Copy, paste, and tweak the receipt signing worker code to use for app
signing.
Update receipt signing handling to match apps.
Update receipt error reporting to match that of packaged apps.
Add support for omitting per file signatures since I'm not 100% certain if the
client will scream bloody murder if the individual file sections are there.
