Bug 78510. Maintain a per-document hashmap from URI-spec-hash to set of pointers to content elements that are links to those URIs whose state has been queried by the style system. Eliminate nsIStyleSet as an XPCOM interface, making the implementation internal to layout. Frame constructor ownership moves to the pres shell, and the style set methods that forward to the frame constructor are gone. Rewrote style set sheet add/remove to reduce code size. We were leaking nsIClassInfo objects because a DOM class info getter returned AddRef()ed raw pointers. Most of our dom/ and content/ QueryInterface() implementations knew this and accounted for it, but not all. The leak fix is thus achieved by fixing the method to not call AddRef() on its return value, and fixing the appropriate QueryInterface() macros/implementations. As a bonus, this macro-izes many QueryInterface() implementations in content/ which were fully written out.
