Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. r=dbaron, sr=dougt. 97667. nsIInterfaceRequestor is now frozen. nsIInterfaceRequestorUtils.h is now required if you want to use do_GetInterface() for convenience. The two were split because the stuff that is now in nsIInterfaceRequestorUtils.h wasn't ready for freezing. Also, .idl files should just contain interface defs anyway.

Fix for bug 46861. nsIDOMWindow is now split into two interfaces: a public interface (still called nsIDOMWindow) for embedders and an internal interface (nsIDOMWindowInternal) for internal use. The union of the two interfaces is still visible from JavaScript. r=jst@netscape.com

Improved full screen implementation; added bold and underline styles. Centralized UI event handling. Added checks to limit number of output lines displayed. Added capability to interrupt hung output data streams. Nearing full backwards compatibility with xterm.
