Frame constructor ownership moves to the pres shell, and the style set methods that forward to the frame constructor are gone. Rewrote style set sheet add/remove to reduce code size. Change StyleRuleChanged notification signatures, and change pres shell's handling of style rule change, addition, and removal to just do reresolution. Remove fine-grained ways of clearing style data. bzbarsky%mit.edu fe53f95e60 Make the CSSLoader correctly order stylesheets as they are dynamically added
and removed via the DOM.  Clean up the nsIDocument stylesheet accessors.  Clean
up nsIDocumentObserver stylesheet stuff a bit.  Make style sheets visible in
the CSSOM (though not completely accessible) from the moment the load is kicked
off.  Make us have sheet objects that can be manipulated via CSSOM even for

It will be set to an empty throw function by default on all platforms except mac CW builds. This will allow us to not crash on systems that expect |operator new| to throw an exception by default and will suppress compiler warnings about how |operator new| should throw an exception instead of returning NULL.

Null-terminate lists of RuleValues rather than using a special mEndValue, and build the lists in the reverse order (reversing the meaning of their index). - Eliminate the _moz-option-selected attribute; move the actual selected state into the option content node.
- Change all users of _moz-option-selected to use :checked.
- Add a third parameter to nsIDocument[Observer]::ContentStatesChanged to indicate which pseudoclass changed, this is used for optimizing handling of :checked state changes.

* cut in half the number of (expensive, universally hashed) selectors needed for many UA-stylesheet :link and :visited rules by implementing :-moz-any-link (bug 83484)
 * remove most of EnumerateTagRules since most of it does no useful work (bug 83506)
 * avoid refcounting of atoms for stack-based hash keys (bug 83511)
 * add logging (turned off, except for me) to evaluate effectiveness of RuleHash
 * add a namespace hash, which will be a big win once bug 35847 is fixed (bug 83497)
 * avoid repeated reallocation of mEnumList by setting the initial length to a minimum of 8
 * remove an unneeded variable in the beginning of SelectorMatches
 * fix one case in CSS error reporting (bug 65469), not part of default build
