Paul McLanahan 89fd76c83b Ensure x-backend-server header appears in all responses 
To accomplish this we simply move the middleware closer to the front
of the list of MIDDLEWARE so that it is not skipped for redirects.

Fix docs build on RTD: More dependencies needed 
This isn't a problem during our docker build because the other
deps are already installed when the docs file installed. But on
RTD it only installs the docs.txt file.
