Bug 300030: Move intrinsic width computation out of nsIFrame::Reflow and into its own methods on nsIFrame. Replace reflow reasons, types, and commands with dirty bits/notifications. Thanks to bzbarsky for almost all of the HTML form controls (mozilla/layout/forms) changes, and many others for help testing and patching. For detailed commit logs, see REFLOW_YYYYMMDD_BRANCH, where YYYYMMDD is one of 20061031, 20060830, 20060603, 20060302, 20060119, 20051011, 20050804, 20050429, 20050315, 20050111, and 20041213.

Remove nsIStyleContext (bug 114713), r/sr=dbaron. Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned. Turn on nsRefPtr (bug 104346), and change nsDerivedSafe to not declare AddRef and Release explicitly virtual, r=jag, sr=dbaron.

Bug 77834: Fix problem where form control state is being restored to the wrong form control or not restored at all when going back and forth through session history. The problem was that the hash key we used was the content ID, which was not consistent from page load to page load. removed where the input element was telling the rado/checkbox to return the opposite value

now it changes it value before the DOM call (script) and then if the event is cancelled
it sets the value back (which doesn't happen very often)
