2. dmsetup suspend pool
3. dmsetup resume pool
4. dmsetup remove_all # Concurrent with 3
The root cause of this UAF bug is that dm_resume() adds timer after
dm_destroy() skips cancelling the timer because of suspend status.
After timeout, it will call run_timer_softirq(), however pool has
already been freed. The concurrency UAF bug will happen.
Therefore, cancelling timer again in __pool_destroy().

If a thinpool set fail_io while suspending, resume will fail with:
device-mapper: resume ioctl on vg-thinpool failed: Invalid argument
The thin-pool also can't be removed if an in-flight bio is in the
deferred list.
This can be easily reproduced using:
echo "offline" > /sys/block/sda/device/state
dd if=/dev/zero of=/dev/mapper/thin bs=4K count=1
dmsetup suspend /dev/mapper/pool
mkfs.ext4 /dev/mapper/thin
dmsetup resume /dev/mapper/pool
The root cause is maybe_resize_data_dev() will check fail_io and return
error before called dm_resume.
Fix this by adding FAIL mode check at the end of pool_preresume().

Following process may generate a broken btree mixed with fresh and
stale btree nodes, which could get dm thin trapped in an infinite loop
while looking up data block:

The pmd->root points to a broken btree, Y may contain stale node
pointing to any block, for example X, which gets dm thin trapped into
a dead loop while looking up Z.
Fix this by setting pmd->root in __open_metadata(), so that dm thin
will use the last transaction's pmd->root if commit failed.

There was a problem that a user burned a dm-integrity image on CDROM
and could not activate it because it had a non-empty journal.
Fix this problem by flushing the journal (done by the previous commit)
and clearing the journal (done by this commit). Once the journal is
cleared, dm-integrity won't attempt to replay it on the next
activation.

This commit flushes the journal on suspend. It is prerequisite for the
next commit that enables activating dm integrity devices in read-only mode.
Note that we deliberately didn't flush the journal on suspend, so that the
journal replay code would be tested. However, the dm-integrity code is 5
years old now, so that journal replay is well-tested, and we can make this
change now.

__list_versions will first estimate the required space using the
"dm_target_iterate(list_version_get_needed, &needed)" call and then will
fill the space using the "dm_target_iterate(list_version_get_info,
&iter_info)" call. Each of these calls locks the targets using the
"down_read(&_lock)" and "up_read(&_lock)" calls, however between the first
and second "dm_target_iterate" there is no lock held and the target
modules can be loaded at this point, so the second "dm_target_iterate"
call may need more space than what was the first "dm_target_iterate"
returned.
The code tries to handle this overflow (see the beginning of
list_version_get_info), however this handling is incorrect.
The code sets "param->data_size = param->data_start + needed" and
"iter_info.end = (char *)vers+len" - "needed" is the size returned by the
first dm_target_iterate call; "len" is the size of the buffer allocated by
userspace.
"len" may be greater than "needed"; in this case, the code will write up
to "len" bytes into the buffer, however param->data_size is set to
"needed", so it may write data past the param->data_size value. The ioctl
interface copies only up to param->data_size into userspace, thus part of
the result will be truncated.
Fix this bug by setting "iter_info.end = (char *)vers + needed;" - this
guarantees that the second "dm_target_iterate" call will write only up to
the "needed" buffer and it will exit with "DM_BUFFER_FULL_FLAG" if it
overflows the "needed" space - in this case, userspace will allocate a
larger buffer and retry.
Note that there is also a bug in list_version_get_needed - we need to add
"strlen(tt->name) + 1" to the needed size, not "strlen(tt->name)".

A complicated deadlock exists when using the journal and an elevated
group_thrtead_cnt. It was found with loop devices, but its not clear
whether it can be seen with real disks. The deadlock can occur simply
by writing data with an fio script.

However, before hanging, the MD_SB_CHANGE_PENDING flag will be
set for sb_flags in r5l_write_super_and_discard_space(). This
flag will never be cleared because the submit_bio() call never
returns.
3) Due to the MD_SB_CHANGE_PENDING flag being set, handle_stripe()
will do no processing on any pending stripes and re-set
STRIPE_HANDLE. This will cause the raid5d thread to enter an
infinite loop, constantly trying to handle the same stripes
stuck in the queue.
The raid5d thread has a blk_plug that holds a number of bios
that are also stuck waiting seeing the thread is in a loop
that never schedules. These bios have been accounted for by
blk-wbt thus preventing the other threads above from
continuing when they try to submit bios. --Deadlock.
To fix this, add the same wait_event() that is used in raid5_do_work()
to raid5d() such that if MD_SB_CHANGE_PENDING is set, the thread will
schedule and wait until the flag is cleared. The schedule action will
flush the plug which will allow the r5l_reclaim thread to continue,
thus preventing the deadlock.
However, md_check_recovery() calls can also clear MD_SB_CHANGE_PENDING
from the same thread and can thus deadlock if the thread is put to
sleep. So avoid waiting if md_check_recovery() is being called in the
loop.
It's not clear when the deadlock was introduced, but the similar
wait_event() call in raid5_do_work() was added in 2017 by this
commit:

Inside set_at_max_writeback_rate() the calculation in following if()
check is wrong,
if (atomic_inc_return(&c->idle_counter) <
atomic_read(&c->attached_dev_nr) * 6)
Because each attached backing device has its own writeback thread
running and increasing c->idle_counter, the counter increates much
faster than expected. The correct calculation should be,
(counter / dev_nr) < dev_nr * 6
which equals to,
counter < dev_nr * dev_nr * 6
This patch fixes the above mistake with correct calculation, and helper
routine idle_counter_exceeded() is added to make code be more clear.

The double free is caused by an unnecessary bio_put() in the
if(is_badblock(...)) error path in raid5_read_one_chunk().
The error path was moved ahead of bio_alloc_clone() in c82aa1b767
("md/raid5: move checking badblock before clone bio in
raid5_read_one_chunk"). The previous code checked and freed align_bio
which required a bio_put. After the move that is no longer needed as
raid_bio is returned to the control of the common io path which
performs its own endio resulting in a double free on bad device blocks.

When doing degrade/recover tests using the journal a kernel BUG
is hit at drivers/md/raid5.c:4381 in handle_parity_checks5():
BUG_ON(!test_bit(R5_UPTODATE, &dev->flags));
This was found to occur because handle_stripe_fill() was skipped
for stripes in the journal due to a condition in that function.
Thus blocks were not fetched and R5_UPTODATE was not set when
the code reached handle_parity_checks5().
To fix this, don't skip handle_stripe_fill() unless the stripe is
for read.

Current code produces a warning as shown below when total characters
in the constituent block device names plus the slashes exceeds 200.
snprintf() returns the number of characters generated from the given
input, which could cause the expression “200 – len” to wrap around
to a large positive number. Fix this by using scnprintf() instead,
which returns the actual number of characters written into the buffer.

A race condition still exists when removing and re-creating md devices
in test cases. However, it is only seen on some setups.
The race condition was tracked down to a reference still being held
to the kobject by the rdev in the md_rdev_misc_wq which will be released
in rdev_delayed_delete().
md_alloc() waits for previous deletions by waiting on the md_misc_wq,
but the md_rdev_misc_wq may still be holding a reference to a recently
removed device.
To fix this, also flush the md_rdev_misc_wq in md_alloc().

In line 2884, "raid5_release_stripe(sh);" drops the reference to sh and
may cause sh to be released. However, sh is subsequently used in lines
2886 "if (sh->batch_head && sh != sh->batch_head)". This may result in an
use-after-free bug.
It can be fixed by moving "raid5_release_stripe(sh);" to the bottom of
the function.

The mdadm test 07layouts randomly produces a kernel hung task deadlock.
The deadlock is caused by the suspend_lo/suspend_hi files being set by
the mdadm background process during reshape and not being cleared
because the process hangs. (Leaving aside the issue of the fragility of
freezing kernel tasks by buggy userspace processes...)
When the background mdadm process hangs it, is waiting (without a
timeout) on a change to the sync_completed file signalling that the
reshape has completed. The process is woken up a couple times when
the reshape finishes but it is woken up before MD_RECOVERY_RUNNING
is cleared so sync_completed_show() reports 0 instead of "none".
To fix this, notify the sysfs file in md_reap_sync_thread() after
MD_RECOVERY_RUNNING has been cleared. This wakes up mdadm and causes
it to continue and write to suspend_lo/suspend_hi to allow IO to
continue.

There is a KASAN warning in raid_resume when running the lvm test
lvconvert-raid.sh. The reason for the warning is that mddev->raid_disks
is greater than rs->raid_disks, so the loop touches one entry beyond
the allocated length.

The warning is caused by reading conf->max_nr_stripes in raid_status. The
code in raid_status reads mddev->private, casts it to struct r5conf and
reads the entry max_nr_stripes.
However, if we have different raid type than 4/5/6, mddev->private
doesn't point to struct r5conf; it may point to struct r0conf, struct
r1conf, struct r10conf or struct mpconf. If we cast a pointer to one
of these structs to struct r5conf, we will be reading invalid memory
and KASAN warns about it.
Fix this bug by reading struct r5conf only if raid type is 4, 5 or 6.

If a metadata commit fails, the transaction will be aborted and the
metadata space maps will be destroyed. If a DM table reload then
happens for this failed thin-pool, a use-after-free will occur in
dm_sm_register_threshold_callback (called from
dm_pool_register_metadata_threshold).
Fix this by in dm_pool_register_metadata_threshold() by returning the
-EINVAL error if the thin-pool is in fail mode. Also fail pool_ctr()
with a new error message: "Error registering metadata threshold".

dm-writecache has the capability to limit the number of writeback jobs
in progress. However, this feature was off by default. As such there
were some out-of-memory crashes observed when lowering the low
watermark while the cache is full.
This commit enables writeback limit by default. It is set to 256MiB or
1/16 of total system memory, whichever is smaller.

When we ran the lvm test "shell/integrity-blocksize-3.sh" on a kernel with
kasan, we got failure in write_page.
The reason for the failure is that md_bitmap_destroy is called before
destroying the thread and the thread may be waiting in the function
write_page for the bio to complete. When the thread finishes waiting, it
executes "if (test_bit(BITMAP_WRITE_ERROR, &bitmap->flags))", which
triggers the kasan warning.
Note that the commit 48df498daf that caused this bug claims that it is
neede for md-cluster, you should check md-cluster and possibly find
another bugfix for it.

On dm-raid table load (using raid_ctr), dm-raid allocates an array
rs->devs[rs->raid_disks] for the raid device members. rs->raid_disks
is defined by the number of raid metadata and image tupples passed
into the target's constructor.
In the case of RAID layout changes being requested, that number can be
different from the current number of members for existing raid sets as
defined in their superblocks. Example RAID layout changes include:
- raid1 legs being added/removed
- raid4/5/6/10 number of stripes changed (stripe reshaping)
- takeover to higher raid level (e.g. raid5 -> raid6)
When accessing array members, rs->raid_disks must be used in control
loops instead of the potentially larger value in rs->md.raid_disks.
Otherwise it will cause memory access beyond the end of the rs->devs
array.
Fix this by changing code that is prone to out-of-bounds access.
Also fix validate_raid_redundancy() to validate all devices that are
added. Also, use braces to help clean up raid_iterate_devices().
The out-of-bounds memory accesses was discovered using KASAN.
This commit was verified to pass all LVM2 RAID tests (with KASAN
enabled).

Commit 85e123c27d ("dm mirror log: round up region bitmap size to
BITS_PER_LONG") introduced a regression on 64-bit architectures in the
lvm testsuite tests: lvcreate-mirror, mirror-names and vgsplit-operation.
If the device is shrunk, we need to clear log bits beyond the end of the
device. The code clears bits up to a 32-bit boundary and then calculates
lc->sync_count by summing set bits up to a 64-bit boundary (the commit
changed that; previously, this boundary was 32-bit too). So, it was using
some non-zeroed bits in the calculation and this caused misbehavior.
Fix this regression by clearing bits up to BITS_PER_LONG boundary.

During postsuspend dm-era does the following:
1. Archives the current era
2. Commits the metadata, as part of the RPC call for archiving the
current era
3. Stops the worker
Until the worker stops, it might write to the metadata again. Moreover,
these writes are not flushed to disk immediately, but are cached by the
dm-bufio client, which writes them back asynchronously.
As a result, the committed metadata of a suspended dm-era device might
not be consistent with the in-core metadata.
In some cases, this can result in the corruption of the on-disk
metadata. Suppose the following sequence of events:
1. Load a new table, e.g. a snapshot-origin table, to a device with a
dm-era table
2. Suspend the device
3. dm-era commits its metadata, but the worker does a few more metadata
writes until it stops, as part of digesting an archived writeset
4. These writes are cached by the dm-bufio client
5. Load the dm-era table to another device.
6. The new instance of the dm-era target loads the committed, on-disk
metadata, which don't include the extra writes done by the worker
after the metadata commit.
7. Resume the new device
8. The new dm-era target instance starts using the metadata
9. Resume the original device
10. The destructor of the old dm-era target instance is called and
destroys the dm-bufio client, which results in flushing the cached
writes to disk
11. These writes might overwrite the writes done by the new dm-era
instance, hence corrupting its metadata.
Fix this by committing the metadata after the worker stops running.
stop_worker uses flush_workqueue to flush the current work. However, the
work item may re-queue itself and flush_workqueue doesn't wait for
re-queued works to finish.
This could result in the worker changing the metadata after they have

The code in dm-log rounds up bitset_size to 32 bits. It then uses
find_next_zero_bit_le on the allocated region. find_next_zero_bit_le
accesses the bitmap using unsigned long pointers. So, on 64-bit
architectures, it may access 4 bytes beyond the allocated size.
Fix this bug by rounding up bitset_size to BITS_PER_LONG.
This bug was found by running the lvm2 testsuite with kasan.

The RAID0 layout is irrelevant if all members have the same size so the
array has only one zone. It is *also* irrelevant if the array has two
zones and the second zone has only one device, for example if the array
has two members of different sizes.
So in that case it makes sense to allow assembly even when the layout is
undefined, like what is done when the array has only one zone.

Generally, the md_unregister_thread is called with reconfig_mutex, but
raid_message in dm-raid doesn't hold reconfig_mutex to unregister thread,
so md_unregister_thread can be called simulitaneously from two call sites
in theory.
Then after previous commit which remove the protection of reconfig_mutex
for md_unregister_thread completely, the potential issue could be worse
than before.
Let's take pers_lock at the beginning of function to ensure reentrancy.

In normal stop process, it does like this:

__md_stop sets mddev->private to NULL after pers->free. The raid device
will be stopped and mddev memory is free. But in reshape, it doesn't
free the mddev and mddev will still be used in new raid.
In reshape, it first sets mddev->private to new_pers and then runs
old_pers->free(). Now raid0 sets mddev->private to NULL in raid0_free.
The new raid can't work anymore. It will panic when dereference
mddev->private because of NULL pointer dereference.

The journal no-space deadlock was reported time to time. Such deadlock
can happen in the following situation.
When all journal buckets are fully filled by active jset with heavy
write I/O load, the cache set registration (after a reboot) will load
all active jsets and inserting them into the btree again (which is
called journal replay). If a journaled bkey is inserted into a btree
node and results btree node split, new journal request might be
triggered. For example, the btree grows one more level after the node
split, then the root node record in cache device super block will be
upgrade by bch_journal_meta() from bch_btree_set_root(). But there is no
space in journal buckets, the journal replay has to wait for new journal
bucket to be reclaimed after at least one journal bucket replayed. This
is one example that how the journal no-space deadlock happens.
The solution to avoid the deadlock is to reserve 1 journal bucket in
run time, and only permit the reserved journal bucket to be used during
cache set registration procedure for things like journal replay. Then
the journal space will never be fully filled, there is no chance for
journal no-space deadlock to happen anymore.
This patch adds a new member "bool do_reserve" in struct journal, it is
inititalized to 0 (false) when struct journal is allocated, and set to
1 (true) by bch_journal_space_reserve() when all initialization done in
run_cache_set(). In the run time when journal_reclaim() tries to
allocate a new journal bucket, free_journal_buckets() is called to check
whether there are enough free journal buckets to use. If there is only
1 free journal bucket and journal->do_reserve is 1 (true), the last
bucket is reserved and free_journal_buckets() will return 0 to indicate
no free journal bucket. Then journal_reclaim() will give up, and try
next time to see whetheer there is free journal bucket to allocate. By
this method, there is always 1 jouranl bucket reserved in run time.
During the cache set registration, journal->do_reserve is 0 (false), so
the reserved journal bucket can be used to avoid the no-space deadlock.

After making bch_sectors_dirty_init() being multithreaded, the existing
incremental dirty sector counting in bch_root_node_dirty_init() doesn't
release btree occupation after iterating 500000 (INIT_KEYS_EACH_TIME)
bkeys. Because a read lock is added on btree root node to prevent the
btree to be split during the dirty sectors counting, other I/O requester
has no chance to gain the write lock even restart bcache_btree().
That is to say, the incremental dirty sectors counting is incompatible
to the multhreaded bch_sectors_dirty_init(). We have to choose one and
drop another one.
In my testing, with 512 bytes random writes, I generate 1.2T dirty data
and a btree with 400K nodes. With single thread and incremental dirty
sectors counting, it takes 30+ minites to register the backing device.
And with multithreaded dirty sectors counting, the backing device
registration can be accomplished within 2 minutes.
The 30+ minutes V.S. 2- minutes difference makes me decide to keep
multithreaded bch_sectors_dirty_init() and drop the incremental dirty
sectors counting. This is what this patch does.
But INIT_KEYS_EACH_TIME is kept, in sectors_dirty_init_fn() the CPU
will be released by cond_resched() after every INIT_KEYS_EACH_TIME keys
iterated. This is to avoid the watchdog reports a bogus soft lockup
warning.

Commit b144e45fc5 ("bcache: make bch_sectors_dirty_init() to be
multithreaded") makes bch_sectors_dirty_init() to be much faster
when counting dirty sectors by iterating all dirty keys in the btree.
But it isn't in ideal shape yet, still can be improved.
This patch does the following changes to improve current parallel dirty
keys iteration on the btree,
- Add read lock to root node when multiple threads iterating the btree,
to prevent the root node gets split by I/Os from other registered
bcache devices.
- Remove local variable "char name[32]" and generate kernel thread name
string directly when calling kthread_run().
- Allocate "struct bch_dirty_init_state state" directly on stack and
avoid the unnecessary dynamic memory allocation for it.
- Decrease BCH_DIRTY_INIT_THRD_MAX from 64 to 12 which is enough indeed.
- Increase &state->started to count created kernel thread after it
succeeds to create.
- When wait for all dirty key counting threads to finish, use
wait_event() to replace wait_event_interruptible().
With the above changes, the code is more clear, and some potential error
conditions are avoided.

all btree nodes during cache device registration. But it isn't in ideal
shap yet, still can be improved.
This patch does the following thing to improve current parallel btree
nodes check by multiple threads in bch_btree_check(),
- Add read lock to root node while checking all the btree nodes with
multiple threads. Although currently it is not mandatory but it is
good to have a read lock in code logic.
- Remove local variable 'char name[32]', and generate kernel thread name
string directly when calling kthread_run().
- Allocate local variable "struct btree_check_state check_state" on the
stack and avoid unnecessary dynamic memory allocation for it.
- Reduce BCH_BTR_CHKTHREAD_MAX from 64 to 12 which is enough indeed.
- Increase check_state->started to count created kernel thread after it
succeeds to create.
- When wait for all checking kernel threads to finish, use wait_event()
to replace wait_event_interruptible().
With this change, the code is more clear, and some potential error

and lead to invalid memory access passing the check.

while using the original variable 'pdev' as a dedicated pointer to
point to the found element.

Raid456 module had allowed to achieve failed state. It was fixed by
fb73b357fb ("raid5: block failing device if raid will be failed").
This fix introduces a bug, now if raid5 fails during IO, it may result
with a hung task without completion. Faulty flag on the device is
necessary to process all requests and is checked many times, mainly in
analyze_stripe().
Allow to set faulty on drive again and set MD_BROKEN if raid is failed.
As a result, this level is allowed to achieve failed state again, but
communication with userspace (via -EBUSY status) will be preserved.
This restores possibility to fail array via #mdadm --set-faulty command
and will be fixed by additional verification on mdadm side.

The device-mapper framework provides a mechanism to mark targets as
immutable (and hence fail table reloads that try to change the target
type). Add the DM_TARGET_IMMUTABLE flag to the dm-verity target's
feature flags to prevent switching the verity target with a different
target type.

The device mapper dm-crypt target is using scnprintf("%02x", cc->key[i]) to
report the current key to userspace. However, this is not a constant-time
operation and it may leak information about the key via timing, via cache
access patterns or via the branch predictor.
Change dm-crypt's key printing to use "%c" instead of "%02x". Also
introduce hex2asc() that carefully avoids any branching or memory
accesses when converting a number in the range 0 ... 15 to an ascii
character.

It is possible to set up dm-integrity in such a way that the
"tag_size" parameter is less than the actual digest size. In this
situation, a part of the digest beyond tag_size is ignored.
In this case, dm-integrity would write beyond the end of the
ic->recalc_tags array and corrupt memory. The corruption happened in
integrity_recalc->integrity_sector_checksum->crypto_shash_final.
Fix this corruption by increasing the tags array so that it has enough
padding at the end to accomodate the loop in integrity_recalc() being
able to write a full digest size for the last member of the tags
array.

Update both bio-based and request-based DM to requeue IO if the
mapping table not available.
This race of IO being submitted before the DM device ready is so
narrow, yet possible for initial table load given that the DM device's
request_queue is created prior, that it best to requeue IO to handle
this unlikely case.

When multiple threads to check btree nodes in parallel, the main
thread wait for all threads to stop or CACHE_SET_IO_DISABLE flag:
wait_event_interruptible(check_state->wait,
atomic_read(&check_state->started) == 0 ||
test_bit(CACHE_SET_IO_DISABLE, &c->flags));
However, the bch_btree_node_read and bch_btree_node_read_done
maybe call bch_cache_set_error, then the CACHE_SET_IO_DISABLE
will be set. If the flag already set, the main thread return
error. At the same time, maybe some threads still running and
read NULL pointer, the kernel will crash.
This patch change the event wait condition, the main thread must
wait for all threads to stop.

Commit f6f72f32c2 ("dm integrity: don't replay journal data past the
end of the device") skips journal replay if the target sector points
beyond the end of the device. Unfortunatelly, it doesn't set the
journal entry unused, which resulted in this BUG being triggered:

Fix this by calling journal_entry_set_unused() for this case.

DM handles a flush with data by first issuing an empty flush and then
once it completes the REQ_PREFLUSH flag is removed and the payload is
issued. The problem fixed by this commit is that both the empty flush
bio and the data payload will account the full extent of the data
payload.
Fix this by factoring out dm_io_acct() and having it wrap all IO
accounting to set the size of bio with REQ_PREFLUSH to 0, account the
IO, and then restore the original size.

Commit d208b89401 ("dm: fix mempool NULL pointer race when
completing IO") didn't go far enough.
When bio_end_io_acct ends the count of in-flight I/Os may reach zero
and the DM device may be suspended. There is a possibility that the
suspend races with dm_stats_account_io.
Fix this by adding percpu "pending_io" counters to track outstanding
dm_io. Move kicking of suspend queue to dm_io_dec_pending(). Also,
rename md_in_flight_bios() to dm_in_flight_bios() and update it to
iterate all pending_io counters.

dm_stats_account_io()'s STAT_PRECISE_TIMESTAMPS support doesn't handle
the fact that with commit b879f915bc ("dm: properly fix redundant
bio-based IO accounting") io->start_time _may_ be in the past (meaning
the start_io_acct() was deferred until later).
Add a new dm_stats_recalc_precise_timestamps() helper that will
set/clear a new 'precise_timestamps' flag in the dm_stats struct based
on whether any configured stats enable STAT_PRECISE_TIMESTAMPS.
And update DM core's alloc_io() to use dm_stats_record_start() to set
stats_aux.duration_ns if stats->precise_timestamps is true.
Also, remove unused 'last_sector' and 'last_rw' members from the
dm_stats struct.

Various block drivers call blk_set_queue_dying to mark a disk as dead due
to surprise removal events, but since commit 8e141f9eb8 that doesn't
work given that the GD_DEAD flag needs to be set to stop I/O.
Replace the driver calls to blk_set_queue_dying with a new (and properly
documented) blk_mark_disk_dead API, and fold blk_set_queue_dying into the
only remaining caller.

Record the start_time for a bio but defer the starting block core's IO
accounting until after IO is submitted using bio_start_io_acct_time().
This approach avoids the need to mess around with any of the
individual IO stats in response to a bio_split() that follows bio
submission.

The superblock of version 1.0 doesn't get moved to the new position on a
device size change. This leads to a rdev without a superblock on a known
position, the raid can't be re-assembled.
The line was removed by mistake and is re-added by this patch.

The above commit replaces page_address(bv->bv_page) by bvec_virt(bv) to
avoid directly access to bv->bv_page, but in situation bv->bv_offset is
not zero and page_address(bv->bv_page) is not equal to bvec_virt(bv). In
such case a memory corruption may happen because memory in next page is
tainted by following line in do_btree_node_write(),
memcpy(bvec_virt(bv), addr, PAGE_SIZE);
This patch reverts the mentioned commit to avoid the memory corruption.

In bcache_device_free(), pointer disk is referenced still in
ida_simple_remove() after blk_cleanup_disk() gets called on this
pointer. This may cause a potential panic by use-after-free on the
disk pointer.
This patch fixes the problem by calling blk_cleanup_disk() after
ida_simple_remove().

Commit 6607cd319b ("raid1: ensure write
behind bio has less than BIO_MAX_VECS sectors") tried to guarantee the
size of behind bio is not bigger than BIO_MAX_VECS sectors.
Unfortunately the same calltrace still could happen since an array could
enable write-behind without write mostly device.
To match the manpage of mdadm (which says "write-behind is only attempted
on drives marked as write-mostly"), we need to check WriteMostly flag to
avoid such unexpected behavior.

DM uses blk-mq's quiesce/unquiesce to stop/start device mapper queue.
But blk-mq's unquiesce may come from outside events, such as elevator
switch, updating nr_requests or others, and request may come during
suspend, so simply ask for blk-mq to requeue it.
Fixes one kernel panic issue when running updating nr_requests and
dm-mpath suspend/resume stress test.

Without FEC, dm-verity won't call verity_handle_err() when I/O fails,
but with FEC enabled, it currently does even if an I/O error has
occurred.
If there is an I/O error and FEC correction fails, return the error
instead of calling verity_handle_err() again.

Commit b0140891a8 ("md: Fix race when creating a new md device.")
not only moved assigning mddev->gendisk before calling add_disk, which
fixes the races described in the commit log, but also added a
mddev->open_mutex critical section over add_disk and creation of the
md kobj. Adding a kobject after add_disk is racy vs deleting the gendisk
right after adding it, but md already prevents against that by holding
a mddev->active reference.
On the other hand taking this lock added a lock order reversal with what
is not disk->open_mutex (used to be bdev->bd_mutex when the commit was
added) for partition devices, which need that lock for the internal open
for the partition scan, and a recent commit also takes it for
non-partitioned devices, leading to further lockdep splatter.

The functions get_online_cpus() and put_online_cpus() have been
deprecated during the CPU hotplug rework. They map directly to
cpus_read_lock() and cpus_read_unlock().
Replace deprecated CPU-hotplug functions with the official version.
The behavior remains unchanged.

There is no point in trying to finding the dax device if the DAX flag is
not set on the queue as none of the users of the device mapper exported
block devices could make use of the DAX capability.

In the first loop of function raid10_handle_discard. It already
determines which disk need to handle discard request and add the

Certain DM targets ('integrity', 'multipath', 'verity') need to update the
way their attributes are recorded in the ima log, so that the attestation
servers can interpret the data correctly and decide if the devices
meet the attestation requirements. For instance, the "mode=%c" attribute
in the 'integrity' target is measured twice, the 'verity' target is
missing the attribute "root_hash_sig_key_desc=%s", and the 'multipath'
target needs to index the attributes properly.
Update 'integrity' target to remove the duplicate measurement of
the attribute "mode=%c". Add "root_hash_sig_key_desc=%s" attribute
for the 'verity' target. Index various attributes in 'multipath'
target. Also, add "nr_priority_groups=%u" attribute to 'multipath'
target to record the number of priority groups.

The end-users of DM devices/targets may remove and re-create the same
device multiple times. IMA does not measure such duplicate events if the
configuration CONFIG_IMA_DISABLE_HTABLE is set to 'n'.
To avoid confusion, the end-users need some indication on the client
if that configuration option is disabled.
Add a one-time warning during dm_init() if CONFIG_IMA_DISABLE_HTABLE
is set to 'n', to notify the end-users that duplicate events will not
be measured in the ima log. Also cleanup some whitespace in dm_init().

The DM events present in the ima log contain various attributes in the
key=value format. The attributes' names/values may change in future,
and new attributes may also get added. The attestation server needs
some versioning to determine which attributes are supported and are
expected in the ima log.
Add version information to the DM events present in the ima log to
help attestation servers to correctly process the attributes across
different versions.

The active/inactive table hashes measured in the ima log do not contain
the information about hash algorithm. This information is useful for the
attestation servers to recreate the hashes and compare them with the ones
present in the ima log to verify the table contents.
Prefix the table hashes in various DM events in ima log with the hash
algorithm used to compute those hashes.

On systems with many cores using dm-crypt, heavy spinlock contention in
percpu_counter_compare() can be observed when the page allocation limit
for a given device is reached or close to be reached. This is due
to percpu_counter_compare() taking a spinlock to compute an exact
result on potentially many CPUs at the same time.
Switch to non-exact comparison of allocated and allowed pages by using
the value returned by percpu_counter_read_positive() to avoid taking
the percpu_counter spinlock.
This may over/under estimate the actual number of allocated pages by at
most (batch-1) * num_online_cpus().
Currently, batch is bounded by 32. The system on which this issue was
first observed has 256 CPUs and 512GB of RAM. With a 4k page size, this
change may over/under estimate by 31MB. With ~10G (2%) allowed dm-crypt
allocations, this seems an acceptable error. Certainly preferred over
running into the spinlock contention.
This behavior was reproduced on an EC2 c5.24xlarge instance with 96 CPUs
and 192GB RAM as follows, but can be provoked on systems with less CPUs
as well.

contention in percpu_counter_compare():

After applying this patch and running the same test, %system usage is
lowered to ~7% and write throughput on the loop device increases
to ~2.7GB/s. perf report shows mempool_alloc() as ~8% rather than ~62%
in the profile and not hitting the percpu_counter() spinlock anymore.

For device mapper targets to take advantage of IMA's measurement
capabilities, the status functions for the individual targets need to be
updated to handle the status_type_t case for value STATUSTYPE_IMA.
Update status functions for the following target types, to log their
respective attributes to be measured using IMA.
01. cache
02. crypt
03. integrity
04. linear
05. mirror
06. multipath
07. raid
08. snapshot
09. striped
10. verity
For rest of the targets, handle the STATUSTYPE_IMA case by setting the
measurement buffer to NULL.
For IMA to measure the data on a given system, the IMA policy on the
system needs to be updated to have the following line, and the system
needs to be restarted for the measurements to take effect.
/etc/ima/ima-policy
measure func=CRITICAL_DATA label=device-mapper template=ima-buf
The measurements will be reflected in the IMA logs, which are located at:
/sys/kernel/security/integrity/ima/ascii_runtime_measurements
/sys/kernel/security/integrity/ima/binary_runtime_measurements
These IMA logs can later be consumed by various attestation clients
running on the system, and send them to external services for attesting
the system.
The DM target data measured by IMA subsystem can alternatively
be queried from userspace by setting DM_IMA_MEASUREMENT_FLAG with
DM_TABLE_STATUS_CMD.

A given block device is identified by it's name and UUID. However, both
these parameters can be renamed. For an external attestation service to
correctly attest a given device, it needs to keep track of these rename
events.
Update the device data with the new values for IMA measurements. Measure
both old and new device name/UUID parameters in the same IMA measurement
event, so that the old and the new values can be connected later.

For a given block device, an inactive table slot contains the parameters
to configure the device with. The inactive table can be cleared
multiple times, accidentally or maliciously, which may impact the
functionality of the device, and compromise the system. Therefore it is
important to measure and log the event when a table is cleared.
Measure device parameters, and table hashes when the inactive table slot
is cleared.

Presence of an active block-device, configured with expected parameters,
is important for an external attestation service to determine if a system
meets the attestation requirements. Therefore it is important for DM to
measure the device remove events.
Measure device parameters and table hashes when the device is removed,
using either remove or remove_all.

A given block device can load a table multiple times, with different
input parameters, before eventually resuming it. Further, a device may
be suspended and then resumed. The device may never resume after a
table-load. Because of the above valid scenarios for a given device,
it is important to measure and log the device resume event using IMA.
Also, if the table is large, measuring it in clear-text each time the
device changes state, will unnecessarily increase the size of IMA log.
Since the table clear-text is already measured during table-load event,
measuring the hash during resume should be sufficient to validate the
table contents.
Measure the device parameters, and hash of the active table, when the
device is resumed.

DM configures a block device with various target specific attributes
passed to it as a table. DM loads the table, and calls each target’s
respective constructors with the attributes as input parameters.
Some of these attributes are critical to ensure the device meets
certain security bar. Thus, IMA should measure these attributes, to
ensure they are not tampered with, during the lifetime of the device.
So that the external services can have high confidence in the
configuration of the block-devices on a given system.
Some devices may have large tables. And a given device may change its
state (table-load, suspend, resume, rename, remove, table-clear etc.)
many times. Measuring these attributes each time when the device
changes its state will significantly increase the size of the IMA logs.
Further, once configured, these attributes are not expected to change
unless a new table is loaded, or a device is removed and recreated.
Therefore the clear-text of the attributes should only be measured
during table load, and the hash of the active/inactive table should be
measured for the remaining device state changes.
Export IMA function ima_measure_critical_data() to allow measurement
of DM device parameters, as well as target specific attributes, during
table load. Compute the hash of the inactive table and store it for
measurements during future state change. If a load is called multiple
times, update the inactive table hash with the hash of the latest
populated table. So that the correct inactive table hash is measured
when the device transitions to different states like resume, remove,
rename, etc.

Add 10 counters for various events (hit, miss, etc) and export them in
the status line (accessed from userspace with "dmsetup status"). Also

device mapper is currently the only outlier that tries to call
register_disk after add_disk, leading to fairly inconsistent state
of these block layer data structures. Instead change device-mapper
to just register the gendisk later now that the holder mechanism
can cope with that.
Note that this introduces a user visible change: the dm kobject is
now only visible after the initial table has been loaded.

option for it so that we don't have to build it without any remapped

Commit 95b88f4d71 ("dm writecache: pause
writeback if cache full and origin being written directly") introduced a
code that pauses cache flushing if we are issuing writes directly to the
origin.
Improve that initial commit by making the timeout code configurable
(via the option "pause_writeback"). Also change the default from 1s to
3s because it performed better.

Implementation reuses dm_io_tracker, that until now was only used by
dm-cache, to track if any writes were issued directly to the origin
(due to cache being full) within the last second. If so writeback is
paused for a second.
This change improves performance for when the cache is full and IO is
issued directly to the origin device (rather than through the cache).

remove_raw() in dm_btree_remove() may fail due to IO read error
(e.g. read the content of origin block fails during shadowing),
and the value of shadow_spine::root is uninitialized, but
the uninitialized value is still assign to new_root in the
end of dm_btree_remove().
For dm-thin, the value of pmd->details_root or pmd->root will become
an uninitialized value, so if trying to read details_info tree again
out-of-bound memory may occur as showed below:

Make sure that the zone write pointer offset array is allocated with a
vmalloc in dm_zone_revalidate_cb() by passing GFP_KERNEL gfp flag to
kvcalloc(). However, since we do not want to trigger IOs while
revalidating zones, change dm_revalidate_zones() to have the zone scan
done in GFP_NOIO context using memalloc_noio_save/restore calls.

Add a "metadata_only" parameter that when present: only metadata is
promoted to the cache. This option improves performance for heavier
REQ_META workloads (e.g. device-mapper-test-suite's "git clone and
checkout" benchmark improves from 341s to 312s).

SSDs perform badly with sub-4k writes (because they perfrorm
read-modify-write internally), so make sure writecache writes at least
4k when committing.

Commit d53f1fafec ("dm writecache: do
direct write if the cache is full") changed dm-writecache, so that it
writes directly to the origin device if the cache is full.
Unfortunately, it doesn't forward flush requests to the origin device,
so that there is a bug where flushes are being ignored.
Fix this by adding missing flush forwarding.
For PMEM mode, we fix this bug by disabling direct writes to the origin
device, because it performs better.

Make dm-writecache wait if the kcopyd workqueue is busy (as will
happen if waiting for page allocation or inside submit_bio).
This change improves performance of "mkfs.ext2" by approximately 20%
on one testbed.

There is a lock contention on device_lock in read_one_chunk().
device_lock is taken to sync conf->active_aligned_reads and
conf->quiesce.
read_one_chunk() takes the lock, then waits for quiesce=0 (resumed)
before incrementing active_aligned_reads.
raid5_quiesce() takes the lock, sets quiesce=2 (in-progress), then waits
for active_aligned_reads to be zero before setting quiesce=1
(suspended).
Introduce a fast (lockless) path in read_one_chunk(): activate aligned
read without taking device_lock. In case quiesce starts while
activating the aligned-read in fast path, deactivate it and revert to
old behavior (take device_lock and wait for quiesce to finish).
Add smp store/load in raid5_quiesce()/read_one_chunk() respectively to
gaurantee that read_one_chunk() does not miss an ongoing quiesce.
My setups:
1. 8 local nvme drives (each up to 250k iops).
2. 8 ram disks (brd).
Each setup with raid6 (6+2), 1024 io threads on a 96 cpu-cores (48 per
socket) system. Record both iops and cpu spent on this contention with
rand-read-4k. Record bw with sequential-read-128k. Note: in most cases
cpu is still busy but due to "new" bottlenecks.

The bio_set (io_acct_set) is used by personalities to clone bio and
trace the timestamp of bio. Some personalities such as raid1/10 don't
need the bio_set, so add check to not create it unconditionally.
Also update the comment for md_account_bio to make it more clear.
