference_kernel_function_descriptor() are identical on the
three architectures implementing them.
Make them common and put them out-of-line in kernel/extable.c
which is one of the users and has similar type of functions.

We have three architectures using function descriptors, each with its
own type and name.
Add a common typedef that can be used in generic code.
Also add a stub typedef for architecture without function descriptors,
to avoid a forest of #ifdefs.
It replaces the similar 'func_desc_t' previously defined in
arch/powerpc/kernel/module_64.c

Replace HAVE_DEREFERENCE_FUNCTION_DESCRIPTOR by a config option
named CONFIG_HAVE_FUNCTION_DESCRIPTORS and use it instead of
'dereference_function_descriptor' macro to know whether an
arch has function descriptors.
To limit churn in one of the following patches, use
an #ifdef/#else construct with empty first part
instead of an #ifndef in asm-generic/sections.h
On powerpc, make sure the config option matches the ABI used
by the compiler with a BUILD_BUG_ON() and add missing _CALL_ELF=2
when calling 'sparse' so that sparse sees the same piece of
code as GCC.
And include a helper to check whether an arch has function
descriptors or not : have_function_descriptors()

Modern compilers are perfectly capable of extracting parallelism from
the XOR routines, provided that the prototypes reflect the nature of the
input accurately, in particular, the fact that the input vectors are
expected not to overlap. This is not documented explicitly, but is
implied by the interchangeability of the various C routines, some of
which use temporary variables while others don't: this means that these
routines only behave identically for non-overlapping inputs.
So let's decorate these input vectors with the __restrict modifier,
which informs the compiler that there is no overlap. While at it, make
the input-only vectors pointer-to-const as well.

In linux-next, IA64_MCA_RECOVERY uses the (new) function
make_task_dead(), which is not exported for use by modules. Instead of
exporting it for one user, convert IA64_MCA_RECOVERY to be a bool
Kconfig symbol.
In a config file from "kernel test robot <lkp@intel.com>" for a
different problem, this linker error was exposed when
CONFIG_IA64_MCA_RECOVERY=m.

Ville reported that the sysfs "rom" file for VGA devices disappeared after
527139d738 ("PCI/sysfs: Convert "rom" to static attribute").
Prior to 527139d738, FINAL fixups, including pci_fixup_video() where we
find shadow ROMs, were run before pci_create_sysfs_dev_files() created the
sysfs "rom" file.
After 527139d738, "rom" is a static attribute and is created before FINAL
fixups are run, so we didn't create "rom" files for shadow ROMs:

When supporting page mapping percpu first chunk allocator on arm64, we

The pcpu_fc_alloc_fn_t/pcpu_fc_free_fn_t is killed, we provide generic
pcpu_fc_alloc() and pcpu_fc_free() function, which are called in the
pcpu_embed/page_first_chunk().
1) For pcpu_embed_first_chunk(), pcpu_fc_cpu_to_node_fn_t is needed to be
provided when archs supported NUMA.
2) For pcpu_page_first_chunk(), the pcpu_fc_populate_pte_fn_t is killed too,
a generic pcpu_populate_pte() which marked '__weak' is provided, if you
need a different function to populate pte on the arch(like x86), please

Pull signal/exit/ptrace updates from Eric Biederman:
"This set of changes deletes some dead code, makes a lot of cleanups
which hopefully make the code easier to follow, and fixes bugs found
along the way.
The end-game which I have not yet reached yet is for fatal signals
that generate coredumps to be short-circuit deliverable from
complete_signal, for force_siginfo_to_task not to require changing
userspace configured signal delivery state, and for the ptrace stops
to always happen in locations where we can guarantee on all
architectures that the all of the registers are saved and available on
the stack.
Removal of profile_task_ext, profile_munmap, and profile_handoff_task
are the big successes for dead code removal this round.
A bunch of small bug fixes are included, as most of the issues
reported were small enough that they would not affect bisection so I
simply added the fixes and did not fold the fixes into the changes
they were fixing.
There was a bug that broke coredumps piped to systemd-coredump. I
dropped the change that caused that bug and replaced it entirely with
something much more restrained. Unfortunately that required some
rebasing.
Some successes after this set of changes: There are few enough calls
to do_exit to audit in a reasonable amount of time. The lifetime of
struct kthread now matches the lifetime of struct task, and the
pointer to struct kthread is no longer stored in set_child_tid. The
flag SIGNAL_GROUP_COREDUMP is removed. The field group_exit_task is
removed. Issues where task->exit_code was examined with
signal->group_exit_code should been examined were fixed.
There are several loosely related changes included because I am
cleaning up and if I don't include them they will probably get lost.

find_bit API and bitmap API are closely related, but inclusion paths
are different - include/asm-generic and include/linux, correspondingly.

Since commit 4064b98270 ("mm: allow VM_FAULT_RETRY for multiple
times") allowed VM_FAULT_RETRY for multiple times, the
FAULT_FLAG_ALLOW_RETRY bit of fault_flag will not be changed in the page
fault path, so the following check is no longer needed:

There are currently two ways to create a set of sysfs files for a kobj_type,
through the default_attrs field, and the default_groups field.
Move the ia64 topology sysfs code to use default_groups field which has
been the preferred way since aa30f47cf6 ("kobject: Add support for

There are two big uses of do_exit. The first is it's design use to be
the guts of the exit(2) system call. The second use is to terminate
a task after something catastrophic has happened like a NULL pointer
in kernel code.
Add a function make_task_dead that is initialy exactly the same as
do_exit to cover the cases where do_exit is called to handle
catastrophic failure. In time this can probably be reduced to just a
light wrapper around do_task_dead. For now keep it exactly the same so
that there will be no behavioral differences introducing this new
concept.
Replace all of the uses of do_exit that use it for catastraphic
task cleanup with make_task_dead to make it clear what the code
is doing.
As part of this rename rewind_stack_do_exit
rewind_stack_and_make_dead.

The printk header file includes ratelimit_types.h for its __ratelimit()
based usage. It is required for the static initializer used in
printk_ratelimited(). It uses a raw_spinlock_t and includes the
spinlock_types.h.
PREEMPT_RT substitutes spinlock_t with a rtmutex based implementation and so
its spinlock_t implmentation (provided by spinlock_rt.h) includes rtmutex.h and
atomic.h which leads to recursive includes where defines are missing.
By including only the raw_spinlock_t defines it avoids the atomic.h
related includes at this stage.

We want to specify flags when hotplugging memory. Let's prepare to pass
flags to memblock_add_node() by adjusting all existing users.
Note that when hotplugging memory the system is already up and running
and we might have concurrent memblock users: for example, while we're
hotplugging memory, kexec_file code might search for suitable memory
regions to place kexec images. It's important to add the memory
directly to memblock via a single call with the right flags, instead of
adding the memory first and apply flags later: otherwise, concurrent
memblock users might temporarily stumble over memblocks with wrong
flags, which will be important in a follow-up patch that introduces a
new flag to properly handle add_memory_driver_managed().

"Current coredumps are mixed up with the exit code, the signal handling
code, and the ptrace code making coredumps much more complicated than
necessary and difficult to follow.
This series of changes starts with ptrace_stop and cleans it up,
making it easier to follow what is happening in ptrace_stop. Then
cleans up the exec interactions with coredumps. Then cleans up the
coredump interactions with exit. Finally the coredump interactions
with the signal handling code is cleaned up.
The first and last changes are bug fixes for minor bugs.
I believe the fact that vfork followed by execve can kill the process
the called vfork if exec fails is sufficient justification to change
the userspace visible behavior.
In previous discussions some of these changes were organized
differently and individually appeared to make the code base worse. As
currently written I believe they all stand on their own as cleanups
and bug fixes.
Which means that even if the worst should happen and the last change
needs to be reverted for some unimaginable reason, the code base will
still be improved.
If the worst does not happen there are a more cleanups that can be
made. Signals that generate coredumps can easily become eligible for
short circuit delivery in complete_signal. The entire rendezvous for
generating a coredump can move into get_signal. The function
force_sig_info_to_task be written in a way that does not modify the
signal handling state of the target task (because coredumps are
eligible for short circuit delivery). Many of these future cleanups
can be done another way but nothing so cleanly as if coredumps become

parisc, ia64 and powerpc32 are the only remaining architectures that
provide custom arch_{spin,read,write}_lock_flags() functions, which are
meant to re-enable interrupts while waiting for a spinlock.
However, none of these can actually run into this codepath, because
it is only called on architectures without CONFIG_GENERIC_LOCKBREAK,
or when CONFIG_DEBUG_LOCK_ALLOC is set without CONFIG_LOCKDEP, and none
of those combinations are possible on the three architectures.
Going back in the git history, it appears that arch/mn10300 may have
been able to run into this code path, but there is a good chance that
it never worked. On the architectures that still exist, it was
already impossible to hit back in 2008 after the introduction of
CONFIG_GENERIC_LOCKBREAK, and possibly earlier.
As this is all dead code, just remove it and the helper functions built
around it. For arch/ia64, the inline asm could be cleaned up, but
it seems safer to leave it untouched.

Documentation/kbuild/makefiles.rst suggests to use "archclean" for
cleaning arch/$(SRCARCH)/boot/, but it is not a hard requirement.
Since commit d92cc4d516 ("kbuild: require all architectures to have
arch/$(SRCARCH)/Kbuild"), we can use the "subdir- += boot" trick for
all architectures. This can take advantage of the parallel option (-j)
for "make clean".
I also cleaned up the comments in arch/$(SRCARCH)/Makefile. The "archdep"
target no longer exists.

Compile-testing drivers that require access to a firmware layer
fails when that firmware symbol is unavailable. This happened
twice this week:
- My proposed to change to rework the QCOM_SCM firmware symbol
broke on ppc64 and others.
- The cs_dsp firmware patch added device specific firmware loader
into drivers/firmware, which broke on the same set of
architectures.
We should probably do the same thing for other subsystems as well,
but fix this one first as this is a dependency for other patches
getting merged.

Both arch_ptrace_stop_needed and arch_ptrace_stop are called with an
exit_code and a siginfo structure. Neither argument is used by any of
the implementations so just remove the unneeded arguments.
The two arechitectures that implement arch_ptrace_stop are ia64 and
sparc. Both architectures flush their register stacks before a
ptrace_stack so that all of the register information can be accessed
by debuggers.
As the question of if a register stack needs to be flushed is
independent of why ptrace is stopping not needing arguments make sense.

When CONFIG_PRINTK is not set, the CMPXCHG_BUGCHECK() macro calls
_printk(), but _printk() is a static inline function, not available
as an extern.
Since the purpose of the macro is to print the BUGCHECK info,
make this config option depend on PRINTK.

Replace audit syscall class magic numbers with macros.
This required putting the macros into new header file
include/linux/audit_arch.h since the syscall macros were
included for both 64 bit and 32 bit in any compat code, causing
redefinition warnings.

The __kretprobe_trampoline_handler() callback, called from low level
arch kprobes methods, has the 'trampoline_address' parameter, which is
entirely superfluous as it basically just replicates:
dereference_kernel_function_descriptor(kretprobe_trampoline)
In fact we had bugs in arch code where it wasn't replicated correctly.
So remove this superfluous parameter and use kretprobe_trampoline_addr()
instead.
