David Gibson 0bb5e19d63 Clean up mostly unused IOSPACE macros
Most architectures defined three macros, MK_IOSPACE_PFN(), GET_IOSPACE()
and GET_PFN() in pgtable.h.  However, the only callers of any of these
macros are in Sparc specific code, either in arch/sparc, arch/sparc64 or
drivers/sbus.

This patch removes the redundant macros from all architectures except
sparc and sparc64.

Christoph Hellwig 1eeb66a1bb move die notifier handling to common code
This patch moves the die notifier handling to common code.  Previous
various architectures had exactly the same code for it.  Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)

arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at.  avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

Linus Torvalds e3ebadd95c Revert "[PATCH] x86: __pa and __pa_symbol address space separation"
This was broken.  It adds complexity, for no good reason.  Rather than
separate __pa() and __pa_symbol(), we should deprecate __pa_symbol(),
and preferably __pa() too - and just use "virt_to_phys()" instead, which
is more readable and has nicer semantics.

However, right now, just undo the separation, and make __pa_symbol() be
the exact same as __pa().  That fixes the bugs this patch introduced,
and we can do the fairly obvious cleanups later.

Do the new __phys_addr() function (which is now the actual workhorse for
the unified __pa()/__pa_symbol()) as a real external function, that way
all the potential issues with compile/link-time optimizations of
constant symbol addresses go away, and we can also, if we choose to, add
more sanity-checking of the argument.

Christoph Lameter 2bff73830c [PATCH] x86-64: use lru instead of page->index and page->private for pgd lists management.
x86_64 currently simulates a list using the index and private fields of the
page struct.  Seems that the code was inherited from i386.  But x86_64 does
not use the slab to allocate pgds and pmds etc.  So the lru field is not
used by the slab and therefore available.

This patch uses standard list operations on page->lru to realize pgd
tracking.

Vivek Goyal 0dbf7028c0 [PATCH] x86: __pa and __pa_symbol address space separation
Currently __pa_symbol is for use with symbols in the kernel address
map and __pa is for use with pointers into the physical memory map.
But the code is implemented so you can usually interchange the two.

__pa which is much more common can be implemented much more cheaply
if it is it doesn't have to worry about any other kernel address
spaces.  This is especially true with a relocatable kernel as
__pa_symbol needs to peform an extra variable read to resolve
the address.

There is a third macro that is added for the vsyscall data
__pa_vsymbol for finding the physical addesses of vsyscall pages.

Most of this patch is simply sorting through the references to
__pa or __pa_symbol and using the proper one.  A little of
it is continuing to use a physical address when we have it
instead of recalculating it several times.

swapper_pgd is now NULL.  leave_mm now uses init_mm.pgd
and init_mm.pgd is initialized at boot (instead of compile time)
to the physmem virtual mapping of init_level4_pgd.  The
physical address changed.

Except for the for EMPTY_ZERO page all of the remaining references
to __pa_symbol appear to be during kernel initialization.  So this
should reduce the cost of __pa in the common case, even on a relocated
kernel.

As this is technically a semantic change we need to be on the lookout
for anything I missed.  But it works for me (tm).

Vivek Goyal cfd243d4af [PATCH] x86-64: Remove the identity mapping as early as possible
With the rewrite of the SMP trampoline and the early page
allocator there is nothing that needs identity mapped pages,
once we start executing C code.

So add zap_identity_mappings into head64.c and remove
zap_low_mappings() from much later in the code.  The functions
 are subtly different thus the name change.

This also kills boot_level4_pgt which was from an earlier
attempt to move the identity mappings as early as possible,
and is now no longer needed.  Essentially I have replaced
boot_level4_pgt with trampoline_level4_pgt in trampoline.S

Vivek Goyal 67dcbb6bc6 [PATCH] x86-64: Clean up the early boot page table
- Merge physmem_pgt and ident_pgt, removing physmem_pgt.  The merge
  is broken as soon as mm/init.c:init_memory_mapping is run.
- As physmem_pgt is gone don't export it in pgtable.h.
- Use defines from pgtable.h for page permissions.
- Fix the physical memory identity mapping so it is at the correct
  address.
- Remove the physical memory mapping from wakeup_level4_pgt it
  is at the wrong address so we can't possibly be usinging it.
- Simply NEXT_PAGE the work to calculate the phys_ alias
  of the labels was very cool.  Unfortuantely it was a brittle
  special purpose hack that makes maitenance more difficult.
  Instead just use label - __START_KERNEL_map like we do
  everywhere else in assembly.

Vivek Goyal 9d291e787b [PATCH] x86-64: Assembly safe page.h and pgtable.h
This patch makes pgtable.h and page.h safe to include
in assembly files like head.S.  Allowing us to use
symbolic constants instead of hard coded numbers when
refering to the page tables.

This patch copies asm-sparc64/const.h to asm-x86_64 to
get a definition of _AC() a very convinient macro that
allows us to force the type when we are compiling the
code in C and to drop all of the type information when
we are using the constant in assembly.  Previously this
was done with multiple definition of the same constant.
const.h was modified slightly so that it works when given
CONFIG options as arguments.

This patch adds #ifndef __ASSEMBLY__ ... #endif
and _AC(1,UL) where appropriate so the assembler won't
choke on the header files.  Otherwise nothing
should have changed.

If function change_page_attr_addr calls revert_page to revert
to original pte value, mk_pte_phys does not mask NX bit. If NX bit
is set on no NX hardware supported x86_64 machine, there is will
be RSVD type page fault and system will crash. This patch adds NX
mask bit for PTE entry.

Dave McCracken 46a82b2d55 [PATCH] Standardize pxx_page macros
One of the changes necessary for shared page tables is to standardize the
pxx_page macros.  pte_page and pmd_page have always returned the struct
page associated with their entry, while pte_page_kernel and pmd_page_kernel
have returned the kernel virtual address.  pud_page and pgd_page, on the
other hand, return the kernel virtual address.

Shared page tables needs pud_page and pgd_page to return the actual page
structures.  There are very few actual users of these functions, so it is
simple to standardize their usage.

Since this is basic cleanup, I am submitting these changes as a standalone
patch.  Per Hugh Dickins' comments about it, I am also changing the
pxx_page_kernel macros to pxx_page_vaddr to clarify their meaning.

Instead of hackish manual parsing

Requires earlier i386 patchkit, but also fixes i386 early_printk again.

I removed some obsolete really early parameters which didn't do anything useful.
Also made a few parameters that needed it early (mostly oops printing setup)

Also removed one panic check that wasn't visible without
early console anyways (the early console is now initialized after that
panic)

This cleans up a lot of code.

Jan Beulich 8c914cb704 [PATCH] x86_64: actively synchronize vmalloc area when registering certain callbacks
While the modular aspect of the respective i386 patch doesn't apply to
x86-64 (as the top level page directory entry is shared between modules
and the base kernel), handlers registered with register_die_notifier()
are still under similar constraints for touching ioremap()ed or
vmalloc()ed memory. The likelihood of this problem becoming visible is
of course significantly lower, as the assigned virtual addresses would
have to cross a 2**39 byte boundary. This is because the callback gets
invoked
(a) in the page fault path before the top level page table propagation
gets carried out (hence a fault to propagate the top level page table
entry/entries mapping to module's code/data would nest infinitly) and
(b) in the NMI path, where nested faults must absolutely not happen,
since otherwise the IRET from the nested fault re-enables NMIs,
potentially resulting in nested NMI occurences.

Akinobu Mita 3d1712c91d [PATCH] x86_64: {set,clear,test}_bit() related cleanup and pci_mmcfg_init() fix
While working on these patch set, I found several possible cleanup on x86-64
and ia64.

akpm: I stole this from Andi's queue.

Not only does it clean up bitops.  It also unrelatedly changes the prototype
of pci_mmcfg_init() and removes its arch_initcall().  It seems that the wrong
two patches got joined together, but this is the one which has been tested.

This patch fixes the current x86_64 build error (the pci_mmcfg_init()
declaration in arch/i386/pci/pci.h disagrees with the definition in
arch/x86_64/pci/mmconfig.c)

This also means that x86_64's pci_mmcfg_init() gets called in the same (new)
manner as x86's: from arch/i386/pci/init.c:pci_access_init(), rather than via
initcall.

The bitops cleanups came along for free.

All this worked OK in -mm testing (since 2.6.16-rc4-mm1) because x86_64 was
tested with both patches applied.

Zhang, Yanmin 8f860591ff [PATCH] Enable mprotect on huge pages
2.6.16-rc3 uses hugetlb on-demand paging, but it doesn_t support hugetlb
mprotect.

  Remove a test from the mprotect() path which checks that the mprotect()ed
  range on a hugepage VMA is hugepage aligned (yes, really, the sense of
  is_aligned_hugepage_range() is the opposite of what you'd guess :-/).

  In fact, we don't need this test.  If the given addresses match the
  beginning/end of a hugepage VMA they must already be suitably aligned.  If
  they don't, then mprotect_fixup() will attempt to split the VMA.  The very
  first test in split_vma() will check for a badly aligned address on a
  hugepage VMA and return -EINVAL if necessary.

  On i386 and x86-64, pte flag _PAGE_PSE collides with _PAGE_PROTNONE.  The
  identify of hugetlb pte is lost when changing page protection via mprotect.
  A page fault occurs later will trigger a bug check in huge_pte_alloc().

  The fix is to always make new pte a hugetlb pte and also to clean up
  legacy code where _PAGE_PRESENT is forced on in the pre-faulting day.

Arjan van de Ven c728252c7a [PATCH] x86/x86_64: mark rodata section read only: generic x86-64 bugfix
Bug fix required for the .rodata work on x86-64:

when change_page_attr() and friends need to break up a 2Mb page into 4Kb
pages, it always set the NX bit on the PMD, which causes the cpu to consider
the entire 2Mb region to be NX regardless of the actual PTE perms.  This is
fine in general, with one big exception: the 2Mb page that covers the last
part of the kernel .text!  The fix is to not invent a new permission for the
new PMD entry, but to just inherit the existing one minus the PSE bit.

Siddha, Suresh B f6c2e3330d [PATCH] x86_64: Unmap NULL during early bootup
We should zap the low mappings, as soon as possible, so that we can catch
kernel bugs more effectively. Previously early boot had NULL mapped
and didn't trap on NULL references.

This patch introduces boot_level4_pgt, which will always have low identity
addresses mapped.  Druing boot, all the processors will use this as their
level4 pgt.  On BP, we will switch to init_level4_pgt as soon as we enter C
code and zap the low mappings as soon as we are done with the usage of
identity low mapped addresses.  On AP's we will zap the low mappings as
soon as we jump to C code.

Tejun Heo 1426d7a81d [PATCH] vm: remove unused/broken page_pte[_prot] macros
This patch removes page_pte_prot and page_pte macros from all
architectures.  Some architectures define both, some only page_pte (broken)
and others none.  These macros are not used anywhere.

page_pte_prot(page, prot) is identical to mk_pte(page, prot) and
page_pte(page) is identical to page_pte_prot(page, __pgprot(0)).

* The following architectures define both page_pte_prot and page_pte

  arm, arm26, ia64, sh64, sparc, sparc64

* The following architectures define only page_pte (broken)

  frv, i386, m32r, mips, sh, x86-64

* All other architectures define neither

Zachary Amsden 61e06037e7 [PATCH] x86_64: avoid some atomic operations during address space destruction
Any architecture that has hardware updated A/D bits that require
synchronization against other processors during PTE operations can benefit
from doing non-atomic PTE updates during address space destruction.
Originally done on i386, now ported to x86_64.

Doing a read/write pair instead of an xchg() operation saves the implicit
lock, which turns out to be a big win on 32-bit (esp w PAE).

Adam Litke 32e51a8c97 [PATCH] hugetlb: add pte_huge() macro

David Gibson 63551ae0fe [PATCH] Hugepage consolidation
A lot of the code in arch/*/mm/hugetlbpage.c is quite similar.  This patch
attempts to consolidate a lot of the code across the arch's, putting the
combined version in mm/hugetlb.c.  There are a couple of uglyish hacks in
order to covert all the hugepage archs, but the result is a very large
reduction in the total amount of code.  It also means things like hugepage
lazy allocation could be implemented in one place, instead of six.

Tested, at least a little, on ppc64, i386 and x86_64.

Notes:
	- this patch changes the meaning of set_huge_pte() to be more
	  analagous to set_pte()
	- does SH4 need s special huge_ptep_get_and_clear()??

Linus Torvalds 1da177e4c3 Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
