This patch does 2 things:
. when SetBodyUsed() is called, the pump for the stream reading is activated.
. Just because of the reading of the stream could end up executing JS code, we
need to pass the JSContext in the correct state down to SetBodyUsed.
All the instances are converted as follows.
- nsAFlatString --> nsString
- nsAFlatCString --> nsCString

This patch also fixes an existing bug where we used to call
RewriteEntriesSchema at the end of each individual migration,
which is wrong since after the first call, sqlite will think
that it's dealing with the final schema. So far we were lucky
that we only changed the actual schema once.
The bulk of this commit was generated with a script, executed at the top
level of a typical source code checkout. The only non-machine-generated
part was modifying MFBT's moz.build to reflect the new naming.
CLOSED TREE makes big refactorings like this a piece of cake.

The ServiceWorkerRegistrationInfo's principal is the principal of the document
that called register(). If we create WorkerPrivate instances based off of
this, they have a valid principal in terms of security and same-origin-ness,
but the URI path is wrong. When fetching the script from the network, the
channel's principal is used to update the worker principal. We need to do the
same when the script is loaded from Cache. This patch adds support to store the
channel principal in the cache.

We are going to infer all RequestContext types from the corresponding
nsContentPolicyType. It no longer makes sense to store both of these on
InternalRequest. This has the extra benefit that we don't need to worry
about keeping them in sync any more.
