Virtio-net driver control queue implementation is not safe
when used with VDUSE. If the VDUSE application does not
reply to control queue messages, it currently ends up
hanging the kernel thread sending this command.
Some work is on-going to make the control queue
implementation robust with VDUSE. Until it is completed,
let's fail features check if control-queue feature is
requested.

This patch is preliminary work to enable network device
type support to VDUSE.
As VIRTIO_BLK_F_CONFIG_WCE shares the same value as
VIRTIO_NET_F_HOST_TSO4, we need to restrict its check
to Virtio-blk device type.

The MLX driver was not updating its control virtqueue size at set_vq_num
and instead always initialized to MLX5_CVQ_MAX_ENT (16) at
setup_cvq_vring.
Qemu would try to set the size to 64 by default, however, because the
CVQ size always was initialized to 16, an error would be thrown when
sending >16 control messages (as used-ring entry 17 is initialized to 0).
For example, starting a guest with x-svq=on and then executing the
following command would produce the error below:

emitted. It is not required to clear the mr so remove the memset call.

The IRQ injection work used spin_lock_irq() to protect the
scheduling of the softirq, but spin_lock_bh() should be
used.
With spin_lock_irq(), we noticed delay of more than 6
seconds between the time a NAPI polling work is scheduled
and the time it is executed.

Starting from an used_idx different than 0 is needed in use cases like
virtual machine migration. Not doing so and letting the caller set an
avail idx different than 0 causes destination device to try to use old
buffers that source driver already recover and are not available
anymore.
Since vdpa_sim does not support receive inflight descriptors as a
destination of a migration, let's set both avail_idx and used_idx the
same at vq start. This is how vhost-user works in a
VHOST_SET_VRING_BASE call.
Although the simple fix is to set last_used_idx at vdpasim_set_vq_state,
it would be reset at vdpasim_queue_ready. The last_avail_idx case is
fixed with commit 0e84f918fa ("vdpa_sim: not reset state in
vdpasim_queue_ready"). Since the only option is to make it equal to
last_avail_idx, adding the only change needed here.
This was discovered and tested live migrating the vdpa_sim_net device.

vdpasim_queue_ready calls vringh_init_iotlb, which resets split indexes.
But it can be called after setting a ring base with
vdpasim_set_vq_state.
Fix it by stashing them. They're still resetted in vdpasim_vq_reset.
This was discovered and tested live migrating the vdpa_sim_net device.

Add a limit to 'config->vq_num' which is user controlled data which
comes from an vduse_ioctl to prevent large memory allocations.
Micheal says - This limit is somewhat arbitrary.
However, currently virtio pci and ccw are limited to a 16 bit vq number.
While MMIO isn't it is also isn't used with lots of VQs due to
current lack of support for per-vq interrupts.
Thus, the 0xffff limit on number of VQs corresponding
to a 16-bit VQ number seems sufficient for now.
This is found using static analysis with smatch.

When we initialize vringh, we should pass the features and the
number of elements in the virtqueue negotiated with the driver,
otherwise operations with vringh may fail.
This was discovered in a case where the driver sets a number of
elements in the virtqueue different from the value returned by
.get_vq_num_max().
In vdpasim_vq_reset() is safe to initialize the vringh with
default values, since the virtqueue will not be used until
vdpasim_queue_ready() is called again.

If the VDUSE application provides a smaller config space
than the driver expects, the driver may use uninitialized
memory from the stack.
This patch prevents it by initializing the buffer passed by
the driver to store the config value.
This fix addresses CVE-2022-2308.

vduse devices are not backed by any real devices such as PCI. Hence it
doesn't have any parent device linked to it.
Kernel driver model in [1] suggests to avoid an empty device
release callback.
Hence tie the mgmtdevice object's life cycle to an allocate dummy struct
device instead of static one.

Currently, CVQ vringh is initialized inside setup_virtqueues() which is
called every time a memory update is done. This is undesirable since it
resets all the context of the vring, including the available and used
indices.
Move the initialization to mlx5_vdpa_set_status() when
VIRTIO_CONFIG_S_DRIVER_OK is set.

If mlx5_vdpa gets unloaded while a VM is running, the workqueue will be
destroyed. However, vhost might still have reference to the kick
function and might attempt to push new works. This could lead to null
pointer dereference.
To fix this, set mvdev->wq to NULL just before destroying and verify
that the workqueue is not NULL in mlx5_vdpa_kick_vq before attempting to
push a new work.

A userspace triggerable infinite loop could happen in
mlx5_cvq_kick_handler() if userspace keeps sending a huge amount of
cvq requests.
Fixing this by introducing a quota and re-queue the work if we're out
of the budget (currently the implicit budget is one) . While at it,
using a per device work struct to avoid on demand memory allocation
for cvq.

Add code to register to hardware asynchronous events. Use this
mechanism to track link status events coming from the device and update
the config struct.
After doing link status change, call the vdpa callback to notify of the
link status change.

Per VIRTIO v1.1 specification, section 5.1.3.1 Feature bit requirements:
"VIRTIO_NET_F_MQ Requires VIRTIO_NET_F_CTRL_VQ".
There's assumption in the mlx5_vdpa multiqueue code that MQ must come
together with CTRL_VQ. However, there's nowhere in the upper layer to
guarantee this assumption would hold. Were there an untrusted driver
sending down MQ without CTRL_VQ, it would compromise various spots for
e.g. is_index_valid() and is_ctrl_vq_idx(). Although this doesn't end
up with immediate panic or security loophole as of today's code, the
chance for this to be taken advantage of due to future code change is
not zero.
Harden the crispy assumption by failing the set_driver_features() call
when seeing (MQ && !CTRL_VQ). For that end, verify_min_features() is
renamed to verify_driver_features() to reflect the fact that it now does
more than just validate the minimum features. verify_driver_features()
is now used to accommodate various checks against the driver features
for set_driver_features().

When control vq receives a VIRTIO_NET_CTRL_MQ_VQ_PAIRS_SET command
request from the driver, presently there is no validation against the
number of queue pairs to configure, or even if multiqueue had been
negotiated or not is unverified. This may lead to kernel panic due to
uninitialized resource for the queues were there any bogus request
sent down by untrusted driver. Tie up the loose ends there.

This commit fixes a misuse of virtio-net device config size issue
for virtio-block devices.
A new member config_size in struct ifcvf_hw is introduced and would
be initialized through vdpa_dev_add() to record correct device
config size.
To be more generic, rename ifcvf_hw.net_config to ifcvf_hw.dev_config,
the helpers ifcvf_read/write_net_config() to ifcvf_read/write_dev_config()

This condition checks "len" but it does not check "offset" and that
could result in an out of bounds read if "offset > dev->config_size".
The problem is that since both variables are unsigned the
"dev->config_size - offset" subtraction would result in a very high
unsigned value.
I think these checks might not be necessary because "len" and "offset"
are supposed to already have been validated using the
vhost_vdpa_config_validate() function. But I do not know the code
perfectly, and I like to be safe.

The "config.offset" comes from the user. There needs to a check to
prevent it being out of bounds. The "config.offset" and
"dev->config_size" variables are both type u32. So if the offset if
out of bounds then the "dev->config_size - config.offset" subtraction
results in a very high u32 value. The out of bounds offset can result
in memory corruption.

Cited patch in the fixes tag clears the features bit during reset.
mlx5 vdpa device feature bits are static decided by device capabilities.
These feature bits (including VIRTIO_NET_F_MAC) are initialized during
device addition time.
Clearing features bit in reset callback cleared the VIRTIO_NET_F_MAC. Due
to this, MAC address provided by the device is not honored.
Fix it by not clearing the static feature bits during reset.

The interrupt might be triggered after a reset since there is
no synchronization between resetting and irq injecting. And it
might break something if the interrupt is delayed until a new
round of device initialization.

The interrupt callback should not be triggered before DRIVER_OK
is set. Otherwise, it might break the virtio device driver.

We should cleanup the old kernel states e.g. interrupt callback
no matter whether the userspace handle the reset correctly or not
since virtio-vdpa can't handle the reset failure now.
Otherwise, the old state might be used after reset which might
break something, e.g. the old interrupt callback might be triggered
by userspace after reset, which can break the virtio device driver.

This VDUSE driver enables implementing software-emulated vDPA
devices in userspace. The vDPA device is created by
ioctl(VDUSE_CREATE_DEV) on /dev/vduse/control. Then a char device
interface (/dev/vduse/$NAME) is exported to userspace for device
emulation.
In order to make the device emulation more secure, the device's
control path is handled in kernel. A message mechnism is introduced
to forward some dataplane related control messages to userspace.
And in the data path, the DMA buffer will be mapped into userspace
address space through different ways depending on the vDPA bus to
which the vDPA device is attached. In virtio-vdpa case, the MMU-based
software IOTLB is used to achieve that. And in vhost-vdpa case, the
DMA buffer is reside in a userspace memory region which can be shared
to the VDUSE userspace processs via transferring the shmfd.

This implements an MMU-based software IOTLB to support mapping
kernel dma buffer into userspace dynamically. The basic idea
behind it is treating MMU (VA->PA) as IOMMU (IOVA->PA). The
software IOTLB will set up MMU mapping instead of IOMMU mapping
for the DMA transfer so that the userspace process is able to
use its virtual address to access the dma buffer in kernel.
To avoid security issue, a bounce-buffering mechanism is
introduced to prevent userspace accessing the original buffer
directly which may contain other kernel data. During the mapping,
unmapping, the software IOTLB will copy the data from the original
buffer to the bounce buffer and back, depending on the direction
of the transfer. And the bounce-buffer addresses will be mapped
into the user address space instead of the original one.

This patch introduces an attribute for vDPA device to indicate
whether virtual address can be used. If vDPA device driver set
it, vhost-vdpa bus driver will not pin user page and transfer
userspace virtual address instead of physical address during
DMA mapping. And corresponding vma->vm_file and offset will be
also passed as an opaque pointer.
