In order to more easily accomodate CSS fallbacks for when native theme rendering is not available, this adds GetWidgetPadding() to nsITheme, which allows the native theme impl to supply padding values which will override the css padding. Don't create new rendering contexts for each caret drawn on *nix platforms since there are no known issues with doing so.  If issues crop up, I will undo this change.
