When a memblock allocation APIs are called with align = 0, the alignment
is implicitly set to SMP_CACHE_BYTES.
Implicit alignment is done deep in the memblock allocator and it can
come as a surprise. Not that such an alignment would be wrong even
when used incorrectly but it is better to be explicit for the sake of
clarity and the prinicple of the least surprise.
Replace all such uses of memblock APIs with the 'align' parameter
explicitly set to SMP_CACHE_BYTES and stop implicit alignment assignment
in the memblock internal allocation functions.
For the case when memblock APIs are used via helper functions, e.g. like
iommu_arena_new_node() in Alpha, the helper functions were detected with
Coccinelle's help and then manually examined and updated where
appropriate.

The alloc_bootmem(size) is a shortcut for allocation of SMP_CACHE_BYTES
aligned memory. When the align parameter of memblock_alloc() is 0, the
alignment is implicitly set to SMP_CACHE_BYTES and thus alloc_bootmem(size)
and memblock_alloc(size, 0) are equivalent.

Drop PLATFORM_DEFAULT_MEM_START from the platform/hardware.h headers.
Provide definition of CONFIG_DEFAULT_MEM_START always, allow changing it
only in noMMU configurations when PLATFORM_WANT_DEFAULT_MEM is selected.
Change prompt and description so that it's clear that it controls
PAGE_OFFSET and PHYS_OFFSET.

It happens often while I'm preparing a patch for a block driver that
I'm wondering: is a definition of SECTOR_SIZE and/or SECTOR_SHIFT
available for this driver? Do I have to introduce definitions of these
constants before I can use these constants? To avoid this confusion,
move the existing definitions of SECTOR_SIZE and SECTOR_SHIFT into the
<linux/blkdev.h> header file such that these become available for all
block drivers. Make the SECTOR_SIZE definition in the uapi msdos_fs.h
header file conditional to avoid that including that header file after
<linux/blkdev.h> causes the compiler to complain about a SECTOR_SIZE
redefinition.
Note: the SECTOR_SIZE / SECTOR_SHIFT / SECTOR_BITS definitions have
not been removed from uapi header files nor from NAND drivers in
which these constants are used for another purpose than converting
block layer offsets and sizes into a number of sectors.

This helper doesn't buy us much over calling kmap_atomic directly.
In fact in the only caller it does a bit of useless work as the
caller already has the bvec at hand, and said caller would even
buggy for a multi-segment bio due to the use of this helper.

There are three simcalls implemented by ISS and QEMU related to
argc/argv processing: SYS_iss_argc (get number of command line
arguments), SYS_iss_argv_size (get size of command line argument block)
and SYS_iss_set_argv (copy command line arguments to virtual guest
address).
Add definitions for these calls to iss/include/platform/simcall.h

ISS kernel by default has only low memory. But it may be configured to
support high memory and started in a simulator with more than 128M of
RAM. Simdisk driver in such configuration can get IO request with a
high memory page. There may be no TLB entry for that page, only page
table entry. However simulators don't do pagewalking, so such IO request
will fail. Touch IO buffer in the buffer read/write loop so that a TLB
entry is likely there when read or write simcall is invoked.

A number of ISS platform functions use inline assembly to invoke
simulator exit, not all correctly. Define simc_exit(exit_code) and use
it instead of inline assembly.

Simulator stdin may be connected to a file, when its end is reached
kernel hangs in infinite loop inside rs_poll, because simc_poll always
signals that descriptor 0 is readable and simc_read always returns 0.
Check simc_read return value and exit loop if it's not positive. Also
don't rewind polling timer if it's zero.

(really) fast devices. The code has been reviewed and has been
sitting on the side for a few releases, but this is now good enough
for coordinated benchmarking and further experimentation.
Currently O_DIRECT sync read/write are supported. A framework is in

- Cleanup of the bio IO error signaling from Christoph. We used to
rely on the uptodate bit and passing around of an error, now we
store the error in the bio itself.
- Improvement of the above from myself, by shrinking the bio size
down again to fit in two cachelines on x86-64.
- Revert of the max_hw_sectors cap removal from a revision again,
from Jeff Moyer. This caused performance regressions in various
tests. Reinstate the limit, bump it to a more reasonable size
instead.
- Make /sys/block/<dev>/queue/discard_max_bytes writeable, by me.
Most devices have huge trim limits, which can cause nasty latencies
when deleting files. Enable the admin to configure the size down.
We will look into having a more sane default instead of UINT_MAX
sectors.
- Improvement of the SGP gaps logic from Keith Busch.
- Enable the block core to handle arbitrarily sized bios, which
enables a nice simplification of bio_add_page() (which is an IO hot

Currently we have two different ways to signal an I/O error on a BIO:
(1) by clearing the BIO_UPTODATE flag
(2) by returning a Linux errno value to the bi_end_io callback
The first one has the drawback of only communicating a single possible
error (-EIO), and the second one has the drawback of not beeing persistent
when bios are queued up, and are not passed along from child to parent
bio in the ever more popular chaining scenario. Having both mechanisms
available has the additional drawback of utterly confusing driver authors
and introducing bugs where various I/O submitters only deal with one of
them, and the others have to add boilerplate code to deal with both kinds
of error returns.
So add a new bi_error field to store an errno value directly in struct
bio and remove the existing mechanisms to clean all this up.

The network.c code is piggybacking off of the arch independent
CONFIG_NET, which is bool. So the code is either built in or
absent. It will never be modular, so using module_init as an
alias for __initcall is rather misleading.
Fix this up now, so that we can relocate module_init from
init.h into module.h in the future. If we don't do this, we'd
have to add module.h to obviously non-modular code, and that
would be a worse thing.
Direct use of __initcall is discouraged, vs prioritized ones.
Use of device_initcall is consistent with what __initcall
maps onto, and hence does not change the init order, making the
impact of this change zero. Should someone with real hardware
for boot testing want to change it later to arch_initcall or
something different, they can do that at a later date.

- don't lock lp->lock in the iss_net_timer for the call of iss_net_poll,
it will lock it itself;
- invert order of lp->lock and opened_lock acquisition in the
iss_net_open to make it consistent with iss_net_poll;
- replace spin_lock with spin_lock_bh when acquiring locks used in
iss_net_timer from non-atomic context;
- replace spin_lock_irqsave with spin_lock_bh in the iss_net_start_xmit
as the driver doesn't use lp->lock in the hard IRQ context;
- replace __SPIN_LOCK_UNLOCKED(lp.lock) with spin_lock_init, otherwise
lockdep is unhappy about using non-static key.
