Bug 792652 - Store Manager() in IProtocol (r=dvander)

This patch stores mManager in IProtocol rather than in each individual
PFoo. It also adds a generic accessor for that field. Note that each
individual protocol still defines a Manager() function that returns
PFooParent or whatever. I tried to get rid of that but it was a lot
of work.

This patch also fixes an existing bug where we used to call
RewriteEntriesSchema at the end of each individual migration,
which is wrong since after the first call, sqlite will think
that it's dealing with the final schema. So far we were lucky
that we only changed the actual schema once.

Firefox OS still needs to use the cache API both to access it, and possibly
store app:// URL resources inside it. See for example code in
https://github.com/fxos-components/fxos-fastlist/blob/master/fxos-fastlist.js
that manipulates the cache. Let's restore this support for now since it's
pretty harmless.
