devm_memremap_pages() is currently used by the PCI P2PDMA code to create
struct page mappings for IO memory. At present, these mappings are
created with PAGE_KERNEL which implies setting the PAT bits to be WB.
However, on x86, an mtrr register will typically override this and force
the cache type to be UC-. In the case firmware doesn't set this
register it is effectively WB and will typically result in a machine
check exception when it's accessed.
Other arches are not currently likely to function correctly seeing they
don't have any MTRR registers to fall back on.
To solve this, provide a way to specify the pgprot value explicitly to
arch_add_memory().
Of the arches that support MEMORY_HOTPLUG: x86_64, and arm64 need a
simple change to pass the pgprot_t down to their respective functions
which set up the page tables. For x86_32, set the page tables
explicitly using _set_memory_prot() (seeing they are already mapped).
For ia64, s390 and sh, reject anything but PAGE_KERNEL settings -- this
should be fine, for now, seeing these architectures don't support
ZONE_DEVICE.
A check in __add_pages() is also added to ensure the pgprot parameter
was set for all arches.

Currently there are many platforms that dont enable ARCH_HAS_PTE_SPECIAL
but required to define quite similar fallback stubs for special page
table entry helpers such as pte_special() and pte_mkspecial(), as they
get build in generic MM without a config check. This creates two
generic fallback stub definitions for these helpers, eliminating much
code duplication.
mips platform has a special case where pte_special() and pte_mkspecial()
visibility is wider than what ARCH_HAS_PTE_SPECIAL enablement requires.
This restricts those symbol visibility in order to avoid redefinitions
which is now exposed through this new generic stubs and subsequent build
failure. arm platform set_pte_at() definition needs to be moved into a
C file just to prevent a build failure.

There are many places where all basic VMA access flags (read, write,
exec) are initialized or checked against as a group. One such example
is during page fault. Existing vma_is_accessible() wrapper already
creates the notion of VMA accessibility as a group access permissions.
Hence lets just create VM_ACCESS_FLAGS (VM_READ|VM_WRITE|VM_EXEC) which
will not only reduce code duplication but also extend the VMA
accessibility concept in general.

There are many platforms with exact same value for VM_DATA_DEFAULT_FLAGS
This creates a default value for VM_DATA_DEFAULT_FLAGS in line with the
existing VM_STACK_DEFAULT_FLAGS. While here, also define some more
macros with standard VMA access flag combinations that are used
frequently across many platforms. Apart from simplification, this
reduces code duplication as well.

pte_index() is either defined as a macro (e.g. sparc64) or as an
inlined function (e.g. x86). vm_insert_pages() depends on pte_index
but it is not defined on all platforms (e.g. m68k).
To fix compilation of vm_insert_pages() on architectures not providing
pte_index(), we perform the following fix:
0. For platforms where it is meaningful, and defined as a macro, no
change is needed.
1. For platforms where it is meaningful and defined as an inlined
function, and we want to use it with vm_insert_pages(), we define
a degenerate macro of the form: #define pte_index pte_index
2. vm_insert_pages() checks for the existence of a pte_index macro
definition. If found, it implements a batched insert. If not found,
it devolves to calling vm_insert_page() in a loop.
This patch implements step 1 for x86.
v3 of this patch fixes a compilation warning for an unused method.
v2 of this patch moved a macro definition to a more readable location.

pte_index() on platforms other than sparc return a numerical index. On
sparc, it returns a pte_t*. This presents an issue for
vm_insert_pages(), which relies on pte_index() to find the offset for a
pte within a pmd, for batched inserts.
This patch:
1. Modifies pte_index() for sparc to return a numerical index, like
other platforms,
2. Defines pte_entry() for sparc which returns a pte_t*
(as pte_index() used to),
3. Converts existing sparc callers for pte_index() to use pte_entry().

Commit 944d9fec8d ("hugetlb: add support for gigantic page allocation
at runtime") has added the run-time allocation of gigantic pages.
However it actually works only at early stages of the system loading,
when the majority of memory is free. After some time the memory gets
fragmented by non-movable pages, so the chances to find a contiguous 1GB
block are getting close to zero. Even dropping caches manually doesn't
help a lot.
At large scale rebooting servers in order to allocate gigantic hugepages
is quite expensive and complex. At the same time keeping some constant
percentage of memory in reserved hugepages even if the workload isn't
using it is a big waste: not all workloads can benefit from using 1 GB
pages.
The following solution can solve the problem:
1) On boot time a dedicated cma area* is reserved. The size is passed
as a kernel argument.
2) Run-time allocations of gigantic hugepages are performed using the
cma allocator and the dedicated cma area
In this case gigantic hugepages can be allocated successfully with a
high probability, however the memory isn't completely wasted if nobody
is using 1GB hugepages: it can be used for pagecache, anon memory, THPs,
etc.
* On a multi-node machine a per-node cma area is allocated on each node.
Following gigantic hugetlb allocation are using the first available
numa node if the mask isn't specified by a user.
Usage:
1) configure the kernel to allocate a cma area for hugetlb allocations:
pass hugetlb_cma=10G as a kernel argument
2) allocate hugetlb pages as usual, e.g.
echo 10 > /sys/kernel/mm/hugepages/hugepages-1048576kB/nr_hugepages
If the option isn't enabled or the allocation of the cma area failed,
the current behavior of the system is preserved.
x86 and arm-64 are covered by this patch, other architectures can be
trivially added later.

The current arm BPF JIT does not correctly compile RSH or ARSH when the
immediate shift amount is 0. This causes the "rsh64 by 0 imm" and "arsh64
by 0 imm" BPF selftests to hang the kernel by reaching an instruction
the verifier determines to be unreachable.
The root cause is in how immediate right shifts are encoded on arm.
For LSR and ASR (logical and arithmetic right shift), a bit-pattern
of 00000 in the immediate encodes a shift amount of 32. When the BPF
immediate is 0, the generated code shifts by 32 instead of the expected
behavior (a no-op).
This patch fixes the bugs by adding an additional check if the BPF
immediate is 0. After the change, the above mentioned BPF selftests pass.

For thumb instructions, call_undef_hook() in traps.c first reads a u16,
and if the u16 indicates a T32 instruction (u16 >= 0xe800), a second
u16 is read, which then makes up the the lower half-word of a T32
instruction. For T16 instructions, the second u16 is not read,
which makes the resulting u32 opcode always have the upper half set to
0.
However, having the upper half of instr_mask in the undef_hook set to 0
masks out the upper half of all thumb instructions - both T16 and T32.
This results in trapped T32 instructions with the lower half-word equal
to the T16 encoding of setend (b650) being matched, even though the upper
half-word is not 0000 and thus indicates a T32 opcode.
An example of such a T32 instruction is eaa0b650, which should raise a
SIGILL since T32 instructions with an eaa prefix are unallocated as per
Arm ARM, but instead works as a SETEND because the second half-word is set
to b650.
This patch fixes the issue by extending instr_mask to include the
upper u32 half, which will still match T16 instructions where the upper
half is 0, but not T32 instructions.

As far as I understood, prom_meminit() in arch/mips/fw/arc/memory.c
is overridden by the one in arch/mips/sgi-ip32/ip32-memory.c if
CONFIG_SGI_IP32 is enabled.
The use of EXPORT_SYMBOL in static libraries potentially causes a
problem for the llvm linker [1]. So, I want to forcibly link lib-y
objects to vmlinux when CONFIG_MODULES=y.
As a groundwork, we must fix multiple definitions that have previously
been hidden by lib-y.
The prom_cleanup() in this file is already marked as __weak (because
it is overridden by the one in arch/mips/sgi-ip22/ip22-mc.c).
I think it should be OK to do the same for these two.

cleaned up by 'make clean'.
Assigning it to the variable 'targets' teaches Kbuild that it is a
generated file. However, this line is not evaluated when cleaning
because scripts/Makefile.clean does not include include/config/auto.conf.
Remove the ifneq-conditional, so this file is correctly cleaned up.

We raise the minimal supported binutils version from time to time.
The last bump was commit 1fb12b35e5 ("kbuild: Raise the minimum
required binutils version to 2.21").
We have these as-instr tests because binutils 2.21 does not support
them.
When we bump the binutils version next time, this will be a good
hint to find out which one can be dropped.
As for the Clang/LLVM builds, we require very new LLVM version,
so the LLVM integrated assembler supports all of them.

Doing this probing inside of the Makefiles means we have a maze of
ifdefs inside the source code and child Makefiles that need to make
proper decisions on this too. Instead, we do it at Kconfig time, like
many other compiler and assembler options, which allows us to set up the
dependencies normally for full compilation units. In the process, the
ADX test changes to use %eax instead of %r10 so that it's valid in both
32-bit and 64-bit mode.

The uncore subsystem in Ice Lake server is similar to previous server.
There are some differences in config register encoding and pci device
IDs. The uncore PMON units in Ice Lake server include Ubox, Chabox, IIO,
IRP, M2PCIE, PCU, M2M, PCIE3 and IMC.
- For CHA, filter 1 register has been removed. The filter 0 register can
be used by and of CHA events to be filterd by Thread/Core-ID. To do
so, the control register's tid_en bit must be set to 1.
- For IIO, there are some changes on event constraints. The MSR address
and MSR offsets among counters are also changed.
- For IRP, the MSR address and MSR offsets among counters are changed.
- For M2PCIE, the counters are accessed by MSR now. Add new MSR address
and MSR offsets. Change event constraints.
- To determine the number of CHAs, have to read CAPID6(Low) and CAPID7
(High) now.
- For M2M, update the PCICFG address and Device ID.
- For UPI, update the PCICFG address, Device ID and counter address.
- For M3UPI, update the PCICFG address, Device ID, counter address and
event constraints.
- For IMC, update the formular to calculate MMIO BAR address, which is
MMIO_BASE + specific MEM_BAR offset.

In order to wake from suspend by ethernet magic packets the GPC
must be used as intc does not have wakeup functionality.
But the FEC DT node currently uses interrupt-extended,
specificying intc, thus breaking WoL.
This problem is probably fallout from the stacked domain conversion
as intc used to chain to GPC.
So replace "interrupts-extended" by "interrupts" to use the default
parent which is GPC.

The existing code in emit_call on RV64 checks that the PC-relative offset
to the function fits in 32 bits before calling emit_jump_and_link to emit
an auipc+jalr pair. However, this check is incorrect because offsets in
the range [2^31 - 2^11, 2^31 - 1] cannot be encoded using auipc+jalr on
RV64 (see discussion [1]). The RISC-V spec has recently been updated
to reflect this fact [2, 3].
This patch fixes the problem by moving the check on the offset into
emit_jump_and_link and modifying it to the correct range of encodable
offsets, which is [-2^31 - 2^11, 2^31 - 2^11). This also enforces the
check on the offset to other uses of emit_jump_and_link (e.g., BPF_JA)
as well.
Currently, this bug is unlikely to be triggered, because the memory
region from which JITed images are allocated is close enough to kernel
text for the offsets to not become too large; and because the bounds on
BPF program size are small enough. This patch prevents this problem from
becoming an issue if either of these change.

The code, #undef CONFIG_OPTIMIZE_INLINING, is not working as expected
because <linux/compiler_types.h> is parsed before vclock_gettime.c since
28128c61e0 ("kconfig.h: Include compiler types to avoid missed struct
attributes").
Since then, <linux/compiler_types.h> is included really early by using the
'-include' option. So, you cannot negate the decision of
<linux/compiler_types.h> in this way.
You can confirm it by checking the pre-processed code, like this:
$ make arch/x86/entry/vdso/vdso32/vclock_gettime.i
There is no difference with/without CONFIG_CC_OPTIMIZE_FOR_SIZE.
It is about two years since 28128c61e0. Nobody has reported a problem
(or, nobody has even noticed the fact that this code is not working).
It is ugly and unreliable to attempt to undefine a CONFIG option from C
files, and anyway the inlining heuristic is up to the compiler.
Just remove the broken code.

Let's always try to online the re-added memory blocks. In case
add_memory() already onlined the added memory blocks, the first
device_online() call will fail and stop processing the remaining memory
blocks.
This avoids manually having to check memhp_auto_online.
Note: PPC always onlines all hotplugged memory directly from the kernel as
well - something that is handled by user space on other architectures.

Accurate userfaultfd WP tracking is possible by tracking exactly which
virtual memory ranges were writeprotected by userland. We can't relay
only on the RW bit of the mapped pagetable because that information is
destroyed by fork() or KSM or swap. If we were to relay on that, we'd
need to stay on the safe side and generate false positive wp faults for
every swapped out page.

It is unlikely that an inaccessible VMA without required permission flags
will get a page fault. Hence lets just append unlikely() directive to
such checks in order to improve performance while also standardizing it
across various platforms.

The root cause is that loaded_vmcss_on_cpu list is not yet initialized,
we initialize it in hardware_enable() but this only happens when we start
a VM.
Previously, we used to have a bitmap with enabled CPUs and that was
preventing [masking] the issue.
Initialized loaded_vmcss_on_cpu list earlier, right before we assign
crash_vmclear_loaded_vmcss pointer. blocked_vcpu_on_cpu list and
blocked_vcpu_on_cpu_lock are moved altogether for consistency.

We have to properly retry again by returning -EINVAL immediately in case
somebody else instantiated the table concurrently. We missed to add the
goto in this function only. The code now matches the other, similar
shadowing functions.
We are overwriting an existing region 2 table entry. All allocated pages
are added to the crst_list to be freed later, so they are not lost
forever. However, when unshadowing the region 2 table, we wouldn't trigger
unshadowing of the original shadowed region 3 table that we replaced. It
would get unshadowed when the original region 3 table is modified. As it's
not connected to the page table hierarchy anymore, it's not going to get
used anymore. However, for a limited time, this page table will stick
around, so it's in some sense a temporary memory leak.
Identified by manual code inspection. I don't think this classifies as
stable material.

Whenever we get an -EFAULT, we failed to read in guest 2 physical
address space. Such addressing exceptions are reported via a program
intercept to the nested hypervisor.
We faked the intercept, we have to return to guest 2. Instead, right
now we would be returning -EFAULT from the intercept handler, eventually
crashing the VM.
the correct thing to do is to return 1 as rc == 1 is the internal
representation of "we have to go back into g2".
Addressing exceptions can only happen if the g2->g3 page tables
reference invalid g2 addresses (say, either a table or the final page is
not accessible - so something that basically never happens in sane
environments.
Identified by manual code inspection.

In case we have a region 1 the following calculation
(31 + ((gmap->asce & _ASCE_TYPE_MASK) >> 2)*11)
results in 64. As shifts beyond the size are undefined the compiler is
free to use instructions like sllg. sllg will only use 6 bits of the
shift value (here 64) resulting in no shift at all. That means that ALL
addresses will be rejected.
The can result in endless loops, e.g. when prefix cannot get mapped.

Upper-layer drivers allocate their SBALs by calling qdio_alloc_buffers()
for each individual queue. But when later passing the SBAL addresses to
qdio_establish(), they need to be in a single array of pointers.
So if the driver uses multiple Input or Output queues, it needs to
allocate a temporary array just to present all its SBAL pointers in this
layout.
This patch slightly changes the format of the QDIO initialization data,
so that drivers can pass a per-queue array where each element points to
a queue's SBAL array.
zfcp doesn't use multiple queues, so the impact there is trivial.
For qeth this brings a nice reduction in complexity, and removes
a page-sized allocation.

All that qdio_allocate() actually uses from the init_data is the cdev,
and the number of Input and Output Queues. Have the driver pass those as
parameters, and defer the init_data processing into qdio_establish().
This includes writing per-device(!) trace entries, and most of the
sanity checks.

The __SYSCALL macro's arguments are system call number,
system call entry name and number of arguments for the
system call.
Argument- nargs in __SYSCALL(nr, entry, nargs) is neither
calculated nor used anywhere. So it would be better to
keep the implementaion as __SYSCALL(nr, entry). This will
unifies the implementation with some other architetures
too.

request_irq() is preferred over setup_irq(). Invocations of setup_irq()
occur after memory allocators are ready.
Per tglx[1], setup_irq() existed in olden days when allocators were not
ready by the time early interrupts were initialized.
Hence replace setup_irq() by request_irq().

Core and userspace API:
- The userspace API KFIFOs have been imoproved with locks that
do not block interrupts. This makes us better at getting
events to userspace without blocking or disturbing new events
arriving in the same time. This was reviewed by the KFIFO
maintainer Stefani. This is a generic improvement which
paves the road for similar improvements in other subsystems.
- We provide a new ioctl() for monitoring changes in the line
information, such as when multiple clients are taking lines
and giving them back, possibly reconfiguring them in the
process: we can now monitor that and not get stuck with stale
static information.
- An example tool 'gpio-watch' is provided to showcase this
functionality.
- Timestamps for events are switched to ktime_get_ns() which is
monotonic. We previously had a 'realtime' stamp which could
move forward and *backward* in time, which probably would just
cause silent bugs and weird behaviour. In the long run we
see two relevant timestamps: ktime_get_ns() or the timestamp
sometimes provided by the GPIO hardware itself, if that
exists.
- Device Tree overlay support for GPIO hogs. On systems that
load overlays, these overlays can now contain hogs, and will
then be respected.
- Handle pin control interaction with nonexisting pin ranges
in the GPIO library core instead of in the individual
drivers.

Add a generic device tree for Kendryte K210 SoC based boards. This is
for now a very simple device tree describing the core elements of the
SoC. This is suitable (and tested) for the Kendryte KD233 development
board, the Sipeed MAIX M1 Dan Dock board and the Sipeed MAIXDUINO board.

Add support for the Kendryte K210 RISC-V SoC. For now, this support
only provides a simple sysctl driver allowing to setup the CPU and
uart clock. This support is enabled through the new Kconfig option
SOC_KENDRYTE and defines the config option CONFIG_K210_SYSCTL
to enable the K210 SoC sysctl driver compilation.
The sysctl driver also registers an early SoC initialization function
allowing enabling the general purpose use of the 2MB of SRAM normally
reserved for the SoC AI engine. This initialization function is
automatically called before the dt early initialization using the flat
dt root node compatible property matching the value "kendryte,k210".

Add a mechanism for early SoC initialization for platforms that need
additional hardware initialization not possible through the regular
device tree and drivers mechanism. With this, a SoC specific
initialization function can be called very early, before DTB parsing
is done by parse_dtb() in Linux RISC-V kernel setup code.
This can be very useful for early hardware initialization for No-MMU
kernels booted directly in M-mode because it is quite likely that no
other booting stage exist prior to the No-MMU kernel.
Example use of a SoC early initialization is as follows:

Add handlers for unaligned load and store traps that may be generated
by applications. Code heavily inspired from the OpenSBI project.
Handling of the unaligned access traps is suitable for applications
compiled with or without compressed instructions and is independent of
the kernel CONFIG_RISCV_ISA_C option value.

The compiler (GCC) does not like the situation, where there is inline
assembly block that clobbers all available machine registers in the
middle of the function. This situation can be found in function
svm_vcpu_run in file kvm/svm.c and results in many register spills and
fills to/from stack frame.
This patch fixes the issue with the same approach as was done for
VMX some time ago. The big inline assembly is moved to a separate
assembly .S file, taking into account all ABI requirements.
There are two main benefits of the above approach:
* elimination of several register spills and fills to/from stack
frame, and consequently smaller function .text size. The binary size
of svm_vcpu_run is lowered from 2019 to 1626 bytes.
* more efficient access to a register save array. Currently, register
save array is accessed as:

That commit does not play well with soft-masked irq state
manipulations in idle, interrupt replay, and possibly others due to
tracing code sometimes using irq_work_queue (e.g., in
trace_hardirqs_on()). That can cause PACA_IRQ_DEC to become set when
it is not expected, and be ignored or cleared or cause warnings.
The net result seems to be missing an irq_work until the next timer
interrupt in the worst case which is usually not going to be noticed,
however it could be a long time if the tick is disabled, which is
against the spirit of irq_work and might cause real problems.
The idea is still solid, but it would need more work. It's not really
clear if it would be worth added complexity, so revert this for
now (not a straight revert, but replace with a comment explaining why
we might see interrupts happening, and gives git blame something to
find).

The PowerPC time code is not a clock provider, and just needs to call
of_clk_init().
Hence it can include <linux/of_clk.h> instead of <linux/clk-provider.h>.
Remove the #ifdef protecting the of_clk_init() call, as a stub is
available for the !CONFIG_COMMON_CLK case.

For the memory size ( > 512MB, < 1GB), the MSA setting is:
- SSEG0: PHY_START , PHY_START + 512MB
- SSEG1: PHY_START + 512MB, PHY_START + 1GB
But the real memory is no more than 1GB, there is a gap between the
end size of memory and border of 1GB. CPU could speculatively
execute to that gap and if the gap of the bus couldn't respond to
the CPU request, then the crash will happen.
Now make the setting with:
- SSEG0: PHY_START , PHY_START + 512MB (no change)
- SSEG1: Disabled (We use highmem to use the memory of 512MB~1GB)
We also deprecated zhole_szie[] settings, it's only used by arm
style CPUs. All memory gap should use Reserved setting of dts in
csky system.

This patch enable kprobes, kretprobes, ftrace interface. It utilized
software breakpoint and single step debug exceptions, instructions
simulation on csky.
We use USR_BKPT replace origin instruction, and the kprobe handler
prepares an excutable memory slot for out-of-line execution with a
copy of the original instruction being probed. Most of instructions
could be executed by single-step, but some instructions need origin
pc value to execute and we need software simulate these instructions.

- Introduce 'zero_page_range' as a dax operation. This facilitates
filesystem-dax operation without a block-device.
- Advertise a persistence-domain for of_pmem and papr_scm. The
persistence domain indicates where cpu-store cycles need to reach in
the platform-memory subsystem before the platform will consider them
power-fail protected.
- Fixup some flexible-array declarations.
- Promote numa_map_to_online_node() to a cross-kernel generic facility.
- Save x86 numa information to allow for node-id lookups for reserved
memory ranges, deploy that capability for the e820-pmem driver.
- Introduce phys_to_target_node() to facilitate drivers that want to
know resulting numa node if a given reserved address range was
onlined.
Unlike normal memory ("memory" compatible type in the FDT), the
persistent memory ("ibm,pmemory" in the FDT) can be mapped anywhere in
the guest physical space and it can be used for DMA.
In order to maintain 1:1 mapping via the huge DMA window, we need to
know the maximum physical address at the time of the window setup. So
far we've been looking at "memory" nodes but "ibm,pmemory" does not
have fixed addresses and the persistent memory may be mapped
afterwards.
Since the persistent memory is still backed with page structs, use
MAX_PHYSMEM_BITS as the upper limit.
This effectively disables huge DMA window in LPAR under pHyp if
persistent memory is present but this is the best we can do for the
moment.

sparc32 is the last platform making dynamic decisions in
get_arch_dma_ops based on the bus passed in. Instead set the
iommu dma_ops at iommu probing and propagate them in
of_propagate_archdata, falling back to the NULL ops for the
direct mapping in the Leon or PCI case.

The BPF JIT fails to build for kernels configured to !MMU. Without an
MMU, the BPF JIT does not make much sense, therefore this patch
disables the JIT for nommu builds.

proc_flush_task, and the work to solve a deadlock between strace and

Fixing proc_flush_task so that it no longer requires a persistent

found and reported by the Android folks. This further allows Alexey

The work on exec starts solving a long standing issue with exec that
it takes mutexes of blocking userspace applications, which makes exec
extremely deadlock prone. For the moment this adds a second mutex with
a narrower scope that handles all of the easy cases. Which makes the

After introducing mem sub section concept, pfn_present() loses its literal
meaning, and will not be necessary a truth on partial populated mem
section.
Since all of the callers use it to judge an absent section, it is better
to rename pfn_present() as pfn_in_present_section().

The idea comes from a discussion between Linus and Andrea [1].
Before this patch we only allow a page fault to retry once. We achieved
this by clearing the FAULT_FLAG_ALLOW_RETRY flag when doing
handle_mm_fault() the second time. This was majorly used to avoid
unexpected starvation of the system by looping over forever to handle the
page fault on a single page. However that should hardly happen, and after
all for each code path to return a VM_FAULT_RETRY we'll first wait for a
condition (during which time we should possibly yield the cpu) to happen
before VM_FAULT_RETRY is really returned.
This patch removes the restriction by keeping the FAULT_FLAG_ALLOW_RETRY
flag when we receive VM_FAULT_RETRY. It means that the page fault handler
now can retry the page fault for multiple times if necessary without the
need to generate another page fault event. Meanwhile we still keep the
FAULT_FLAG_TRIED flag so page fault handler can still identify whether a
page fault is the first attempt or not.
Then we'll have these combinations of fault flags (only considering
ALLOW_RETRY flag and TRIED flag):
- ALLOW_RETRY and !TRIED: this means the page fault allows to
retry, and this is the first try
- ALLOW_RETRY and TRIED: this means the page fault allows to
retry, and this is not the first try
- !ALLOW_RETRY and !TRIED: this means the page fault does not allow
to retry at all
- !ALLOW_RETRY and TRIED: this is forbidden and should never be used
In existing code we have multiple places that has taken special care of
the first condition above by checking against (fault_flags &
FAULT_FLAG_ALLOW_RETRY). This patch introduces a simple helper to detect
the first retry of a page fault by checking against both (fault_flags &
FAULT_FLAG_ALLOW_RETRY) and !(fault_flag & FAULT_FLAG_TRIED) because now
even the 2nd try will have the ALLOW_RETRY set, then use that helper in
all existing special paths. One example is in __lock_page_or_retry(), now
we'll drop the mmap_sem only in the first attempt of page fault and we'll
keep it in follow up retries, so old locking behavior will be retained.
This will be a nice enhancement for current code [2] at the same time a
supporting material for the future userfaultfd-writeprotect work, since in
that work there will always be an explicit userfault writeprotect retry
for protected pages, and if that cannot resolve the page fault (e.g., when
userfaultfd-writeprotect is used in conjunction with swapped pages) then
we'll possibly need a 3rd retry of the page fault. It might also benefit
other potential users who will have similar requirement like userfault
write-protection.
GUP code is not touched yet and will be covered in follow up patch.

Although there're tons of arch-specific page fault handlers, most of them
are still sharing the same initial value of the page fault flags. Say,
merely all of the page fault handlers would allow the fault to be retried,
and they also allow the fault to respond to SIGKILL.
Let's define a default value for the fault flags to replace those initial
page fault flags that were copied over. With this, it'll be far easier to
introduce new fault flag that can be used by all the architectures instead
of touching all the archs.

Let SH to use the new fault_signal_pending() helper. Here we'll need to
move the up_read() out because that's actually needed as long as !RETRY
cases. At the meantime we can drop all the rest of up_read()s now (which
seems to be cleaner).

For most architectures, we've got a quick path to detect fatal signal
after a handle_mm_fault(). Introduce a helper for that quick path.
It cleans the current codes a bit so we don't need to duplicate the same
check across archs. More importantly, this will be an unified place that
we handle the signal immediately right after an interrupted page fault, so
it'll be much easier for us if we want to change the behavior of handling
signals later on for all the archs.
Note that currently only part of the archs are using this new helper,
because some archs have their own way to handle signals. In the follow up
patches, we'll try to apply this helper to all the rest of archs.
Another note is that the "regs" parameter in the new helper is not used
yet. It'll be used very soon. Now we kept it in this patch only to avoid
touching all the archs again in the follow up patches.

Idea of a foreign VMA with respect to the present context is very generic.
But currently there are two identical definitions for this in powerpc and
x86 platforms. Lets consolidate those redundant definitions while making
vma_is_foreign() available for general use later. This should not cause
any functional change.

It is tedious to list generic-y for each arch that needs it.
So, mandatory-y works like a fallback default (by just wrapping
asm-generic one) when arch does not have a specific header
implementation.
