Ira Weiny 487cfade12 mm/highmem: fix CONFIG_DEBUG_KMAP_LOCAL_FORCE_MAP
The kernel test robot found that __kmap_local_sched_out() was not
correctly skipping the guard pages when DEBUG_KMAP_LOCAL_FORCE_MAP was
set.[1] This was due to DEBUG_HIGHMEM check being used.

Change the configuration check to be correct.

OGAWA Hirofumi 184cee516f mm/highmem.c: fix zero_user_segments() with start > end
zero_user_segments() is used from __block_write_begin_int(), for example
like the following

	zero_user_segments(page, 4096, 1024, 512, 918)

But new the zero_user_segments() implementation for for HIGHMEM +
TRANSPARENT_HUGEPAGE doesn't handle "start > end" case correctly, and hits
BUG_ON().  (we can fix __block_write_begin_int() instead though, it is the
old and multiple usage)

Also it calls kmap_atomic() unnecessarily while start == end == 0.

The generic kmap_local() map function uses set_pte_at(), but MIPS requires
set_pte() and PowerPC wants __set_pte_at().

Matthew Wilcox (Oracle) 0060ef3b4e mm: support THPs in zero_user_segments
We can only kmap() one subpage of a THP at a time, so loop over all
relevant subpages, skipping ones which don't need to be zeroed.  This is
too large to inline when THPs are enabled and we actually need highmem, so
put it in highmem.c.

Thomas Gleixner f3ba3c710a mm/highmem: Provide kmap_local*
Now that the kmap atomic index is stored in task struct provide a
preemptible variant. On context switch the maps of an outgoing task are
removed and the map of the incoming task are restored. That's obviously
slow, but highmem is slow anyway.

The kmap_local.*() functions can be invoked from both preemptible and
atomic context. kmap local sections disable migration to keep the resulting
virtual mapping address correct, but disable neither pagefaults nor
preemption.

A wholesale conversion of kmap_atomic to be fully preemptible is not
possible because some of the usage sites might rely on the preemption
disable for serialization or on the implicit pagefault disable. Needs to be
done on a case by case basis.

Thomas Gleixner 5fbda3ecd1 sched: highmem: Store local kmaps in task struct
Instead of storing the map per CPU provide and use per task storage. That
prepares for local kmaps which are preemptible.

The context switch code is preparatory and not yet in use because
kmap_atomic() runs with preemption disabled. Will be made usable in the
next step.

The context switch logic is safe even when an interrupt happens after
clearing or before restoring the kmaps. The kmap index in task struct is
not modified so any nesting kmap in an interrupt will use unused indices
and on return the counter is the same as before.

Also add an assert into the return to user space code. Going back to user
space with an active kmap local is a nono.

Thomas Gleixner 0e91a0c698 mm/highmem: Provide CONFIG_DEBUG_KMAP_LOCAL_FORCE_MAP
CONFIG_DEBUG_KMAP_LOCAL, which is selected by CONFIG_DEBUG_HIGHMEM is only
providing guard pages, but does not provide a mechanism to enforce the
usage of the kmap_local() infrastructure.

Provide CONFIG_DEBUG_KMAP_LOCAL_FORCE_MAP which forces the temporary
mapping even for lowmem pages. This needs to be a seperate config switch
because this only works on architectures which do not have cache aliasing
problems.

Thomas Gleixner 6e799cb69a mm/highmem: Provide and use CONFIG_DEBUG_KMAP_LOCAL
CONFIG_KMAP_LOCAL can be enabled by x86/32bit even if CONFIG_HIGHMEM is not
enabled for temporary MMIO space mappings.

Provide it as a seperate config option which depends on CONFIG_KMAP_LOCAL
and let CONFIG_DEBUG_HIGHMEM select it.

This won't increase the debug coverage of this significantly but it paves
the way to do so.

Thomas Gleixner 2a656cad33 mm/highmem: Take kmap_high_get() properly into account
kunmap_local() warns when the virtual address to unmap is below
PAGE_OFFSET. This is correct except for the case that the mapping was
obtained via kmap_high_get() because the PKMAP addresses are right below
PAGE_OFFSET.

Cure it by skipping the WARN_ON() when the unmap was handled by
kunmap_high().

Thomas Gleixner 389755c250 highmem: Make DEBUG_HIGHMEM functional
For some obscure reason when CONFIG_DEBUG_HIGHMEM is enabled the stack
depth is increased from 20 to 41. But the only thing DEBUG_HIGHMEM does is
to enable a few BUG_ON()'s in the mapping code.

That's a leftover from the historical mapping code which had fixed entries
for various purposes. DEBUG_HIGHMEM inserted guard mappings between the map
types. But that got all ditched when kmap_atomic() switched to a stack
based map management. Though the WITH_KM_FENCE magic survived without being
functional. All the thing does today is to increase the stack depth.

Add a working implementation to the generic kmap_local* implementation.

Ingo Molnar 186525bd6b mm, x86/mm: Untangle address space layout definitions from basic pgtable type definitions
- Untangle the somewhat incestous way of how VMALLOC_START is used all across the
  kernel, but is, on x86, defined deep inside one of the lowest level page table headers.
  It doesn't help that vmalloc.h only includes a single asm header:

     #include <asm/page.h>           /* pgprot_t */

  So there was no existing cross-arch way to decouple address layout
  definitions from page.h details. I used this:

   #ifndef VMALLOC_START
   # include <asm/vmalloc.h>
   #endif

  This way every architecture that wants to simplify page.h can do so.

- Also on x86 we had a couple of LDT related inline functions that used
  the late-stage address space layout positions - but these could be
  uninlined without real trouble - the end result is cleaner this way as
  well.

Arun KS ca79b0c211 mm: convert totalram_pages and totalhigh_pages variables to atomic
totalram_pages and totalhigh_pages are made static inline function.

Main motivation was that managed_page_count_lock handling was complicating
things.  It was discussed in length here,

better to remove the lock and convert variables to atomic, with preventing
poteintial store-to-read tearing as a bonus.

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   SPDX license identifier                            # files

   and resulted in the first patch in this series.

   If that file was a */uapi/* path one, it was "GPL-2.0 WITH

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Joonsoo Kim 33499bfe50 mm/highmem: make nr_free_highpages() handles all highmem zones by itself
nr_free_highpages() manually adds statistics per each highmem zone and
returns a total value for them.  Whenever we add a new highmem zone, we
need to consider this function and it's really troublesome.  Make it
handle all highmem zones by itself.

Max Filippov 15de36a4c3 mm/highmem: make kmap cache coloring aware
User-visible effect:
 Architectures that choose this method of maintaining cache coherency
(MIPS and xtensa currently) are able to use high memory on cores with
aliasing data cache.  Without this fix such architectures can not use
high memory (in case of xtensa it means that at most 128 MBytes of
physical memory is available).

The problem:
 VIPT cache with way size larger than MMU page size may suffer from
aliasing problem: a single physical address accessed via different
virtual addresses may end up in multiple locations in the cache.
Virtual mappings of a physical address that always get cached in
different cache locations are said to have different colors.  L1 caching
hardware usually doesn't handle this situation leaving it up to
software.  Software must avoid this situation as it leads to data
corruption.

What can be done:
 One way to handle this is to flush and invalidate data cache every time
page mapping changes color.  The other way is to always map physical
page at a virtual address with the same color.  Low memory pages already
have this property.  Giving architecture a way to control color of high
memory page mapping allows reusing of existing low memory cache alias
handling code.

How this is done with this patch:
 Provide hooks that allow architectures with aliasing cache to align
mapping address of high pages according to their color.  Such
architectures may enforce similar coloring of low- and high-memory page
mappings and reuse existing cache management functions to support
highmem.

This code is based on the implementation of similar feature for MIPS by
Leonid Yegoshin.

Will Deacon 498c228021 mm: highmem: don't treat PKMAP_ADDR(LAST_PKMAP) as a highmem address
kmap_to_page returns the corresponding struct page for a virtual address
of an arbitrary mapping.  This works by checking whether the address
falls in the pkmap region and using the pkmap page tables instead of the
linear mapping if appropriate.

Unfortunately, the bounds checking means that PKMAP_ADDR(LAST_PKMAP) is
incorrectly treated as a highmem address and we can end up walking off
the end of pkmap_page_table and subsequently passing junk to pte_page.

This patch fixes the bound check to stay within the pkmap tables.

Mel Gorman 5a178119b0 mm: add support for direct_IO to highmem pages
The patch "mm: add support for a filesystem to activate swap files and use
direct_IO for writing swap pages" added support for using direct_IO to
write swap pages but it is insufficient for highmem pages.

To support highmem pages, this patch kmaps() the page before calling the
direct_IO() handler.  As direct_IO deals with virtual addresses an
additional helper is necessary for get_kernel_pages() to lookup the struct
page for a kmap virtual address.

Peter Zijlstra 3e4d3af501 mm: stack based kmap_atomic()
Keep the current interface but ignore the KM_type and use a stack based
approach.

The advantage is that we get rid of crappy code like:

and in general can stop worrying about what context we're in and what kmap
slots might be appropriate for that.

The downside is that FRV kmap_atomic() gets more expensive.

For now we use a CPP trick suggested by Andrew:

to avoid having to touch all kmap_atomic() users in a single patch.

Soeren Sandmann 5ebd4c2289 highmem: Fix race in debug_kmap_atomic() which could cause warn_count to underflow
debug_kmap_atomic() tries to prevent ever printing more than 10
warnings, but it does so by testing whether an unsigned integer
is equal to 0. However, if the warning is caused by a nested
IRQ, then this counter may underflow and the stream of warnings
will never end.

Fix that by using a signed integer instead.

Akinobu Mita f4112de6b6 mm: introduce debug_kmap_atomic
x86 has debug_kmap_atomic_prot() which is error checking function for
kmap_atomic.  It is usefull for the other architectures, although it needs
CONFIG_TRACE_IRQFLAGS_SUPPORT.

This patch exposes it to the other architectures.

Nicolas Pitre 3297e76077 highmem: atomic highmem kmap page pinning
Most ARM machines have a non IO coherent cache, meaning that the
dma_map_*() set of functions must clean and/or invalidate the affected
memory manually before DMA occurs.  And because the majority of those
machines have a VIVT cache, the cache maintenance operations must be
performed using virtual
addresses.

When a highmem page is kunmap'd, its mapping (and cache) remains in place
in case it is kmap'd again. However if dma_map_page() is then called with
such a page, some cache maintenance on the remaining mapping must be
performed. In that case, page_address(page) is non null and we can use
that to synchronize the cache.

It is unlikely but still possible for kmap() to race and recycle the
virtual address obtained above, and use it for another page before some
on-going cache invalidation loop in dma_map_page() is done. In that case,
the new mapping could end up with dirty cache lines for another page,
and the unsuspecting cache invalidation loop in dma_map_page() might
simply discard those dirty cache lines resulting in data loss.

For example, let's consider this sequence of events:

	- dma_map_page(..., DMA_FROM_DEVICE) is called on a highmem page.

	-->	- vaddr = page_address(page) is non null. In this case
		it is likely that the page has valid cache lines
		associated with vaddr. Remember that the cache is VIVT.

		-->	for (i = vaddr; i < vaddr + PAGE_SIZE; i += 32)
				invalidate_cache_line(i);

	*** preemption occurs in the middle of the loop above ***

	- kmap_high() is called for a different page.

	-->	- last_pkmap_nr wraps to zero and flush_all_zero_pkmaps()
		  is called.  The pkmap_count value for the page passed
		  to dma_map_page() above happens to be 1, so the page
		  is unmapped.  But prior to that, flush_cache_kmaps()
		  cleared the cache for it.  So far so good.

		- A fresh pkmap entry is assigned for this kmap request.
		  The Murphy law says this pkmap entry will eventually
		  happen to use the same vaddr as the one which used to
		  belong to the other page being processed by
		  dma_map_page() in the preempted thread above.

	- The kmap_high() caller start dirtying the cache using the
	  just assigned virtual mapping for its page.

	*** the first thread is rescheduled ***

			- The for(...) loop is resumed, but now cached
			  data belonging to a different physical page is
			  being discarded !

And this is not only a preemption issue as ARM can be SMP as well,
making the above scenario just as likely. Hence the need for some kind
of pkmap page pinning which can be used in any context, primarily for
the benefit of dma_map_page() on ARM.

This provides the necessary interface to cope with the above issue if
ARCH_NEEDS_KMAP_HIGH_GET is defined, otherwise the resulting code is
unchanged.

Nick Piggin 5843d9a4d0 x86, pat: avoid highmem cache attribute aliasing
Highmem code can leave ptes and tlb entries around for a given page even after
kunmap, and after it has been freed.

>From what I can gather, the PAT code may change the cache attributes of
arbitrary physical addresses (ie. including highmem pages), which would result
in aliases in the case that it operates on one of these lazy tlb highmem
pages.

Flushing kmaps should solve the problem.

I've also just added code for conditional flushing if we haven't got
any dangling highmem aliases -- this should help performance if we
change page attributes frequently or systems that aren't using much
highmem pages (eg. if < 4G RAM). Should be turned into 2 patches, but
just for RFC...

David S. Miller db7a94d60f highmem: Export totalhigh_pages.
Hash et al. sizing code in SCTP wants to make the
calculation totalram_pages - totalhigh_pages, just
like TCP.  But this requires an export for the
CONFIG_HIGHMEM case to work.
