CellData keeps track of row versus col spans.
nsColFrame keeps track of what cell contributed to constrained width.
nsCellMap::GetCellFrameAt replace with GetCellInfoAt
simplified nsTableCellFrame::RecalcLayoutData and removed FindLargestMargin, CalculateMargins, CalculateMargins
simplified nsTableFrame::RecalcLayoutData and removed AppendLayoutData
Added gsDebugReflow capability, remove all existing debug printf

hyatt%netscape.com fe2af9709a Fix for assertions being thrown when fixed layout tables are flowed at an
unconstrained width.  Instead of asserting, intrinsic first row cell widths will be
used to determine the widths of columns with percentage-specified sizes.

buster%netscape.com 1fba5ac7dc all frame initialization and related logic has moved into the various table frames' Init() methods
from their respective InitalReflow() methods.
table-layout=fixed now completely skips pass1 reflow and doesn't require maxElementSize
information, saving a tremendous amount of time during initial reflow.
