The code will try to access dev->iotlb when processing
VHOST_IOTLB_INVALIDATE even if it was not initialized which may lead
to NULL pointer dereference. Fixes this by check dev->iotlb before.

We used to call mutex_lock() in vhost_dev_lock_vqs() which tries to
hold mutexes of all virtqueues. This may confuse lockdep to report a
possible deadlock because of trying to hold locks belong to same
class. Switch to use mutex_lock_nested() to avoid false positive.

This patch tries to batched used ring update during RX. This is pretty
fit for the case when guest is much faster (e.g dpdk based
backend). In this case, used ring is almost empty:
- we may get serious cache line misses/contending on both used ring
and used idx.
- at most 1 packet could be dequeued at one time, batching in guest
does not make much effect.
Update used ring in a batch can help since guest won't access the used
ring until used idx was advanced for several descriptors and since we
advance used ring for every N packets, guest will only need to access
used idx for every N packet since it can cache the used idx. To have a
better interaction for both batch dequeuing and dpdk batching,
VHOST_RX_BATCH was used as the maximum number of descriptors that
could be batched.
Test were done between two machines with 2.40GHz Intel(R) Xeon(R) CPU
E5-2630 connected back to back through ixgbe. Traffic were generated
on one remote ixgbe through MoonGen and measure the RX pps through
testpmd in guest when do xdp_redirect_map from local ixgbe to
tap. RX pps were increased from 3.05 Mpps to 4.00 Mpps (about 31%
improvement).
One possible concern for this is the implications for TCP (especially
latency sensitive workload). Result[1] does not show obvious changes
for most of the netperf test (RR, TX, and RX). And we do get some
improvements for RX on some specific size.

This patch implements XDP transmission for TAP. Since we can't create
new queues for TAP during XDP set, exist ptr_ring was reused for
queuing XDP buffers. To differ xdp_buff from sk_buff, TUN_XDP_FLAG
(0x1UL) was encoded into lowest bit of xpd_buff pointer during
ptr_ring_produce, and was decoded during consuming. XDP metadata was
stored in the headroom of the packet which should work in most of
cases since driver usually reserve enough headroom. Very minor changes
were done for vhost_net: it just need to peek the length depends on
the type of pointer.
Tests were done on two Intel E5-2630 2.40GHz machines connected back
to back through two 82599ES. Traffic were generated/received through
MoonGen/testpmd(rxonly). It reports ~20% improvements when
xdp_redirect_map is doing redirection from ixgbe to TAP (from 2.50Mpps
to 3.05Mpps)

Matthew found a roughly 40% tcp throughput regression with commit
c67df11f(vhost_net: try batch dequing from skb array) as discussed

Eventually we figured out that it was a skb leak in handle_rx()
when sending packets to the VM. This usually happens when a guest
can not drain out vq as fast as vhost fills in, afterwards it sets
off the traffic jam and leaks skb(s) which occurs as no headcount
to send on the vq from vhost side.
This can be avoided by making sure we have got enough headcount
before actually consuming a skb from the batched rx array while
transmitting, which is simply done by moving checking the zero
headcount a bit ahead.

We always poll tx for socket, this is sub optimal since this will
slightly increase the waitqueue traversing time and more important,
vhost could not benefit from commit 9e641bdcfa ("net-tun:
restructure tun_do_read for better sleep/wakeup efficiency") even if
we've stopped rx polling during handle_rx(), tx poll were still left
in the waitqueue.
Pktgen from a remote host to VM over mlx4 on two 2.00GHz Xeon E5-2650
shows 11.7% improvements on rx PPS. (from 1.28Mpps to 1.44Mpps)

The vhost_vsock->guest_cid field is uninitialized when /dev/vhost-vsock
is opened until the VHOST_VSOCK_SET_GUEST_CID ioctl is called.
kvmalloc(..., GFP_KERNEL | __GFP_RETRY_MAYFAIL) does not zero memory.
All other vhost_vsock fields are initialized explicitly so just
initialize this field too.

During access_ok checks, addr increases as we iterate over the data
structure, thus addr + len - 1 will point beyond the end of region we
are translating. Harmless since we then verify that the region covers
addr, but let's not waste cpu cycles.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Please do not apply this to mainline directly, instead please re-run the
coccinelle script shown below and apply its output.
For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write
accessors must be used. This patch migrates (most) remaining
ACCESS_ONCE() instances to {READ,WRITE}_ONCE(), using the following

Vhost-net has a hard limit on the number of zerocopy skbs in flight.
When reached, transmission stalls. Stalls cause latency, as well as
head-of-line blocking of other flows that do not use zerocopy.
Instead of stalling, revert to copy-based transmission.
Tested by sending two udp flows from guest to host, one with payload
of VHOST_GOODCOPY_LEN, the other too small for zerocopy (1B). The
large flow is redirected to a netem instance with 1MBps rate limit
and deep 1000 entry queue.

Before the delay, both flows process around 80K pps. With the delay,
before this patch, both process around 400. After this patch, the
large flow is still rate limited, while the small reverts to its
original rate. See also discussion in the first link, below.
Without rate limiting, {1, 10, 100}x TCP_STREAM tests continued to
send at 100% zerocopy.
The limit in vhost_exceeds_maxpend must be carefully chosen. With
vq->num >> 1, the flows remain correlated. This value happens to
correspond to VHOST_MAX_PENDING for vq->num == 256. Allow smaller
fractions and ensure correctness also for much smaller values of
vq->num, by testing the min() of both explicitly. See also the
discussion in the second link below.

Allow interval trees to quickly check for overlaps to avoid unnecesary
tree lookups in interval_tree_iter_first().
As of this patch, all interval tree flavors will require using a
'rb_root_cached' such that we can have the leftmost node easily
available. While most users will make use of this feature, those with
special functions (in addition to the generic insert, delete, search
calls) will avoid using the cached option as they can do funky things
with insertions -- for example, vma_interval_tree_insert_after().

We check tx avail through vhost_enable_notify() in the past which is
wrong since it only checks whether or not guest has filled more
available buffer since last avail idx synchronization which was just
done by vhost_vq_avail_empty() before. What we really want is checking
pending buffers in the avail ring. Fix this by calling
vhost_vq_avail_empty() instead.
This issue could be noticed by doing netperf TCP_RR benchmark as
client from guest (but not host). With this fix, TCP_RR from guest to
localhost restores from 1375.91 trans per sec to 55235.28 trans per
sec on my laptop (Intel(R) Core(TM) i7-5600U CPU @ 2.60GHz).

refcount_t type and corresponding API should be
used instead of atomic_t when the variable is used as
a reference counter. This allows to avoid accidental
refcounter overflows that might lead to use-after-free
situations.

Prepare the datapath for refcounted ubuf_info. Clone ubuf_info with
skb_zerocopy_clone() wherever needed due to skb split, merge, resize
or clone.
Split skb_orphan_frags into two variants. The split, merge, .. paths
support reference counted zerocopy buffers, so do not do a deep copy.
Add skb_orphan_frags_rx for paths that may loop packets to receive
sockets. That is not allowed, as it may cause unbounded latency.
Deep copy all zerocopy copy buffers, ref-counted or not, in this path.
The exact locations to modify were chosen by exhaustively searching
through all code that might modify skb_frag references and/or the
the SKBTX_DEV_ZEROCOPY tx_flags bit.
The changes err on the safe side, in two ways.
(1) legacy ubuf_info paths virtio and tap are not modified. They keep
a 1:1 ubuf_info to sk_buff relationship. Calls to skb_orphan_frags
still call skb_copy_ubufs and thus copy frags in this case.
(2) not all copies deep in the stack are addressed yet. skb_shift,
skb_split and skb_try_coalesce can be refined to avoid copying.
These are not in the hot path and this patch is hairy enough as
is, so that is left for future refinement.

was reported to break vhost_net. We want to cache used event and use
it to check for notification. The assumption was that guest won't move
the event idx back, but this could happen in fact when 16 bit index
wraps around after 64K entries.

__GFP_REPEAT was designed to allow retry-but-eventually-fail semantic to
the page allocator. This has been true but only for allocations
requests larger than PAGE_ALLOC_COSTLY_ORDER. It has been always
ignored for smaller sizes. This is a bit unfortunate because there is
no way to express the same semantic for those requests and they are
considered too important to fail so they might end up looping in the
page allocator for ever, similarly to GFP_NOFAIL requests.
Now that the whole tree has been cleaned up and accidental or misled
usage of __GFP_REPEAT flag has been removed for !costly requests we can
give the original flag a better name and more importantly a more useful
semantic. Let's rename it to __GFP_RETRY_MAYFAIL which tells the user
that the allocator would try really hard but there is no promise of a
success. This will work independent of the order and overrides the
default allocator behavior. Page allocator users have several levels of
guarantee vs. cost options (take GFP_KERNEL as an example)
- GFP_KERNEL & ~__GFP_RECLAIM - optimistic allocation without _any_
attempt to free memory at all. The most light weight mode which even
doesn't kick the background reclaim. Should be used carefully because
it might deplete the memory and the next user might hit the more
aggressive reclaim
- GFP_KERNEL & ~__GFP_DIRECT_RECLAIM (or GFP_NOWAIT)- optimistic
allocation without any attempt to free memory from the current
context but can wake kswapd to reclaim memory if the zone is below
the low watermark. Can be used from either atomic contexts or when
the request is a performance optimization and there is another
fallback for a slow path.
- (GFP_KERNEL|__GFP_HIGH) & ~__GFP_DIRECT_RECLAIM (aka GFP_ATOMIC) -
non sleeping allocation with an expensive fallback so it can access
some portion of memory reserves. Usually used from interrupt/bh
context with an expensive slow path fallback.
- GFP_KERNEL - both background and direct reclaim are allowed and the
_default_ page allocator behavior is used. That means that !costly
allocation requests are basically nofail but there is no guarantee of
that behavior so failures have to be checked properly by callers
(e.g. OOM killer victim is allowed to fail currently).
- GFP_KERNEL | __GFP_NORETRY - overrides the default allocator behavior
and all allocation requests fail early rather than cause disruptive
reclaim (one round of reclaim in this implementation). The OOM killer
is not invoked.
- GFP_KERNEL | __GFP_RETRY_MAYFAIL - overrides the default allocator
behavior and all allocation requests try really hard. The request
will fail if the reclaim cannot make any progress. The OOM killer
won't be triggered.
- GFP_KERNEL | __GFP_NOFAIL - overrides the default allocator behavior
and all allocation requests will loop endlessly until they succeed.
This might be really dangerous especially for larger orders.
Existing users of __GFP_REPEAT are changed to __GFP_RETRY_MAYFAIL
because they already had their semantic. No new users are added.
__alloc_pages_slowpath is changed to bail out for __GFP_RETRY_MAYFAIL if
there is no progress and we have already passed the OOM point.
This means that all the reclaim opportunities have been exhausted except
the most disruptive one (the OOM killer) and a user defined fallback
behavior is more sensible than keep retrying in the page allocator.

Rename:
wait_queue_t => wait_queue_entry_t
'wait_queue_t' was always a slight misnomer: its name implies that it's a "queue",
but in reality it's a queue *entry*. The 'real' queue is the wait queue head,
which had to carry the name.
Start sorting this out by renaming it to 'wait_queue_entry_t'.
This also allows the real structure name 'struct __wait_queue' to
lose its double underscore and become 'struct wait_queue_entry',
which is the more canonical nomenclature for such data types.

Vhost-vsock is a software device so there is no probe call that causes
the driver to register its misc char device node. This creates a
chicken and egg problem: userspace applications must open
/dev/vhost-vsock to use the driver but the file doesn't exist until the
kernel module has been loaded.
Use the devname modalias mechanism so that /dev/vhost-vsock is created
at boot. The vhost_vsock kernel module is automatically loaded when the
first application opens /dev/host-vsock.
Note that the "reserved for local use" range in
Documentation/admin-guide/devices.txt is incorrect. The userio driver
already occupies part of that range. I've updated the documentation
accordingly.

We used to dequeue one skb during recvmsg() from skb_array, this could
be inefficient because of the bad cache utilization and spinlock
touching for each packet. This patch tries to batch them by calling
batch dequeuing helpers explicitly on the exported skb array and pass
the skb back through msg_control for underlayer socket to finish the
userspace copying. Batch dequeuing is also the requirement for more
batching improvement on receive path.
Tests were done by pktgen on tap with XDP1 in guest. Host is Intel(R)
Xeon(R) CPU E5-2650 0 @ 2.00GHz.

vhost code uses __GFP_REPEAT when allocating vhost_virtqueue resp.
vhost_vsock because it would really like to prefer kmalloc to the
vmalloc fallback - see 23cc5a991c ("vhost-net: extend device
allocation to vmalloc") for more context. Michael Tsirkin has also
noted:
"__GFP_REPEAT overhead is during allocation time. Using vmalloc means
all accesses are slowed down. Allocation is not on data path, accesses
are."
The similar applies to other vhost_kvzalloc users.
Let's teach kvmalloc_node to handle __GFP_REPEAT properly. There are
two things to be careful about. First we should prevent from the OOM
killer and so have to involve __GFP_NORETRY by default and secondly
override __GFP_REPEAT for !costly order requests as the __GFP_REPEAT is
ignored for !costly orders.
Supporting __GFP_REPEAT like semantic for !costly request is possible it
would require changes in the page allocator. This is out of scope of
this patch.
This patch shouldn't introduce any functional change.

The virtio drivers deal with struct virtio_vsock_pkt. Add
virtio_transport_deliver_tap_pkt(pkt) for handing packets to the
vsockmon device.
We call virtio_transport_deliver_tap_pkt(pkt) from
net/vmw_vsock/virtio_transport.c and drivers/vhost/vsock.c instead of
common code. This is because the drivers may drop packets before
handing them to common code - we still want to capture them.

"The point of these changes is to significantly reduce the
<linux/sched.h> header footprint, to speed up the kernel build and to
have a cleaner header structure.
After these changes the new <linux/sched.h>'s typical preprocessed
size goes down from a previous ~0.68 MB (~22K lines) to ~0.45 MB (~15K
lines), which is around 40% faster to build on typical configs.
Not much changed from the last version (-v2) posted three weeks ago: I
eliminated quirks, backmerged fixes plus I rebased it to an upstream
SHA1 from yesterday that includes most changes queued up in -next plus
all sched.h changes that were pending from Andrew.
I've re-tested the series both on x86 and on cross-arch defconfigs,
and did a bisectability test at a number of random points.
I tried to test as many build configurations as possible, but some
build breakage is probably still left - but it should be mostly
limited to architectures that have no cross-compiler binaries
available on kernel.org, and non-default configurations"

When device IOTLB is enabled, all address translations were stored in
interval tree. O(lgN) searching time could be slow for virtqueue
metadata (avail, used and descriptors) since they were accessed much
often than other addresses. So this patch introduces an O(1) array
which points to the interval tree nodes that store the translations of
vq metadata. Those array were update during vq IOTLB prefetching and
were reset during each invalidation and tlb update. Each time we want
to access vq metadata, this small array were queried before interval
tree. This would be sufficient for static mappings but not dynamic
mappings, we could do optimizations on top.
Test were done with l2fwd in guest (2M hugepage):
noiommu | before | after
tx 1.32Mpps | 1.06Mpps(82%) | 1.30Mpps(98%)
rx 2.33Mpps | 1.46Mpps(63%) | 2.29Mpps(98%)
We can almost reach the same performance as noiommu mode.

If last avail idx is not equal to cached avail idx, we're sure there's
still available buffers in the virtqueue so there's no need to re-read
avail idx. So let's skip this to avoid unnecessary userspace memory
access and memory barrier. Pktgen test show about 3% improvement on rx
pps.

in 'net-next'.

Currently, under certain circumstances vhost_init_is_le does just a part
of the initialization job, and depends on vhost_reset_is_le being called
too. For this reason vhost_vq_init_access used to call vhost_reset_is_le
when vq->private_data is NULL. This is not only counter intuitive, but
also real a problem because it breaks vhost_net. The bug was introduced to
vhost_net with commit 2751c9882b ("vhost: cross-endian support for
legacy devices"). The symptom is corruption of the vq's used.idx field
(virtio) after VHOST_NET_SET_BACKEND was issued as a part of the vhost
shutdown on a vq with pending descriptors.
Let us make sure the outcome of vhost_init_is_le never depend on the state
it is actually supposed to initialize, and fix virtio_net by removing the
reset from vhost_vq_init_access.
With the above, there is no reason for vhost_reset_is_le to do just half
of the job. Let us make vhost_reset_is_le reinitialize is_le.

This is to silence an uninitialized variable warning in debug output.
The problem is this line:
pr_debug("vhost_get_vq_desc: head: %d, out: %u in: %u\n",
head, out, in);
If "head == vq->num" is true on the first iteration then "out" and "in"
aren't initialized. We handle that a few lines after the printk. I was
tempted to just delete the pr_debug() but I decided to just initialize
them to zero instead.
Also checkpatch.pl complains if variables are declared as just
"unsigned" without the "int".

Declare target_core_fabric_ops strucrues as const as they are only
passed as an argument to the functions target_register_template and

This patch tries to utilize tuntap rx batching by peeking the tx
virtqueue during transmission, if there's more available buffers in
the virtqueue, set MSG_MORE flag for a hint for backend (e.g tuntap)
to batch the packets.

This patch tries to do several tweaks on vhost_vq_avail_empty() for a
better performance:
- check cached avail index first which could avoid userspace memory access.
- using unlikely() for the failure of userspace access
- check vq->last_avail_idx instead of cached avail index as the last
step.
This patch is need for batching supports which needs to peek whether
or not there's still available buffers in the ring.

Despite living under drivers/ vringh.c is also used as part of the userspace
virtio tools. Before we can kill off the ACCESS_ONCE()definition in the tools,
we must convert vringh.c to use {READ,WRITE}_ONCE().
This patch does so, along with the required include of <linux/compiler.h> for
the relevant definitions. The userspace tools provide their own definitions in
their own <linux/compiler.h>.

When event index was enabled, we need to fetch used event from
userspace memory each time. This userspace fetch (with memory
barrier) could be saved sometime when 1) caching used event and 2)
if used event is ahead of new and old to new updating does not cross
it, we're sure there's no need to notify guest.
This will be useful for heavy tx load e.g guest pktgen test with Linux
driver shows ~3.5% improvement.

on pointers, causing sparse warnings. Fix this up.

copy_from_iter_full(), copy_from_iter_full_nocache() and
csum_and_copy_from_iter_full() - counterparts of copy_from_iter()
et.al., advancing iterator only in case of successful full copy
and returning whether it had been successful or not.
Convert some obvious users. *NOTE* - do not blindly assume that
something is a good candidate for those unless you are sure that
not advancing iov_iter in failure case is the right thing in
this case. Anything that does short read/short write kind of
stuff (or is in a loop, etc.) is unlikely to be a good one.

Many modules call misc_register and misc_deregister in its module init
and exit methods without any additional code. This ends up being
boilerplate. This patch adds helper macro module_misc_device(), that
replaces module_init()/ module_exit() with template functions.
This patch also converts drivers to use new macro.

The address of the iovec &vq->iov[out] is not guaranteed to contain the scsi
command's response iovec throughout the lifetime of the command. Rather, it
is more likely to contain an iovec from an immediately following command
after looping back around to vhost_get_vq_desc(). Pass along the iovec
entirely instead.

Stash the packet length in a local variable before handing over
ownership of the packet to virtio_transport_recv_pkt() or
virtio_transport_free_pkt().

This patch tries to implement an device IOTLB for vhost. This could be
used with userspace(qemu) implementation of DMA remapping
to emulate an IOMMU for the guest.
The idea is simple, cache the translation in a software device IOTLB
(which is implemented as an interval tree) in vhost and use vhost_net
file descriptor for reporting IOTLB miss and IOTLB
update/invalidation. When vhost meets an IOTLB miss, the fault
address, size and access can be read from the file. After userspace
finishes the translation, it writes the translated address to the
vhost_net file to update the device IOTLB.
When device IOTLB is enabled by setting VIRTIO_F_IOMMU_PLATFORM all vq
addresses set by ioctl are treated as iova instead of virtual address and
the accessing can only be done through IOTLB instead of direct userspace
memory access. Before each round or vq processing, all vq metadata is
prefetched in device IOTLB to make sure no translation fault happens
during vq processing.
In most cases, virtqueues are contiguous even in virtual address space.
The IOTLB translation for virtqueue itself may make it a little
slower. We might add fast path cache on top of this patch.

Current pre-sorted memory region array has some limitations for future
device IOTLB conversion:
1) need extra work for adding and removing a single region, and it's
expected to be slow because of sorting or memory re-allocation.
2) need extra work of removing a large range which may intersect
several regions with different size.
3) need trick for a replacement policy like LRU
To overcome the above shortcomings, this patch convert it to interval
tree which can easily address the above issue with almost no extra
work.
The patch could be used for:
- Extend the current API and only let the userspace to send diffs of
memory table.
- Simplify Device IOTLB implementation.

We use spinlock to synchronize the work list now which may cause
unnecessary contentions. So this patch switch to use llist to remove
this contention. Pktgen tests shows about 5% improvement:

done seq, and the number of flushing. This patch simplify this by just
implement work flushing through another kind of vhost work with

We used to queue tx packets in sk_receive_queue, this is less
efficient since it requires spinlocks to synchronize between producer
and consumer.
This patch tries to address this by:
- switch from sk_receive_queue to a skb_array, and resize it when
tx_queue_len was changed.
- introduce a new proto_ops peek_len which was used for peeking the
skb length.
- implement a tun version of peek_len for vhost_net to use and convert
vhost_net to use peek_len if possible.
Pktgen test shows about 15.3% improvement on guest receiving pps for small
buffers:

We don't stop rx polling socket during rx processing, this will lead
unnecessary wakeups from under layer net devices (E.g
sock_def_readable() form tun). Rx will be slowed down in this
way. This patch avoids this by stop polling socket during rx
processing. A small drawback is that this introduces some overheads in
light load case because of the extra start/stop polling, but single
netperf TCP_RR does not notice any change. In a super heavy load case,
e.g using pktgen to inject packet to guest, we get about ~8.8%
improvement on pps:

Turns out the template and thus many drivers got the return value wrong:
0 means the fabrics driver needs to put a session reference, which no
driver except for the iSCSI target drivers did. Fortunately none of these
drivers supports explicit Node ACLs, so the bug was harmless.
Even without that only qla2xxx and iscsi every did real work in
shutdown_session, so get rid of the boilerplate code in all other
drivers.

Looking at how callers use this, maybe we should just rename init_used
to vhost_vq_init_access. The _used suffix was a hint that we
access the vq used ring. But maybe what callers care about is
that it must be called after access_ok.
Also, this function manipulates the vq->is_le field which isn't related
to the vq used ring.
This patch simply renames vhost_init_used() to vhost_vq_init_access() as
suggested by Michael.
No behaviour change.

The default use case for vhost is when the host and the vring have the
same endianness (default native endianness). But there are cases where
they differ and vhost should byteswap when accessing the vring.
The first case is when the host is big endian and the vring belongs to
a virtio 1.0 device, which is always little endian.
This is covered by the vq->is_le field. This field is initialized when
userspace calls the VHOST_SET_FEATURES ioctl. It is reset when the device
stops.
We already have a vhost_init_is_le() helper, but the reset operation is
opencoded as follows:
vq->is_le = virtio_legacy_is_little_endian();
It isn't clear that we are resetting vq->is_le here.
This patch moves the code to a helper with a more explicit name.
The other case where we may have to byteswap is when the architecture can
switch endianness at runtime (bi-endian). If endianness differs in the host
and in the guest, then legacy devices need to be used in cross-endian mode.
This mode is available with CONFIG_VHOST_CROSS_ENDIAN_LEGACY=y, which
introduces a vq->user_be field. Userspace may enable cross-endian mode
by calling the SET_VRING_ENDIAN ioctl before the device is started. The
cross-endian mode is disabled when the device is stopped.
The current names of the helpers that manipulate vq->user_be are unclear.
This patch renames those helpers to clearly show that this is cross-endian
stuff and with explicit enable/disable semantics.
No behaviour change.

show_attribute and store_attribute methods in commit 517982229f
("configfs: remove old API") from this pull. As Alexander says about
that patch:
"There's no need to keep an extra wrapper structure per item and the
awkward show_attribute/store_attribute item ops are no longer needed.
This patch converts policy code to the new api, all the while making
the code quite a bit smaller and easier on the eyes.

With well over 200+ users of this api, there are a mere 12 users that
actually checked the return value of this function. And all of them
really didn't do anything with that information as the system or module
was shutting down no matter what.
So stop pretending like it matters, and just return void from
misc_deregister(). If something goes wrong in the call, you will get a
WARNING splat in the syslog so you know how to fix up your driver.
Other than that, there's nothing that can go wrong.

slots, which is used by memory hotplug for
registering hotplugged memory.
However QEMU crashes if it's used with more than ~60

in module vhost-net refuses to accept more than 64

with large number of memory regions we could end up with
high order allocations and kmalloc could fail if
host is under memory pressure.
Considering that memory regions array is used on hot path
try harder to allocate using kmalloc and if it fails resort
to vmalloc.
It's still better than just failing vhost_set_memory() and
causing guest crash due to it when a new memory hotplugged
to guest.
I'll still look at QEMU side solution to reduce amount of
memory regions it feeds to vhost to make things even better,
but it doesn't hurt for kernel to behave smarter and don't
crash older QEMU's which could use large amount of memory
regions.

For default region layouts performance stays the same
as linear search i.e. it takes around 210ns average for
translate_desc() that inlines find_region().
But it scales better with larger amount of regions,
235ns BS vs 300ns LS with 55 memory regions
and it will be about the same values when allowed number
of slots is increased to 509 like it has been done in kvm.

This patch drops unnecessary target_core_fabric_ops parameter usage
for core_tpg_register() during fabric driver TFO->fabric_make_tpg()
se_portal_group creation callback execution.
Instead, use the existing se_wwn->wwn_tf->tf_ops pointer to ensure
fabric driver is really using the same TFO provided at module_init
time.
Also go ahead and drop the forward TFO declarations tree-wide, and
handling the special case for iscsi-target discovery TPG.

Only include SCSI initiator header files in target code that needs
these header files, namely the SCSI pass-through code and the tcm_loop
driver. Change SCSI_SENSE_BUFFERSIZE into TRANSPORT_SENSE_BUFFER in
target code because the former is intended for initiator code and the
latter for target code. With this patch the only initiator include
directives in target code that remain are as follows:

This patch brings cross-endian support to vhost when used to implement
legacy virtio devices. Since it is a relatively rare situation, the
feature availability is controlled by a kernel config option (not set
by default).
The vq->is_le boolean field is added to cache the endianness to be
used for ring accesses. It defaults to native endian, as expected
by legacy virtio devices. When the ring gets active, we force little
endian if the device is modern. When the ring is deactivated, we
revert to the native endian default.
If cross-endian was compiled in, a vq->user_be boolean field is added
so that userspace may request a specific endianness. This field is
used to override the default when activating the ring of a legacy
device. It has no effect on modern devices.

The current memory accessors logic is:
- little endian if little_endian
- native endian (i.e. no byteswap) if !little_endian
If we want to fully support cross-endian vhost, we also need to be
able to convert to big endian.
Instead of changing the little_endian argument to some 3-value enum, this
patch changes the logic to:
- little endian if little_endian
- big endian if !little_endian
The native endian case is handled by all users with a trivial helper. This
patch doesn't change any functionality, nor it does add overhead.

By always allocating and adding, respectively removing and freeing
the se_node_acl structure in core code we can remove tons of repeated
code in the init_nodeacl and drop_nodeacl routines. Additionally
this now respects the get_default_queue_depth method in this code
path as well.

There is just one configfs subsystem in the target code, so we might as
well add two helpers to reference / unreference it from the core code
instead of passing pointers to it around.
This fixes a regression introduced for v4.1-rc1 with commit 9ac8928e6,
where configfs_depend_item() callers using se_tpg_tfo->tf_subsys would
fail, because the assignment from the original target_core_subsystem[]
is no longer happening at target_register_template() time.

All of these files were only building on non-x86 because of
the indirect of inclusion of vmalloc.h by, of all things,
"net/inet_hashtables.h"
None of this got caught during build testing, because on x86
there is an implicit vmalloc.h include via on of the arch asm/
headers.

Instead of calling target_fabric_configfs_init() +
target_fabric_configfs_register() / target_fabric_configfs_deregister()
target_fabric_configfs_free() from every target driver, rewrite the API
so that we have simple register/unregister functions that operate on
a const operations vector.
This patch also fixes a memory leak in several target drivers. Several
target drivers namely called target_fabric_configfs_deregister()
without calling target_fabric_configfs_free().
A large part of this patch is based on earlier changes from
Bart Van Assche <bart.vanassche@sandisk.com>.

This patch updates vhost-scsi to add a new fabric_prot_type TPG
attribute, used for controlling LLD level protection into LIO when
the backend device does not support T10-PI.
This is required for vhost-scsi to enable WRITE_STRIP + READ_INSERT
operations using software emulation + crct10dif instruction offload.
It's disabled by default and controls which se_sesion->sess_prot_type
are set at vhost_scsi_make_nexus() session registration time.

This patch changes loopback, usb-gadget, vhost-scsi and xen-scsiback
fabric code to invoke transport_register_session() instead of the
unprotected flavour, to ensure se_tpg->session_lock is taken when
adding new session list nodes to se_tpg->tpg_sess_list.
Note that since these four fabric drivers already hold their own
internal TPG mutexes when accessing se_tpg->tpg_sess_list, and
consist of a single se_session created through configfs attribute
access, no list corruption can currently occur.
So for correctness sake, go ahead and use the se_tpg->session_lock
protected version for these four fabric drivers.

After TIPC doesn't depend on iocb argument in its internal
implementations of sendmsg() and recvmsg() hooks defined in proto
structure, no any user is using iocb argument in them at all now.
Then we can drop the redundant iocb argument completely from kinds of
implementations of both sendmsg() and recvmsg() in the entire
networking stack.

The 2 that we use for copy_to_iter comes from sizeof(u16),
it used to be that way before the iov iter update.
Fix it up, making it obvious the size of stack access
is right.

Recent iterator-related changes in vhost made it
harder to follow the logic fixing up the header.
In fact, the fixup always happens at the same
offset: sizeof(virtio_net_hdr): sometimes the
fixup iterator is updated by copy_to_iter,
sometimes-by iov_iter_advance.
Rearrange code to make this obvious.

In commit ba7438aed9 ("vhost: don't bother copying iovecs in
handle_rx(), kill memcpy_toiovecend()"), we advance iov iter fixup
sizeof(struct virtio_net_hdr) bytes and fill the number of buffers
after doing the socket recvmsg(). This work well but was broken after
commit 6e03f896b5 ("Merge
git://git.kernel.org/pub/scm/linux/kernel/git/davem/net") which tries
to advance sizeof(struct virtio_net_hdr_mrg_rxbuf). It will fill the
number of buffers at the wrong place. This patch fixes this.

This code in vhost_scsi_make_tpg() is confusing because we limit "tpgt"
to UINT_MAX but the data type of "tpg->tport_tpgt" and that is a u16.
I looked at the context and it turns out that in
vhost_scsi_set_endpoint(), "tpg->tport_tpgt" is used as an offset into
the vs_tpg[] array which has VHOST_SCSI_MAX_TARGET (256) elements so
anything higher than 255 then it is invalid. I have made that the limit
now.
In vhost_scsi_send_evt() we mask away values higher than 255, but now
that the limit has changed, we don't need the mask.

The net/core/dev.c conflict was the overlap of one commit marking an
existing function static whilst another was adding a new function.
In the include/linux/if_vlan.h case, the type used for a local
variable was changed in 'net', whereas the function got rewritten
to fix a stacked vlan bug in 'net-next'.
In drivers/vhost/net.c, Al Viro's iov_iter conversions in 'net-next'
overlapped with an endainness fix for VHOST 1.0 in 'net'.
In drivers/net/vxlan.c, vxlan_find_vni() added a 'flags' parameter
in 'net-next' whereas in 'net' there was a bug fix to pass in the
correct network namespace pointer in calls to this function.

In virtio 1.0 mode, when mergeable buffers are enabled on a big-endian
host, num_buffers wasn't byte-swapped correctly, so large incoming
packets got corrupted.
To fix, fill it in within hdr - this also makes sure it gets
the correct type.

This patch adds ANY_LAYOUT compatible support within the existing
vhost_scsi_handle_vq() ->handle_kick() callback.
It calculates data_direction + exp_data_len for the new tcm_vhost_cmd
descriptor by walking both outgoing + incoming iovecs using iov_iter,
assuming the layout of outgoing request header + T10_PI + Data payload
comes first.
It also uses copy_from_iter() to copy leading virtio-scsi request header
that may or may not include SCSI CDB, that returns a re-calculated iovec
to start of T10_PI or Data SGL memory.
Also, go ahead and drop the legacy pre virtio v1.0 !ANY_LAYOUT logic.

This patch adds ANY_LAYOUT prerequisites logic for accepting a set of
protection + data payloads via iov_iter. Also includes helpers for
calcuating SGLs + invoking vhost_scsi_map_to_sgl() with a known number
of iovecs.
Required by ANY_LAYOUT processing when struct iovec may be offset into
the first outgoing virtio-scsi request header.

This patch fixes vhost_scsi_handle_vq() failure cases that result in BUG_ON()
getting triggered when vhost_scsi_free_cmd() is called, and ->tvc_se_cmd has
not been initialized by target_submit_cmd_map_sgls().
It changes tcm_vhost_release_cmd() to use tcm_vhost_cmd->tvc_nexus for obtaining
se_session pointer reference. Also, avoid calling put_page() on NULL sg->page
entries in vhost_scsi_map_to_sgl() failure path.

header + fixed size sense buffer payload may span more than a single

While looking at hch's recent conversion to drop the MSG_*_TAG
definitions, I noticed a long standing bug in vhost-scsi where
the VIRTIO_SCSI_S_* attribute definitions where incorrectly
being passed directly into target_submit_cmd_map_sgls().
This patch adds the missing virtio-scsi to TCM/SAM task attribute
conversion.

virtio 1.0 only requires used address to be 4 byte aligned,
vhost required 8 bytes (size of vring_used_elem).
Fix up vhost to match that.
Additionally, while vhost correctly requires 8 byte
alignment for log, it's unconnected to used ring:
it's a consequence that log has u64 entries.
Tweak code to make that clearer.

triggers sparse warnings when used with __bitwise tag (when same tag is on both

arches that don't already have it (many do), when that's
merged these warnings will go away.

Most places in vhost can use __get/__put_user rather than
get/put_user since addresses are pre-validated.
This should be good for performance, but this also
will help make code sparse-clean: get/put_user macros
don't play well with __virtioXX bitwise tags.
Switch to get/put_user to __ variants everywhere in vhost.
There's one exception - for consistency switch that
as well, and add an explicit access_ok check.

vhost/net keeps a copy of the used ring in host memory but (ab)uses
the length field for internal house-keeping. This works because the
length in the used ring for tx is always 0. In order to suppress sparse
warnings, we force native endianness here.
Note that these values are never exposed to guests.

This patch addresses a bug where individual vhost-scsi configfs endpoint
groups can be removed from below while active exports to QEMU userspace
still exist, resulting in an OOPs.
It adds a configfs_depend_item() in vhost_scsi_set_endpoint() to obtain
an explicit dependency on se_tpg->tpg_group in order to prevent individual
vhost-scsi WWPN endpoints from being released via normal configfs methods
while an QEMU ioctl reference still exists.
Also, add matching configfs_undepend_item() in vhost_scsi_clear_endpoint()
to release the dependency, once QEMU's reference to the individual group
at /sys/kernel/config/target/vhost/$WWPN/$TPGT is released.

This reworks vhost core dropping unnecessary RCU uses in favor of VQ mutexes
which are used on fast path anyway. This fixes worst-case latency for users
which change the memory mappings a lot.
Memory allocation for vhost-net now supports fallback on vmalloc (same as for
vhost-scsi) this makes it possible to create the device on systems where memory
is very fragmented, with slightly lower performance.

vhost: replace rcu with mutex
replaced rcu sync for memory accesses with VQ mutex locl/unlock.
This is correct since all accesses are under VQ mutex, but incomplete:
we still do useless rcu lock/unlock operations, someone might copy this
code into some other context where this won't be right.
This use of RCU is also non standard and hard to understand.
Let's copy the pointer to each VQ structure, this way
the access rules become straight-forward, and there's
no need for RCU anymore.

All memory accesses are done under some VQ mutex.
So lock/unlock all VQs is a faster equivalent of synchronize_rcu()
for memory access changes.
Some guests cause a lot of these changes, so it's helpful
to make them faster.

Michael Mueller provided a patch to reduce the size of
vhost-net structure as some allocations could fail under
memory pressure/fragmentation. We are still left with
high order allocations though.
This patch is handling the problem at the core level, allowing
vhost structures to use vmalloc() if kmalloc() failed.
As vmalloc() adds overhead on a critical network path, add __GFP_REPEAT
to kzalloc() flags to do this fallback only when really needed.
People are still looking at cleaner ways to handle the problem
at the API level, probably passing in multiple iovecs.
This hack seems consistent with approaches
taken since then by drivers/vhost/scsi.c and net/core/dev.c

This patch adds vhost_scsi_map_iov_to_prot() to perform the mapping of
T10 data integrity memory between virtio iov + struct scatterlist using
get_user_pages_fast() following existing code.
As with vhost_scsi_map_iov_to_sgl(), this does sanity checks against the
total prot_sgl_count vs. pre-allocated SGLs, and loops across protection
iovs using vhost_scsi_map_to_sgl() to perform the actual memory mapping.
Also update tcm_vhost_release_cmd() to release associated tvc_prot_sgl[]
struct page.

In order to support local WRITE_INSERT + READ_STRIP operations for
non PI enabled fabrics, the fabric driver needs to be able signal
what protection offload operations are supported.
This is done at session initialization time so the modes can be
signaled by individual se_wwn + se_portal_group endpoints, as well
as optionally across different transports on the same endpoint.
For iser-target, set TARGET_PROT_ALL if the underlying ib_device
has already signaled PI offload support, and allow this to be
exposed via a new iscsit_transport->iscsit_get_sup_prot_ops()
callback.
For loopback, set TARGET_PROT_ALL to signal SCSI initiator mode
operation.
For all other drivers, set TARGET_PROT_NORMAL to disable fabric
level PI.

Now that TASK_ABORTED status is not generated for all cases by
TMR ABORT_TASK + LUN_RESET, a new TFO->abort_task() caller is
necessary in order to give fabric drivers a chance to unmap
hardware / software resources before the se_cmd descriptor is
released via the normal TFO->release_cmd() codepath.
This patch adds TFO->aborted_task() in core_tmr_abort_task()
in place of the original transport_send_task_abort(), and
also updates all fabric drivers to implement this caller.
The fabric drivers that include changes to perform cleanup
via ->aborted_task() are:
- iscsi-target
- iser-target
- srpt
- tcm_qla2xxx
The fabric drivers that currently set ->aborted_task() to
NOPs are:
- loopback
- tcm_fc
- usb-gadget
- sbp-target
- vhost-scsi
For the latter five, there appears to be no additional cleanup
required before invoking TFO->release_cmd() to release the
se_cmd descriptor.

When mergeable buffers are disabled, and the
incoming packet is too large for the rx buffer,
get_rx_bufs returns success.
This was intentional in order for make recvmsg
truncate the packet and then handle_rx would
detect err != sock_len and drop it.
Unfortunately we pass the original sock_len to
recvmsg - which means we use parts of iov not fully
validated.
Fix this up by detecting this overrun and doing packet drop
immediately.

vhost_zerocopy_callback accesses VQ right after it drops a ubuf
reference. In theory, this could race with device removal which waits
on the ubuf kref, and crash on use after free.
Do all accesses within rcu read side critical section, and synchronize
on release.
Since callbacks are always invoked from bh, synchronize_rcu_bh seems
enough and will help release complete a bit faster.

vhost checked the counter within the refcnt before decrementing. It
really wanted to know that it is the one that has the last reference, as
a way to batch freeing resources a bit more efficiently.
Note: we only let refcount go to 0 on device release.
This works well but we now access the ref counter twice so there's a
race: all users might see a high count and decide to defer freeing
resources.
In the end no one initiates freeing resources until the last reference
is gone (which is on VM shotdown so might happen after a looooong time).
Let's do what we probably should have done straight away:
switch from kref to plain atomic, documenting the
semantics, return the refcount value atomically after decrement,
then use that to avoid the deadlock.

This patch changes percpu_ida_alloc() + callers to accept task state
bitmask for prepare_to_wait() for code like target/iscsi that needs
it for interruptible sleep, that is provided in a subsequent patch.
It now expects TASK_UNINTERRUPTIBLE when the caller is able to sleep
waiting for a new tag, or TASK_RUNNING when the caller cannot sleep,
and is forced to return a negative value when no tags are available.

This patch adds support to target_submit_cmd_map_sgls() for
accepting 'sgl_prot' + 'sgl_prot_count' parameters for
DIF protection information.
Note the passed parameters are stored at se_cmd->t_prot_sg
and se_cmd->t_prot_nents respectively.
Also, update tcm_loop and vhost-scsi fabrics usage of
target_submit_cmd_map_sgls() to take into account the
new parameters.

This patch addresses a long-standing bug where the get_user_pages_fast()
write parameter used for setting the underlying page table entry permission
bits was incorrectly set to write=1 for data_direction=DMA_TO_DEVICE, and
passed into get_user_pages_fast() via vhost_scsi_map_iov_to_sgl().
However, this parameter is intended to signal WRITEs to pinned userspace
PTEs for the virtio-scsi DMA_FROM_DEVICE -> READ payload case, and *not*
for the virtio-scsi DMA_TO_DEVICE -> WRITE payload case.
This bug would manifest itself as random process segmentation faults on
KVM host after repeated vhost starts + stops and/or with lots of vhost
endpoints + LUNs.

As vhost scsi device struct is large, if the device is
created on a busy system, kzalloc() might fail, so this patch does a
fallback to vzalloc().
As vmalloc() adds overhead on data-path, add __GFP_REPEAT
to kzalloc() flags to do this fallback only when really needed.

This patch adds support for pre-allocation of per tv_cmd descriptor
scatterlist + user-space page pointer memory using se_sess->sess_cmd_map
within tcm_vhost_make_nexus() code.
This includes sanity checks within vhost_scsi_map_to_sgl()
to reject I/O that exceeds these initial hardcoded values, and
the necessary cleanup in tcm_vhost_make_nexus() failure path +
tcm_vhost_drop_nexus().

As Michael point out, We used to limit the max pending DMAs to get better cache
utilization. But it was not done correctly since it was one done when there's no
new buffers submitted from guest. Guest can easily exceeds the limitation by
keeping sending packets.
So this patch moves the check into main loop. Tests shows about 5%-10%
improvement on per cpu throughput for guest tx.

We used to poll vhost queue before making DMA is done, this is racy if vhost
thread were waked up before marking DMA is done which can result the signal to
be missed. Fix this by always polling the vhost thread before DMA is done.

Currently, even if the packet length is smaller than VHOST_GOODCOPY_LEN, if
upend_idx != done_idx we still set zcopy_used to true and rollback this choice
later. This could be avoided by determining zerocopy once by checking all
conditions at one time before.

We tend to batch the used adding and signaling in vhost_zerocopy_callback()
which may result more than 100 used buffers to be updated in
vhost_zerocopy_signal_used() in some cases. So switch to use
vhost_add_used_and_signal_n() to avoid multiple calls to
vhost_add_used_and_signal(). Which means much less times of used index
updating and memory barriers.
2% performance improvement were seen on netperf TCP_RR test.

The return value wasn't checked by any of the callers. Assuming this is
correct behaviour, we can simplify some code by not bothering to
generate it.

Currently, vhost-net and vhost-scsi are sharing the vhost core code.
However, vhost-scsi shares the code by including the vhost.c file
directly.
Making vhost a separate module makes it is easier to share code with
other vhost devices.

vhost_net_ubuf_put_and_wait has a confusing name:
it will actually also free it's argument.
Thus since commit 1280c27f8e
"vhost-net: flush outstanding DMAs on memory change"
vhost_net_flush tries to use the argument after passing it
to vhost_net_ubuf_put_and_wait, this results
in use after free.
To fix, don't free the argument in vhost_net_ubuf_put_and_wait,
add an new API for callers that want to free ubufs.

This patch coverts vhost/scsi to se_cmd->cmd_kref TARGET_SCF_ACK_KREF
usage, instead of assuming that vhost_scsi_free_cmd() is always called
before TCM processing is completed in the response fast path.
This includes adding vhost_scsi_check_stop_free() -> target_put_sess_cmd()
to perform the second se_cmd->cmd_kref put, and moving vhost_scsi_free_cmd()
resource release into tcm_vhost_release_cmd() that is invoked once the last
se_cmd->cmd_kref put occurs.

This patch changes vhost_scsi_free_cmd() to call transport_generic_free_cmd()
with wait_for_tasks=false in order to avoid the extra se_cmd->t_state_lock
access for the wait_for_tasks=true case.
This is unnecessary because vhost_scsi_free_cmd() is only ever called by
vhost_scsi_complete_cmd_work() after TCM completion handoff, and by
vhost_scsi_handle_vq() exception code before TCM submission handoff, so
there is never a case where se_cmd is still active from TCM's perspective
when transport_generic_free_cmd() is called.

vhost_net_clear_ubuf_info didn't clear ubuf_info
after kfree, this could trigger double free.
Fix this and simplify this code to make it more robust: make sure
ubuf info is always freed through vhost_net_clear_ubuf_info.

When we decide not use zero-copy, msg.control should be set to NULL otherwise
macvtap/tap may set zerocopy callbacks which may decrease the kref of ubufs
wrongly.
Bug were introduced by commit cedb9bdce0
(vhost-net: skip head management if no outstanding).

RESET_OWNER ioctl would leave the fd in a bad state if
memory allocation failed: device is stopped
but owner is not reset. Make state changes
after allocating memory, such that a failed
ioctl has no effect.

Unlike tcm_vhost_evt requests, tcm_vhost_cmd requests are passed to the
target core system, we can not make sure all the pending requests will
be finished by flushing the virt queue.
In this patch, we do refcount for every tcm_vhost_cmd requests to make
vhost_scsi_flush() wait for all the pending requests issued before the
flush operation to be finished.
This is useful when we call vhost_scsi_clear_endpoint() to stop
tcm_vhost. No new requests will be passed to target core system because
we clear the endpoint by setting vs_tpg to NULL. And we wait for all the
old requests. These guarantee no requests will be leaked and existing
requests will be completed.

This is useful for any device who wants device specific fields per vq.
For example, tcm_vhost wants a per vq field to track requests which are
in flight on the vq. Also, on top of this we can add patches to move
things like ubufs from vhost.h out to net.c.

In commit 365a715009 ([SCSI] virtio-scsi: hotplug support for
virtio-scsi), hotplug support is added to virtio-scsi.
This patch adds hotplug and hotunplug support to tcm_vhost.
You can create or delete a LUN in targetcli to hotplug or hotunplug a
LUN in guest.

The e{uid,gid} --> {uid,gid} credentials fix conflicted with the
cleanup in net-next to now pass cred structs around.
The be2net driver had a bug fix in 'net' that overlapped with the VLAN
interface changes by Patrick McHardy in net-next.
An IGB conflict existed because in 'net' the build_skb() support was
reverted, and in 'net-next' there was a comment style fix within that
code.
Several batman-adv conflicts were resolved by making sure that all
calls to batadv_is_my_mac() are changed to have a new bat_priv first
argument.
Eric Dumazet's TS ECR fix in TCP in 'net' conflicted with the F-RTO
rewrite in 'net-next', mostly overlapping changes.

After commit 2b8b328b61 (vhost_net: handle polling
errors when setting backend), we in fact track the polling state through
poll->wqh, so there's no need to duplicate the work with an extra
vhost_net_polling_state. So this patch removes this and make the code simpler.
This patch also removes the all tx starting/stopping code in tx path according
to Michael's suggestion.
Netperf test shows almost the same result in stream test, but gets improvements
on TCP_RR tests (both zerocopy or copy) especially on low load cases.
Tested between multiqueue kvm guest and external host with two direct
connected 82599s.

This patch fixes guest hang when booting seabios and guest.

vq->last_used_idx is initialized only when /dev/vhost-scsi is
opened or closed.
vhost_scsi_open -> vhost_dev_init() -> vhost_vq_reset()
vhost_scsi_release() -> vhost_dev_cleanup -> vhost_vq_reset()
So, when guest talks to tcm_vhost after seabios does, vq->last_used_idx
still contains the old valule for seabios. This confuses guest.
Fix this by calling vhost_init_used() to init vq->last_used_idx when
we set endpoint.

not. It is set or cleared in vhost_scsi_set_endpoint() or
vhost_scsi_clear_endpoint() under the vs->dev.mutex lock. However, when
we check it in vhost_scsi_handle_vq(), we ignored the lock.
Instead of using the vs->vs_endpoint and the vs->dev.mutex lock to
indicate the status of the endpoint, we use per virtqueue
vq->private_data to indicate it. In this way, we can only take the
vq->mutex lock which is per queue and make the concurrent multiqueue
process having less lock contention. Further, in the read side of
vq->private_data, we can even do not take the lock if it is accessed in
the vhost worker thread, because it is protected by "vhost rcu".

"This includes the bug-fix for a >= v3.8-rc1 regression specific to
iscsi-target persistent reservation conflict handling (CC'ed to
stable), and a tcm_vhost patch to drop VIRTIO_RING_F_EVENT_IDX usage
so that in-flight qemu vhost-scsi-pci device code can detect the
proper vhost feature bits.
Also, there are two more tcm_vhost patches still being discussed by
MST and Asias for v3.9 that will be required for the in-flight qemu
vhost-scsi-pci device patch to function properly, and that should
(hopefully) be the last target fixes for this round."

This patch adds a VHOST_SCSI_FEATURES mask minus VIRTIO_RING_F_EVENT_IDX
so that vhost-scsi-pci userspace will strip this feature bit once
GET_FEATURES reports it as being unsupported on the host.
This is to avoid a bug where ->handle_kicks() are missed when EVENT_IDX
is enabled by default in userspace code.

Getting use of virtio rings correct is tricky, and a recent patch saw
an implementation of in-kernel rings (as separate from userspace).
This abstracts the business of dealing with the virtio ring layout
from the access (userspace or direct); to do this, we use function
pointers, which gcc inlines correctly.
