Use pageref->offset instead of page->index for deferred-I/O writeback
where appropriate. Distinguishes between file-mapping offset and video-
memory offset. While at it, also remove unnecessary references to
struct page.
Fbdev's deferred-I/O code uses the two related page->index and
pageref->offset. The former is the page offset in the mapped file,
the latter is the byte offset in the video memory (or fbdev screen
buffer). It's the same value for fbdev drivers, but for DRM the values
can be different. Because GEM buffer objects are mapped at an offset
in the DRM device file, page->index has this offset added to it as well.
We currently don't hit this case in DRM, because all affected mappings
of GEM memory are performed with an internal, intermediate shadow buffer.
The value of page->index is required by page_mkclean(), which we
call to reset the mappings during the writeback phase of the deferred
I/O. The value of pageref->offset is for conveniently getting an offset
into video memory in fb helpers.

Rename various instances of pagelist to pagereflist. The list now
stores pageref structures, so the new name is more appropriate.
In their write-back helpers, several fbdev drivers refer to the
pageref list in struct fb_deferred_io instead of using the one
supplied as argument to the function. Convert them over to the
supplied one. It's the same instance, so no change of behavior
occurs.

Store the per-page state for fbdev's deferred I/O in struct
fb_deferred_io_pageref. Maintain a list of pagerefs for the pages
that have to be written back to video memory. Update all affected
drivers.
As with pages before, fbdev acquires a pageref when an mmaped page
of the framebuffer is being written to. It holds the pageref in a
list of all currently written pagerefs until it flushes the written
pages to video memory. Writeback occurs periodically. After writeback
fbdev releases all pagerefs and builds up a new dirty list until the
next writeback occurs.
Using pagerefs has a number of benefits.
For pages of the framebuffer, the deferred I/O code used struct
page.lru as an entry into the list of dirty pages. The lru field is
owned by the page cache, which makes deferred I/O incompatible with
some memory pages (e.g., most notably DRM's GEM SHMEM allocator).
struct fb_deferred_io_pageref now provides an entry into a list of
dirty framebuffer pages, freeing lru for use with the page cache.
Drivers also assumed that struct page.index is the page offset into
the framebuffer. This is not true for DRM buffers, which are located
at various offset within a mapped area. struct fb_deferred_io_pageref
explicitly stores an offset into the framebuffer. struct page.index
is now only the page offset into the mapped area.
These changes will allow DRM to use fbdev deferred I/O without an
intermediate shadow buffer.

The fbdev mmap function fb_mmap() unconditionally overrides the
driver's implementation if deferred I/O has been activated. This
makes it hard to implement mmap with anything but a vmalloc()'ed
software buffer. That is specifically a problem for DRM, where
video memory is maintained by a memory manager.
Leave the mmap handling to drivers and expect them to call the
helper for deferred I/O by thmeselves.

Fbdev's deferred I/O sorts all dirty pages by default, which incurs a
significant overhead. Make the sorting step optional and update the few
drivers that require it. Use a FIFO list by default.
Most fbdev drivers with deferred I/O build a bounding rectangle around
the dirty pages or simply flush the whole screen. The only two affected
DRM drivers, generic fbdev and vmwgfx, both use a bounding rectangle.
In those cases, the exact order of the pages doesn't matter. The other
drivers look at the page index or handle pages one-by-one. The patch
sets the sort_pagelist flag for those, even though some of them would
probably work correctly without sorting. Driver maintainers should update
their driver accordingly.
Sorting pages by memory offset for deferred I/O performs an implicit
bubble-sort step on the list of dirty pages. The algorithm goes through
the list of dirty pages and inserts each new page according to its
index field. Even worse, list traversal always starts at the first
entry. As video memory is most likely updated scanline by scanline, the
algorithm traverses through the complete list for each updated page.
For example, with 1024x768x32bpp each page covers exactly one scanline.
Writing a single screen update from top to bottom requires updating
768 pages. With an average list length of 384 entries, a screen update
creates (768 * 384 =) 294912 compare operation.
Fix this by making the sorting step opt-in and update the few drivers
that require it. All other drivers work with unsorted page lists. Pages
are appended to the list. Therefore, in the common case of writing the
framebuffer top to bottom, pages are still sorted by offset, which may
have a positive effect on performance.
Playing a video [1] in mplayer's benchmark mode shows the difference
(i7-4790, FullHD, simpledrm, kernel with debugging).
mplayer -benchmark -nosound -vo fbdev ./big_buck_bunny_720p_stereo.ogg
With sorted page lists:
BENCHMARKs: VC: 32.960s VO: 73.068s A: 0.000s Sys: 2.413s = 108.441s
BENCHMARK%: VC: 30.3947% VO: 67.3802% A: 0.0000% Sys: 2.2251% = 100.0000%
With unsorted page lists:
BENCHMARKs: VC: 31.005s VO: 42.889s A: 0.000s Sys: 2.256s = 76.150s
BENCHMARK%: VC: 40.7156% VO: 56.3219% A: 0.0000% Sys: 2.9625% = 100.0000%
VC shows the overhead of video decoding, VO shows the overhead of the
video output. Using unsorted page lists reduces the benchmark's run time
by ~32s/~25%.

Rendering calls may be done simultaneously from the workqueue,
dlfb_ops_write, dlfb_ops_ioctl, dlfb_ops_set_par and dlfb_dpy_deferred_io.
The code is robust enough so that it won't crash on concurrent rendering.
However, concurrent rendering may cause display corruption if the same
pixel is simultaneously being rendered. In order to avoid this corruption,
this patch adds a mutex around the rendering calls.

If a framebuffer device is used as a console, the rendering calls
(copyarea, fillrect, imageblit) may be done with the console spinlock
held. On udlfb, these function call dlfb_handle_damage that takes a
blocking semaphore before acquiring an URB.
In order to fix the bug, this patch changes the calls copyarea, fillrect
and imageblit to offload USB work to a workqueue.
A side effect of this patch is 3x improvement in console scrolling speed
because the device doesn't have to be updated after each copyarea call.

In the current kernel, then kzalloc() can't fail for small allocations,
but if it did fail then we would have a NULL dereference in the error

would cause an Oops inside the unregister_framebuffer() function but it
can't be NULL so let's remove that check.

The udlfb driver maintained an open count and cleaned up itself when the
count reached zero. But the console is also counted in the reference count
- so, if the user unplugged the device, the open count would not drop to
zero and the driver stayed loaded with console attached. If the user
re-plugged the adapter, it would create a device /dev/fb1, show green
screen and the access to the console would be lost.
The framebuffer subsystem has reference counting on its own - in order to
fix the unplug bug, we rely the framebuffer reference counting. When the
user unplugs the adapter, we call unregister_framebuffer unconditionally.
unregister_framebuffer will unbind the console, wait until all users stop
using the framebuffer and then call the fb_destroy method. The fb_destroy
cleans up the USB driver.
This patch makes the following changes:
* Drop dlfb->kref and rely on implicit framebuffer reference counting
instead.
* dlfb_usb_disconnect calls unregister_framebuffer, the rest of driver
cleanup is done in the function dlfb_ops_destroy. dlfb_ops_destroy will
be called by the framebuffer subsystem when no processes have the
framebuffer open or mapped.
* We don't use workqueue during initialization, but initialize directly
from dlfb_usb_probe. The workqueue could race with dlfb_usb_disconnect
and this racing would produce various kinds of memory corruption.
* We use usb_get_dev and usb_put_dev to make sure that the USB subsystem
doesn't free the device under us.

as interrupt handlers). From a process context, we should call
spin_lock_irq and spin_unlock_irq, that avoids the costly pushf and popf
instructions.

Currently, the udlfb driver only tests for identical bytes at the
beginning or at the end of a page and renders anything between the first
and last mismatching pixel. But pages are not the same as lines, so this
is quite suboptimal - if there is something modified at the beginning of a
page and at the end of a page, the whole page is rendered, even if most of
the page is not modified.
This patch makes it test for identical pixels at the beginning and end of
each rendering command. This patch improves identical byte detection by
41% when playing video in a window.
This patch also fixes a possible screen corruption if the user is writing
to the framebuffer while dlfb_render_hline is in progress - the pixel data
that is copied to the backbuffer with memcpy may be different from the
pixel data that is actually rendered to the hardware (because the content
of the framebuffer may change between memcpy and the rendering command).
We must make sure that we copy exactly the same pixel as the pixel that is
being rendered.

This patch changes udlfb so that it may reallocate the framebuffer when
setting higher-resolution mode. If we boot the system without monitor
attached, udlfb creates a framebuffer with the size 800x600. This patch
makes it possible to select higher videomode with the fbset command when
a monitor is attached.
Note that there is no reliable way to prevent the system from touching the
old framebuffer, so we must not free it. We add it to the list
dlfb->deferred_free and free it when the driver is unloaded.

The defio subsystem overwrites the method fb_osp->mmap. That method is
stored in module's static data - and that means that if we have multiple
diplaylink adapters, they will over write each other's method.
In order to avoid interference between multiple adapters, we copy the
fb_ops structure to a device-local memory.

The udlfb driver reprograms the hardware everytime the user switches the
console, that makes quite unusable when working on the console.
This patch makes the driver remember the videomode we are in and avoid
reprogramming the hardware if we switch to the same videomode.
We mask the "activate" field and the "FB_VMODE_SMOOTH_XPAN" flag when
comparing the videomode, because they cause spurious switches when
switching to and from the Xserver.

The displaylink hardware has such a peculiarity that it doesn't render a
command until next command is received. This produces occasional
corruption, such as when setting 22x11 font on the console, only the first
line of the cursor will be blinking if the cursor is located at some
specific columns.
When we end up with a repeating pixel, the driver has a bug that it leaves
one uninitialized byte after the command (and this byte is enough to flush
the command and render it - thus it fixes the screen corruption), however
whe we end up with a non-repeating pixel, there is no byte appended and
this results in temporary screen corruption.
This patch fixes the screen corruption by always appending a byte 0xAF at
the end of URB. It also removes the uninitialized byte.

I observed that the performance of the udl fb driver degrades over time.
On a freshly booted machine, it takes 6 seconds to do "ls -la /usr/bin";
after some time of use, the same operation takes 14 seconds.
The reason is that the value of "limit_sem" decays over time.
The udl driver uses a semaphore "limit_set" to specify how many free urbs
are there on dlfb->urbs.list. If the count is zero, the "down" operation
will sleep until some urbs are added to the freelist.
In order to avoid some hypothetical deadlock, the driver will not call
"up" immediately, but it will offload it to a workqueue. The problem is
that if we call "schedule_delayed_work" on the same work item multiple
times, the work item may only be executed once.
This is happening:
* some urb completes
* dlfb_urb_completion adds it to the free list
* dlfb_urb_completion calls schedule_delayed_work to schedule the function
dlfb_release_urb_work to increase the semaphore count
* as the urb is on the free list, some other task grabs it and submits it
* the submitted urb completes, dlfb_urb_completion is called again
* dlfb_urb_completion calls schedule_delayed_work, but the work is already
scheduled, so it does nothing
* finally, dlfb_release_urb_work is called, it increases the semaphore
count by 1, although it should increase it by 2
So, the semaphore count is decreasing over time, and this causes gradual
performance degradation.
Note that in the current kernel, the "up" function may be called from
interrupt and it may race with the "down" function called by another
thread, so we don't have to offload the call of "up" to a workqueue at
all. This patch removes the workqueue code. The patch also changes
"down_interruptible" to "down" in dlfb_free_urb_list, so that we will
clean up the driver properly even if a signal arrives.
With this patch, the performance of udlfb no longer degrades.
