Appana Durga Kedareswara rao adc4cefae9 microblaze: Add support for error injection
To inject the error using the tmr inject IP reset vectors need to be placed
in lmb(bram) due to the limitation in HW when this code runs out of DDR.
Below code adds the error inject code to the .init.ivt section to copy
it in machine_early_init to lmb/Bram location. C_BASE_VECTORS which allow
moving reset vectors out of 0 location is not currently supported by
Microblaze architecture, that's why all the time reset vectors with
injection code is all the time copied to address 0.

As of now getting this functionality working CPU switches to real mode
and simply jumps to bram, which causes triggering of fault which continues
to call_xmb_manager_break break handler which will at the end calls the
error count callback function and performs recovery.

Appana Durga Kedareswara rao a5e3aaa654 microblaze: Add xmb_manager_register function
Triple Modular Redundancy (TMR) Microblaze solution provides soft error
injection, detection, correction and recovery for Microblaze cores in the
system. The Xilinx/AMD Triple Modular Redundancy (TMR) solution in Vivado
provides all the necessary building blocks to implement a redundant
triplicated MicroBlaze subsystem. This processing subsystem is
fault-tolerant and continues to operate nominally after encountering an
error. Together with the capability to detect and recover from errors,
the implementation ensures the reliability of the entire subsystem.

When the break vector gets asserted because of error injection,
the break signal must be blocked before exiting from the break handler,
This commit adds support for xmb_manager_register api which updates the
TMR manager address and control register and error count and reset callback
function arguments, which will be used by the break handler to block the
break and call the error count callback function and reset callback
function.

Stafford Horne abb4970ac3 PCI: Move isa_dma_bridge_buggy out of asm/dma.h
The isa_dma_bridge_buggy symbol is only used for x86_32, and only x86_32
platforms or quirks ever set it.

Add a new linux/isa-dma.h header that #defines isa_dma_bridge_buggy to 0
except on x86_32, where we keep it as a variable, and remove all the arch-
specific definitions.

Anshuman Khandual fa3f9f4a91 microblaze/mm: enable ARCH_HAS_VM_GET_PAGE_PROT
This enables ARCH_HAS_VM_GET_PAGE_PROT on the platform and exports
standard vm_get_page_prot() implementation via DECLARE_VM_GET_PAGE_PROT,
which looks up a private and static protection_map[] array.  Subsequently
all __SXXX and __PXXX macros can be dropped which are no longer needed.

Arnd Bergmann 4313a24985 arch/*/: remove CONFIG_VIRT_TO_BUS
All architecture-independent users of virt_to_bus() and bus_to_virt()
have been fixed to use the dma mapping interfaces or have been
removed now.  This means the definitions on most architectures, and the
CONFIG_VIRT_TO_BUS symbol are now obsolete and can be removed.

The only exceptions to this are a few network and scsi drivers for m68k
Amiga and VME machines and ppc32 Macintosh. These drivers work correctly
with the old interfaces and are probably not worth changing.

On alpha and parisc, virt_to_bus() were still used in asm/floppy.h.
alpha can use isa_virt_to_bus() like x86 does, and parisc can just
open-code the virt_to_phys() here, as this is architecture specific
code.

I tried updating the bus-virt-phys-mapping.rst documentation, which
started as an email from Linus to explain some details of the Linux-2.0
driver interfaces. The bits about virt_to_bus() were declared obsolete
backin 2000, and the rest is not all that relevant any more, so in the
end I just decided to remove the file completely.

Michal Simek 61a4e653ca microblaze: Use simple memmove/memcpy implementation from lib/string.c
This is based on previous commit ("microblaze: Use simple memset
implementation from lib/string.c") where generic memset implementation is
used when OPT_LIB_FUNCTION is not defined. The same change can be done for
memset/memcpy implementation where doesn't make sense to have generic
implementation in architecture code.

Michal Simek 8f0f265e6c microblaze: Use simple memset implementation from lib/string.c
On microblaze systems which are not using OPT_LIB_FUNCTION only simple
memset is used. This function is already implemented in lib/string.c that's
why it should be used instead.
This change is done in respect of issue fixed by commit 33d0f96ffd
("lib/string.c: Use freestanding environment") where gcc-10.x moved
-ftree-loop-distribute-patterns optimization is to O2 optimization level.
This optimization causes GCC to convert the while loop in memset.c into a
call to memset. So This optimization is transforming a loop in a
memset/memcpy into a call to the function itself. This makes the memset
implementation as recursive.

Based on fix above -ffreestanding was used and it needs to be used on
Microblaze too but the patch is not adding this flag it removes simple
implementation to cause that generic implementation is used where this flag
is already setup.

Mike Rapoport 7106c51ee9 arch: Add pmd_pfn() where it is missing
We need to use this function in common code, so define it for
architectures and/or configrations that miss it.  The result of
pmd_pfn() will only be used if TRANSPARENT_HUGEPAGE is enabled,
but a function or macro called pmd_pfn() must be defined, even
on machines with two level page tables.

Arnd Bergmann 967747bbc0 uaccess: remove CONFIG_SET_FS
There are no remaining callers of set_fs(), so CONFIG_SET_FS
can be removed globally, along with the thread_info field and
any references to it.

This turns access_ok() into a cheaper check against TASK_SIZE_MAX.

As CONFIG_SET_FS is now gone, drop all remaining references to
set_fs()/get_fs(), mm_segment_t, user_addr_max() and uaccess_kernel().

Arnd Bergmann 12700c17fc uaccess: generalize access_ok()
There are many different ways that access_ok() is defined across
architectures, but in the end, they all just compare against the
user_addr_max() value or they accept anything.

Provide one definition that works for most architectures, checking
against TASK_SIZE_MAX for user processes or skipping the check inside
of uaccess_kernel() sections.

For architectures without CONFIG_SET_FS(), this should be the fastest
check, as it comes down to a single comparison of a pointer against a
compile-time constant, while the architecture specific versions tend to
do something more complex for historic reasons or get something wrong.

Type checking for __user annotations is handled inconsistently across
architectures, but this is easily simplified as well by using an inline
function that takes a 'const void __user *' argument. A handful of
callers need an extra __user annotation for this.

Some architectures had trick to use 33-bit or 65-bit arithmetic on the
addresses to calculate the overflow, however this simpler version uses
fewer registers, which means it can produce better object code in the
end despite needing a second (statically predicted) branch.

address limit without taking a possible overflow into account.
Passing a negative length or another overflow in here returns
success when it should not.

Use the most common correct implementation here, which optimizes

Mike Rapoport 22e7878102 microblaze: simplify pte_alloc_one_kernel()
The microblaze's implementation of pte_alloc_one_kernel() used
memblock_alloc_try_nid_raw() along with clear_page() to allocated a zeroed
page during early setup.

Replace calls of these functions with a call to memblock_alloc_try_nid()
that already returns zeroed page and respects the same allocation limits
as memblock_alloc_try_nid_raw().

While on it drop early_get_page() wrapper that was only used in
pte_alloc_one_kernel().

Mike Rapoport c3ab6baf6a mm/page_alloc: always initialize memory map for the holes
Patch series "mm: ensure consistency of memory map poisoning".

Currently memory map allocation for FLATMEM case does not poison the
struct pages regardless of CONFIG_PAGE_POISON setting.

This happens because allocation of the memory map for FLATMEM and SPARSMEM
use different memblock functions and those that are used for SPARSMEM case
(namely memblock_alloc_try_nid_raw() and memblock_alloc_exact_nid_raw())
implicitly poison the allocated memory.

Another side effect of this implicit poisoning is that early setup code
that uses the same functions to allocate memory burns cycles for the
memory poisoning even if it was not intended.

These patches introduce memmap_alloc() wrapper that ensure that the memory
map allocation is consistent for different memory models.

This patch (of 4):

Currently memory map for the holes is initialized only when SPARSEMEM
memory model is used.  Yet, even with FLATMEM there could be holes in the
physical memory layout that have memory map entries.

For instance, the memory reserved using e820 API on i386 or
"reserved-memory" nodes in device tree would not appear in memblock.memory
and hence the struct pages for such holes will be skipped during memory
map initialization.

These struct pages will be zeroed because the memory map for FLATMEM
systems is allocated with memblock_alloc_node() that clears the allocated
memory.  While zeroed struct pages do not cause immediate problems, the
correct behaviour is to initialize every page using __init_single_page().
Besides, enabling page poison for FLATMEM case will trigger
PF_POISONED_CHECK() unless the memory map is properly initialized.

Make sure init_unavailable_range() is called for both SPARSEMEM and
FLATMEM so that struct pages representing memory holes would appear as
PG_Reserved with any memory layout.

Anshuman Khandual 1c2f7d14d8 mm/thp: define default pmd_pgtable()
Currently most platforms define pmd_pgtable() as pmd_page() duplicating
the same code all over.  Instead just define a default value i.e
pmd_page() for pmd_pgtable() and let platforms override when required via
<asm/pgtable.h>.  All the existing platform that override pmd_pgtable()
have been moved into their respective <asm/pgtable.h> header in order to
precede before the new generic definition.  This makes it much cleaner
with reduced code.

Anshuman Khandual fac7757e1f mm: define default value for FIRST_USER_ADDRESS

Mark Rutland b68622a86c locking/atomic: microblaze: use asm-generic exclusively
Microblaze provides its own implementation of atomic_dec_if_positive(),
but nothing else. For a while now, the conditional inc/dec ops have been
optional, and the core code will provide generic implementations using
the code templates in scripts/atomic/fallbacks/.

For simplicity, and for consistency with the other conditional atomic
ops, let's drop the microblaze implementation of
atomic_dec_if_positive(), and use the generic implementation.

With that, we can also drop the local asm/atomic.h and asm/cmpxchg.h
headers, as asm-generic/atomic.h is mandatory-y, and we can pull in
asm-generic/cmpxchg.h via generic-y. This matches what nios2 and nds32
do today.

There should be no functional change as a result of this patch.

Arnd Bergmann 637be9183e asm-generic: use asm-generic/unaligned.h for most architectures
There are several architectures that just duplicate the contents
of asm-generic/unaligned.h, so change those over to use the
file directly, to make future modifications easier.

The exceptions are:

- arm32 sets HAVE_EFFICIENT_UNALIGNED_ACCESS, but wants the
  unaligned-struct version

- ppc64le disables HAVE_EFFICIENT_UNALIGNED_ACCESS but includes
  the access-ok version

- most m68k also uses the access-ok version without setting
  HAVE_EFFICIENT_UNALIGNED_ACCESS.

- sh4a has a custom inline asm version

- openrisc is the only one using the memmove version that
  generally leads to worse code.

Mike Rapoport ca15ca406f mm: remove unneeded includes of <asm/pgalloc.h>
Patch series "mm: cleanup usage of <asm/pgalloc.h>"

Most architectures have very similar versions of pXd_alloc_one() and
pXd_free_one() for intermediate levels of page table.  These patches add
generic versions of these functions in <asm-generic/pgalloc.h> and enable
use of the generic functions where appropriate.

In addition, functions declared and defined in <asm/pgalloc.h> headers are
used mostly by core mm and early mm initialization in arch and there is no
actual reason to have the <asm/pgalloc.h> included all over the place.
The first patch in this series removes unneeded includes of
<asm/pgalloc.h>

In the end it didn't work out as neatly as I hoped and moving
pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require
unnecessary changes to arches that have custom page table allocations, so
I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local
to mm/.

This patch (of 8):

In most cases <asm/pgalloc.h> header is required only for allocations of
page table memory.  Most of the .c files that include that header do not
use symbols declared in <asm/pgalloc.h> and do not require that header.

As for the other header files that used to include <asm/pgalloc.h>, it is
possible to move that include into the .c file that actually uses symbols
from <asm/pgalloc.h> and drop the include from the header file.

The process was somewhat automated using

	sed -i -E '/[<"]asm\/pgalloc\.h/d' \
                $(grep -L -w -f /tmp/xx \
                        $(git grep -E -l '[<"]asm/pgalloc\.h'))

where /tmp/xx contains all the symbols defined in
arch/*/include/asm/pgalloc.h.

Mike Rapoport 974b9b2c68 mm: consolidate pte_index() and pte_offset_*() definitions
All architectures define pte_index() as

	(address >> PAGE_SHIFT) & (PTRS_PER_PTE - 1)

and all architectures define pte_offset_kernel() as an entry in the array
of PTEs indexed by the pte_index().

For the most architectures the pte_offset_kernel() implementation relies
on the availability of pmd_page_vaddr() that converts a PMD entry value to
the virtual address of the page containing PTEs array.

Let's move x86 definitions of the PTE accessors to the generic place in
<linux/pgtable.h> and then simply drop the respective definitions from the
other architectures.

The architectures that didn't provide pmd_page_vaddr() are updated to have
that defined.

The generic implementation of pte_offset_kernel() can be overridden by an
architecture and alpha makes use of this because it has special ordering
requirements for its version of pte_offset_kernel().
