Summary: Really sorry for the size of the patch. It's mostly automatic
s/nsIDocument/Document/ but I had to fix up in a bunch of places manually to
add the right namespacing and such.
Overall it's not a very interesting patch I think.
nsDocument.cpp turns into Document.cpp, nsIDocument.h into Document.h and
nsIDocumentInlines.h into DocumentInlines.h.
I also changed a bunch of nsCOMPtr usage to RefPtr, but not all of it.
While fixing up some of the bits I also removed some unneeded OwnerDoc() null
checks and such, but I didn't do anything riskier than that.
If we can't reach the outer window from the document, then also can't reach the chrome <browser> either.
Therefore, there's no point in firing the event.

This patch converts all the prefs in MediaPrefs to the new StaticPrefs system.
Note that the "media.wmf.skip-blacklist" pref was present in both MediaPrefs
and gfxPrefs. The copy in MediaPrefs was never used; this explains why this
patch does not add an entry for it to StaticPrefList.h.
Note also that the patch removes themedia.rust.mp4parser pref, because it's
unused.

This class contains a bunch of Gecko DOM specific stuff, and it would make
keeping the copy of MediaDecoder in Servo up to date easier if the Gecko
DOM stuff wasn't in MediaDecoder (and the other classes we import).
