"absolutely necessary" in this context means "needs PRUnichar", which is
the reason that nsString.h now #includes prtypes.h.

We can't use NS_MAX or std::max because we lack uniform constexpr
support across our supported compilers.  But we can do a simple inline
max ourselves.
