The only actual user of qdio.no_input_queues is qeth_qdio_establish(),
and there we already have full awareness of the current Input Queue
configuration (1 RX queue, plus potentially 1 TX Completion queue).
So avoid this state tracking, and the ambiguity it brings with it.

For none of the users we are under risk of running in HW IRQ context or
or with IRQs disabled. Thus we always end up in consume_skb().
But the two occurences in the RX path should really report the dropped
packet to dropmon, so have them use kfree_skb() instead. That's also
consistent with what napi_free_frags() does internally.

qdio.ko no longer needs to care about how the QAOBs are allocated,
from its perspective they are merely another parameter to do_QDIO().
So for a start, shift the cache into the only qdio driver that uses
QAOBs (ie. qeth). Here there's further opportunity to optimize its
usage in the future - eg. make it per-{device, TX queue}, or only
compile it when the driver is built with CQ/QAOB support.

With commit 18787eeebd ("qeth: use ndo_siocdevprivate") this callback
is now actually used to handle transport mode-specific _private_ ioctls.
We only have such ioctls for L3 devices. So wire up a L3-specific
.ndo_siocdevprivate() callback that handles those ioctls, and defers to
the core qeth_siocdevprivate() for all other private ioctls.
This takes the discipline one step closer to its original purpose of
providing an internal extension for the qeth_core_ccwgroup_driver.

Commit 406f42fa0d ("net-next: When a bond have a massive amount
of VLANs...") introduced a rbtree for faster Ethernet address look
up. To maintain netdev->dev_addr in this tree we need to make all
the writes to it got through appropriate helpers.
Make sure local references to netdev->dev_addr are constant.

__dasd_device_check_path_events() calls the discipline path event handler.
This handler can leave the 'to be verified pathmask' populated for an
additional verification.
There is a race window where the worker has finished before
dasd_path_clear_all_verify() is called which resets the tbvpm.
Due to this there could be outstanding path verifications missed.
Fix by clearing the pathmasks before calling the handler and add them
again in case of an error.

dasd_eckd_path_available_action() does a memory allocation to store
the per path configuration data permanently.
In the unlikely case that this allocation fails there is no conf_data
stored for the corresponding path.
This is OK since this is not necessary for an operational path but some
features like control unit initiated reconfiguration (CUIR) do not work.
To fix this add the path to the 'to be verified pathmask' again and
schedule the handler again.

Replace the blk_poll interface that requires the caller to keep a queue
and cookie from the submissions with polling based on the bio.
Polling for the bio itself leads to a few advantages:
- the cookie construction can made entirely private in blk-mq.c
- the caller does not need to remember the request_queue and cookie
separately and thus sidesteps their lifetime issues
- keeping the device and the cookie inside the bio allows to trivially
support polling BIOs remapping by stacking drivers
- a lot of code to propagate the cookie back up the submission path can
be removed entirely.

The cio_ignore list is used to create and maintain the list of devices
which is to be ignored by Linux. During boot-time, this list is adjusted
and accommodate all the devices which are configured on the HMC
interface. Once these devices are accessible, they are then available to
Linux and set online.
cio_ignore purge function should align with this functionality. But
currently, the subchannel associated with the offline-devices are not
unregistered during purge. Add an explicit subchannel-unregister function
in the purge_fn callback.

Currently when removing a device from cio_ignore list, we trigger a
path-verification for all the subchannels available in the system. This
could lead to path-verification requests on subchannels with an online
device, which could cause unwanted delay. Instead of all the
subchannels, trigger the path-verifications to those without an online
device.

Without this call an xarray entry is leaked when the vfio_ap device is
unprobed. It was missed when the below patch was rebased across the

Commit 0b9902c1fc ("s390/qeth: fix deadlock during recovery") removed
taking discipline_mutex inside qeth_do_reset(), fixing potential
deadlocks. An error path was missed though, that still takes
discipline_mutex and thus has the original deadlock potential.
Intermittent deadlocks were seen when a qeth channel path is configured
offline, causing a race between qeth_do_reset and ccwgroup_remove.
Call qeth_set_offline() directly in the qeth_do_reset() error case and
then a new variant of ccwgroup_set_offline(), without taking
discipline_mutex.

Problem: qeth_close_dev_handler is a worker that tries to acquire
card->discipline_mutex via drv->set_offline() in ccwgroup_set_offline().
Since commit b41b554c1e
("s390/qeth: fix locking for discipline setup / removal")
qeth_remove_discipline() is called under card->discipline_mutex and
cancels the work and waits for it to finish.
STOPLAN reception with reason code IPA_RC_VEPA_TO_VEB_TRANSITION is the
only situation that schedules close_dev_work. In that situation scheduling
qeth recovery will also result in an offline interface, when resetting the
isolation mode fails, if the external switch is still set to VEB.
And since commit 0b9902c1fc ("s390/qeth: fix deadlock during recovery")
qeth recovery does not aquire card->discipline_mutex anymore.
So we accept the longer pathlength of qeth_schedule_recovery in this
error situation and re-use the existing function.
As a side-benefit this changes the hwtrap to behave like during recovery
instead of like during a user-triggered set_offline.

When qeth_set_online() calls qeth_clear_working_pool_list() to roll
back after an error exit from qeth_hardsetup_card(), we are at risk of
accessing card->qdio.in_q before it was allocated by
qeth_alloc_qdio_queues() via qeth_mpc_initialize().
qeth_clear_working_pool_list() then dereferences NULL, and by writing to
queue->bufs[i].pool_entry scribbles all over the CPU's lowcore.
Resulting in a crash when those lowcore areas are used next (eg. on
the next machine-check interrupt).
Such a scenario would typically happen when the device is first set
online and its queues aren't allocated yet. An early IO error or certain
misconfigs (eg. mismatched transport mode, bad portno) then cause us to
error out from qeth_hardsetup_card() with card->qdio.in_q still being
NULL.
Fix it by checking the pointer for NULL before accessing it.
Note that we also have (rare) paths inside qeth_mpc_initialize() where
a configuration change can cause us to free the existing queues,
expecting that subsequent code will allocate them again. If we then
error out before that re-allocation happens, the same bug occurs.

- Fix stack unwinding so that the correct number of entries are omitted like
expected by common code. This fixes KCSAN selftests.

Support for expanded storage was only available until z13 and z/VM 6.3
respectively. However there haven't been any use cases a long time
before for this device driver.

This series consists of the usual driver updates (ufs, qla2xxx,
target, smartpqi, lpfc, mpt3sas). The core change causing the most
churn was replacing the command request field request with a macro,
allowing us to offset map to it and remove the redundant field; the
same was also done for the tag field. The most impactful change is
the final removal of scsi_ioctl, which has been deprecated for over a
decade.

- changed the bus remove callback to return void

The latter one will cause a tiny merge issue with your tree, as there
was a last-minute fix for this in 5.14 in your tree, but the fixup
should be "obvious". If you want me to provide a fixed merge for this,
please let me know.
Other than those two things, there's only a few small things in here:

The devices owned by the parent-driver (css) was getting unregistered
from the io-subchannel driver is clearly a layering violation.
Remove the subchannel unregistration from the child-drivers.
This also means, if the device connected to the subchannel is not
operational, or not accessible, the subchannel will not be unregistered.
Instead the CIO layer will allow valid subchannels without any operational
devices in sysfs. And the userspace tooling might need to be modified to
optimally handle this new situation.

If for any reason the interrupt enable for an ap queue fails the
state machine run for the queue returned wrong return codes to the
caller. So the caller assumed interrupt support for this queue in
enabled and thus did not re-establish the high resolution timer used
for polling. In the end this let to a hang for the user space process
waiting "forever" for the reply.
This patch reworks these return codes to return correct indications
for the caller to re-establish the timer when a queue runs without
interrupt support.
Please note that this is fixing a wrong behavior after a first
failure (enable interrupt support for the queue) failed. However,
looks like this occasionally happens on KVM systems.

This is straightforward conversion, the ap_matrix_mdev is actually serving
as the vfio_device and we can replace all the mdev_get_drvdata()'s with a
simple container_of() or a dev_get_drvdata() for sysfs paths.

Add tracing of interactions between the SCLP base driver, firmware and
other drivers to support problem determination in case of SCLP-related
issues.
For that purpose this patch introduces two new s390dbf debug areas:
- sclp: An abbreviated log of all common interactions
- sclp_err: A full log of failed or abnormal interactions
Tracing of full SCCB contents can be enabled for the sclp area by
setting its debug level to maximum (6).

- SRV1: Service call about to be issued
- SRV2: Service call was issued
- INT: Interrupt received
* Driver interaction:

Since the SCLP interface is used by console drivers this patch also

area locks to prevent a potential deadlock that would otherwise be

The device struct provides a pointer for driver-private data. Use this
in the zcrypt drivers (as vfio_ap already does), and then remove the
custom pointer from the AP device structs.
As really_probe() will always clear the drvdata pointer on error, we
no longer have to do so ourselves.

It was pointed out during an unrelated patch review that locks should not
be open coded - i.e., writing the algorithm of a standard lock in a
function instead of using a lock from the standard library. The setting and
testing of a busy flag and sleeping on a wait_event is the same thing
a lock does. The open coded locks are invisible to lockdep, so potential
locking problems are not detected.
This patch removes the open coded locks used during
VFIO_GROUP_NOTIFY_SET_KVM notification. The busy flag
and wait queue were introduced to resolve a possible circular locking
dependency reported by lockdep when starting a secure execution guest
configured with AP adapters and domains. Reversing the order in which
the kvm->lock mutex and matrix_dev->lock mutex are locked resolves the
issue reported by lockdep, thus enabling the removal of the open coded
locks.

The function pointer to the interception handler for the PQAP instruction
can get changed during the interception process. Let's add a
semaphore to struct kvm_s390_crypto to control read/write access to the
function pointer contained therein.
The semaphore must be locked for write access by the vfio_ap device driver
when notified that the KVM pointer has been set or cleared. It must be
locked for read access by the interception framework when the PQAP
instruction is intercepted.

Most drivers should use and have been converted to use blk_alloc_disk
and blk_mq_alloc_disk. Only the scsi ULPs and dasd still allocate
a disk separately from the request_queue, so don't bother with
convenience macros for something that should not see significant
new users and remove these wrappers.

The memory block occupied by the SCLP early buffer that is allocated
by the decompressor and then handed over to the decompressed kernel,
must be reserved to prevent it from being reused for other purposes.
This is necessary because the SCLP early buffer is still in use
during kernel initialization.

Tests showed a mismatch between what the CCA tool reports about
the APKA master key state and what's displayed by the zcrypt dd
in sysfs. After some investigation, we found out that the
documentation which was the source for the zcrypt dd implementation
lacks the listing of 3 fields. So this patch now moves the
evaluation of the APKA master key state to the correct offset.

Move all QIB-related code into qdio_setup_qib(), and slightly re-order
it according to the order of the struct's fields. This makes it easier
to understand what the QIB actually looks like before we send it to HW.
Also get rid of the qebsm_possible() helper - as 31-bit support is long
gone, the comment doesn't make any sense. And while removing some stale
QIB-related comment, also move the clearing of the QDR into its proper
place.

Push the sync check from qdio_inspect_queue() down into the two
get_*_buffer_frontier() code paths, where we actually need the sync to
look at the current queue state. This lets us avoid the check when we
know that there is no work on the queue (ie. when q->nr_buf_used is 0).
While at it introduce the qdio_sync_*_queue() helpers, so that we can
avoid the branch on q->is_input_q when we already know the queue type.

Don't bother with translating the SIGA-related capability bits into
our own internal format, just cache the full qdioac1 field instead.
Also adjust the helper macros so that they take a qdio_irq argument
and can be used everywhere, instead of taking a qdio_q and then
internally dereferencing the parent pointer.

The queue processing is fully decoupled from any preceding interrupt,
so we're no longer making any use of the sync-after-IRQ HW capabilities.
And as SIGA-sync is a legacy feature, there's also not much point in
re-designing the driver & qdio-layer code just so that we can
potentially avoid a few syncs. So just remove all the leftover code.

Introduce dev_busid, which exports the device-id associated with the
io-subchannel (and message-subchannel). The dev_busid indicates that of
the device which may be physically installed on the corrosponding
subchannel. The dev_busid value "none" indicates that the subchannel
is not valid, there is no I/O device currently associated with the
subchannel.
The dev_busid information would be helpful to write device-specific
udev-rules associated with the subchannel. The dev_busid interface would
be available even when the sch is not bound to any driver or if there is
no operational device connected on it. Hence this attribute can be used to
write udev-rules which are specific to the device associated with the
subchannel.

This patch introduces a new rescan sys-interface for channel-subsystem.
The rescan interface allows the user to invoke an evaluation of all
subchannels defined in the I/O configuration.
The new rescan interface can be found at /sys/devices/css0/rescan
and can be triggered by,
echo > /sys/devices/css0/rescan
Writing to this interface triggers subchannel evaluation. The write
request completes only after scan-related work has completed
This user-invoked subchannel evaluation allows manual recovery in error
situations such as:
- restart of device discovery after resolution of temporary device
error
- inconsistent OS view of subchannel state due to missing state-change
interrupts (CRWs)

The user can open multiple device FDs if it likes, however these open()
functions call vfio_register_notifier() on some device global
state. Calling vfio_register_notifier() twice in will trigger a WARN_ON
from notifier_chain_register() and the first close will wrongly delete the
notifier and more.
Since these really want the new open/close_device() semantics just change
the functions over.

The blamed commit added a new field to struct switchdev_notifier_fdb_info,
but did not make sure that all call paths set it to something valid.
For example, a switchdev driver may emit a SWITCHDEV_FDB_ADD_TO_BRIDGE
notifier, and since the 'is_local' flag is not set, it contains junk
from the stack, so the bridge might interpret those notifications as
being for local FDB entries when that was not intended.
To avoid that now and in the future, zero-initialize all
switchdev_notifier_fdb_info structures created by drivers such that all
newly added fields to not need to touch drivers again.

Update the MAC addresses that are registered with a LEARNING_SYNC qeth
device with the events announced by the attached software bridge.
Typically the LEARNING_SYNC qeth bridge port has an isolated sibling (the
default interface of an 'HiperSockets Converged Interface' (HSCI)). Update
the MACs of isolated siblings as well, to avoid unnecessary flooding in
the attached virtualized switches.

QETH HiperSockets devices with LEARNING_SYNC capability can be used
to construct a linux bridge with:
2 isolated southbound interfaces:
a) a default network interface
b) a LEARNING-SYNC HiperSockets interface
and 1 non-isolated northbound interface. This is called a 'HiperSockets
Converged Interface' (HSCI).
The existing LEARNING_SYNC functionality is used to update the bridge fdb
with MAC addresses that should be sent-out via the HiperSockets interface,
instead of the default network interface.
Add handling of switchdev events SWITCHDEV_FDB_ADD_TO_DEVICE and
SWITCHDEV_FDB_DEL_TO_DEVICE to the qeth LEARNING_SYNC functionality. Thus
if the northbound bridgeport of an HSCI doesn't only have a single static
MAC address, but instead is a learning bridgeport, work is enqueued, so
the HiperSockets virtual switch (that is external to this Linux instance)
can update its fdb.
When BRIDGE is a loadable module, QETH_L2 mustn't be built-in:

The functions get_online_cpus() and put_online_cpus() have been
deprecated during the CPU hotplug rework. They map directly to
cpus_read_lock() and cpus_read_unlock().
Replace deprecated CPU-hotplug functions with the official version.
The behavior remains unchanged.

When new configuration data is obtained after a path event it is stored
in the per path array. The old data needs to be freed.
The first valid configuration data is also referenced in the device
private structure to identify the device.
When the old per path configuration data was freed the device still
pointed to the already freed data leading to a use after free.
Fix by replacing also the device configuration data with the newly
obtained one before the old data gets freed.

Most users of ndo_do_ioctl are ethernet drivers that implement
the MII commands SIOCGMIIPHY/SIOCGMIIREG/SIOCSMIIREG, or hardware
timestamping with SIOCSHWTSTAMP/SIOCGHWTSTAMP.
Separate these from the few drivers that use ndo_do_ioctl to
implement SIOCBOND, SIOCBR and SIOCWANDEV commands.
This is a purely cosmetic change intended to help readers find
their way through the implementation.

alloc_tty_driver was deprecated by tty_alloc_driver in commit
7f0bc6a68e (TTY: pass flags to alloc_tty_driver) in 2012.
I never got into eliminating alloc_tty_driver until now. So we still
have two functions for allocating drivers which might be confusing. So
get rid of alloc_tty_driver uses to eliminate it for good in the next
patch.
Note we need to switch return value checking as tty_alloc_driver uses
ERR_PTR. And flags are now a parameter of tty_alloc_driver.

Now that all drivers use qdio_inspect_queue() and qdio's internal
queue tasklets are gone, the driver-specified queue handlers are
only called for async error reporting (eg. for an error condition in
the QEBSM code).
So take a moment to clean up the Output Queue handlers (they are
_always_ called with qdio_error != 0), and clarify which error types
can be reported through what interface. As Benjamin already suggested
a while ago, we should turn these into distinct enums at some point.

If the ESTABLISH ccw fails (ie. the qdio_irq is set to
QDIO_IRQ_STATE_ERR), we don't need to call qdio_shutdown() for rolling
back our earlier actions. All the needed logic is already available in
qdio_establish()'s error chain, and using it means we don't have to
temporarily drop the setup_mutex either.
This makes qdio_shutdown() a purely external function, that should only
be called by the driver if an earlier qdio_establish() succeeded.

When the ESTABLISH ccw does not complete within the specified timeout,
try our best to cancel the ccw program that is still active on the
device. Otherwise the IO subsystem might be accessing it even after
the driver eg. called qdio_free().

When qdio_establish() times out while waiting for the ESTABLISH ccw to
complete, it calls qdio_shutdown() to roll back all of its previous
actions. But at this point the qdio_irq's state is still
QDIO_IRQ_STATE_INACTIVE, so qdio_shutdown() will exit immediately
without doing any actual work.
Which means that eg. the qdio_irq's thinint-indicator stays registered,
and cdev->handler isn't restored to its old value. And since
commit 954d6235be ("s390/qdio: make thinint registration symmetric")
the qdio_irq also stays on the tiq_list, so on the next qdio_establish()
we might get a helpful BUG from the list-debugging code:

The new boot data struct shall replace global variables OLDMEM_BASE and
OLDMEM_SIZE. It is initialized in the decompressor and passed
to the decompressed kernel. In comparison to the old solution, this one
doesn't access data at fixed physical addresses which will become important
when the decompressor becomes relocatable.

To make the decompressor relocatable, the early SCLP buffer with a fixed
address must be replaced with a relocatable C buffer of the according size
and alignment as required by SCLP.
Introduce a new function sclp_early_set_buffer() into the SCLP driver
which enables the decompressor to change the SCLP early buffer at any time.
This will be useful when the decompressor becomes fully relocatable and
might need to change the SCLP early buffer to one with an address < 2G
as required by SCLP because it was loaded at an address >= 2G.

A buffer that can be used for communication with SCLP is required
to lie below 2GB memory address. Therefore, both sclp_info_sccb
and sclp_early_sccb must fulfill this requirement if passed directly
to the sclp_early_cmd() function. Instead, use only sclp_early_sccb
for communication with SCLP. This allows the buffer sclp_info_sccb
to be placed anywhere in the memory address space and, therefore,
simplifies the process of making the decompressor relocatable later on,
one thing less to relocate. And make sure that the length of the new unified
early SCLP buffer is no less than the length of the removed sclp_info_sccb
buffer which might be larger than the length of the sclp_early_sccb buffer.

The driver core ignores the return value of this callback because there
is only little it can do when a device disappears.
This is the final bit of a long lasting cleanup quest where several
buses were converted to also return void from their remove callback.
Additionally some resource leaks were fixed that were caused by drivers
returning an error code in the expectation that the driver won't go
away.
With struct bus_type::remove returning void it's prevented that newly
implemented buses return an ignored error code and so don't anticipate
wrong expectations for driver authors.

qeth uses three device_type structs - a generic one, and one for each
sub-driver (which is used for fixed-layer devices only). Instead of
exporting these device_types back&forth between the driver's modules,
make all the logic self-contained within the sub-drivers.
On disc->setup() they either install their own device_type, or add the
sysfs attributes that are missing in the generic device_type. Later on
disc->remove() these attributes are removed again from any device that
has the generic device_type.

Commit fb64de1bc3 ("s390/qeth: phase out OSN support") spelled out
why the OSN support in qeth is in a bad shape, and put any remaining
interested parties on notice to speak up before it gets ripped out.
It's 2021 now, so make true on that promise and remove all the
OSN-specific parts from qeth. This also means that we no longer need to
export various parts of the cmd & data path internals to the L2 driver.

On remote cable pull, a zfcp_port keeps its status and only gets
ZFCP_STATUS_PORT_LINK_TEST added. Only after an ADISC timeout, we would
actually start port recovery and remove ZFCP_STATUS_COMMON_UNBLOCKED which
zfcp_sysfs_port_fc_security_show() detected and reported as "unknown"
instead of the old and possibly stale zfcp_port->connection_info.
Add check for ZFCP_STATUS_PORT_LINK_TEST for timely "unknown" report.

Rework of the ap_dqap() inline function with the dqap inline assembler
invocation and the caller code in ap_queue.c to be able to handle
replies which exceed the receive buffer size.
ap_dqap() now provides two additional parameters to handle together
with the caller the case where a reply in the firmware queue entry
exceeds the given message buffer size. It depends on the caller how to
exactly handle this. The behavior implemented now by ap_sm_recv() in
ap_queue.c is to simple purge this entry from the firmware queue and
let the caller 'receive' a -EMSGSIZE for the request without
delivering any reply data - not even a truncated reply message.
However, the reworked ap_dqap() could now get invoked in a way that
the message is received in multiple parts and the caller assembles the
parts into one reply message.

This patch provides support for new dynamic AP bus message limit
with the existing zcrypt device driver and AP bus core code.
There is support for a new field 'ml' from TAPQ query. The field
gives if != 0 the AP bus limit for this card in 4k chunk units.
The actual message size limit per card is shown as a new read-only
sysfs attribute. The sysfs attribute
/sys/devices/ap/cardxx/max_msg_size
shows the upper limit in bytes used by the AP bus and zcrypt device
driver for requests and replies send to and received from this card.
Currently up to CEX7 support only max 12kB msg size and thus the field
shows 12288 meaning the upper limit of a valid msg for this card is
12kB. Please note that the usable payload is somewhat lower and
depends on the msg type and thus the header struct which is to be
prepended by the zcrypt dd.
The dispatcher responsible for choosing the right card and queue is
aware of the individual card AP bus message limit. So a request is
only assigned to a queue of a card which is able to handle the size of
the request (e.g. a 14kB request will never go to a max 12kB card).
If no such card is found the ioctl will fail with ENODEV.
The reply buffer held by the device driver is determined by the ml
field of the TAPQ for this card. If a response from the card exceeds
this limit however, the response is not truncated but the ioctl for
this request will fail with errno EMSGSIZE to indicate that the device
driver has dropped the response because it would overflow the buffer
limit.
If the request size does not indicate to the dispatcher that an
adapter with extended limit is to be used, a random card will be
chosen when no specific card is addressed (ANY addressing). This may
result in an ioctl failure when the reply size needs an adapter with
extended limit but the randomly chosen one is not capable of handling
the broader reply size. The user space application needs to use
dedicated addressing to forward such a request only to suitable cards
to get requests like this processed properly.

kernel.h is being used as a dump for all kinds of stuff for a long time.
Here is the attempt to start cleaning it up by splitting out panic and
oops helpers.
There are several purposes of doing this:
- dropping dependency in bug.h
- dropping a loop by moving out panic_notifier.h
- unload kernel.h from something which has its own domain
At the same time convert users tree-wide to use new headers, although for
the time being include new header back to kernel.h to avoid twisted
indirected includes for existing users.

In preparation for FORTIFY_SOURCE performing compile-time and run-time
field array bounds checking for memcpy(), memmove(), and memset(),
avoid intentionally reading across neighboring array fields.
Add a wrapping structure to serve as the memcpy() source, so the compiler
can do appropriate bounds checking, avoiding this future warning:

Using register asm statements has been proven to be very error prone,
especially when using code instrumentation where gcc may add function
calls, which clobbers register contents in an unexpected way.
Therefore get rid of register asm statement in dasd code, even though
there is currently nothing wrong with it. This way we know for sure
that the above mentioned bug class won't be introduced here.

The mdev remove callback for the vfio_ap device driver bails out with
-EBUSY if the mdev is in use by a KVM guest (i.e., the KVM pointer in the
struct ap_matrix_mdev is not NULL). The intended purpose was
to prevent the mdev from being removed while in use. There are two
problems with this scenario:
1. Returning a non-zero return code from the remove callback does not
prevent the removal of the mdev.
2. The KVM pointer in the struct ap_matrix_mdev will always be NULL because
the remove callback will not get invoked until the mdev fd is closed.
When the mdev fd is closed, the mdev release callback is invoked and
clears the KVM pointer from the struct ap_matrix_mdev.
Let's go ahead and remove the check for KVM in the remove callback and
allow the cleanup of mdev resources to proceed.

Currently css_wait_for_slow_path() gets called inside the chp->lock.
The path-verification-loop of slowpath inside this lock could lead to
deadlock as reported by the lockdep validator.
The ccw_device_get_chp_desc() during the instance of a device-set-online
would try to acquire the same 'chp->lock' to read the chp->desc.
The instance of this function can get called from multiple scenario,
like probing or setting-device online manually. This could, in some
corner-cases lead to the deadlock.

The chp->lock was introduced to serialize the access of struct

When a AP queue is switched to soft offline, all pending
requests are purged out of the pending requests list and
'received' by the upper layer like zcrypt device drivers.
This is also done for requests which are already enqueued
into the firmware queue. A request in a firmware queue
may eventually produce an response message, but there is
no waiting process any more. However, the response was
counted with the queue_counter and as this counter was
reset to 0 with the offline switch, the pending response
caused the queue_counter to get negative. The next request
increased this counter to 0 (instead of 1) which caused
the ap code to assume there is nothing to receive and so
the response for this valid request was never tried to
fetch from the firmware queue.
This all caused a queue to not work properly after a
switch offline/online and in the end processes to hang
forever when trying to send a crypto request after an
queue offline/online switch cicle.
Fixed by a) making sure the counter does not drop below 0
and b) on a successful enqueue of a message has at least
a value of 1.
Additionally a warning is emitted, when a reply can't get
assigned to a waiting process. This may be normal operation
(process had timeout or has been killed) but may give a
hint that something unexpected happened (like this odd
behavior described above).

Without the SWITCHDEV module, the bridgeport attribute LEARNING_SYNC
of the physical device (self) does not provide any functionality.
Instead of calling the no-op stub version of the switchdev functions,
fail the setting of the attribute with an appropriate message.
While at it, also add an error message for the 'not supported by HW'
case.

Maintaining a pointer inside the aob's user-definable area is fragile
and unnecessary. At this stage we only need it to overload the buffer's
state field, and to access the buffer's TX queue.
The first part is easily solved by tracking the aob's state within the
aob itself. This also feels much cleaner and self-contained.
For enabling the access to the associated TX queue, we can store the
queue's index in the aob.

With commit 396c100472 ("s390/qdio: let driver manage the QAOB")
a pending TX buffer now has access to its associated QAOB during
TX completion processing. We can thus reduce the amount of work & state
propagation that needs to be done by qeth_qdio_handle_aob().
Move all this logic into the respective TX completion paths. Doing so
even allows us to determine more precise TX_NOTIFY_* values via
qeth_compute_cq_notification(aob->aorc, ...).

We have one field to track _whether_ a cmd is active on a ccw device
('irq_pending'), and one to track _which_ cmd it is ('active_cmd').

Set scan_threshold = 0 to opt out from the qdio layer's internal tasklet
& timer mechanism for TX completions, and replace it with the TX NAPI
infrastructure that qeth already uses for IQD devices. This avoids the
fragile logic in qdio_check_output_queue(), enables tighter integration
and gives us more tuning options via ethtool in the future.
For now we continue to apply the same policy as the qdio layer:
scan for completions if 32 TX buffers are in use, or after 1 sec.
A re-scan is done after 10 sec, but only if no TX interrupt is pending.
With scan_threshold = 0 we no longer get TX completion scans from
within qdio_get_next_buffers(). So trigger these manually in qeth_poll()
and in the RX path switch to the equivalent qdio_inspect_queue().

For future work we need the struct tpi_info declaration in asm/ptrace.h.
Due to circular dependencies it cannot stay in asm/lowcore.h or asm/cio.h,
which would be the preferred location. Therefore add it in its own header
file.
Also fix a typo in the length of a reserved field that did not have a
functional effect beyond an incorrect field value in the s390_cio_tpi
tracepoint.

ccwgroup_notifier() currently listens for BUS_NOTIFY_UNBIND_DRIVER
events, and triggers an ungrouping for the affected device.
Looking at __device_release_driver(), we can wait for a little longer
until the driver has been fully unbound and eg. bus->remove() has been
called. So listen for BUS_NOTIFY_UNBOUND_DRIVER instead. Due to locking
the current code should work just fine, but this clarifies our intent.

driver_unregister() ends up calling __device_release_driver() for
each device that is bound to the driver. Thus ccwgroup_notifier() will
see a BUS_NOTIFY_UNBIND_DRIVER event for these ccwgroup devices, and
trigger the ungrouping.
So there's no need to also trigger the ungrouping from within
ccwgroup_driver_unregister(), remove it.

Userspace udev rules need an indication about the very first AP change
BINDINGS=complete uevent.
So now this uevent is extend with an additional key-value entry
COMPLETECOUNT=<counter>. The very first uevent will show counter=1 and
the following BINDINGS=complete uevents increase this counter by 1.
Here is an example how the very first BINDINGS=complete uevent
looks like:

DEVPATH=/devices/ap

Wiring up the cdevs is an essential part of the gdev creation. So
release them during the gdev destruction, ie. on the last put_device().
This could cause us to hold on to the cdevs a tiny bit longer, but
that's not a real concern.
As we have much less certainty of what context this put_device() is
executed from, switch to irqsave locking.

Hijacking the device_driver's probe/remove callbacks for purely
bus-internal logic is a very unconvential construct. Instead just set
up our callbacks in the AP bus struct, and really_probe() will call them
in the same way as before.

Fix crash with illegal operation exception in dasd_device_tasklet.
Commit b729493288 ("s390/dasd: Prepare for additional path event handling")
renamed the verify_path function for ECKD but not for FBA and DIAG.
This leads to a panic when the path verification function is called for a
FBA or DIAG device.
Fix by defining a wrapper function for dasd_generic_verify_path().

tty_operations::flush_buffer is optional. If it doesn't exist, nothing
is called. So remove almost¹ empty flush_buffer implementations.
