'nobats' kernel parameter or some options like CONFIG_DEBUG_PAGEALLOC
deny the use of BATS for mapping memory.

This patch makes sure that the specific wii RAM mapping function
takes it into account as well.

Paul Mackerras 19f8a5b5be powerpc/powernv: Don't reprogram SLW image on every KVM guest entry/exit
Commit 24be85a23d ("powerpc/powernv: Clear PECE1 in LPCR via stop-api
only on Hotplug", 2017-07-21) added two calls to opal_slw_set_reg()
inside pnv_cpu_offline(), with the aim of changing the LPCR value in
the SLW image to disable wakeups from the decrementer while a CPU is
offline.  However, pnv_cpu_offline() gets called each time a secondary
CPU thread is woken up to participate in running a KVM guest, that is,
not just when a CPU is offlined.

Since opal_slw_set_reg() is a very slow operation (with observed
execution times around 20 milliseconds), this means that an offline
secondary CPU can often be busy doing the opal_slw_set_reg() call
when the primary CPU wants to grab all the secondary threads so that
it can run a KVM guest.  This leads to messages like "KVM: couldn't
grab CPU n" being printed and guest execution failing.

There is no need to reprogram the SLW image on every KVM guest entry
and exit.  So that we do it only when a CPU is really transitioning
between online and offline, this moves the calls to
pnv_program_cpu_hotplug_lpcr() into pnv_smp_cpu_kill_self().

Tyrel Datwyler 9f3ba362d8 powerpc/pseries: export timebase register sample in lparcfg
The Processor Utilzation of Resource Registers (PURR) provide an
estimate of resources used by a cpu thread. Section 7.6 in Book III of
the ISA outlines how to calculate the percentage of shared resources
for threads using the ratio of the PURR delta and Timebase Register
delta for a sampled period.

This calculation is currently done erroneously by the lparstat tool
from the powerpc-utils package. This patch exports the current
timebase value after we sample the PURRs and exposes it to userspace
accounting tools via /proc/ppc64/lparcfg.

Michael Ellerman aa7150ba37 powerpc/44x: Force PCI on for CURRITUCK
The recent rework of PCI kconfig symbols exposed an existing bug in
the CURRITUCK kconfig logic.

It selects PPC4xx_PCI_EXPRESS which depends on PCI, but PCI is user
selectable and might be disabled, leading to a warning:

Prior to commit eb01d42a77 ("PCI: consolidate PCI config entry in
drivers/pci") PCI was enabled by default for currituck_defconfig so we
didn't see the warning. The bad logic was still there, it just
required someone disabling PCI in their .config to hit it.

Fix it by forcing PCI on for CURRITUCK, which seems was always the
expectation anyway.

Peter Xu 1b58a975be powerpc/powernv/npu: Remove redundant change_pte() hook
The change_pte() notifier was designed to use as a quick path to
update secondary MMU PTEs on write permission changes or PFN changes.
For KVM, it could reduce the vm-exits when vcpu faults on the pages
that was touched up by KSM. It's not used to do cache invalidations,
for example, if we see the notifier will be called before the real PTE
update after all (please see set_pte_at_notify that set_pte_at was
called later).

All the necessary cache invalidation should all be done in
invalidate_range() already.

Alexey Kardashevskiy 8f5b27347e powerpc/powernv/sriov: Register IOMMU groups for VFs
The compound IOMMU group rework moved iommu_register_group() together
in pnv_pci_ioda_setup_iommu_api() (which is a part of
ppc_md.pcibios_fixup). As the result, pnv_ioda_setup_bus_iommu_group()
does not create groups any more, it only adds devices to groups.

This works fine for boot time devices. However IOMMU groups for
SRIOV's VFs were added by pnv_ioda_setup_bus_iommu_group() so this got
broken: pnv_tce_iommu_bus_notifier() expects a group to be registered
for VF and it is not.

This adds missing group registration and adds a NULL pointer check
into the bus notifier so we won't crash if there is no group, although
it is not expected to happen now because of the change above.

Now that we've switched all the powerpc nommu and swiotlb methods to
use the generic dma_direct_* calls we can remove these ops vectors
entirely and rely on the common direct mapping bypass that avoids
indirect function calls entirely.  This also allows to remove a whole
lot of boilerplate code related to setting up these operations.

vio_dma_mapping_ops currently does a lot of indirect calls through
dma_iommu_ops, which not only make the code harder to follow but are
also expensive in the post-spectre world.  Unwind the indirect calls
by calling the ppc_iommu_* or iommu_* APIs directly applicable, or
just use the dma_iommu_* methods directly where we can.

Oliver O'Halloran b174b4fb91 powerpc/powernv: Escalate reset when IODA reset fails
The IODA reset is used to flush out any OS controlled state from the PHB.
This reset can fail if a PHB fatal error has occurred in early boot,
probably due to a because of a bad device. We already do a fundemental
reset of the device in some cases, so this patch just adds a test to force
a full reset if firmware reports an error when performing the IODA reset.

Oliver O'Halloran 5a3840a470 powerpc/papr_scm: Use the correct bind address
When binding an SCM volume to a physical address the hypervisor has the
option to return early with a continue token with the expectation that
the guest will resume the bind operation until it completes. A quirk of
this interface is that the bind address will only be returned by the
first bind h-call and the subsequent calls will return
0xFFFF_FFFF_FFFF_FFFF for the bind address.

We currently do not save the address returned by the first h-call. As a
result we will use the junk address as the base of the bound region if
the hypervisor decides to split the bind across multiple h-calls. This
bug was found when testing with very large SCM volumes where the bind
process would take more time than they hypervisor's internal h-call time
limit would allow. This patch fixes the issue by saving the bind address
from the first call.

Nathan Fontenot 81b6132492 powerpc/pseries: Perform full re-add of CPU for topology update post-migration
On pseries systems, performing a partition migration can result in
altering the nodes a CPU is assigned to on the destination system. For
exampl, pre-migration on the source system CPUs are in node 1 and 3,
post-migration on the destination system CPUs are in nodes 2 and 3.

Handling the node change for a CPU can cause corruption in the slab
cache if we hit a timing where a CPUs node is changed while cache_reap()
is invoked. The corruption occurs because the slab cache code appears
to rely on the CPU and slab cache pages being on the same node.

The current dynamic updating of a CPUs node done in arch/powerpc/mm/numa.c
does not prevent us from hitting this scenario.

Changing the device tree property update notification handler that
recognizes an affinity change for a CPU to do a full DLPAR remove and
add of the CPU instead of dynamically changing its node resolves this
issue.

Finn Thain 20e07af71f powerpc: Adopt nvram module for PPC64
Adopt nvram module to reduce code duplication. This means CONFIG_NVRAM
becomes available to PPC64 builds. Previously it was only available to
PPC32 builds because it depended on CONFIG_GENERIC_NVRAM.

The IOC_NVRAM_GET_OFFSET ioctl as implemented on PPC64 validates the
offset returned by pmac_get_partition(). Do the same in the nvram module.

Note that the old PPC32 generic_nvram module lacked this test.
So when CONFIG_PPC32 && CONFIG_PPC_PMAC, the IOC_NVRAM_GET_OFFSET ioctl
would have returned 0 (always). But when CONFIG_PPC64 && CONFIG_PPC_PMAC,
the IOC_NVRAM_GET_OFFSET ioctl would have returned -1 (which is -EPERM)
when the requested partition was not found.

With this patch, the result is now -EINVAL on both PPC32 and PPC64 when
the requested PowerMac NVRAM partition is not found. This is a userspace-
visible change, in the non-existent partition case, which would be in
an error path for an IOC_NVRAM_GET_OFFSET ioctl syscall.

Add the nvram_size() function to those PowerPC platforms that don't already
have one: CHRP and PowerMac. This means that the ppc_md.nvram_size()
function can be called by nvram_get_size().

Since we are addressing CHRP inconsistencies here, rename chrp_nvram_read
and chrp_nvram_write, which break the naming convention used across
powerpc platforms for NVRAM accessor functions.

An opencapi device is using a device PE, so the current code breaks
because pe->pbus is not defined.

More generally, there's no need to define an IOMMU group for opencapi,
as the device sends real addresses directly (admittedly, the
virtualization story is yet to be written). So let's fix it by
skipping the IOMMU group setup for opencapi PHBs.

Dan Williams 8fc5c73554 acpi/nfit, device-dax: Identify differentiated memory with a unique numa-node
Persistent memory, as described by the ACPI NFIT (NVDIMM Firmware
Interface Table), is the first known instance of a memory range
described by a unique "target" proximity domain. Where "initiator" and
"target" proximity domains is an approach that the ACPI HMAT
(Heterogeneous Memory Attributes Table) uses to described the unique
performance properties of a memory range relative to a given initiator
(e.g. CPU or DMA device).

Currently the numa-node for a /dev/pmemX block-device or /dev/daxX.Y
char-device follows the traditional notion of 'numa-node' where the
attribute conveys the closest online numa-node. That numa-node attribute
is useful for cpu-binding and memory-binding processes *near* the
device. However, when the memory range backing a 'pmem', or 'dax' device
is onlined (memory hot-add) the memory-only-numa-node representing that
address needs to be differentiated from the set of online nodes. In
other words, the numa-node association of the device depends on whether
you can bind processes *near* the cpu-numa-node in the offline
device-case, or bind process *on* the memory-range directly after the
backing address range is onlined.

Allow for the case that platform firmware describes persistent memory
with a unique proximity domain, i.e. when it is distinct from the
proximity of DRAM and CPUs that are on the same socket. Plumb the Linux
numa-node translation of that proximity through the libnvdimm region
device to namespaces that are in device-dax mode. With this in place the
proposed kmem driver [1] can optionally discover a unique numa-node
number for the address range as it transitions the memory from an
offline state managed by a device-driver to an online memory range
managed by the core-mm.

The jump label is controlled by HAVE_JUMP_LABEL, which is defined

Linus Torvalds 96d4f267e4 Remove 'type' argument from access_ok() function
Nobody has actually used the type (VERIFY_READ vs VERIFY_WRITE) argument
of the user address range verification function since we got rid of the
old racy i386-only code to walk page tables by hand.

It existed because the original 80386 would not honor the write protect
bit when in kernel mode, so you had to do COW by hand before doing any
user access.  But we haven't supported that in a long time, and these
days the 'type' argument is a purely historical artifact.

A discussion about extending 'user_access_begin()' to do the range
checking resulted this patch, because there is no way we're going to
move the old VERIFY_xyz interface to that model.  And it's best done at
the end of the merge window when I've done most of my merges, so let's
just get this done once and for all.

This patch was mostly done with a sed-script, with manual fix-ups for
the cases that weren't of the trivial 'access_ok(VERIFY_xyz' form.

There were a couple of notable cases:

 - csky still had the old "verify_area()" name as an alias.

 - the iter_iov code had magical hardcoded knowledge of the actual
   values of VERIFY_{READ,WRITE} (not that they mattered, since nothing
   really used it)

 - microblaze used the type argument for a debug printout

but other than those oddities this should be a total no-op patch.

I tried to fix up all architectures, did fairly extensive grepping for
access_ok() uses, and the changes are trivial, but I may have missed
something.  Any missed conversion should be trivially fixable, though.

Arun KS ca79b0c211 mm: convert totalram_pages and totalhigh_pages variables to atomic
totalram_pages and totalhigh_pages are made static inline function.

Main motivation was that managed_page_count_lock handling was complicating
things.  It was discussed in length here,
https://lore.kernel.org/patchwork/patch/995739/#1181785 So it seemes
better to remove the lock and convert variables to atomic, with preventing
poteintial store-to-read tearing as a bonus.

When the watchdog timer is set in interrupt mode, it causes a
machine check when it times out. The purpose of this mode is to
ease debugging, not to crash the kernel and reboot the machine.

This patch implements a special handling for that, in order to not
crash the kernel if the watchdog times out while in interrupt or
within the idle task.

Alexey Kardashevskiy 58629c0dc3 powerpc/powernv/npu: Fault user page into the hypervisor's pagetable
When a page fault happens in a GPU, the GPU signals the OS and the GPU
driver calls the fault handler which populated a page table; this allows
the GPU to complete an ATS request.

On the bare metal get_user_pages() is enough as it adds a pte to
the kernel page table but under KVM the partition scope tree does not get
updated so ATS will still fail.

This reads a byte from an effective address which causes HV storage
interrupt and KVM updates the partition scope tree.

Alexey Kardashevskiy 1b785611e1 powerpc/powernv/npu: Add release_ownership hook
In order to make ATS work and translate addresses for arbitrary
LPID and PID, we need to program an NPU with LPID and allow PID wildcard
matching with a specific MSR mask.

This implements a helper to assign a GPU to LPAR and program the NPU
with a wildcard for PID and a helper to do clean-up. The helper takes
MSR (only DR/HV/PR/SF bits are allowed) to program them into NPU2 for
ATS checkout requests support.

This exports pnv_npu2_unmap_lpar_dev() as following patches will use it
from the VFIO driver.

Alexey Kardashevskiy 0bd971676e powerpc/powernv/npu: Add compound IOMMU groups
At the moment the powernv platform registers an IOMMU group for each
PE. There is an exception though: an NVLink bridge which is attached
to the corresponding GPU's IOMMU group making it a master.

Now we have POWER9 systems with GPUs connected to each other directly
bypassing PCI. At the moment we do not control state of these links so
we have to put such interconnected GPUs to one IOMMU group which means
that the old scheme with one GPU as a master won't work - there will
be up to 3 GPUs in such group.

This introduces a npu_comp struct which represents a compound IOMMU
group made of multiple PEs - PCI PEs (for GPUs) and NPU PEs (for
NVLink bridges). This converts the existing NVLink1 code to use the
new scheme. >From now on, each PE must have a valid
iommu_table_group_ops which will either be called directly (for a
single PE group) or indirectly from a compound group handlers.

This moves IOMMU group registration for NVLink-connected GPUs to
npu-dma.c. For POWER8, this stores a new compound group pointer in the
PE (so a GPU is still a master); for POWER9 the new group pointer is
stored in an NPU (which is allocated per a PCI host controller).

Alexey Kardashevskiy 83fb8ccf97 powerpc/powernv/npu: Convert NPU IOMMU helpers to iommu_table_group_ops
At the moment NPU IOMMU is manipulated directly from the IODA2 PCI
PE code; PCI PE acts as a master to NPU PE. Soon we will have compound
IOMMU groups with several PEs from several different PHB (such as
interconnected GPUs and NPUs) so there will be no single master but
a one big IOMMU group.

This makes a first step and converts an NPU PE with a set of extern
function to a table group.

This should cause no behavioral change. Note that
pnv_npu_release_ownership() has never been implemented.

Normal PCI PEs have 2 TVEs, one per a DMA window; however NPU PE has only
one which points to one of two tables of the corresponding PCI PE.

So whenever a new DMA window is programmed to PEs, the NPU PE needs to
release old table in order to use the new one.

Commit d41ce7b1bc ("powerpc/powernv/npu: Do not try invalidating 32bit
table when 64bit table is enabled") did just that but in pci-ioda.c
while it actually belongs to npu-dma.c.

This moves the single TVE handling to npu-dma.c. This does not implement
restoring though as it is highly unlikely that we can set the table to
PCI PE and cannot to NPU PE and if that fails, we could only set 32bit
table to NPU PE and this configuration is not really supported or wanted.

Registering new IOMMU groups and adding devices to them are separated in
code and the latter is dug in the DMA setup code which it does not
really belong to.

This moved IOMMU groups setup to a separate helper which registers a group
and adds devices as before. This does not make a difference as IOMMU
groups are not used anyway; the only dependency here is that
iommu_add_device() requires a valid pointer to an iommu_table
(set by set_iommu_table_base()).

To keep the old behaviour, this does not add new IOMMU groups for PEs
with no DMA weight and also skips NVLink bridges which do not have

Alexey Kardashevskiy c4e9d3c1e6 powerpc/powernv/pseries: Rework device adding to IOMMU groups
The powernv platform registers IOMMU groups and adds devices to them
from the pci_controller_ops::setup_bridge() hook except one case when
virtual functions (SRIOV VFs) are added from a bus notifier.

The pseries platform registers IOMMU groups from
the pci_controller_ops::dma_bus_setup() hook and adds devices from
the pci_controller_ops::dma_dev_setup() hook. The very same bus notifier
used for powernv does not add devices for pseries though as
__of_scan_bus() adds devices first, then it does the bus/dev DMA setup.

Both platforms use iommu_add_device() which takes a device and expects
it to have a valid IOMMU table struct with an iommu_table_group pointer
which in turn points the iommu_group struct (which represents
an IOMMU group). Although the helper seems easy to use, it relies on
some pre-existing device configuration and associated data structures
which it does not really need.

This simplifies iommu_add_device() to take the table_group pointer
directly. Pseries already has a table_group pointer handy and the bus
notified is not used anyway. For powernv, this copies the existing bus
notifier, makes it work for powernv only which means an easy way of
getting to the table_group pointer. This was tested on VFs but should
also support physical PCI hotplug.

Since iommu_add_device() receives the table_group pointer directly,
pseries does not do TCE cache invalidation (the hypervisor does) nor
allow multiple groups per a VFIO container (in other words sharing
an IOMMU table between partitionable endpoints), this removes
iommu_table_group_link from pseries.

Alexey Kardashevskiy 68c0449ea1 powerpc/pseries/iommu: Use memory@ nodes in max RAM address calculation
We might have memory@ nodes with "linux,usable-memory" set to zero
(for example, to replicate powernv's behaviour for GPU coherent memory)
which means that the memory needs an extra initialization but since
it can be used afterwards, the pseries platform will try mapping it
for DMA so the DMA window needs to cover those memory regions too;
if the window cannot cover new memory regions, the memory onlining fails.

This walks through the memory nodes to find the highest RAM address to
let a huge DMA window cover that too in case this memory gets onlined
later.

Alexey Kardashevskiy 0e759bd752 powerpc/powernv/npu: Move OPAL calls away from context manipulation
When introduced, the NPU context init/destroy helpers called OPAL which
enabled/disabled PID (a userspace memory context ID) filtering in an NPU
per a GPU; this was a requirement for P9 DD1.0. However newer chip
revision added a PID wildcard support so there is no more need to
call OPAL every time a new context is initialized. Also, since the PID
wildcard support was added, skiboot does not clear wildcard entries
in the NPU so these remain in the hardware till the system reboot.

This moves LPID and wildcard programming to the PE setup code which
executes once during the booting process so NPU2 context init/destroy
won't need to do additional configuration.

This replaces the check for FW_FEATURE_OPAL with a check for npu!=NULL as
this is the way to tell if the NPU support is present and configured.

This moves pnv_npu2_init() declaration as pseries should be able to use it.
This keeps pnv_npu2_map_lpar() in powernv as pseries is not allowed to
call that. This exports pnv_npu2_map_lpar_dev() as following patches
will use it from the VFIO driver.

While at it, replace redundant list_for_each_entry_safe() with
a simpler list_for_each_entry().

The powernv PCI code stores NPU data in the pnv_phb struct. The latter
is referenced by pci_controller::private_data. We are going to have NPU2
support in the pseries platform as well but it does not store any
private_data in in the pci_controller struct; and even if it did,
it would be a different data structure.

This makes npu a pointer and stores it one level higher in
the pci_controller struct.

The skiboot firmware has a hot reset handler which fences the NVIDIA V100
GPU RAM on Witherspoons and makes accesses no-op instead of throwing HMIs:

Now we are going to pass V100 via VFIO which most certainly involves
KVM guests which are often terminated without getting a chance to offline
GPU RAM so we end up with a running machine with misconfigured memory.
Accessing this memory produces hardware management interrupts (HMI)
which bring the host down.

To suppress HMIs, this wires up this hot reset hook to vfio_pci_disable()
via pci_disable_device() which switches NPU2 to a safe mode and prevents
HMIs.

will be included by kernel/systbl.S file.

Mahesh Salgaonkar 0db6896ff6 powerpc/fadump: Do not allow hot-remove memory from fadump reserved area.
For fadump to work successfully there should not be any holes in reserved
memory ranges where kernel has asked firmware to move the content of old
kernel memory in event of crash. Now that fadump uses CMA for reserved
area, this memory area is now not protected from hot-remove operations
unless it is cma allocated. Hence, fadump service can fail to re-register
after the hot-remove operation, if hot-removed memory belongs to fadump
reserved region. To avoid this make sure that memory from fadump reserved
area is not hot-removable if fadump is registered.

However, if user still wants to remove that memory, he can do so by
manually stopping fadump service before hot-remove operation.

opal_power_control_init() depends on opal message notifier to be
initialized, which is done in opal_init()->opal_message_init(). But both
these initialization are called through machine initcalls and it all
depends on in which order they being called. So far these are called in
correct order (may be we got lucky) and never saw any issue. But it is
clearer to control initialization order explicitly by moving
opal_power_control_init() into opal_init().

The powernv platform maintains 2 TCE tables for VFIO - a hardware TCE
table and a table with userspace addresses; the latter is used for
marking pages dirty when corresponging TCEs are unmapped from
the hardware table.

a68bd1267b ("powerpc/powernv/ioda: Allocate indirect TCE levels
on demand") enabled on-demand allocation of the hardware table,
however it missed the other table so it has still been fully allocated
at the boot time. This fixes the issue by allocating a single level,
just like we do for the hardware table.

The A-Eon Amigaone X1000's Nemo motherboard has an AMD SB600
connected to one of the PCI-e root ports on its PaSemi
Pwrficient 1628M SoC. Normally the SB600 southbridge would be
connected to a hidden PCI-e port on the system's northbridge,
and as a result doesn't fully comply with the PCI-e spec.

Add code to relax the PCI-e detection in both the root port
and the Linux kernel allowing on board devices to be detected.

Christoph Hellwig 25078dc1f7 powerpc: use mm zones more sensibly
Powerpc has somewhat odd usage where ZONE_DMA is used for all memory on
common 64-bit configfs, and ZONE_DMA32 is used for 31-bit schemes.

Move to a scheme closer to what other architectures use (and I dare to
say the intent of the system):

 - ZONE_DMA: optionally for memory < 31-bit (64-bit embedded only)
 - ZONE_NORMAL: everything addressable by the kernel
 - ZONE_HIGHMEM: memory > 32-bit for 32-bit kernels

Also provide information on how ZONE_DMA is used by defining
ARCH_ZONE_DMA_BITS.

Contains various fixes from Benjamin Herrenschmidt.

Oliver O'Halloran 43001c52b6 powerpc/papr_scm: Use ibm,unit-guid as the iset cookie
The interleave set cookie is used to determine if a label stored in the
metadata space should be applied to the current region. This is
important in the case of NVDIMMs since the firmware may change the
interleaving configuration of a DIMM which would invalidate the existing
labels. In our case the hypervisor hides those details from us so we
don't really care, but libnvdimm still requires the interleave set
cookie to be non-zero.

For our purposes we just need the set cookie to be unique and fixed for
a given PAPR SCM region and using the unit-guid (really a UUID) is fine
for this purpose.

Oliver O'Halloran b0d65a8cbc powerpc/papr_scm: Fix DIMM device registration race
When a new nvdimm device is registered with libnvdimm via
nvdimm_create() it is added as a device on the nvdimm bus. The probe
function for the DIMM driver is potentially quite slow so actually
registering and probing the device is done in an async domain rather
than immediately after device creation. This can result in a race where
the region device (created 2nd) is probed first and fails to activate at
boot.

To fix this we use the same approach as the ACPI/NFIT driver which is to
check that all the DIMM devices registered successfully. LibNVDIMM
provides the nvdimm_bus_count_dimms() function which synchronises with
the async domain and verifies that the dimm was successfully registered
with the bus.

If either of these does not occur then we bail.

Oliver O'Halloran 409dd7dc83 powerpc/papr_scm: Remove endian conversions
The return values of a h-call are returned in the CPU registers and
written to the provided buffer by the plpar_hcall() wrapper. As a result
the values written to memory are always in the native endian and should
not be byte swapped.

The inital implementation of the H-Call interface was done in qemu and
the returned values were byte swapped unnecessarily in both the
hypervisor and in the driver so this was only noticed when bringing up
the PowerVM implementation.

The ibm,unit-sizes property was originally specified as an array of two
u32s corresponding to the memory block size, and the number of blocks
available in that region. A fairly last-minute change to the SCM DT
specification was splitting that into two seperate u64 properties:
ibm,block-sizes and ibm,number-of-blocks that convey the same
information. No firmware / hypervisor that emitted the ibm,unit-size
property ever appeared in the wild.

Stephen Rothwell 8ad940217c powerpc: annotate implicit fall throughs
There is a plan to build the kernel with -Wimplicit-fallthrough and these
places in the code produced warnings, but because we build arch/powerpc
with -Werror, they became errors.  Fix them up.

This patch produces no change in behaviour, but should be reviewed in

Christoph Hellwig 1753d50c9f rapidio: consolidate RAPIDIO config entry in drivers/rapidio
There is no good reason to duplicate the RAPIDIO menu in various
architectures.  Instead provide a selectable HAVE_RAPIDIO symbol
that indicates native availability of RAPIDIO support and the handle
the rest in drivers/pci.  This also means we now provide support
for PCI(e) to Rapidio bridges for every architecture instead of a
limited subset.

The previous commit, "of: overlay: add missing of_node_get() in
__of_attach_node_sysfs" added a missing of_node_get() to
__of_attach_node_sysfs().  This results in a refcount imbalance
for nodes attached with dlpar_attach_node().  The calling sequence
from dlpar_attach_node() to __of_attach_node_sysfs() is:

   dlpar_attach_node()
      of_attach_node()
         __of_attach_node_sysfs()

Alistair Popple 3182215dd0 powerpc/powernv/npu: Remove NPU DMA ops
The NPU IOMMU is setup to mirror the parent PCIe device IOMMU
setup. Therefore it does not make sense to call dma operations such as
dma_map_page(), etc. directly on these devices. The existing dma_ops
simply print a warning if they are ever called, however this is
unnecessary and the warnings are likely to go unnoticed.

It is instead simpler to remove these operations and let the generic
DMA code print warnings (eg. via a NULL pointer deref) in cases of
buggy drivers attempting dma operations on NVLink devices.

Various powerpc boards select the PCI_MSI config option without selecting
PCI, resulting in potentially not compilable configurations if the by

David Hildenbrand 8df1d0e4a2 mm/memory_hotplug: make add_memory() take the device_hotplug_lock
add_memory() currently does not take the device_hotplug_lock, however
is aleady called under the lock from
	arch/powerpc/platforms/pseries/hotplug-memory.c
	drivers/acpi/acpi_memhotplug.c
to synchronize against CPU hot-remove and similar.

In general, we should hold the device_hotplug_lock when adding memory to
synchronize against online/offline request (e.g.  from user space) - which
already resulted in lock inversions due to device_lock() and
mem_hotplug_lock - see 30467e0b3b ("mm, hotplug: fix concurrent memory
hot-add deadlock").  add_memory()/add_memory_resource() will create memory
block devices, so this really feels like the right thing to do.

Holding the device_hotplug_lock makes sure that a memory block device
can really only be accessed (e.g. via .online/.state) from user space,
once the memory has been fully added to the system.

The lock is not held yet in
	drivers/xen/balloon.c
	arch/powerpc/platforms/powernv/memtrace.c
	drivers/s390/char/sclp_cmd.c
	drivers/hv/hv_balloon.c
So, let's either use the locked variants or take the lock.

Don't export add_memory_resource(), as it once was exported to be used by
XEN, which is never built as a module.  If somebody requires it, we also
have to export a locked variant (as device_hotplug_lock is never
exported).

David Hildenbrand d15e59260f mm/memory_hotplug: make remove_memory() take the device_hotplug_lock
Patch series "mm: online/offline_pages called w.o. mem_hotplug_lock", v3.

Reading through the code and studying how mem_hotplug_lock is to be used,
I noticed that there are two places where we can end up calling
device_online()/device_offline() - online_pages()/offline_pages() without
the mem_hotplug_lock.  And there are other places where we call
device_online()/device_offline() without the device_hotplug_lock.

While e.g.
	echo "online" > /sys/devices/system/memory/memory9/state
is fine, e.g.
	echo 1 > /sys/devices/system/memory/memory9/online
Will not take the mem_hotplug_lock. However the device_lock() and
device_hotplug_lock.

E.g.  via memory_probe_store(), we can end up calling
add_memory()->online_pages() without the device_hotplug_lock.  So we can
have concurrent callers in online_pages().  We e.g.  touch in
online_pages() basically unprotected zone->present_pages then.

Looks like there is a longer history to that (see Patch #2 for details),
and fixing it to work the way it was intended is not really possible.  We
would e.g.  have to take the mem_hotplug_lock in device/base/core.c, which
sounds wrong.

Summary: We had a lock inversion on mem_hotplug_lock and device_lock().
More details can be found in patch 3 and patch 6.

I propose the general rules (documentation added in patch 6):

1. add_memory/add_memory_resource() must only be called with
   device_hotplug_lock.
2. remove_memory() must only be called with device_hotplug_lock. This is
   already documented and holds for all callers.
3. device_online()/device_offline() must only be called with
   device_hotplug_lock. This is already documented and true for now in core
   code. Other callers (related to memory hotplug) have to be fixed up.
4. mem_hotplug_lock is taken inside of add_memory/remove_memory/
   online_pages/offline_pages.

To me, this looks way cleaner than what we have right now (and easier to
verify).  And looking at the documentation of remove_memory, using
lock_device_hotplug also for add_memory() feels natural.

This patch (of 6):

remove_memory() is exported right now but requires the
device_hotplug_lock, which is not exported.  So let's provide a variant
that takes the lock and only export that one.

The lock is already held in
	arch/powerpc/platforms/pseries/hotplug-memory.c
	drivers/acpi/acpi_memhotplug.c
	arch/powerpc/platforms/powernv/memtrace.c

Apart from that, there are not other users in the tree.

Mike Rapoport 7e1c4e2792 memblock: stop using implicit alignment to SMP_CACHE_BYTES
When a memblock allocation APIs are called with align = 0, the alignment
is implicitly set to SMP_CACHE_BYTES.

Implicit alignment is done deep in the memblock allocator and it can
come as a surprise.  Not that such an alignment would be wrong even
when used incorrectly but it is better to be explicit for the sake of
clarity and the prinicple of the least surprise.

Replace all such uses of memblock APIs with the 'align' parameter
explicitly set to SMP_CACHE_BYTES and stop implicit alignment assignment
in the memblock internal allocation functions.

For the case when memblock APIs are used via helper functions, e.g.  like
iommu_arena_new_node() in Alpha, the helper functions were detected with
Coccinelle's help and then manually examined and updated where
appropriate.

The direct memblock APIs users were updated using the semantic patch below:

 "I have been slowly sorting out siginfo and this is the culmination of
  that work.

  The primary result is in several ways the signal infrastructure has
  been made less error prone. The code has been updated so that manually
  specifying SEND_SIG_FORCED is never necessary. The conversion to the
  new siginfo sending functions is now complete, which makes it
  difficult to send a signal without filling in the proper siginfo
  fields.

  At the tail end of the patchset comes the optimization of decreasing
  the size of struct siginfo in the kernel from 128 bytes to about 48
  bytes on 64bit. The fundamental observation that enables this is by
  definition none of the known ways to use struct siginfo uses the extra
  bytes.

  This comes at the cost of a small user space observable difference.
  For the rare case of siginfo being injected into the kernel only what
  can be copied into kernel_siginfo is delivered to the destination, the
  rest of the bytes are set to 0. For cases where the signal and the
  si_code are known this is safe, because we know those bytes are not
  used. For cases where the signal and si_code combination is unknown
  the bits that won't fit into struct kernel_siginfo are tested to
  verify they are zero, and the send fails if they are not.

  I made an extensive search through userspace code and I could not find
  anything that would break because of the above change. If it turns out
  I did break something it will take just the revert of a single change
  to restore kernel_siginfo to the same size as userspace siginfo.

  Testing did reveal dependencies on preferring the signo passed to
  sigqueueinfo over si->signo, so bit the bullet and added the
  complexity necessary to handle that case.

  Testing also revealed bad things can happen if a negative signal
  number is passed into the system calls. Something no sane application
  will do but something a malicious program or a fuzzer might do. So I
  have fixed the code that performs the bounds checks to ensure negative
  signal numbers are handled"

A per-CPU file is created which exports the VPA data of

Michael Ellerman 23ad1a2700 powerpc: Add -Werror at arch/powerpc level
Back when I added -Werror in commit ba55bd7436 ("powerpc: Add
configurable -Werror for arch/powerpc") I did it by adding it to most
of the arch Makefiles.

At the time we excluded math-emu, because apparently it didn't build
cleanly. But that seems to have been fixed somewhere in the interim.

So move the -Werror addition to the top-level of the arch, this saves
us from repeating it in every Makefile and means we won't forget to
add it to any new sub-dirs.

Oliver O'Halloran b5beae5e22 powerpc/pseries: Add driver for PAPR SCM regions
Adds a driver that implements support for enabling and accessing PAPR
SCM regions. Unfortunately due to how the PAPR interface works we can't
use the existing of_pmem driver (yet) because:

 a) The guest is required to use the H_SCM_BIND_MEM h-call to add
    add the SCM region to it's physical address space, and
 b) There is currently no mechanism for relating a bare of_pmem region
    to the backing DIMM (or not-a-DIMM for our case).

Both of these are easily handled by rolling the functionality into a
seperate driver so here we are...

Michael Bringmann 65b9fdadfc powerpc/pseries/mobility: Extend start/stop topology update scope
The powerpc mobility code may receive RTAS requests to perform PRRN
(Platform Resource Reassignment Notification) topology changes at any
time, including during LPAR migration operations.

In some configurations where the affinity of CPUs or memory is being
changed on that platform, the PRRN requests may apply or refer to
outdated information prior to the complete update of the device-tree.

This patch changes the duration for which topology updates are
suppressed during LPAR migrations from just the rtas_ibm_suspend_me()
/ 'ibm,suspend-me' call(s) to cover the entire migration_store()
operation to allow all changes to the device-tree to be applied prior
to accepting and applying any PRRN requests.

For tracking purposes, pr_info notices are added to the functions
start_topology_update() and stop_topology_update() of 'numa.c'.

Sam Bobroff fef7f90552 powerpc/eeh: Cleanup eeh_ops.wait_state()
The wait_state member of eeh_ops does not need to be platform
dependent; it's just logic around eeh_ops.get_state(). Therefore,
merge the two (slightly different!) platform versions into a new
function, eeh_wait_state() and remove the eeh_ops member.

While doing this, also correct:
* The wait logic, so that it never waits longer than max_wait.
* The wait logic, so that it never waits less than
  EEH_STATE_MIN_WAIT_TIME.
* One call site where the result is treated like a bit field before
  it's checked for negative error values.
* In pseries_eeh_get_state(), rename the "state" parameter to "delay"
  because that's what it is.

fields "child_list" and "child", so place these next to each other
in the definition.

The field "child" is a list entry, so remove the unnecessary and

The eeh_dev struct contains two list entry fields, called "list" and
"rmv_list". Rename them to "entry" and "rmv_entry" and, as above, stop
initializing them with LIST_HEAD().

Sam Bobroff bffc0176e7 powerpc/eeh: Cleanup EEH_POSTPONED_PROBE
Currently a flag, EEH_POSTPONED_PROBE, is used to prevent an incorrect
message "EEH: No capable adapters found" from being displayed during
the boot of powernv systems.

It is necessary because, on powernv, the call to eeh_probe_devices()
made from eeh_init() is too early and EEH can't yet be enabled. A
second call is made later from eeh_pnv_post_init(), which succeeds.

(On pseries, the first call succeeds because PCI devices are set up
early enough and no second call is made.)

This can be simplified by moving the early call to eeh_probe_devices()
from eeh_init() (where it's seen by both platforms) to
pSeries_final_fixup(), so that each platform only calls
eeh_probe_devices() once, at a point where it can succeed.
This is slightly later in the boot sequence, but but still early
enough and it is now in the same place in the sequence for both
platforms (the pcibios_fixup hook).

The display of the message can be cleaned up as well, by moving it
into eeh_probe_devices().

Dan Carpenter 014704e6f5 powerpc: Fix signedness bug in update_flash_db()
The "count < sizeof(struct os_area_db)" comparison is type promoted to
size_t so negative values of "count" are treated as very high values
and we accidentally return success instead of a negative error code.

This doesn't really change runtime much but it fixes a static checker
warning.

Mark Hairgrove 3689c37d23 powerpc/powernv/npu: Use size-based ATSD invalidates
Prior to this change only two types of ATSDs were issued to the NPU:
invalidates targeting a single page and invalidates targeting the whole
address space. The crossover point happened at the configurable
atsd_threshold which defaulted to 2M. Invalidates that size or smaller
would issue per-page invalidates for the whole range.

The NPU supports more invalidation sizes however: 64K, 2M, 1G, and all.
These invalidates target addresses aligned to their size. 2M is a common
invalidation size for GPU-enabled applications because that is a GPU
page size, so reducing the number of invalidates by 32x in that case is a
clear improvement.

ATSD latency is high in general so now we always issue a single invalidate
rather than multiple. This will over-invalidate in some cases, but for any
invalidation size over 2M it matches or improves the prior behavior.
There's also an improvement for single-page invalidates since the prior
version issued two invalidates for that case instead of one.

With this change all issued ATSDs now perform a flush, so the flush
parameter has been removed from all the helpers.

To show the benefit here are some performance numbers from a
microbenchmark which creates a 1G allocation then uses mprotect with
PROT_NONE to trigger invalidates in strides across the allocation.

One NPU (1 GPU):

Mark Hairgrove 7ead15a144 powerpc/powernv/npu: Reduce eieio usage when issuing ATSD invalidates
There are two types of ATSDs issued to the NPU: invalidates targeting a
specific virtual address and invalidates targeting the whole address
space. In both cases prior to this change, the sequence was:

    for each NPU
        - Write the target address to the XTS_ATSD_AVA register
        - EIEIO
        - Write the launch value to issue the ATSD

First, a target address is not required when invalidating the whole
address space, so that write and the EIEIO have been removed. The AP
(size) field in the launch is not needed either.

Second, for per-address invalidates the above sequence is inefficient in
the common case of multiple NPUs because an EIEIO is issued per NPU. This
unnecessarily forces the launches of later ATSDs to be ordered with the
launches of earlier ones. The new sequence only issues a single EIEIO:

    for each NPU
        - Write the target address to the XTS_ATSD_AVA register
    EIEIO
    for each NPU
        - Write the launch value to issue the ATSD

Performance results were gathered using a microbenchmark which creates a
1G allocation then uses mprotect with PROT_NONE to trigger invalidates in
strides across the allocation.

With only a single NPU active (one GPU) the difference is in the noise for
both types of invalidates (+/-1%).

With two NPUs active (on a 6-GPU system) the effect is more noticeable:

Breno Leitao 62dea077f5 powerpc/powernv: Mark function as __noreturn
There is a mismatch between function pnv_platform_error_reboot() definition
and declaration regarding function modifiers. In the declaration part, it
contains the function attribute __noreturn, while function definition
itself lacks it.

This was reported by sparse tool as an error:

  arch/powerpc/platforms/powernv/opal.c:538:6: error: symbol 'pnv_platform_error_reboot' redeclared with different type (originally declared at arch/powerpc/platforms/powernv/powernv.h:11) - different modifiers

I checked and the function is already being considered as being 'noreturn'
by the compiler, thus, I understand this patch does not change any code
being generated.

Christophe Leroy c3ff2a5193 powerpc/32: add stack protector support
This functionality was tentatively added in the past
(commit 6533b7c16e ("powerpc: Initial stack protector
(-fstack-protector) support")) but had to be reverted
(commit f2574030b0 ("powerpc: Revert the initial stack
protector support") because of GCC implementing it differently
whether it had been built with libc support or not.

Now, GCC offers the possibility to manually set the
stack-protector mode (global or tls) regardless of libc support.

This time, the patch selects HAVE_STACKPROTECTOR only if
-mstack-protector-guard=tls is supported by GCC.

On PPC32, as register r2 points to current task_struct at
all time, the stack_canary located inside task_struct can be
used directly by using the following GCC options:
-mstack-protector-guard=tls
-mstack-protector-guard-reg=r2
-mstack-protector-guard-offset=offsetof(struct task_struct, stack_canary))

The protector is disabled for prom_init and bootx_init as
it is too early to handle it properly.

Naveen N. Rao 9258227e9d powerpc/pseries: Fix how we iterate over the DTL entries
When CONFIG_VIRT_CPU_ACCOUNTING_NATIVE is not set, we look up dtl_idx in
the lppaca to determine the number of entries in the buffer. Since
lppaca is in big endian, we need to do an endian conversion before using
this in our calculation to determine the number of entries in the
buffer. Without this, we do not iterate over the existing entries in the
DTL buffer properly.

Naveen N. Rao db787af1b8 powerpc/pseries: Fix DTL buffer registration
When CONFIG_VIRT_CPU_ACCOUNTING_NATIVE is not set, we register the DTL
buffer for a cpu when the associated file under powerpc/dtl in debugfs
is opened. When doing so, we need to set the size of the buffer being
registered in the second u32 word of the buffer. This needs to be in big
endian, but we are not doing the conversion resulting in the below error
showing up in dmesg:

	dtl_start: DTL registration for cpu 0 (hw 0) failed with -4

Fix this in the obvious manner.

Vaibhav Jain 8139046a5a powerpc/powernv: Make possible for user to force a full ipl cec reboot
Ever since fast reboot is enabled by default in opal,
opal_cec_reboot() will use fast-reset instead of full IPL to perform
system reboot. This leaves the user with no direct way to force a full
IPL reboot except changing an nvram setting that persistently disables
fast-reset for all subsequent reboots.

This patch provides a more direct way for the user to force a one-shot
full IPL reboot by passing the command line argument 'full' to the
reboot command. So the user will be able to tweak the reboot behavior
via:

  $ sudo reboot full	# Force a full ipl reboot skipping fast-reset

  or
  $ sudo reboot  	# default reboot path (usually fast-reset)

The reboot command passes the un-parsed command argument to the kernel
via the 'Reboot' syscall which is then passed on to the arch function
pnv_restart(). The patch updates pnv_restart() to handle this cmd-arg
and issues opal_cec_reboot2 with OPAL_REBOOT_FULL_IPL to force a full
IPL reset.

brought in changes that moved the code to a new file. So reapply it to
the new file.

Original commit message follows:

We use PHB in mode1 which uses bit 59 to select a correct DMA window.
However there is mode2 which uses bits 59:55 and allows up to 32 DMA
windows per a PE.

Even though documentation does not clearly specify that, it seems that
the actual hardware does not support bits 59:55 even in mode1, in
other words we can create a window as big as 1<<58 but DMA simply
won't work.

This reduces the upper limit from 59 to 55 bits to let the userspace
know about the hardware limits.

Nathan Fontenot fd12527a1d powerpc/pseries: Remove unneeded uses of dlpar work queue
There are three instances in which dlpar hotplug events are invoked;
handling a hotplug interrupt (in a kvm guest), handling a dlpar
request through sysfs, and updating LMB affinity when handling a
PRRN event. Only in the case of handling a hotplug interrupt do we
have to put the work on a workqueue, the other cases can handle the
dlpar request directly.

This patch exports the handle_dlpar_errorlog() function so that
dlpar hotplug events can be handled directly and updates the two
instances mentioned above to use the direct invocation.

Nathan Fontenot 063b8b1251 powerpc/pseries/memory-hotplug: Only update DT once per memory DLPAR request
The updates to powerpc numa and memory hotplug code now use the
in-kernel LMB array instead of the device tree. This change allows the
pseries memory DLPAR code to only update the device tree once after
successfully handling a DLPAR request.

Prior to the in-kernel LMB array, the numa code looked up the affinity
for memory being added in the device tree, the code now looks this up
in the LMB array. This change means the memory hotplug code can just
update the affinity for an LMB in the LMB array instead of updating
the device tree.

This also provides a savings in kernel memory. When updating the
device tree old properties are never free'ed since there is no
usecount on properties. This behavior leads to a new copy of the
property being allocated every time a LMB is added or removed (i.e. a
request to add 100 LMBs creates 100 new copies of the property). With
this update only a single new property is created when a DLPAR request
completes successfully.

Mahesh Salgaonkar c6d15258cd powerpc/pseries: Dump the SLB contents on SLB MCE errors.
If we get a machine check exceptions due to SLB errors then dump the
current SLB contents which will be very much helpful in debugging the
root cause of SLB errors. Introduce an exclusive buffer per cpu to hold
faulty SLB entries. In real mode mce handler saves the old SLB contents
into this buffer accessible through paca and print it out later in virtual
mode.

exceptions due to SLB errors. These are soft errors and can be fixed
by flushing the SLBs so the kernel can continue to function instead of
system crash. We do this in real mode before turning on MMU. Otherwise
we would run into nested machine checks. This patch now fetches the
rtas error log in real mode and flushes the SLBs on SLB/ERAT errors.

Rashmica Gupta 3f7daf3d75 powerpc/memtrace: Remove memory in chunks
When hot-removing memory release_mem_region_adjustable() splits iomem
resources if they are not the exact size of the memory being
hot-deleted. Adding this memory back to the kernel adds a new resource.

Eg a node has memory 0x0 - 0xfffffffff. Hot-removing 1GB from
0xf40000000 results in the single resource 0x0-0xfffffffff being split
into two resources: 0x0-0xf3fffffff and 0xf80000000-0xfffffffff.

When we hot-add the memory back we now have three resources:
0x0-0xf3fffffff, 0xf40000000-0xf7fffffff, and 0xf80000000-0xfffffffff.

This is an issue if we try to remove some memory that overlaps
resources. Eg when trying to remove 2GB at address 0xf40000000,
release_mem_region_adjustable() fails as it expects the chunk of memory
to be within the boundaries of a single resource. We then get the
warning: "Unable to release resource" and attempting to use memtrace
again gives us this error: "bash: echo: write error: Resource
temporarily unavailable"

This patch makes memtrace remove memory in chunks that are always the
same size from an address that is always equal to end_of_memory -
n*size, for some n. So hotremoving and hotadding memory of different
sizes will now not attempt to remove memory that spans multiple
resources.

Benjamin Herrenschmidt db2173198b powerpc/powernv/pci: Work around races in PCI bridge enabling
The generic code is racy when multiple children of a PCI bridge try to
enable it simultaneously.

This leads to drivers trying to access a device through a
not-yet-enabled bridge, and this EEH errors under various
circumstances when using parallel driver probing.

There is work going on to fix that properly in the PCI core but it
will take some time.

x86 gets away with it because (outside of hotplug), the BIOS enables
all the bridges at boot time.

This patch does the same thing on powernv by enabling all bridges that
have child devices at boot time, thus avoiding subsequent races. It's
suitable for backporting to stable and distros, while the proper PCI
fix will probably be significantly more invasive.

Souptick Joarder 50a7ca3c6f mm: convert return type of handle_mm_fault() caller to vm_fault_t
Use new return type vm_fault_t for fault handler.  For now, this is just
documenting that the function returns a VM_FAULT value rather than an
errno.  Once all instances are converted, vm_fault_t will become a
distinct type.

In this patch all the caller of handle_mm_fault() are changed to return

   - A series to improve the hvc driver & related OPAL console code,
     which have been seen to cause hardlockups at times. The hvc driver
     changes in particular have been in linux-next for ~month.

   - Increase our MAX_PHYSMEM_BITS to 128TB when SPARSEMEM_VMEMMAP=y.

   - Remove Power8 DD1 and Power9 DD1 support, neither chip should be in
     use anywhere other than as a paper weight.

   - An optimised memcmp implementation using Power7-or-later VMX
     instructions

   - Support for barrier_nospec on some NXP CPUs.

   - Support for flushing the count cache on context switch on some IBM
     CPUs (controlled by firmware), as a Spectre v2 mitigation.

   - A series to enhance the information we print on unhandled signals
     to bring it into line with other arches, including showing the
     offending VMA and dumping the instructions around the fault.
