Fix silly argument processing brokenness.
Prepare for IDL_tree_to_IDL when it arrives.

Dead code trimming (ident, etc.).
Typedef support, including arrays.
nsVarArgs instead of ... in C++ headers.
Fix #include scanning bug that caused pain when we skipped an include that
had already been processed.

nsIVarargs should look like).
IDL return value now becomes a final (but before varargs!) out parameter
_retval in C++.

More header-generation niceties: #ifndef __nsIFoo_h__, DO NOT EDIT warning
now mentions source filename.
Generate #include in header to match #include in IDL, not by grovelling for
referenced interfaces.
(XXX: Need to polish up memory management.)
