Final patch for bug 195350. Make XUL use new nsAttrAndChildren class. Kill nsXULAttributeValue and nsXULAttributes in favour of nsAttrValue resp. nsDOMAttributeMap. Should fix a few bugs, improve performance and reduce bloat.

Eliminate nsIStyleSet as an XPCOM interface, making the implementation internal to layout. Frame constructor ownership moves to the pres shell, and the style set methods that forward to the frame constructor are gone. Rewrote style set sheet add/remove to reduce code size. Fixing bug 124412. Making nsXULDocument inherit nsXMLDocument to avoid the need to duplicate a large amount of code in nsXULDocument. Also removing nsMarkupDocument and nsIDOMEventCapturer that are no longer needed. - convert atoms to UTF8 because 99% of the time, that is what they are used for
- add support for static atoms, where the strings are declared in a large const array and not copied into the heap
