nsISHEntry.index is readonly, but if you pass `true` as getEntryAtIndex()'s
second argument, nsISHEntry.index will be modified. This is pretty gross.
This patch changes `index` so it's not readonly (because it's not!) and removes
getEntryAtIndex()'s second argument.

I generally tried to preserve the behavior of consumers where they treated an
exception from getInterface(Ci.nsIContentFrameMessageManager) as a signal to use
some sort of fallback.
I did change the behavior of consumers that walked up to the root same-type
docshell before getting the message manager to just get it directly from the
docshell they have. Please review those parts carefully, and let me know if you
want me to ask some subject area experts to review those.

Summary:
This patch adds the infrastructure to move Activity Stream (about:newtab, about:home,
and about:welcome) into its own special content process - the privileged content
process. This feature of running Activity Stream in the privileged content process
is disabled by default. (See "browser.tabs.remote.separatePrivilegedContentProcess"
preference.) We can deal with other about: pages in a follow-up.

Extract logic around whether a browser needs to change processes, get a new
frameloader because of preloading, etc. from `_loadURI` in `browser.js` to
`E10SUtils.jsm` where it can be shared with other code paths.
The side effect paths (trying to handle in chrome, removing preloaded state) are
left behind in `browser.js` so that the `E10SUtils` version can be a pure
function.

Importing this object is unnecessary after the updates to the WebIDL console from Bug 1425574
and the follow-ups blocking Bug 1430810. There are still callers that access Console.jsm

It covers almost every file with the extension js, jsm, html, py,
xhtml, or xul.
It removes blank lines after removed lines, when the removed lines are
preceded by either blank lines or the start of a new block. The "start
of a new block" is defined fairly hackily: either the line starts with
//, ends with */, ends with {, <![CDATA[, """ or '''. The first two
cover comments, the third one covers JS, the fourth covers JS embedded
in XUL, and the final two cover JS embedded in Python. This also
applies if the removed line was the first line of the file.
It covers the pattern matching cases like "var {classes: Cc,
interfaces: Ci, utils: Cu, results: Cr} = Components;". It'll remove
the entire thing if they are all either Ci, Cr, Cc or Cu, or it will
remove the appropriate ones and leave the residue behind. If there's
only one behind, then it will turn it into a normal, non-pattern
matching variable definition. (For instance, "const { classes: Cc,
Constructor: CC, interfaces: Ci, utils: Cu } = Components" becomes
"const CC = Components.Constructor".)
