improvements to patient onboarding per feedback (#130) 
* printed SMART Health card, also contains human readable text

partly addresses #125

* add example of human readable SHC

* better QR code example

I can't figure out how to resize image with gh markdown: ![image](./reference_smart_health_card_pdf_vaccine.png)

* wordsmithing

* health system maybe can re-issue

* patient demographics no longer contain country

* promote privacy paragraph further up in the doc

partly fixes #125

* address patient proxy use-cases

* get SHC after the fact.
