When inserting frames into a list of out-of-flow frames, finding the correct position by comparing positions in the content tree doesn't always work, in particular when the frames are for anonymous content. bzbarsky%mit.edu 52aff8ddeb ConstructDocElementFrame can return null if the XBL hasn't loaded yet. Handle 
that XBL loading later by changing how XBL triggers the frame construction and
making it possible to recreate the doc element hierarchy even if there is no

Replace reflow reasons, types, and commands with dirty bits/notifications. Thanks to bzbarsky for almost all of the HTML form controls (mozilla/layout/forms) changes, and many others for help testing and patching.
