Julian Wiedmann 88bf319fc2 s390/qdio: split up CCQ handling for EQBS / SQBS
Get rid of the confusing two-stage translation in a hot path, and only
handle CCQs that we anticipate for the respective command. Any
unexpected value (such as CCQ 97 (rc == 1) for SQBS) should be
considered a severe HW/driver bug, and traced as such.

Julian Wiedmann dae55b6fef s390/qdio: don't retry EQBS after CCQ 96
Immediate retry of EQBS after CCQ 96 means that we potentially misreport
the state of buffers inspected during the first EQBS call.

This occurs when
1. the first EQBS finds all inspected buffers still in the initial state
   set by the driver (ie INPUT EMPTY or OUTPUT PRIMED),
2. the EQBS terminates early with CCQ 96, and
3. by the time that the second EQBS comes around, the state of those
   previously inspected buffers has changed.

If the state reported by the second EQBS is 'driver-owned', all we know
is that the previous buffers are driver-owned now as well. But we can't
tell if they all have the same state. So for instance
- the second EQBS reports OUTPUT EMPTY, but any number of the previous
  buffers could be OUTPUT ERROR by now,
- the second EQBS reports OUTPUT ERROR, but any number of the previous
  buffers could be OUTPUT EMPTY by now.

Effectively, this can result in both over- and underreporting of errors.

If the state reported by the second EQBS is 'HW-owned', that doesn't
guarantee that the previous buffers have not been switched to
driver-owned in the mean time. So for instance
- the second EQBS reports INPUT EMPTY, but any number of the previous
  buffers could be INPUT PRIMED (or INPUT ERROR) by now.

This would result in failure to process pending work on the queue. If
it's the final check before yielding initiative, this can cause
a (temporary) queue stall due to IRQ avoidance.

Julian Wiedmann 0cf1e05157 s390/qdio: don't merge ERROR output buffers
On an Output queue, both EMPTY and PENDING buffer states imply that the
buffer is ready for completion-processing by the upper-layer drivers.

So for a non-QEBSM Output queue, get_buf_states() merges mixed
batches of PENDING and EMPTY buffers into one large batch of EMPTY
buffers. The upper-layer driver (ie. qeth) later distuingishes PENDING
from EMPTY by inspecting the slsb_state for
QDIO_OUTBUF_STATE_FLAG_PENDING.

But the merge logic in get_buf_states() contains a bug that causes us to
erronously also merge ERROR buffers into such a batch of EMPTY buffers
(ERROR is 0xaf, EMPTY is 0xa1; so ERROR & EMPTY == EMPTY).
Effectively, most outbound ERROR buffers are currently discarded
silently and processed as if they had succeeded.

Note that this affects _all_ non-QEBSM device types, not just IQD with CQ.

Fix it by explicitly spelling out the exact conditions for merging.

For extracting the "get initial state" part out of the loop, this relies
on the fact that get_buf_states() is never called with a count of 0. The
QEBSM path already strictly requires this, and the two callers with
variable 'count' make sure of it.

Julian Wiedmann 152485bf76 s390/qdio: simplify math in get_*_buffer_frontier()
When determining the buffer count that get_buf_states() should
be queried for, 'count' is capped at 127 buffers.
So the check
	q->first_to_check == (q->first_to_check + count) % 128
can be reduced to
	count == 0

This helps to emphasize that get_buf_states() is really only
called with count > 0.

Cornelia Huck 9851bc77e6 vfio-ccw: fence off transport mode
vfio-ccw only supports command mode for channel programs, not transport
mode. User space is supposed to already take care of that and pass us
command-mode ORBs only, but better make sure and return an error to
the caller instead of trying to process tcws as ccws.

Sebastian Ott 770b55c995 s390/cio: fix return code after missing interrupt
When a timeout occurs for users of ccw_device_start_timeout
we will stop the IO and call the drivers int handler with
the irb pointer set to ERR_PTR(-ETIMEDOUT). Sometimes
however we'd set the irb pointer to ERR_PTR(-EIO) which is
not intended. Just set the correct value in all codepaths.

Sebastian Ott f97a6b6c47 s390/cio: fix ccw_device_start_timeout API
There are cases a device driver can't start IO because the device is
currently in use by cio. In this case the device driver is notified
when the device is usable again.

Using ccw_device_start_timeout we would set the timeout (and change
an existing timeout) before we test for internal usage. Worst case
this could lead to an unexpected timer deletion.

Fix this by setting the timeout after we test for internal usage.

Greg Kroah-Hartman 13d1d559f0 s390: drivers: Remove redundant license text
Now that the SPDX tag is in all drivers/s390/ files, that identifies the
license in a specific and legally-defined manner.  So the extra GPL text
wording can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text.  And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/s390/cio/ files with the correct SPDX license
identifier based on the license text in the file itself.  The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe

Jason J. Herne 408358b50d s390: vfio-ccw: Do not attempt to free no-op, test and tic cda.
Because we do not make use of the cda (channel data address) for test,
no-op ccws no address translation takes place. This means cda could
contain a guest address which we do not want to attempt to free. Let's
check the command type and skip cda free when it is not needed.

For a TIC ccw, ccw->cda points to either a ccw in an existing chain or
it points to a whole new allocated chain. In either case the data will
be freed when the owning chain is freed.

Sebastian Ott 30e8eb8671 s390/qdio: sanitize put_indicator
qdio maintains an array of struct indicator_t. put_indicator takes a pointer
to a member of a struct indicator_t within that array, calculates the index,
and uses the array and the index to get the struct indicator_t.

Simply use the pointer directly.

Although the pointer happens to point to the first member of that struct
use the container_of macro.

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   SPDX license identifier                            # files
   ---------------------------------------------------|-------

   and resulted in the first patch in this series.

   If that file was a */uapi/* path one, it was "GPL-2.0 WITH
   Linux-syscall-note" otherwise it was "GPL-2.0".  Results of that was:

   and resulted in the second patch in this series.

 - if a file had some form of licensing information in it, and was one
   of the */uapi/* ones, it was denoted with the Linux-syscall-note if

   it (per prior point).  Results summary:

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Dong Jia Shi 688c29533f vfio: ccw: bypass bad idaw address when fetching IDAL ccws
We currently return the same error code (-EFAULT) to indicate two
different error cases:
1. a bug in vfio-ccw implementation has been found.
2. a buggy channel program has been detected.

This brings difficulty for userland program (specifically Qemu) to
handle.

Let's use -EFAULT to only indicate the first case. For the second
case, we simply hand over the ccws to lower level for further
handling.

Notice:
Once a bad idaw address is detected, the current behavior is to
suppress the ssch. With this fix, the channel program will be
accepted, and part of the channel program (the part ahead of
the bad idaw) could possibly be executed by the device before
I/O conclusion.

Julian Wiedmann f9a5d70cfa s390/ccwgroup: tie a ccwgroup driver to its ccw driver
When grouping devices, the ccwgroup core only checks whether all of the
devices are bound to the same ccw_driver. It has no means of checking
if the requesting ccwgroup driver actually supports this device type.
qeth implements its own device matching in qeth_core_probe_device(),
while ctcm and lcs currently have no sanity-checking at all.

Enable ccwgroup drivers to optionally defer the device type checking to
the ccwgroup core, by specifying their supported ccw_driver.
This allows us drop the device type matching from qeth, and improves
the robustness of ctcm and lcs.

When enabling channel measurement fails with a busy condition we wait
for the next interrupt to arrive before we retry the operation. For
devices which usually don't create interrupts we wait forever.

Although the waiting is done interruptible that behavior is not
expected and confused some users. Abort the operation after a 10s
timeout.

Jason J. Herne c389377c01 vfio: ccw: fix bad ptr math for TIC cda translation
When we are translating channel data addresses from guest to host
address space for TIC instructions we are getting incorrect
addresses because of a pointer arithmetic error.

We currently calculate the offset of the TIC's cda from the start
of the channel program chain (ccw->cda - ccw_head). We then add
that to the address of the ccw chain in host memory (iter->ch_ccw).
The problem is that iter->ch_ccw is a pointer to struct ccw1 so
when we increment it we are actually incrementing by the size of
struct ccw1 which is 8 bytes. The intent was to increment by
n-bytes, not n*8.

The fix: cast iter->ch_ccw to char* so it will be incremented by
n*1.

Christoph Hellwig 2a842acab1 block: introduce new block status code type
Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings.  This patch
instead introduces a new  blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning.  Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.

For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.

blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

Dong Jia Shi 129cc19a94 vfio: ccw: improve error handling for vfio_ccw_mdev_remove
When vfio_ccw_mdev_reset fails during the remove process of the mdev,
the current implementation simply returns.

The failure indicates that the subchannel device is in a NOT_OPER state,
thus the right thing to do should be removing the mdev.

While we are at here, reverse the condition check to make the code more
concise and readable.

Dong Jia Shi d686f21ace vfio: ccw: introduce support for ccw0
Although Linux does not use format-0 channel command words (CCW0)
these are a non-optional part of the platform spec, and for the sake
of platform compliance, and possibly some non-Linux guests, we have
to support CCW0.

Making the kernel execute a format 0 channel program is too much hassle
because we would need to allocate and use memory which can be addressed
by 24 bit physical addresses (because of CCW0.cda). So we implement CCW0
support by translating the channel program into an equivalent CCW1
program instead.

Based upon an orginal patch by Kai Yue Wang.

To provide user-space a set of interfaces to:
1. pass in a ccw program to perform an I/O operation.
2. read back I/O results of the completed I/O operations.
We introduce an MMIO region for the vfio-ccw device here.

This region is defined to content:
1. areas to store arguments that an ssch required.
2. areas to store the I/O results.

Using pwrite/pread to the device on this region, a user-space program
could write/read data to/from the vfio-ccw device.

Introduce ccwchain structure and helper functions that can be used to
handle a channel program issued from a virtual machine.

The following limitations apply:
1. Supports only prefetch enabled mode.
2. Supports idal(c64) ccw chaining.
3. Supports 4k idaw.
4. Supports ccw1.
5. Supports direct ccw chaining by translating them to idal ccws.

CCW translation requires to leverage the vfio_(un)pin_pages interfaces
to pin/unpin sets of mem pages frequently. Currently we have a lack of
support to do this in an efficient way. So we introduce pfn_array data
structure and helper functions to handle pin/unpin operations here.

Julian Wiedmann 1e4a382fdc s390/qdio: clear DSCI prior to scanning multiple input queues
For devices with multiple input queues, tiqdio_call_inq_handlers()
iterates over all input queues and clears the device's DSCI
during each iteration. If the DSCI is re-armed during one
of the later iterations, we therefore do not scan the previous
queues again.
The re-arming also raises a new adapter interrupt. But its
handler does not trigger a rescan for the device, as the DSCI
has already been erroneously cleared.
This can result in queue stalls on devices with multiple
input queues.

Fix it by clearing the DSCI just once, prior to scanning the queues.

As the code is moved in front of the loop, we also need to access
the DSCI directly (ie irq->dsci) instead of going via each queue's
parent pointer to the same irq. This is not a functional change,
and a follow-up patch will clean up the other users.

In practice, this bug only affects CQ-enabled HiperSockets devices,
ie. devices with sysfs-attribute "hsuid" set. Setting a hsuid is
needed for AF_IUCV socket applications that use HiperSockets
communication.

Sebastian Ott 98cc43ab6b s390/cio: clarify cssid usage
Currently the cssid in various structures is used as the id of
the respective channel subsystem. Sometimes however we call the
index in the channel_subsystems array cssid. In some places the
id is even used as the index.

Provide a new define MAX_CSS_IDX and use it where appropriate.
In addition to that provide a dummy function to find a channel
subsystem by its id and a macro to iterate over the channel
subsystems.

Lets remove the couple traces of modular infrastructure use, so that
when reading the code there is no doubt it is builtin-only.

Since module_init translates to device_initcall in the non-modular
case, the init ordering remains unchanged with this commit.

Also note that MODULE_DEVICE_TABLE is a no-op for non-modular code.

We delete the MODULE_LICENSE tag etc. since all that information
was (or is now) contained at the top of the file in the comments.

We replace module.h with export.h where the file does export some
symbols.

Sebastian Ott d53c51f261 s390/cio: fix accidental interrupt enabling during resume
Since commit 9f3d6d7 chsc_get_channel_measurement_chars is called with
interrupts disabled during resume from hibernate. Since this function
used spin_unlock_irq, interrupts have been enabled accidentally. Fix
this by using the irqsave variant.

Since we can't guarantee the IRQ-enablement state for all (future/
external) callers, change the locking in related functions to prevent
similar bugs in the future.

Ursula Braun 9bce8b2cbe s390/qdio: avoid reschedule of outbound tasklet once killed
During qdio_shutdown the queue tasklets are killed for all inbound and
outbound queues. The queue structures might be freed after
qdio_shutdown.
Thus it must be guaranteed that these queue tasklets are not rescheduled
after that. In addition the outbound queue timers are deleted and it
must
be guaranteed that these timers are not restarted after qdio_shutdown
processing. Timer deletion should make use of del_timer_sync() to make
sure qdio_outbound_timer() is finished on other CPUs as well. Queue
tasklets should be scheduled in state QDIO_IRQ_STATE_ACTIVE only.

Sebastian Ott 4475aeb8b7 s390/cio: fix premature wakeup during chp configure
We store requests for channel path configure operations in an array but
maintain an additional cfg_busy variable (indicating if we have requests
stored in said array). When 2 tasks request a channel path configure
operation cfg_busy could be set to 0 even if we still have unprocessed
requests. This would lead to the second task being woken up although its
request was not processed yet.

Fix that by getting rid of cfg_busy and use the chp_cfg_task array
in the wake up condition.

Bhaktipriya Shridhar 7b1058bc59 s390/cio/chp : Remove deprecated create_singlethread_workqueue
The workqueue "chp_wq" is involved in performing pending
configure tasks for channel paths.

It has a single work item(&cfg_work) and hence doesn't require
ordering. Also, it is not being used on a memory reclaim path.
Hence, the singlethreaded workqueue has been replaced with the use of
system_wq.

System workqueues have been able to handle high level of concurrency
for a long time now and hence it's not required to have a singlethreaded
workqueue just to gain concurrency. Unlike a dedicated per-cpu workqueue
created with create_singlethread_workqueue(), system_wq allows multiple
work items to overlap executions even on the same CPU; however, a
per-cpu workqueue doesn't have any CPU locality or global ordering
guarantee unless the target CPU is explicitly specified and thus the
increase of local concurrency shouldn't make any difference.

Sebastian Ott f9773768f1 s390/chsc: sanitize fmt check for chp_desc determination
When fetching channel path descriptors we've only evaluated the rfmt
parameter which could lead us to trigger the chsc even though the
machine doesn't support the specific format or to not trigger the
chsc and report a failure to userspace even though the machine would've
supported it.

Improve these checks and change the parameters of the in-kernel
user to be less confusing.

Sebastian Ott 0b60137377 s390/cio: make fmt1 channel path descriptor optional
Not all machines / hypervisors support the chsc commands to fetch
the fmt1 descriptor. When these commands fail the channel path would
currently not be available to linux.

Since users of these descriptors can already deal with invalid data
make fetching it optional. The only data that is mandatory for us is
the fmt0 channel path descriptor.

Also make the return code for missing facilities in
chsc_get_channel_measurement_chars consistent to other functions.

Sebastian Ott 0f5d050cea s390/cio: allow to reset channel measurement block
Prior to commit 1bc6664bdf a call to
enable_cmf for a device for which channel measurement was already
enabled resulted in a reset of the measurement data.

What looked like bugs at the time (a 2nd allocation was triggered
but failed, reset was called regardless of previous failures, and
errors have not been reported to userspace) was actually something
at least one userspace tool depended on. Restore that behavior in
a sane way.

Adding tracepoints to inline functions adds tracepoint invocation code
for each instance where the function is inlined. The resulting increase
in kernel code size can have negative impact:
 - More cache misses in instruction cache
 - Reduced amount of DMA-capable memory

Therefore change all functions implementing I/O instructions from inline
to normal functions.

Peter Oberparleiter ac357c4105 s390/cio: Fix incorrect xsch opcode specification
The numeric representation of the xsch instruction was incorrectly
specified, resulting in reserved fields of the instruction opcode
potentially being set to a non-zero value. While this doesn't currently
cause any problem, a future architecture might make use of these fields
so that the current specification could result in an exception or
unwanted side-effects.

Fix this by using the xsch instruction code for which support in
binutils was added in 2003.

Martin Schwidefsky 1a36a39e22 s390/dump: rework CPU register dump code
To collect the CPU registers of the crashed system allocated a single
page with memblock_alloc_base and use it as a copy buffer. Replace the
stop-and-store-status sigp with a store-status-at-address sigp in
smp_save_dump_cpus() and smp_store_status(). In both cases the target
CPU is already stopped and store-status-at-address avoids the detour
via the absolute zero page.

For kexec simplify s390_reset_system and call store_status() before
the prefix register of the boot CPU has been set to zero. Use STPX
to store the prefix register and remove dump_prefix_page.

Remove all the casts to and from the machine check interruption code.
This patch changes struct mci to a union, which contains an anonymous
structure with the already known bits and in addition an unsigned
long field, which contains the raw machine check interruption code.

This allows to simply assign and decoce the interruption code value
without the need for all those casts we had all the time.

Extended measurement blocks need to be 64 byte aligned. To achieve that
128 bytes for each measurement block are allocated and an align callback
returns a 64 byte aligned address inside this area.

Sebastian Ott 616503d1d8 s390/cio: improve locking during cmbe allocation
During allocation of extended measurement blocks we check if the device is
already active for channel measurement and add the device to a list of
devices with active channel measurement. The check is done under ccwlock
protection and the list modification is guarded by a different lock.

To guarantee that both states are in sync make sure that both locks
are held during the allocation process (like it's already done for the
"normal" measurement block allocation).

Sebastian Ott a6ef15652d s390/cio: fix use after free in cmb processing
Devices with active channel measurement are included in a list. When a
device is removed without deactivating channel measurement first the
list_head is freed but still used. Fix this by making sure that
channel measurement is deactivated during device deregistration.

For devices that we deregister because they are no longer accessible
deactivating channel measurement will fail. In this case we can report
success because the FW will no longer access the measurement block.

In addition to these steps keep an extra device reference while
channel measurement is active.

Peter Oberparleiter 1d2334cb7d s390/cio: Implement proper Link Incident Record handling
A hardware problem on a FICON link is reported by the Channel Subsystem
to the operating system via a Link Incident Record (LIR). In response,
the operating system should issue a message that enables hardware
service personnel to identify and repair the failing component.

Current Linux LIR handling is broken because LIR data is incorrectly
interpreted and no log message is generated.

This patch fixes Linux LIR handling by implementing a new log message
for LIRs indicating a degraded or non-operational link. Also LIRs are
no longer used to deactivate channel paths because the available data
does not reliably allow to determine the affected channel path.

Heiko Carstens 5a79859ae0 s390: remove 31 bit support
Remove the 31 bit support in order to reduce maintenance cost and
effectively remove dead code. Since a couple of years there is no
distribution left that comes with a 31 bit kernel.

The 31 bit kernel also has been broken since more than a year before
anybody noticed. In addition I added a removal warning to the kernel
shown at ipl for 5 minutes: a960062e58 ("s390: add 31 bit warning
message") which let everybody know about the plan to remove 31 bit
code. We didn't get any response.

Given that the last 31 bit only machine was introduced in 1999 let's
remove the code.
Anybody with 31 bit user space code can still use the compat mode.

Martin Schwidefsky 10ad34bc76 s390: add SMT support
The multi-threading facility is introduced with the z13 processor family.
This patch adds code to detect the multi-threading facility. With the
facility enabled each core will surface multiple hardware threads to the
system. Each hardware threads looks like a normal CPU to the operating
system with all its registers and properties.

The SCLP interface reports the SMT topology indirectly via the maximum
thread id. Each reported CPU in the result of a read-scp-information
is a core representing a number of hardware threads.

To reflect the reduced CPU capacity if two hardware threads run on a
single core the MT utilization counter set is used to normalize the
raw cputime obtained by the CPU timer deltas. This scaled cputime is
reported via the taskstats interface. The normal /proc/stat numbers
are based on the raw cputime and are not affected by the normalization.

Rasmus Villemoes 3f8bfd9a75 s390/cio: replace strnicmp with strncasecmp
The kernel used to contain two functions for length-delimited,
case-insensitive string comparison, strnicmp with correct semantics and
a slightly buggy strncasecmp.  The latter is the POSIX name, so strnicmp
was renamed to strncasecmp, and strnicmp made into a wrapper for the new
strncasecmp to avoid breaking existing users.

To allow the compat wrapper strnicmp to be removed at some point in the
future, and to avoid the extra indirection cost, do
s/strnicmp/strncasecmp/g.

buffers. The qeth driver uses huge contiguous buffers which leads
to high order allocations with all their downsides.

Sebastian Ott 2c3e7e15c7 s390/cio: set device name as early as possible
Currently we set the device name at the time we call device_add after
we receive the interrupt for the first I/O. When something is not working
as expected during that first I/O (e.g. we don't receive an interrupt) we
print a message including the device name which has not yet been
initialized.

Set the device name after calling device_initialize (prior to starting
the first I/O) so that we have the name present if some unexpected error
occurs during that first I/O.

Sebastian Ott dbe33fc9ad s390/cio: silence lockdep warning
On systems where a ccw based console device is used a lockdep false alarm
could be triggered when a device driver calls printk while holding a
subchannels lock (e.g. in it's irq handler). Since this is valid behavior
fix this by introducing a separate lock class for the console subchannels
lock.

The lockdep warning was revealed by "printk: enable interrupts before calling
console_trylock_for_printk()" which changed console_unlock() to be called with
lockdep enabled.

Frederic Weisbecker bfc3f0281e cputime: Default implementation of nsecs -> cputime conversion
The architectures that override cputime_t (s390, ppc) don't provide
any version of nsecs_to_cputime(). Indeed this cputime_t implementation
by backend only happens when CONFIG_VIRT_CPU_ACCOUNTING_NATIVE=y under
which the core code doesn't make any use of nsecs_to_cputime().

At least for now.

We are going to make a broader use of it so lets provide a default
version with a per usecs granularity. It should be good enough for most
usecases.

Sebastian Ott f7e1e65d29 s390: improve debug feature usage
The maximum usable buffer size of the s390 debug feature (when using
the sprintf_view) is 11 * sizeof(long) (1 pointer for the format
string + 10 arguments). When a larger buffer size is specified the
additional memory is unused and wasted per debug entry. So reducing
the buffer size to its maximum (or to the actual buffer size used)
will make more precious debug feature space usable.

For pci_msg, chsc_msg, and cio_crw we use the additional usable dbf
space to reduce the number of allocated pages.

Sebastian Ott 1e53209605 s390/cio: reorder initialization of ccw consoles
Drivers for ccw consoles use ccw_device_probe_console to receive
an initialized ccw device which is already enabled for interrupts.
After that the device driver does the initialization of its private
data. This can race with unsolicited interrupts which can happen
once the device is enabled for interrupts.

Split ccw_device_probe_console into ccw_device_create_console and
ccw_device_enable_console and reorder the initialization of the ccw
console drivers.

While at it mark these functions as __init.

Sebastian Ott 2253e8d792 s390/cio: fix driver callback initialization for ccw consoles
ccw consoles are in use before they can be properly registered with
the driver core. For devices which are in use by a device driver we
rely on the ccw_device's pointer to the driver callbacks to be valid.
For ccw consoles this pointer is NULL until they are registered later
during boot and we dereferenced this pointer. This worked by
chance on 64 bit builds (cdev->drv was NULL but the optional callback
cdev->drv->path_event was also NULL by coincidence) and was unnoticed
until we received reports about boot failures on 31 bit systems.
Fix it by initializing the driver pointer for ccw consoles.

Peter Oberparleiter 9955e8d15f s390/cio: Fix missing subchannels after CHPID configure on
Performing a Channel-Path configure on operation on a Channel-Path ID
(CHPID) does not trigger a scan for subchannels that might have become
available through that CHPID. As a result, some subchannels and
associated I/O devices might be missing. Fix this by adding the missing
scan.

This problem was introduced by commit c820de39, "[S390] cio: Rework
css driver.", but wasn't noticed earlier because the machine usually
also generates a Channel-Report-Word when the first CHPID of a
subchannel is configured on, resulting in a separate scan for that
subchannel. The problem only becomes apparent when this first CHPID is
not working properly and additional working CHPIDs are subsequently
configured on without any effect on the availability of the affected
subchannel.

Tejun Heo 0b60f9ead5 s390: use device_remove_file_self() instead of device_schedule_callback()
driver-core now supports synchrnous self-deletion of attributes and
the asynchrnous removal mechanism is scheduled for removal.  Use it
instead of device_schedule_callback().

* Conversions in arch/s390/pci/pci_sysfs.c and
  drivers/s390/block/dcssblk.c are straightforward.

* drivers/s390/cio/ccwgroup.c is a bit more tricky because
  ccwgroup_notifier() was (ab)using device_schedule_callback() to
  purely obtain a process context to kick off ungroup operation which
  may block from a notifier callback.

  Rename ccwgroup_ungroup_callback() to ccwgroup_ungroup() and make it
  take ccwgroup_device * instead.  The new function is now called
  directly from ccwgroup_ungroup_store().

  ccwgroup_notifier() chain is updated to explicitly bounce through
  ccwgroup_device->ungroup_work.  This also removes possible failure
  from memory pressure.

Sebastian Ott 1bc8927cc5 s390/cio: improve cio_commit_config
The modify subchannel wrapper cio_commit_config can fail when
(unexpected) status is pending on the subchannel.

Callers of cio_commit_config (that operated on enabled subchannels)
needed to do error handling for that case (clear the unexpected
status with test subchannel and retry). This error handling is
missing in some code paths and caused online setting of devices to
fail.

Fix this for all callers by moving the error handling inside
cio_commit_config.

Jose Alonso dbb0dd021d s390/qdio: for_each macro correctness
I observed that there are for_each macros that do an extra memory access
beyond the defined area.
Normally this does not cause problems.
But, this can cause exceptions. For example: if the area is allocated at
the end of a page and the next page is not accessible.

For correctness, I suggest changing the arguments of the 'for loop' like
others 'for_each' do in the kernel.

Peter Oberparleiter 175746eb06 s390/cio: Delay scan for newly available I/O devices
The CIO layer scans for newly available I/O devices by performing a scan
of available subchannels using the Store Subchannel (STSCH) instruction.
Performing too many STSCH instructions in a tight loop can cause high
Hypervisor overhead which can negatively impact the performance of the
virtual machine as a whole.

A subchannel scan is triggered for example during a hardware event that
indicates that a channel path has become available. It is also triggered
by the DASD device driver for each device that is set online.

This patch reduces the number of STSCH instructions being performed by
delaying the start of the actual subchannel scan by 1 second. Multiple
scan requests that are scheduled during this time will be merged into a
single scan loop.

The trade-off consists of a short delay that is introduced between
the time that the event is processed and a newly available device
becoming usable. This delay should be acceptable since it only
affects devices that have not been in use before.

This processing can take a significant amount of time during which no
other task can run on the same CPU (unless CONFIG_PREEMPT has been
enabled). As a result, scheduling latencies for other tasks are
increased noticeably, especially on a single-CPU system.

Fix this problem by explicitly allowing other tasks to be scheduled
each time a subchannel has been processed.

This patch reduces the number of STSCH instructions performed while
processing a resource accessibility event and while varying a CHPID
online.

In both cases, Linux first performs a STSCH instruction on each unused
subchannel to see if the subchannel has become available. If the STSCH
instruction indicates that the subchannel is available, a full
evaluation of this subchannel is scheduled. Since the full evaluation
includes performing a STSCH instruction, the initial STSCH is
unnecessary and can be removed.

Stop hiding scm_block's dependency to the eadm subchannel driver
(by using functions provided by the eadm subchannel instead of
wrappers provided by the scm bus).

This will help userspace recognizing module dependencies (e.g. for
building a ramdisk). As a side effect we can get rid of some code
reimplementing refcounting between those modules.

Heiko Carstens 7d7c7b24e4 s390/bitops: rename find_first_bit_left() to find_first_bit_inv()
find_first_bit_left() and friends have nothing to do with the normal
LSB0 bit numbering for big endian machines used in Linux (least
significant bit has bit number 0).
Instead they use MSB0 bit numbering, where the most signficant bit has
bit number 0. So rename find_first_bit_left() and friends to
find_first_bit_inv(), to avoid any confusion.
Also provide inv versions of set_bit, clear_bit and test_bit.

This also removes the confusing use of e.g. set_bit() in airq.c which
uses a "be_to_le" bit number conversion, which could imply that instead
set_bit_le() could be used. But that is entirely wrong since the _le
bitops variant uses yet another bit numbering scheme.

get_inbound_buffer_frontier() makes use of the return value of atomic_sub()
which shouldn't work, since atomic_sub() is supposed to return void.
This only works on s390 because atomic_sub() gets mapped to atomic_sub_return()
with a define without changing it's return value to void.

So use atomic_sub_return() instead of atomic_sub() in qeth code before fixing
atomic ops.

Martin Schwidefsky 8c071b0f19 s390/time: correct use of store clock fast
The result of the store-clock-fast (STCKF) instruction is a bit fuzzy.
It can happen that the value stored on one CPU is smaller than the value
stored on another CPU, although the order of the stores is the other
way around. This can cause deltas of get_tod_clock() values to become
negative when they should not be.

We need to be more careful with store-clock-fast, this patch partially
reverts git commit e4b7b4238e666682555461fa52eecd74652f36bb "time:
always use stckf instead of stck if available". The get_tod_clock()
function now uses the store-clock-extended (STCKE) instruction.
get_tod_clock_fast() can be used if the fuzziness of store-clock-fast
is acceptable e.g. for wait loops local to a CPU.

Martin Schwidefsky 1f44a22577 s390: convert interrupt handling to use generic hardirq
With the introduction of PCI it became apparent that s390 should
convert to generic hardirqs as too many drivers do not have the
correct dependency for GENERIC_HARDIRQS. On the architecture
level s390 does not have irq lines. It has external interrupts,
I/O interrupts and adapter interrupts. This patch hard-codes all
external interrupts as irq #1, all I/O interrupts as irq #2 and
all adapter interrupts as irq #3. The additional information from
the lowcore associated with the interrupt is stored in the
pt_regs of the interrupt frame, where the interrupt handler can
pick it up. For PCI/MSI interrupts the adapter interrupt handler
scans the relevant bit fields and calls generic_handle_irq with
the virtual irq number for the MSI interrupt.

There are three users of adapter interrupts: AP, QDIO and PCI. Each
registers a single adapter interrupt with independent ISCs. Define
a "struct airq" with the interrupt handler, a pointer and a mask for
the local summary indicator and the ISC for the adapter interrupt
source. Convert the indicator array with its fixed number of adapter
interrupt sources per ISE to an array of hlists. This removes the
limitation to 32 adapter interrupts per ISC and allows for arbitrary
memory locations for the local summary indicator.

Sebastian Ott 3ad19b8fb0 s390/cio: fix irq statistics
When we fetch an interrupt on the CCW console using tsch (via
ccw_device_wait_idle formerly known as wait_cons_dev) we increment
the irq count for the affected interruption class but it is not
accounted as an IO interrupt.

This is broken since commit b603d258a4
"s390: remove superfluous tpi from wait_cons_dev"

Fix it so that the sum of the individual interrupts per class matches
the number of IO interrupts again.

Via ccw_device_init_count we keep track of how many devices are in
asynchronous device recognition/initialization. For early devices this
variable was not only used prior to its initialization but used
incorrectly (incremented but never decremented). Fix this by using static
initialization for this variable (and friends), make them visible to
device.c only, and decrement the counter after recognition of early
devices is finished.

Do not abort path verification when waiting for an interrupt timed out.
Use path_noirq_mask to keep track of the paths used for this (also
maintain a path_notoper_mask for debugging purposes). If the timeout
happend to be during an operation where we query or alter the state of
path groups set the pgid_unknown flag.

With this change we allow usage of devices which have such ill-behaved
paths (if at least one path is operational).

Sebastian Ott 509d97b6f9 s390/chsc: fix SEI usage
cbc0dd1 "s390/pci: CHSC PCI support for error and availability events"
introduced a new SEI notification type as part of pci support.
The way SEI was called with nt2 and nt0 consecutive broke the nt0
stuff used for channel subsystem notifications.

The reason why this was broken with the mentioned patch is that you
cannot selectively disable type 0 notifications (so even when asked
for type 2 only, type 0 could be presented).

The way to do it is to tell SEI which types of notification you can
process and -this is the important part- look at the SEI result which
notification type you actually received.

Heiko Carstens 420f42ecf4 s390/irq: remove split irq fields from /proc/stat
Now that irq sum accounting for /proc/stat's "intr" line works again we
have the oddity that the sum field (first field) contains only the sum
of the second (external irqs) and third field (I/O interrupts).
The reason for that is that these two fields are already sums of all other
fields. So if we would sum up everything we would count every interrupt
twice.
This is broken since the split interrupt accounting was merged two years
ago: 052ff461c8 "[S390] irq: have detailed
statistics for interrupt types".
To fix this remove the split interrupt fields from /proc/stat's "intr"
line again and only have them in /proc/interrupts.

This restores the old behaviour, seems to be the only sane fix and mimics
a behaviour from other architectures where /proc/interrupts also contains
more than /proc/stat's "intr" line does.

Heiko Carstens add9bde216 s390/irq: enable irq sum accounting for /proc/stat again
For more than two years, since f2c66cd8ee
"/proc/stat: scalability of irq num per cpu" the output of /proc/stat is
broken.
The first field in the "intr" line should contain the sum of all interrupts,
however since the above mentioned change it is always zero.

The reason for that is that a per cpu irq sum variable had been introduced
which got incremented when calling kstat_incr_irqs_this_cpu(). However
on s390 we directly incremented only the per cpu per irq counter by accessing
the array element via kstat_cpu(smp_processor_id()).irqs[...].
So fix this and use the kstat_incr_irqs_this_cpu() wrapper which increments
both: the per cpu per irq counter and the per cpu irq sum counter.

Jan Glauber 6726a807c3 s390/qdio: rename the misleading PCI flag of qdio devices
For each detected qdio device a line like to following is printed:
qdio: 0.0.4102 OSA on SC 1045 using AI:1 QEBSM:0 PCI:1 TDD:1 SIGA: W AP

The PCI flag is misleading as this stands for "program controlled interrupt".
Rename it to PRI "program requested interrupt" which is more accurate and
does not interfere with another popular piece of technology.

Leave the pci string in the code since changing that would result in a huge
patch.

Sebastian Ott d99e79ec55 s390/cio: fix pgid reserved check
The check to whom a device is reserved is done by checking the path
state of the affected channel paths. If it turns out that one path is
flagged as reserved by someone else the whole device is marked as such.

However the meaning of the RESVD_ELSE bit is that the addressed device
is reserved to a different pathgroup (and not reserved to a different
LPAR). If we do this test on a path which is currently not a member of
the pathgroup we could erroneously mark the device as reserved to
someone else.

To fix this collect the reserved state for all potential members of the
pathgroup and only mark the device as reserved if all of those potential
members have the RESVD_ELSE bit set.
