Bug 513082. Instead of passing aDirtyRect to nsDisplayItem::Paint, have nsDisplayList::OptimizeVisibility (renamed to ComputeVisibility) compute the intersection of the visible region with the bounds of each display item and store the result in the item's mVisibleRect. This lets us do less work to paint display items which have a small intersection with the visible region but a large intersection with the bounding-box of the visible region. r=dbaron

More nsIPresContext deCOMtamination (bug 229371). Removed GetBaseURL, rename GetMedium to Medium and inline it, remove virtual style resolution methods that forward to style set (callers will call style set methods directly). Remove nsIStyleContext (bug 114713), r/sr=dbaron. Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned. Turn on nsRefPtr (bug 104346), and change nsDerivedSafe to not declare AddRef and Release explicitly virtual, r=jag, sr=dbaron.

Correcting our background painting code per the latest CSS specs. By default, backgrounds should be painted to the border

area, and not the padding area as per the CSS 2 Errata, CSS2.1 and CSS3. Also, implementing the CSS3 'background-clip'
and 'background-origin' properties (currently with -moz- prefixes) to control this behavior.

Remove |aForceUnique| parameter from style context resolving methods, and corresponding flag from nsStyleContext, since the only caller was ReResolveStyleContext (on the assumption that other callers used it). Replace the BodyFixupRule with code in nsCSSRendering to fix all the problems with dynamic style changes and backgrounds on the root element and the BODY element.
