This replaces sizeof sense_buffer with SCSI_SENSE_BUFFERSIZE in
several LLDs. It's a preparation for the future changes to remove
sense_buffer array in scsi_cmnd structure.

This patch fixes the module unload problem in flash less 1030
controller environment where firmware download boot functionality is
invoked. The problem is due to the firmware download is being done in
the reverse order, which this patch solves by insureing the download
occurs to the last controller being reset.

NB = number of blocks. This represents the number of blocks to
transfer. The block size is based on the message frame size provided
in the ioc_facts. A value of zero indicates the entire message frame
should be copied. This is two bit value. So by setting this to
non-zero vaule, you increase performance by reducing amount of data
needing to be dma'd. The value that is stored in ioc->ReqeustNB is
sometimes a non-zero vaule, which creates a bug in mptlan, where not
the entire message frame is getting transfer to firware, resulting in
corruption. This fix sets the default to zero, thus entire message
frame is copied.

Cleaning up prints that use the xxx_printk API, in that the fusion
preamble "mptbase: iocX" follows the info provided by the print API.
The way its currently coded, the [H:C:T] print in sdev_printk will be
inbetween "mptbase" and "iocX", instead of before.

ScsiLookup is an array of pending scmd pointers that the scsi lld
maintains. This array is touched from queuecommand, eh threads, and
interrupt context. This array should put under locks, hence this patch
to synchronize its access. I've added some nice little function
wrappers for this, and moved the ScsiLookup array over to MPT_ADAPTER
struct.

alloc_fcdev() calls alloc_netdev() which uses kzalloc to alloc all the
memory together with dev->priv region hence no zeroing of structs inside
struct mpt_lan_priv needed.

The Coverity checker spotted two potential memory leaks in
drivers/message/fusion/mptbase.c::mpt_attach().
There are two returns that may leak the storage allocated for 'ioc'
(sizeof(MPT_ADAPTER) bytes).
A simple fix would be to simply add two kfree() calls before the return
statements, but a better fix (that this patch implements) is to reorder the
code so that if we hit the first return condition we don't have to do the
allocation at all and then just add a kfree() call for the second case.

It's been a useless no-op for long enough in 2.6 so I figured it's time to
remove it. The number of people that could object because they're
maintaining unified 2.4 and 2.6 drivers is probably rather small.

This patch adds a new file mptdebug.h in the fusion source directory,
which contains different debug macros. The existing debug macros and
flags are removed from the mptbase.h and Makefile In Kconfig a new
configuration parameter FUSION_LOGGING is added to enable/disable the
logging support during compile time.

If there is IO going to the volume while a hidden disk is being torn down,
there is a case where we would return a DID_NO_CONNECT for IO sent to the
volume. The end result is the volume goes read-only. This problem is due to the
fact the firmware mapped target ids saved in per device object is phys_disk_num
for hidden raid components, and target_id for the volume. There is a single
case when both phys_disk_num and target_id are equal, so enters this issue. We
fix this issue by checking the tflags when the device is torned down,
insuring the IO being completed is meant for hidden raid component, not the
volume. In addition to this fix, there are a couple other cases to address
hidden raid components. For instance task_abort and device reset are not
supported by mpt fw for hidden raid components, a bus reset would be required
or target reset to volume.
