The list_for_each_entry_safe() macro saves the current item (n) and
the item after (n+1), so that n can be safely removed without
corrupting the list. However, when traversing the list and removing
items using gadget giveback, the DWC3 lock is briefly released,
allowing other routines to execute. There is a situation where, while
items are being removed from the cancelled_list using
dwc3_gadget_ep_cleanup_cancelled_requests(), the pullup disable
routine is running in parallel (due to UDC unbind). As the cleanup
routine removes n, and the pullup disable removes n+1, once the
cleanup retakes the DWC3 lock, it references a request who was already
removed/handled. With list debug enabled, this leads to a panic.
Ensure all instances of the macro are replaced where gadget giveback
is used.

Fix this condition by utilizing list_replace_init(), and traversing
through a local copy of the current elements in the endpoint lists.
This will also set the parent list as empty, so if another thread is
also looping through the list, it will be empty on the next iteration.

In case of dwc3 it is possible that the sysdev is the parent of the
controller. To ensure the right dev is set to the gadget's dev parent we
will hand over sysdev instead of dev, which will always point to the
controller.

Some devices have USB compositions which may require multiple endpoints
that support EP bursting. HW defined TX FIFO sizes may not always be
sufficient for these compositions. By utilizing flexible TX FIFO
allocation, this allows for endpoints to request the required FIFO depth to
achieve higher bandwidth. With some higher bMaxBurst configurations, using
a larger TX FIFO size results in better TX throughput.
By introducing the check_config() callback, the resizing logic can fetch
the maximum number of endpoints used in the USB composition (can contain
multiple configurations), which helps ensure that the resizing logic can
fulfill the configuration(s), or return an error to the gadget layer
otherwise during bind time.

There is no need to keep around the debugfs "root" directory for the
dwc3 device. Instead, look it up anytime we need to find it. This will
help when callers get out-of-order and we had the potential to have a
"stale" pointer around for the root dentry, as has happened in the past.

Currently, the dwc3 platform driver does not explicitly ask for
a DMA mask. This makes it fall back to the default 32-bit mask which
breaks the driver on systems that only have RAM starting above the
first 4G like the Apple M1 SoC.
Fix this by calling dma_set_mask_and_coherent with a 64bit mask.

There is no validation of the index from dwc3_wIndex_to_dep() and we might
be referring a non-existing ep and trigger a NULL pointer exception. In
certain configurations we might use fewer eps and the index might wrongly
indicate a larger ep index than existing.
By adding this validation from the patch we can actually report a wrong
index back to the caller.
In our usecase we are using a composite device on an older kernel, but
upstream might use this fix also. Unfortunately, I cannot describe the
hardware for others to reproduce the issue as it is a proprietary
implementation.

If an error occurs after a successful 'regulator_enable()' call,
'regulator_disable()' must be called.
Fix the error handling path of the probe accordingly.
The remove function doesn't need to be fixed, because the
'regulator_disable()' call is already hidden in 'dwc3_meson_g12a_suspend()'

during probe. This includes creation of subdirectories for each
of the gadget's endpoints. This works fine for peripheral-only
controllers, as dwc3_core_init_mode() calls dwc3_gadget_init()
just prior to calling dwc3_debugfs_init().
However, for dual-role controllers, dwc3_core_init_mode() will
instead call dwc3_drd_init() which is problematic in a few ways.
First, the initial state must be determined, then dwc3_set_mode()
will have to schedule drd_work and by then dwc3_debugfs_init()
could have already been invoked. Even if the initial mode is
peripheral, dwc3_gadget_init() happens after the DebugFS files
are created, and worse so if the initial state is host and the
controller switches to peripheral much later. And secondly,
even if the gadget endpoints' debug entries were successfully
created, if the controller exits peripheral mode, its dwc3_eps
are freed so the debug files would now hold stale references.
So it is best if the DebugFS endpoint entries are created and
removed dynamically at the same time the underlying dwc3_eps are.
Do this by calling dwc3_debugfs_create_endpoint_dir() as each
endpoint is created, and conversely remove the DebugFS entry when
the endpoint is freed.

There exists a possible scenario in which dwc3_gadget_init() can fail:
during during host -> peripheral mode switch in dwc3_set_mode(), and
a pending gadget driver fails to bind. Then, if the DRD undergoes
another mode switch from peripheral->host the resulting
dwc3_gadget_exit() will attempt to reference an invalid and dangling
dwc->gadget pointer as well as call dma_free_coherent() on unmapped
DMA pointers.
The exact scenario can be reproduced as follows:
- Start DWC3 in peripheral mode
- Configure ConfigFS gadget with FunctionFS instance (or use g_ffs)
- Run FunctionFS userspace application (open EPs, write descriptors, etc)
- Bind gadget driver to DWC3's UDC
- Switch DWC3 to host mode
=> dwc3_gadget_exit() is called. usb_del_gadget() will put the
ConfigFS driver instance on the gadget_driver_pending_list
- Stop FunctionFS application (closes the ep files)
- Switch DWC3 to peripheral mode
=> dwc3_gadget_init() fails as usb_add_gadget() calls
check_pending_gadget_drivers() and attempts to rebind the UDC
to the ConfigFS gadget but fails with -19 (-ENODEV) because the
FFS instance is not in FFS_ACTIVE state (userspace has not
re-opened and written the descriptors yet, i.e. desc_ready!=0).
- Switch DWC3 back to host mode
=> dwc3_gadget_exit() is called again, but this time dwc->gadget
is invalid.
Although it can be argued that userspace should take responsibility
for ensuring that the FunctionFS application be ready prior to
allowing the composite driver bind to the UDC, failure to do so
should not result in a panic from the kernel driver.
Fix this by setting dwc->gadget to NULL in the failure path of
dwc3_gadget_init() and add a check to dwc3_gadget_exit() to bail out
unless the gadget pointer is valid.

Current sequence utilizes dwc3_gadget_disable_irq() alongside
synchronize_irq() to ensure that no further DWC3 events are generated.
However, the dwc3_gadget_disable_irq() API only disables device
specific events. Endpoint events can still be generated. Briefly
disable the interrupt line, so that the cleanup code can run to
prevent device and endpoint events. (i.e. __dwc3_gadget_stop() and
dwc3_stop_active_transfers() respectively)
Without doing so, it can lead to both the interrupt handler and the
pullup disable routine both writing to the GEVNTCOUNT register, which
will cause an incorrect count being read from future interrupts.

Commit 0bd0f6d201 ("usb: dwc3: gadget: remove allocated/queued request
tracking") removed the allocated & queued fields from struct dwc3_ep
but neglected to also remove them from the dwc3_log_trb event class's
TP_STRUCT definition which are now unused. Remove them to save eight

The driver incorrectly uses req->num_pending_sgs to track both the
number of pending and queued SG entries. It only prepares the next
request if the previous is done, and it doesn't update num_pending_sgs
until there is TRB completion interrupt. This may starve the controller
of more TRBs until the num_pending_sgs is decremented.
Fix this by decrementing the num_pending_sgs after they are queued and
properly track both num_mapped_sgs and num_queued_sgs.

When extcon is used in combination with dwc3, it is assumed that the dwc3
registers are untouched and as such are only configured if VBUS is valid
or ID is tied to ground.
In case VBUS is not valid or ID is floating, the registers are not
configured as such during driver initialization, causing a wrong
default state during boot.
If the registers are not in a default state, because they are for
instance touched by a boot loader, this can cause for a kernel error.

If an error is received when issuing a start or update transfer
command, the error handler will stop all active requests (including
the current USB request), and call dwc3_gadget_giveback() to notify
function drivers of the requests which have been stopped. Avoid
returning an error for kick transfer during EP queue, to remove
duplicate cleanup operations on the request being queued.

As part of commit e81a7018d9 ("usb: dwc3: allocate gadget structure
dynamically") the dwc3_gadget_release() was added which will free
the dwc->gadget structure upon the device's removal when
usb_del_gadget_udc() is called in dwc3_gadget_exit().
However, simply freeing the gadget results a dangling pointer
situation: the endpoints created in dwc3_gadget_init_endpoints()
have their dep->endpoint.ep_list members chained off the list_head
anchored at dwc->gadget->ep_list. Thus when dwc->gadget is freed,
the first dwc3_ep in the list now has a dangling prev pointer and
likewise for the next pointer of the dwc3_ep at the tail of the list.
The dwc3_gadget_free_endpoints() that follows will result in a
use-after-free when it calls list_del().
This was caught by enabling KASAN and performing a driver unbind.
The recent commit 568262bf54 ("usb: dwc3: core: Add shutdown
callback for dwc3") also exposes this as a panic during shutdown.
There are a few possibilities to fix this. One could be to perform
a list_del() of the gadget->ep_list itself which removes it from
the rest of the dwc3_ep chain.
Another approach is what this patch does, by splitting up the
usb_del_gadget_udc() call into its separate "del" and "put"
components. This allows dwc3_gadget_free_endpoints() to be
called before the gadget is finally freed with usb_put_gadget().

The device event corresponding to End of Periodic Frame is only
found on older IP revisions (2.10a and prior, according to a
cursory SNPS databook search). On revisions 2.30a and newer,
including DWC3.1, the same event value and corresponding DEVTEN
bit were repurposed to indicate that the link has gone into
suspend state (U3 or L2/L1).
EOPF events had never been enabled before in this driver, and
going forward we expect current and future DWC3-based devices
won't likely to be using such old DWC3 IP revisions either.
Hence rather than keeping the deprecated EOPF macro names let's
rename them to indicate their usage for suspend events.

commit 72704f876f ("dwc3: gadget: Implement the suspend entry event
handler") introduced (nearly 5 years ago!) an interrupt handler for
U3/L1-L2 suspend events. The problem is that these events aren't
currently enabled in the DEVTEN register so the handler is never
even invoked. Fix this simply by enabling the corresponding bit
in dwc3_gadget_enable_irq() using the same revision check as found
in the handler.

DWC_usb32 IP introduces a new behavior when handling NoStream event for
IN endpoints. If the controller is capable of DEV_TXF_FLUSH_BYPASS, then
the driver does not need to force to restart stream for IN endpoints.
The controller will generate ERDY and restart the stream periodically.

According to the programming guide, to switch mode for DRD controller,
the driver needs to do the following.
To switch from device to host:
1. Reset controller with GCTL.CoreSoftReset
2. Set GCTL.PrtCapDir(host mode)
3. Reset the host with USBCMD.HCRESET
4. Then follow up with the initializing host registers sequence
To switch from host to device:
1. Reset controller with GCTL.CoreSoftReset
2. Set GCTL.PrtCapDir(device mode)
3. Reset the device with DCTL.CSftRst
4. Then follow up with the initializing registers sequence
Currently we're missing step 1) to do GCTL.CoreSoftReset and step 3) of
switching from host to device. John Stult reported a lockup issue seen
with HiKey960 platform without these steps[1]. Similar issue is observed
with Ferry's testing platform[2].
So, apply the required steps along with some fixes to Yu Chen's and John
Stultz's version. The main fixes to their versions are the missing wait
for clocks synchronization before clearing GCTL.CoreSoftReset and only
apply DCTL.CSftRst when switching from host to device.

The programming guide incorrectly stated that the DCFG.bInterval_m1 must
be set to 0 when operating in fullspeed. There's no such limitation for

The START_TRANSFER command needs to be executed while in ON/U0 link
state (with an exception during register initialization). Don't use
dwc->link_state to check this since the driver only tracks the link
state when the link state change interrupt is enabled. Check the link
state from DSTS register instead.
Note that often the host already brings the device out of low power
before it sends/requests the next transfer. So, the user won't see any
issue when the device starts transfer then. This issue is more
noticeable in cases when the device delays starting transfer, which can
happen during delayed control status after the host put the device in
low power.

This patch adds a shutdown callback to USB DWC core driver to ensure that
it is properly shutdown in reboot/shutdown path. This is required
where SMMU address translation is enabled like on SC7180
SoC and few others. If the hardware is still accessing memory after
SMMU translation is disabled as part of SMMU shutdown callback in
system reboot or shutdown path, then IOVAs(I/O virtual address)
which it was using will go on the bus as the physical addresses which
might result in unknown crashes (NoC/interconnect errors).

Currently the controller handles single stream only. So, Ignore Packet
Pending bit for stream selection and don't search for another stream if
the host sends Data Packet with PP=0 (for OUT direction) or ACK with
NumP=0 and PP=0 (for IN direction). This slightly improves the stream
performance.

In accordance with the USB HCD/DRD schema all the USB controllers are
supposed to have DT-nodes named with prefix "^usb(@.*)?". Since the
existing DT-nodes will be renamed in a subsequent patch let's fix the DWC3
Qcom-specific code to detect the DWC3 sub-node just by checking its
compatible string to match the "snps,dwc3". The semantic of the code
won't change seeing all the DWC USB3 nodes are supposed to have the
compatible property with any of those strings set.

case of any error the clocks freeing will be handled automatically.

Currently, when dwc3 handles request cancelled, dwc3 just returns
-ECONNRESET for all requests. It will cause USB function drivers
can't know if the requests are cancelled by other reasons.
This patch will replace DWC3_REQUEST_STATUS_CANCELLED with the
reasons below.

Currently dwc3 only prints the virtual address of a register when doing
register read/write. However, these hashed addresses are difficult to read.
Also, since we use %p, we may get some useless (___ptrval___) prints if the
address is not randomized enough. Let's include the register offset to help
read the register read and write tracepoints.

It seems that on Intel Merrifield platform the USB PHY shouldn't be suspended.
Otherwise it can't be enabled by simply change the cable in the connector.
Enable corresponding quirk for the platform in question.

If the gadget driver doesn't specify a max_speed, then use the
controller's maximum supported speed as default. For DWC_usb32 IP, the
gadget's speed maybe limited to gen2x1 rate only if the driver's
max_speed is unknown. This scenario should not occur with the current
implementation since the default gadget driver's max_speed should always
be specified. However, to make the driver more robust and help with
readability, let's cover all the scenarios in __dwc3_gadget_set_speed().

The ACPI probe starts failing since commit bea46b9815 ("usb: dwc3:
qcom: Add interconnect support in dwc3 driver"), because there is no
interconnect support for ACPI, and of_icc_get() call in
dwc3_qcom_interconnect_init() will just return -EINVAL.
Fix the problem by skipping interconnect init for ACPI probe, and then
the NULL icc_path_ddr will simply just scheild all ICC calls.

The current dwc3_gadget_reset_interrupt() will stop any active
transfers, but only addresses blocking of EP queuing for while we are
coming from a disconnected scenario, i.e. after receiving the disconnect
event. If the host decides to issue a bus reset on the device, the
connected parameter will still be set to true, allowing for EP queuing
to continue while we are disabling the functions. To avoid this, set the
connected flag to false until the stop active transfers is complete.

Add checks similar to dwc3_gadget_ep_queue() before kicking off
transfers after getting an endpoint completion event. Since cleaning
up completed requests will momentarily unlock dwc->lock, there is a
chance for a sequence like pullup disable to be executed. This can
lead to preparing a TRB, which will be removed by the pullup disable
routine.

Add a new driver for supporting Xilinx platforms. This driver is used
for some sequence of operations required for Xilinx USB controllers.
This driver is also used to choose between PIPE clock coming from SerDes
and the Suspend Clock. Before the controller is out of reset, the clock
selection should be changed to PIPE clock in order to make the USB
controller work. There is a register added in Xilinx USB controller
register space for the same.

None of the DWC_usb3x IPs (and all their versions) supports low-speed
setting in device mode. In the early days, our "Early Adopter Edition"
DWC_usb3 databook shows that the controller may be configured to operate
in low-speed, but it was revised on release. Let's remove this invalid
speed setting to avoid any confusion.

In the situations where the DWC3 gadget stops active transfers, once
calling the dwc3_gadget_giveback(), there is a chance where a function
driver can queue a new USB request in between the time where the dwc3
lock has been released and re-aquired. This occurs after we've already
issued an ENDXFER command. When the stop active transfers continues
to remove USB requests from all dep lists, the newly added request will
also be removed, while controller still has an active TRB for it.
This can lead to the controller accessing an unmapped memory address.
Fix this by ensuring parameters to prevent EP queuing are set before
calling the stop active transfers API.

of_get_child_by_name() increments the reference counter of the OF node it
managed to find. So after the code is done using the device node, the
refcount must be decremented. Add missing of_node_put() invocation then
to the dwc3_qcom_of_register_core() method, since DWC3 OF node is being
used only there.

The dwc3-qcom currently enables wakeup interrupts unconditionally
when suspending, however this should not be done when wakeup is
disabled (e.g. through the sysfs attribute power/wakeup). Only
enable wakeup interrupts when device_may_wakeup() returns true.

The dep->interval captures the number of frames/microframes per interval
from bInterval. Fullspeed interrupt endpoint bInterval is the number of
frames per interval and not 2^(bInterval - 1). So fix it here. This
change is only for debugging purpose and should not affect the interrupt
endpoint operation.

The check for bounded gadget driver in dwc3_gadget_start() was to
prevent going through the initialization again without any cleanup. The
recent commit 49d08cfc78 ("usb: udc: core: Introduce started state")
updated the UDC framework and guarantees this won't happen while the UDC
is started. Also, this check doesn't prevent requesting threaded irq to
the same dev_id, which will mess up the irq freeing logic. Let's remove
it.

The DWC_usb32 controller supports dual-lane SuperSpeed Plus. Check the
maximum_speed property for any limitation in the HW to initialize and
validate the maximum number of lanes and speed the device will operate.
Currently the controller has no visibility into the HW parameter to
determine the maximum number of lanes the HW supports. If the number of
lanes is not specified for SSP, then set the default rate to gen2x2 for
DWC_usb32 and gen2x1 for DWC_usb31.

Commit fe8abf332b ("usb: dwc3: support clocks and resets for DWC3
core") introduced clock support and a new function named
dwc3_core_init_for_resume() which enables the clock before calling
dwc3_core_init() during resume as clocks get disabled during suspend.
Unfortunately in this commit the DWC3_GCTL_PRTCAP_OTG case was forgotten
and therefore during resume, a platform could call dwc3_core_init()
without re-enabling the clocks first, preventing to resume properly.
So update the resume path to call dwc3_core_init_for_resume() as it
should.

For sdm845 ACPI boot, the URS (USB Role Switch) node in ACPI DSDT table
holds the memory resource, while interrupt resources reside in the child
nodes USB0 and UFN0. It adds USB0 host support by probing URS node,
creating platform device for USB0 node, and then retrieve interrupt
resources from USB0 platform device.

Tiger Lake SOC (the versions of it that have integrated USB4
controller) may have two DWC3 controllers. One is part of
the PCH (Platform Controller Hub, i.e. the chipset) as
usual, and the other is inside the actual CPU block.
On all Intel platforms that have the two separate DWC3
controllers, the one inside the CPU handles USB3 and only
USB3 traffic, while the PCH version handles USB2 and USB2
alone. The reason for splitting the two busses like this is
to allow easy USB3 tunneling over USB4 connections. As USB2
is not tunneled over USB4, it has dedicated USB controllers
(both xHCI and DWC3).

usb_gadget_deactivate/usb_gadget_activate does not execute the UDC start
operation, which may leave EP0 disabled and event IRQs disabled when
re-activating the function. Move the enabling/disabling of USB EP0 and
device event IRQs to be performed in the pullup routine.

The DWC3 runtime suspend routine checks for the USB connected parameter to
determine if the controller can enter into a low power state. The
connected state is only set to false after receiving a disconnect event.
However, in the case of a device initiated disconnect (i.e. UDC unbind),
the controller is halted and a disconnect event is never generated. Set
the connected flag to false if issuing a device initiated disconnect to
allow the controller to be suspended.

Some devices support charging while in device mode. In these situations,
the USB gadget will notify the DWC3 gadget driver to modify the current
based on the enumeration and device state. The usb_phy_set_power() API
will allow external charger entities to adjust the charge current through
the notifier block.

First of all the commit e0082698b6 ("usb: dwc3: ulpi: conditionally
resume ULPI PHY") introduced the Suspend USB2.0 HS/FS/LS PHY regression,
as by design of the fix any attempt to read/write from/to the PHY control
registers will completely disable the PHY suspension, which consequently
will increase the USB bus power consumption. Secondly the fix won't work
well for the very first attempt of the ULPI PHY control registers IO,
because after disabling the USB2.0 PHY suspension functionality it will
still take some time for the bus to resume from the sleep state if one has
been reached before it. So the very first PHY register read/write
operation will take more time than the busy-loop provides and the IO
timeout error might be returned anyway.
Here we suggest to fix the denoted problems in the following way. First of
all let's not disable the Suspend USB2.0 HS/FS/LS PHY functionality so to
make the controller and the USB2.0 bus more power efficient. Secondly
instead of that we'll extend the PHY IO op wait procedure with 1 - 1.2 ms
sleep if the PHY suspension is enabled (1ms should be enough as by LPM
specification it is at most how long it takes for the USB2.0 bus to resume
from L1 (Sleep) state). Finally in case if the USB2.0 PHY suspension
functionality has been disabled on the DWC USB3 controller setup procedure
we'll compensate the USB bus resume process latency by extending the
busy-loop attempts counter.

Originally the procedure of the ULPI transaction finish detection has been
developed as a simple busy-loop with just decrementing counter and no
delays. It's wrong since on different systems the loop will take a
different time to complete. So if the system bus and CPU are fast enough
to overtake the ULPI bus and the companion PHY reaction, then we'll get to
take a false timeout error. Fix this by converting the busy-loop procedure
to take the standard bus speed, address value and the registers access
mode into account for the busy-loop delay calculation.
Here is the way the fix works. It's known that the ULPI bus is clocked
with 60MHz signal. In accordance with [1] the ULPI bus protocol is created
so to spend 5 and 6 clock periods for immediate register write and read
operations respectively, and 6 and 7 clock periods - for the extended
register writes and reads. Based on that we can easily pre-calculate the
time which will be needed for the controller to perform a requested IO
operation. Note we'll still preserve the attempts counter in case if the
DWC USB3 controller has got some internals delays.

In accordance with [1] the DWC_usb3 core sets the GUSB2PHYACCn.VStsDone
bit when the PHY vendor control access is done and clears it when the
application initiates a new transaction. The doc doesn't say anything
about the GUSB2PHYACCn.VStsBsy flag serving for the same purpose. Moreover
we've discovered that the VStsBsy flag can be cleared before the VStsDone
bit. So using the former as a signal of the PHY control registers
completion might be dangerous. Let's have the VStsDone flag utilized
instead then.

If "port" node is missing in PHY controller node, dwc3_get_extcon()
isn't able to find extcon devices. This is perfectly fine in case when
"usb-role-switch" or OTG is used, but next misleading error message is
printed in that case, from of_graph_get_remote_node():

Avoid printing that message by checking if the port node exists in PHY
node before calling of_graph_get_remote_node(). While at it, add the
comment from mentioned code block, explaining how checking the port
availability helps to avoid the misleading error.

If we want to send a control status on our own time (through
delayed_status), make sure to handle a case where we may queue the
delayed status before the host requesting for it (when XferNotReady
is generated). Otherwise, the driver won't send anything because it's
not EP0_STATUS_PHASE yet. To resolve this, regardless whether
dwc->ep0state is EP0_STATUS_PHASE, make sure to clear the
dwc->delayed_status flag if dwc3_ep0_send_delayed_status() is called.
The control status can be sent when the host requests it later.

DWC3 IPs can use the maximum stream id (up to 2^16) specified by the
USB 3.x specs. Don't limit to stream id 2^15 only. Note that this does
not reflect the number of concurrent streams the controller handles
internally.

If the transfer had already started and there's no TRB to update, then
there's no need to go through __dwc3_gadget_kick_transfer(). There is
no problem reissuing UPDATE_TRANSFER command. This change just saves
the driver from doing a few operations. This happens when we run out of
TRB and function driver still queues for more requests.

If we couldn't finish preparing all the TRBs of a request, don't prepare
the next request. Otherwise, the TRBs order will be mixed up and the
controller will process the wrong TRB. This is a corner case where
there's not enough TRBs for a request that needs the extra TRB but
there's still 1 available TRB in the pool.

To keep the setting of interrupt-on-completion (IOC) when out of TRBs
consistent and easier to read, the caller of dwc3_prepare_one_trb()
will determine if the TRB must have IOC bit set. This also reduces the
number of times we need to call dwc3_calc_trbs_left(). Note that we only
care about setting IOC from insufficient number of TRBs for SG and not
linear requests (because we don't need to split linear requests).

Previously if we run out of TRBs for the last SG entry that requires
an extra TRB, we set interrupt-on-completion (IOC) bit to an already
prepared TRB (i.e. HWO=1). This logic is not clean, and it's not a
typical way to prepare TRB. Also, this prevents showing IOC setup in
tracepoint when __dwc3_prepare_one_trb() is executed. Instead, let's
look ahead when preparing TRB to know whether to set the IOC bit before
the last SG entry. This requires adding a new parameter "must_interrupt"
to dwc3_prepare_one_trb().

It's possible that there's no new TRBs prepared when kicking a transfer.
This happens when we need to stop and restart a transfer such as in the
case of reinitiating a stream or retrying isoc transfer. For streams,
sometime host may reject a stream and the device may need to reinitiate
that stream by stopping and restarting a transfer. In this case, all the
TRBs may have already been prepared. Allow the function
__dwc3_gadget_kick_transfer() to go through even though there's no new
TRB.

In the DWC3 databook, for a device initiated disconnect or bus reset, the
driver is required to send dependxfer commands for any pending transfers.
In addition, before the controller can move to the halted state, the SW
needs to acknowledge any pending events. If the controller is not halted
properly, there is a chance the controller will continue accessing stale or
freed TRBs and buffers.

By returning the number of TRBs prepared, we know whether to execute
__dwc3_gadget_kick_transfer(). This allows us to check if we ran out of
TRBs when extra TRBs are needed for OUT transfers. It also allows us to
properly handle usb_gadget_map_request_by_dev() error.

The current ZLP handling for ep0 requests is only for control IN
requests. For OUT direction, DWC3 needs to check and setup for MPS
alignment.
Usually, control OUT requests can indicate its transfer size via the
wLength field of the control message. So usb_request->zero is usually
not needed for OUT direction. To handle ZLP OUT for control endpoint,
make sure the TRB is MPS size.

When the driver prepares the extra TRB, it uses bounce buffer. If we
just add a new parameter to dwc3_prepare_one_trb() to indicate this,
then we can refactor and simplify the driver quite a bit.
dwc3_prepare_one_trb() also checks if a request had been moved to the
started list. This is a prerequisite to subsequence patches improving
the handling of extra TRBs.

An SG request may be partially completed (due to no available TRBs).
Don't reclaim extra TRBs and clear the needs_extra_trb flag until the
request is fully completed. Otherwise, the driver will reclaim the wrong
TRB.

resume, don't use the remaining request length to calculate for MPS
alignment. Use the entire request->length to do that.

The current code uses commit fac323471d ("usb: udc: allow adding
and removing the same gadget device") as the workaround to let
the gadget device is re-used, but it is not allowed from driver
core point. In this commit, we allocate gadget structure dynamically,
and free it at its release function. Since the gadget device's
driver_data has already occupied by usb_composite_dev structure, we have
to use gadget device's platform data to store dwc3 structure.

Thus, we add the PCI_DEVICE_ID_INTEL_EHLLP to the list of devices that

The Amlogic AXG is close to the GXL Glue but with a single OTG PHY.
It needs the same init sequence as GXL & GXM, but it seems it doesn't need
the host disconnect bit.
