this change allows servers to be unique by {userid, hostname, type} combination and allows nsIMsgAccountManager::FindServer() to search for servers this way.

includes dropping "const"ness from parameters
some nsString routines were converted back to char* for the time being.
also added nsIMsg.h, it seemed to be missing

Also many fixes to get Unix building again.
